Abstract

Pendahuluan: Konsensus dalam literatur menunjukkan bahwa individu dengan obesitas mengalami penurunan kekuatan otot quadriceps yang relatif terhadap massa tubuh dibandingkan pada individu non-obesitas. Hal ini terbukti meningkatkan risiko terjadinya osteoarthritis, gangguan keseimbangan, dan potensi keterbatasan fungsional. Program latihan retrowalking menunjukkan manfaat dalam meningkatkan kekuatan otot quadriceps. Tujuan dari studi ini adalah untuk mengetahui efek latihan retrowalking terhadap kekuatan otot quadricep pada individu dengan obessitas. Metode: Studi ini merekrut subjek sehat dengan obesitas derajat I di Departemen Rehabilitasi Medik, RSCM-Jakarta. Latihan retrowalking dilakukan selama 4 minggu (2 sesi/minggu, 15 menit/sesi). Kekuatan otot quadriceps diukur oleh satu penguji tunggal menggunakan handheld dynamometer dalam beberapa interval waktu (pretest, minggu ke-2, minggu ke-4). Analisi data dan uji hipotesis menggunakan uji T berpasangan. Hasil: Sebanyak 7 individu yang direkrut dengan rerata usia adalah 29,57 ± 2,94 tahun dan Indeks Massa Tubuh 26,47 ± 0,8. Kekuatan otot quadriceps meningkat secara signifikan pada akhir minggu ke-2 (D = +3,04, p <0,001) dan minggu ke-4 (D = +5,59, p<0,001) dibandingkan sebelum latihan. Tidak ada efek samping yang dilaporkan selama latihan retrowalking pada studi ini. Kesimpulan: Latihan retrowalking terbukti meningkatkan kekuatan otot quadricep pada individu dengan obesitas. Secara bersamaan, latihan retrowalking merupakan latihan yang aman.

Abstract

BackgroundPain from knee and hip osteoarthritis (OA) can have a significant impact on the physical function and quality of life of affected individuals worldwide. The objective of this study was to evaluate the effect of extracorporeal shockwave therapy (ESWT) on pain, flexibility, function, and medial collateral ligament (MCL) thickness in knee osteoarthritis (KOA). MethodsA study of quasi experimental design was performed involving 15 subjects aged 50 – 70 years with Kellgren-Lawrence grade 2-3 KOA. All subjects were evaluated regarding baseline -pain using visual analogue scale (VAS), range of motion (ROM), functional outcome using Western Ontario and McMaster Universities Arthritis Index (WOMAC), and MCL size. Extracorporeal shock wave therapy was given 3 times, at baseline, and 4 and 8 weeks after intervention. All subjects were given 4000 shocks at intensities of 1.5 – 4 Bar (raised gradually) per session. The shocks were given in the supine position, knee flexed 90o, without topical anesthetic. Statistical analyses were conducted using a dependent t-test. ResultsAfter 8 weeks of intervention, ESWT significantly improved pain score (p<0.01), WOMAC (p<0.01) and MCL thickness (p<0.01) in patients with OA of the knee. However, there was no significant difference in knee ROM, both for degree of flexion and extension (p>0.05). ConclusionThe use of ESWT for treatment of knee OA had a beneficial effect on pain relief, function outcome and MCL thickness. However, there remains a lack of clarity regarding the frequency and dosage levels of ESWT required to achieve maximum improvement.

Abstract

Introduction: The consensus within the literature show that obese individuals have reduced maximum quadriceps muscle strength relative to body mass ascompared to non-obese individuals. These effects were shown to increase therisk of developing osteoarthritis, balance impairment, and potentially cause functional limitations. Retrowalking training programs have beenfound to in-crease quadriceps strength. The aim of this study is to examinethe effect ofretrowalking on Quadriceps strength in obese individuals. Methods: This study recruited healthy subjects withgrade I obesity at Reha-bilitation Departemen, RSCM- Jakarta. The subjects performed retrowalking training for 4 weeks (2 sessions/week, 15 min/session). Quadriceps strengthwas examined by one single examiner with handheld dynamometer inseveraltime intervals until training is completed (pretest, 2th week, 4th week). To ana-lyze the data and test the hypotheses, paired t-test was used. Result: Seven, obese grade I individuals were recruited in thisstudy. Theirmean age was 29,57±2,94 years old and Body Mass Index was 26,47±0,8. The Quadriceps strength improved significantly at the end of 2th week (D = +3,04,p less than 0,001) and 4th week (D = +5,59, p less than 0,001) as compared with their pretreat-ment values. There is no adverse effect that was reported during the wholecourse of training. Conclusion: a 2 weeks and 4 weeks retrowalking training is proven to be aneffective quadriceps strenghtening for obese individuals. Simultaneously, retrowalking training is a safe exercise to be performed.

Abstract

Indeks massa tubuh (IMT) merupakan parameter seorang anak kurus, normal, gemuk ataupun obese. Kegiatan anak mempengaruhi kadar lemak tubuh karena konsumsi karbohidrat yang berlebihan tanpa disertai aktivitas yang seimbang menyebabkan penumpukan lemak sebaliknya bila energi tersebut tidak dapat memenuhi kebutuhan maka lemak akan digunakan untuk memenuhi kebutuhan tersebut yang mengakibatkan berkurangnya kadar lemak tubuh. Demikian juga dengan distribusi tekanan plantar karena anak obese dengan aktivitas rendah, tekanan plantar lebih tinggi dibandingkan anak obese dengan aktivitas tinggi sehingga aktivitas subyek penelitian harus dihomogenisasi untuk memperoleh hasil yang akurat. Penelitian ini bertujuan untuk mengetahui hubungan IMT (kurus, normal, gemuk dan obese) dengan lemak tubuh dan distribusi tekanan plantar saat berdiri dan berjalan pada anak usia 8-10 tahun. Metode : desain penelitian adalah observasional cross sectional / potong lintang dengan jumlah 33 anak sebagai subyek penelitian dengan lifestyle sedentary. Penelitian dilakukan dengan mengukur kadar lemak tubuh menggunakan timbangan Tanita dan puncak tekanan (peak pressure) dengan menggunakan alat Matscan. Tekanan plantar diukur saat berdiri dan berjalan. Anak dengan IMT gemuk mempunyai korelasi yang kuat dengan lemak tubuh (r=0,6333) dan anak dengan IMT obese  mempunyai korelasi yang sangat kuat  terhadap lemak tubuh (r=0,8) sedangkan anak dengan IMT kurus juga mempunyai korelasi terhadap lemak tubuh tetapi korelasinya lemah (r=0,2582). IMT juga berhubungan dengan distribusi tekanan plantar saat berdiri dan berjalan terutama daerah midfoot sedangkan untuk anak kurus ditemukan adanya peningkatan tekanan pada daerah hindfoot sewaktu heelstrike. IMT berhubungan dengan kadar lemak tubuh dan distribusi tekanan plantar terutama pada anak dengan IMT gemuk dan obese.

Abstract

The COVID-19 pandemic pushed physicians to modify conventional practices to reduce the exposure and risk of infection among patients and health workers. Telemedicine is one of the safest methods, and telerehabilitation could prevent the sequelae of COVID-19. A quasi-experimental study with randomized sampling without masking/blinding was conducted. The study was conducted from August 2021 to March 2022 at Hospital A in Pekanbaru, Hospital B in Jayapura, and Hospital C in Jakarta, Indonesia. A total of 27 patients were recruited and divided into control and intervention groups. The control group was given conventional education on therapeutic exercise at home, while the intervention group was shown educational videos about therapeutic exercise at home. The comparison of all functional outcomes between the two groups after the intervention showed a significant difference. The intervention group improved more than the control group, except for the fatigue severity scale. Most of the responses showed that this video was feasible and useful and did not need to be supervised by health workers. Therapeutic exercise educational videos can be an option to deliver rehabilitation programs for post-COVID-19 hospitalized patients.
