Abstract

The goal of an antiseptic is to eliminate or greatly reduce the number of microorganisms in the surgical field at the time of the surgery. The objective of this study was to verify the effectiveness of 20% Piper betle leaf infusion as an antiseptic solution in pre-surgery cataract patients. A clinical trial with partner-matching design was conducted on 31 pairs of eyelids. From each pair of eyelids, one eyelid was asigned to the Piper betle infusion group and the opposite one to the povidone-iodine group. The microorganisms were collected by swab from the patient’s palpebral skin, inoculated on nutrient agar, and incubated at 37oC for 20 hours. The antiseptic effectiveness was measured by counting the microbial colonies before and after administration of the antiseptic solutions. This study demonstrates that the mean colony counts after application of 20% Piper betle leaf infusion showed a significant reduction of 27-100% compared with those before administration (p=0.001). Mean colony counts after 10% povidone-iodine administration showed a significant reduction of 88-100% compared with the mean counts before the solution was applied (p=0.000). The 20% Piper betle infusion has an antiseptic potential. Nevertheless, the 10% povidone-iodine solution has more effective antiseptic capability.

Abstract

Background: Retinal artery caliber (RAC) is narrower in human immunodeficiency virus (HIV)-infected patients beginning antiretroviral therapy (ART). We aimed to assess associations between variations in genes encoding inflammatory mediators and natural killer receptors and retinal artery caliber (RAC) in HIV patients beginning ART.Materials and Methods: Seventy-nine HIV positive patients beginning ART with less than 200 cluster of differentiation (CD) 4 T-cells/μL were recruited. Examinations were performed before ART (V0) and at months 3, 6 and 12 (V3, V6, V12). The study was approved by ethics committees and informed consent was obtained from each subject. Results: Right and left RAC of the HIV patients were narrower than healthy controls (p=0.016 for right RAC) and narrowed further on ART, but demographic associations with the right and left RAC were not identical. Here we show that polymorphisms in genes encoding NK receptors or TNF activity had no significant impact, but right RAC was associated with carriage of allele 2 at IL1A+4845 (p=0.037 after 12 months on ART).Conclusion: Overall the paradoxical reduction in the RAC in HIV patients responding to ART was not modified by genotypes known to affect NK cell function or TNF responses, but IL1A genotype may modify the decline in the right RAC.Keywords: anti-retroviral therapy, CMV, HIV, IL1A, retinal artery caliber

Abstract

Diabetic keratopathy is one of the most common ocular complications in diabetes mellitus. Protocol for diabetes induction in rat model also has been established in many centers. Nonetheless, method in developing diabetic keratopathy rat model has not been well covered. Streptozotocin (STZ) -induced diabetes is widely being used as animal diabetic model. The purpose of this study is to obtain an animal model of diabetic keratopathy that can be used to study the morphology, metabolism, and function of cornea in cases where human samples can be difficult to obtain. A single dose STZ (50 mg/kg) was injected intraperitoneally to control and intervention group. Plasma glucose level concentrations were tested in day 3 postinjections. Obtained animal model of diabetic keratopathy, with significant difference of blood glucose level between intervention and control group (P<0.00). Sensibility of cornea was decreased by week 14th in intervention group. Epithelial defect were more prominent in diabetic group. Despite the differences between human and animal characteristic of diabetic keratopathy, the use of animal models has contributed to better understanding of this disease and to examine more effective treatment.

Abstract

Globaly, the prevalence of refractive errors, especially myopia in children, has increased significantly. The cause of myopia is multifactorial. Near-work activities and family history are some suggested factors associated with myopia. However, there has not been enough data on these factors and their association with myopia in Indonesia. This study aims to determine the prevalence and factors associated with myopia in primary and middle schoolaged children in Pamijahan Village, Bogor District, West Java, Indonesia. Data were taken in a cross-sectional design based on the visual acuity, anterior and posterior eye examination. A 14-item questionnaire regarding myopia-associated factors including family history, playing outdoors, and near-work activities, was distributed to the subjects. The data were taken from June to September 2018. Analysis was conducted using SPSS version 20. The prevalence of myopia in Pamijahan Village is 4,9% out of 474 subjects. Family history, including parental (p=0.001, OR 4.49) and sibling history (p=0.001, OR 5.58), was shown to be associated with the increasing risk of myopia in children while near-work activities and playing outdoors were not associated with myopia in children of Pamijahan Village.

Abstract

Mitochondria is an important organelle in the cell acting as its powerhouse. Damage in the mitochondria leads to cell death thus it is crucial to organs utilizing plenty of energy such as the eyes. Mitochondrial disease in the eye can be primary or secondary. Primary mitochondrial diseases of the eye include dominant optic atrophy,  leber hereditary optic neuropathy, chronic progressive external ophthalmoplegia,  and retinitis pigmentosa. Secondary mitochondrial eye diseases include diabetic retinopathy, age-macular degeneration and glaucoma. Mitochondrial damage can also trigger growth of ocular neoplasm (retinoblastoma and uveal melanoma) are also part of mitochondrial eye diseases. However, it is still a challenge to diagnose mitochondrial eye diseases. Methods that can be used are genetic sequencing, radiological examinations to look for other syndromic signs, as well as biochemistry and histochemistry assays. Definitive therapy for mitochondrial eye diseases are still under research and to date the management is supportive in the form of food supplementation.  Keywords: primary mitochondrial diseases, secondary mitochondrial disease, mtDNA.ROS, ocular tumour

Abstract

Purpose : To evaluate the efficacy and safety of cationic emulsion and Carboxymethyl-Cellulose (CMC) 0,5% eye drops in post-menopausal women with dry eyeMethods : A randomized, double-masked clinical trial was conducted in polyclinic Ophthalmology Department during March-April 2009. Forty-six post menopausal women with dry eye were included in the study. The subjects were randomized into two groups. Group 1 received cationic emulsion and group 2 received CMC 0,5%. Tear Film Break-Up Time (TFBUT), Inter-Blink Interval (IBI) and Ocular Protection Index (OPI) were evaluated before treatment, day 1, 7 and 14 after treatment. The dry eye symptom severity score was evaluated before treatment, day 7 and 14 after treatment with Ocular Surface Disease Index (OSDI) questionnaire Results : The cationic emulsion had significantly better OPI score and TFBUT value on day 1, 7 and 14 compared to CMC 0,5% group (p<0,001). There were no significant differences between groups on IBI value between groups on day 7 and 14 (p>0,05). The severity symptom score on cationic emulsion group was significantly lower compared to CMC 0,5% on day 14 (p<0,001). The subjective complaints after using both eye drops were visual disturbance, foreign body sensation, and itchy eyes. All complaints were only reported first follow-up (day 7). Conclusion : Cationic emulsion in this study had a clearly beneficial effect both in increasing OPI score and lowering subjective symptom score in the treatment of dry eye for post menopausal women.

Abstract

Introduction & Objectives : Essential blepharospasm is a neurological condition that is defined by uncontrolled and abnormal eyelid closure from a central cerebral disorder. Between 40%-85% of blepharospasm occurs in conjunction with dry eye symptoms. Botulinum neurotoxin (BoNT) injections are commonly used treatment for reducing spasms in blepharospasm. Furthermore, it is often suggested for dry eye therapy due to their ability to reduce lacrimal drainage. The purpose of this review is to assess the effectiveness of BoNT treatment for blepharospasm with dry eye symptoms. Methods : A literature research was conducted on online databases such as PubMed, Cochrane library and Google scholar. All studies of BoNT and dry eye with full-text journals published in English within the last 10 years were included. We assessed Tear Breakup Time (TBUT), Schirmer Test, and Blepharospasm Disability Index (BSDI). Results : 204 patients from 6 publications were analyzed. In five trials, the BoNT treatment statistically significant increased TBUT (p<0,5), with mean final TBUT 5,7 ±1.1 s. Furthermore, the Schirmer test also increased with the mean final score 6,29 ± 4,6 mm. The mean BSDI score dropped by 6,05 ± 3,7 in two studies. Conclusion : It is confirmed that BoNT injection is an effective treatment for essential blepharospasm with dry eye symptoms. Clinicians should be screened for dry eye in all blepharospasm patients prior to BoNT treatment.

Abstract

Introduction: HLA-B27 is a genetic marker strongly associated with spondyloarthropathy (SpA) and acute anterior uveitis (AAU). Detection of this allele can support earlier diagnosis and targeted management. However, commercially available HLA-B27 tests are costly and often inaccessible in low- and middle-income countries, including Indonesia. Methods: A cross-sectional study was conducted involving 42 subjects: 14 with SpA, 19 with AAU, and 9 healthy controls. DNA was extracted from peripheral blood samples and analyzed using both conventional PCR (targeting exon 3 of HLA-B27) and a commercial HLA-B27 strip assay. Sensitivity, specificity, and diagnostic accuracy of conventional PCR were calculated against the commercial kit as the reference standard. Results: Conventional PCR showed high sensitivity (100%) and accuracy (85.71%) in SpA patients, indicating its potential as a reliable screening tool in this group. However, its performance in AAU patients was suboptimal, with lower sensitivity (40%) and specificity (55.56%). False positives and false negatives were observed, likely due to limitations in allele coverage by conventional primers. Conclusion: Conventional PCR is a promising, affordable alternative for HLA-B27 detection in SpA patients, particularly in resource-limited settings. However, its lower reliability in AAU cases highlights the need for careful clinical application and further optimization. Larger studies and local allele mapping are recommended to enhance diagnostic precision in diverse populations.

Abstract

Background: Keratomycosis, a fungal corneal infection prevalent in tropical regions, frequently leads to visual impairment and blindness. Liposomal amphotericin B (L-AmB) offers improved efficacy and reduced toxicity compared to conventional amphotericin B deoxycholate and represents a promising option for topical ophthalmic use. Objective: To evaluate and compare the therapeutic efficacy of L-AmB eye drops at 0.15% and 0.5% concentrations in experimental Aspergillus keratomycosis in New Zealand White rabbits. Methods: Four rabbits received midstromal corneal inoculation with a mixed suspension of Aspergillus flavus, A. fumigatus, A. niger, and A. terreus (2 x 10⁴ CFU/0.05 mL). L-AmB eye drops were applied hourly from day 8 post-inoculation until corneal cultures became negative. Antifungal susceptibility was assessed by disc diffusion. Clinical response was monitored by slit lamp examination using a modified scoring system. Results: Negative corneal cultures were achieved in all treated eyes within 10 to 18 days (mean 15 days, L-AmB 0.15%) and 11 to 15 days (mean 14 days, L-AmB 0.5%). No toxic effects were observed. Complete corneal transparency without cicatrix was achieved in one eye treated with L-AmB 0.15%. Conclusion: Both concentrations were effective and safe. Liposomal AmB 0.15% appears as efficacious as 0.5% for experimental Aspergillus keratomycosis. Clinical studies in humans are warranted.
