Abstract

Objective: To describe the characteristic of genogram of CL/P cases in Aceh with sociodemographic features and maternal pregnancy history. Methods: cross sectional study with retrospective approach. Data of patients underwent CL/P surgery at the Yayasan Senyum Aceh from 2021-2023 were collected and family member interview was conducted via telephone. Results: 155 patients met our study criteria. Results showed that the majority of patients were male(56.1%), with left unilateral CLP type(43.2%), and live at an altitude of 0-500 meter-ASL(67.09%). Regarding family characteristics, data was obtained for each father and mother, the majority had a secondary education 47.1% and 49.7% respectively, Acehnese 78.7% and 80.6%, working 100% and 29%, with medium category household-income(32.9%), mothers were more than 25-35 years old (57.4%) age group and fathers were less than 40 years old(52.2%). For the pregnancy history:72.2% of mother aged 21-34 years old, 65.8% had less than 2 children, 96.8% had no comorbidities, 94.2% no history of trauma  and folic acid consumption during pregnancy. A family history found in 20% of all cases, with 11% was from 2nd generation. Left unilateral CLP was diagnosed in 1st generation (6.06%), 2nd generation (21.22%), and 3rd generation(18.18%). Conclusion: The characteristics of CL/P patients in Aceh were mostly: male, with left unilateral CLP type, live at altitude of 0-500meter-ASL, from parents with secondary education, working fathers with medium category household-income, mothers aged more than 25-35 while fathers were less than 40 years old. Pregnancy history : mothers were 21-34 years old, had less than  2 children, with no comorbidities, trauma, nor folic acid consumption during pregnancy. The majority of CL/P family history was found from the second generation.

Abstract

Retroperitoneal abscess is one of the rare abscesses within the retroperitoneal space that has higher mortality and morbidity. The obscurity of exact clinical manifestations and diagnosis of the disease causes neglected inflammatory process which leads to worse progression. In this case series and literature review we describes four patients with retroperitoneal abscess. We report the clinical manifestation and treatment of retroperitoneal abscess. The patient often came with chief complaints of pain and feeling of fullness in the waist. CT scan usually revealed a hypodense lesion. The treatment consists of pharmacological treatment and surgical procedures. Nonspecific clinical manifestations often lead to misdiagnosis which causes the progression of  disease. Early diagnosis and treatment is crucial for better outcome.

Abstract

Cancer is still one of the health problems around the world. Cancer is an uncontrolled (abnormal) cell division and can invade surrounding tissues and can also spread to other parts of the body through the blood and lymphatic system known as metastasis. This disease is often diagnosed in men aged 50-80 years with an average age of 73 years. Bladder cancer is divided into transitional cell carcinoma (95%), squamous cell carcinoma (3%), adenocarcinoma (2%), and less than 1% small cell tumors (paraneoplastic syndrome). Risk factors for this disease are smoking, exposure to carcinogenic substances, drugs, infection with the parasite Schistosoma haematobium, chronic irritation (stone disease), physical trauma (in the uroepithelial layer), infectious diseases and those that have not been proven to be the cause are coffee, alcohol, saccharin and cyclamate sweeteners. The prognosis of the disease depends on histologic examination to see the stage of the disease and by tissue biopsy. Methods: This paper is based on a literature search of clinical practice guidelines, scientific literature, websites, and textbooks on the topic of bladder cancer. Results and Discussion: Hematuria is the main clinical symptom in addition to other symptoms as a complaint of bladder cancer. The disease is divided into non-muscle-invasive bladder cancer (NMIBC) and muscle-invasive bladder cancer (MIBC). Non-invasive and invasive cancer conditions play an important role in the treatment and prognosis of the disease. MIBC is a disease condition with high morbidity and mortality. Conclusion: Cystoscopy followed by biopsy resection is the diagnostic standard followed by anatomic pathology examination (histology and cytology) for definitive diagnosis of the disease. The prognosis will be better if the disease is still at superficial and non-invasive stage (Ta), so that only transurethral tumor resection followed by chemotherapy drugs, intravesical and results will be more satisfactory.

Abstract

This study aims to determine the effect of administering Chromolaena odorata L. leaf extract gel on increasing angiogenesis in the healing of open incisional wounds in rats (Rattus norvegicus). In this study, 27 healthy male rats, aged 3-4 months with a body weight of 150-200 g were used. The rats were anesthetized and incised in the back area with a size of 2x2 cm (WxL) and a depth reaching subcutaneous. The rats were divided into three groups (n= 9), the group only given gel (P0), the group of rats that were given 10% Chromolaena odorata leaf extract gel (P1), and the group of rats that were given 20% Chromolaena odorata leaf extract gel (P2). Collection of wound skin tissue samples for histopathological preparations was carried out on days 3, 7, and 14 after treatment, 3 individuals per group for each collection. The results showed that application of Chromolaena odorata leaf extract gel on P1 and P2 could increase angiogenesis significantly (P0.05) compared to P0 on days 3 and 7, but was not significantly different (P0.05) on day 14. The average angiogenesis in P1 and P2 on day 3 was similar, while on day 7 the average angiogenesis on P1 was significantly different compared to P2. It was concluded that administration of Chromolaena odorata leaf extract gel could increase angiogenesis in rats during phase II of the open incision wound healing process.

Abstract

Objective: This study aims to provide an evaluation of the outcome of patients with history of vesicovaginal fistula repair by assessing the success rate and complications that occur. Material & Methods: This study was a retrospective cohort study to describe the etiology, approach, and outcome in patients undergoing vesicovagina fistula repair. We analyzed the complication with vesicovaginal fistula type, etiology and technique used. Results: There were 10 vesicovaginal fistula patients, with mean age 33.4 ± 14.5 years. Both transabdominal and transvaginal approach were equally used. Sixty percent patient was successfully treated without complication. Forty percent patient with incontinence came with mild urinary incontinence and need further repair. Conclusion: Both transvaginal and transabdominal approach were equally effective to repair fistula with relatively low complication rate and high successful rate. Abstract

Introductions: Acute appendicitis is an inflammation that occurs in the Vermiformis appendix. Inflammatory response to acute appendicitis is characterized by the activation of cellular and molecular effectors, including recruitment and activation of neutrophils. The Neutrophil Lymphocytes Ratio (NLR) is a comparison of neutrophil counts with lymphocytes used as a simple benchmark. Methods: we analysed with observational diagnostic test analytical design of retrospective cross sectional studies. The study subjects were acute appendicitis patients at Zainoel Abidin General Hospital in 2018-2019. Subjects upon arrival performed a blood test, and then calculated NLR, the result of NLR count compared to post-appendectomy anatomical pathology examination. The data is analyzed with descriptive statistical analysis, ROC curve analysis and diagnostic tests.Results: There were 176 samples was came to emergency room with diagnosed appendicitis, the most age was 17-25 years (31.2%), 81 male (46%), 95 female (54%). From patological anatomy result were 129 acute appendicitis and 47 chronic appendicitis. From the AUC ROC 0.96 obtained cut off point NLR greater than 3,209 in acute appendicitis. Diagnostic tests obtained sensitivity 83,72% specificity 82,98%, positive guess value 97,71% and negative guess value 65,00% with 83,52% accuracy.Conclusion: we concluded NLR may predict diagnosis of acute appendicitis with NLR greater than 3,209, sensitivity scores of 83,72% specificity 82,98%.

Abstract

Introduction: Tamoxifen (selective estrogen receptor) is the standard hormonal therapy in women with breast cancer with positive estrogen receptor. The use of this regimen proved to reduce the incidence of relapse at local, collateral, or distant body area, and also decrease breast cancer mortality. One of the effects of tamoxifen is to induce endometrial proliferation, cause various abnormalities in endometrial tissue.Method: This study is an unpaired categorical comparative analytic study with a retrospective single cohort (internal comparison group) design. All breast cancer patients at dr. Zainoel Abidin Banda Aceh from 2017 - 2019 was involved in this research. Evaluation of the incidence of endometriosis was carried out based on the duration of tamoxifen using the Mann-Whitney U test. This study is an unpaired categorical comparative analytic study with a retrospective single cohort (internal comparison group) design. All breast cancer patients at dr. Zainoel Abidin Banda Aceh from 2017 - 2019 was involved in this research. Evaluation of the incidence of endometriosis was carried out based on the duration of tamoxifen using the Mann-Whitney U test.Result: A total of 114 patients (46.15%) out of a total of 247 breast cancer patients using tamoxifen reported experiencing endometriosis with a mean age of 49 years old. Most of the patients diagnosed with stage IIIB breast cancer of ductal cell carcinoma. A total of 111 people (97.37%) experienced endometriosis after taking tamoxifen for 30-36 months and the remaining 3 (2.63%) after 24-29 months. The mean duration of tamoxifen use associated with endometriosis was 34 months (p less than 0.001).Conclusion: Long-term use of tamoxifen in breast cancer patients increases the risk of endometriosis

Abstract

Uric acid (UA) and cystine urolithiasis pose unique treatment challenges due to their metabolic origin and high recurrence rates. Oral chemolysis, which targets their underlying pathophysiology, is an alternative to surgical intervention. This study aims to provide a quantitative synthesis of its dissolution efficacy and impact on recurrence based on contemporary primary data. Following PRISMA guidelines, we systematically searched PubMed, Scopus, and the Cochrane Library from January 2015 to October 2025. We included randomized controlled trials (RCTs) and observational studies evaluating oral chemolysis (alkalizing agents for UA; thiol-based agents for cystine) in adults. Primary outcomes (complete stone dissolution) and secondary outcomes (recurrence) were pooled using a random-effects model with Restricted Maximum Likelihood (REML) estimation. Nine primary studies involving 812 patients met the inclusion criteria. For uric acid stones (6 studies, n=597), the pooled proportion of complete dissolution was 69.2% (95% CI: 62.1% - 76.3%; I²=48%). Continuous alkalization therapy was associated with a 66% reduction in 2-year recurrence (Risk Ratio [RR] 0.34; 95% CI: 0.23 - 0.50). For cystine stones (3 studies, n=215), the pooled dissolution rate based on a small cohort was 33.1% (95% CI: 25.0% - 41.2%; I²=29%). Thiol-based therapy was associated with a 40% reduction in 2-year recurrence (RR 0.60; 95% CI: 0.41 - 0.88). In conclusion, oral alkalization is a highly effective primary therapy for appropriately selected patients with uric acid stones. For the more recalcitrant cystine stones, dissolution efficacy is modest and based on limited data, positioning chemolysis as a critical adjunctive therapy. These findings reinforce clinical guideline recommendations for a pathophysiology-based medical approach in non-obstructing metabolic stones.
