Abstract

Abstract: The number of elderly people (over 60 years old) is growing rapidly in this 21th century, reaching as many as 425 millions (+ 6,8%) worldwide in 2000. This figure is estimated almost twice in 2025. In Indonesia, percentage the elderly people in 1995 was as much as 7.5%. In line with the increasing of live expectancy the number of the elderly will grow bigger. This is related to greater need of health service for the elderly. In older people, nutrition problem is closely associated with disease. One factor that causes nutrition problem in the elderly is the increase of morbidity. Increased risk for disease and nutrition problem in the elderly requires early identification of risk for malnutrition in the elderly. Routine assessment of preliminary nutrition status of patients being hospitalized is essential in order to get an overview of nutrition status patients at a time, detect high risk patients and help to identify nutrition treatment specifically for each patient so that appropriate nutrition support can be given to improve nutrition status of patient. The study aimed to identify impact of result screening nutrition based on MNA  method to length of stay and discharge status of elderly patients at inpatient ward of internal medicine and neurology of Dr Sardjito Hospital Yogyakarta. The study was observational that used prospective cohort design and was undertaken at inpatient ward of internal in medicine and neurology of Dr. Sardjito,  Yogyakarta in August-November 2009. Data were collected by the researcher with the help of an enumerator, i.e. nutritionist at inpatient ward. The result of study showed that impact of result screening during initial hospitalization to length of stay of elderly patients based on MNA method was RR 1.63. This indicated that malnourished patients were at risk for being hospitalized > 7 days 1.63 times longer than those not malnourished. Impact  of  result screening during initial hospitalization to discharge status  of elderly patients based on MNA method was RR 1.29. This indicated that malnourished patient were at risk for uncovered discharged as much as 1.29 greater than those not malnourished. There was impact of result screening in admission to length of stay. There was impact of nutrition status to length of discharged status.  Keywords: discharge home, length of stay, nutritional status in initial admission   Abstrak: Pertumbuhan penduduk lanjut usia (umur ≥60 tahun) meningkat secara cepat pada abad 21 ini, yang pada 2000 di seluruh dunia telah mencapai 425 juta jiwa (± 6,8%). Jumlah ini diperkirakan akanmengalami peningkatan hampir dua kali lipat pada 2025. Di Indonesia, persentase lanjut usia pada 1995 mencapai 7,5%. Dengan meningkatnya angka harapan hidup, jumlah lanjut usia pun akan bertambah banyak. Hal ini terkait dengan perlunya peningkatan pelayanan kesehatan lanjut usia. Pada lanjut usia, masalah gizi erat kaitannya dengan penyakit. Salah satu faktor yang menyebabkan lanjut usia menjadi rawan gizi yaitu peningkatan morbiditas penyakit (Darmojo, 2006). Dengan meningkatnya risiko penyakit dan disertai gangguan nutrisi pada lanjut usia, sehingga perlu dilakukan identifikasi risiko malnutrisi pada lanjut usia sedini mungkin. Penilaian status gizi awal pasien masuk rumah sakit sangat penting  dilakukan secara rutin karena dapat menggambarkan status gizi pasien saat itu, mendeteksi pasien-pasien yang beresiko tinggi dan membantu mengidentifikasi perawatan gizi secara spesifik pada masing-masing pasien sehingga dukungan nutrisi yang tepat dapat diterapkan untuk meningkatan status gizi pasien. Untuk mengetahui pengaruh hasil skrining awal berdasarkan metode MNA (Mini Nutritional Assessment) terhadap lama rawat inap dan status pulang pasien lanjut pada ruang rawat inap penyakit dalam dan saraf di RSUP Dr.Sardjito Yogyakarta. Jenis penelitian ini adalah penelitian observasional dengan menggunakan rancangan kohort prospektif. Penelitian ini dilakukan di ruang rawat inap penyakit dalam dan  saraf pada pasien lanjut usia  di Rumah Sakit Umum Pusat Dr. Sardjito Yogyakarta. Penelitian dilaksanakan pada bulan Agustus-November 2009. Pengumpulan data dilakukan oleh peneliti dengan bantuan enumerator yaitu  ahli gizi yang bertugas di ruang rawat inap. Berdasarkan hasil skrining dengan metode MNA pada  pasien lanjut usia terhadap lama rawat inap, maka diketahui bahwa nilai RR=1,63. Hal ini menunjukkan bahwa pasien yang terpapar (malnutrisi) berisiko dirawat selama ≥ 7 hari adalah 1,63 kali lebih besar daripada pasien yang tidak terpapar (tidak malnutrisi). Berdasarkan hasil regresi logistik tidak ada pengaruh antara hasil skrining dengan lama rawat inap. Ada pengaruh secara statistik antara usia, jenis penyakit dan kelas perawatan terhadap lama rawat inap. Berdasarkan hasil regresi logistik, jenis penyakit memiliki pengaruh yang paling dominan dengan nilai RR  3,88 terhadap lama rawat inap.Berdasarkan hasil skrining awal masuk rumah sakit terhadap status pulang pasien lanjut usia berdasarkan metode MNA, maka diketahui nilai RR=1,29. Hal ini menunjukkan bahwa pasien yang terpapar (malnutrisi) berisiko keluar dalam keadaan tidak sembuh sebesar 1,29 kali lebih besar daripada pasien yang tidak terpapar (tidak malnutrisi). Berdasarkan hasil uji regresi logistik ada pengaruh antara hasil skrining dengan status pulang dengan nilai OR 9,21. Demikian pula ada pengaruh antara usia dan jenis kelamin dengan status pulang (p< 0,05). Tidak ada pengaruh antara hasil skrining dengan lama rawat inap. Ada pengaruh antara usia, jenis penyakit dan kelas perawatan terhadap lama rawat inap. Abstract

ABSTRACTBackground: Adequate nutrient intake for in patients is much required in efforts to prevent declining nutrition status during hospitalization. Malnutrition may occur before hospitalization due to the disease or inadequate nutrient intake; however malnutrition may also occur during hospitalization. The result of a preliminary study shows that 75% of patients undergo declining nutrition status during hospitalization. Nutrition is an integral part of medication or recovery process and shortens length of stay.Objectives: To identify effect of changes in nutrient intake to nutrition status and length of stay of adult in patients.Methods: The study was observational with prospective cohort approach using subject that got inadequate intake (<80%) in the beginning of hospitalization as exposed group and those that got adequate intake (≥80%) in the beginning of hospitalization as non exposed group. Assessment of inpatients was made after three days in hospital and then comparison was made to average intake prior to discharge.Results: Changes in intake and nutritional status did not significantly (p>0.05) affect length of stay of adult inpatients at internal medicine and neurology wards of Dr.Sardjito Hospital Yogyakarta. Nutrition therapy significantly (p=0.04) affected change in nutritional status of in patients with RR=2.24.Conclusions: Changes in intake and nutritional status did not affect length of stay. Nutrition therapy affected as much as 2.24 times to changes in nutritional status of adult inpatients at internal medicine and neurology wards of Dr. Sardjito Hospital Yogyakarta.KEYWORDS: length of stay, nutritional status, nutrient intake, nutrition therapyABSTRAKLatar belakang: Asupan zat gizi yang cukup bagi pasien sangat diperlukan untuk mencegah penurunan status gizi selama dirawat di rumah sakit. Malnutrisi juga dapat terjadi sejak sebelum masuk rumah sakit karena penyakit yang diderita maupun asupan zat gizi yang tidak adekuat, namun malnutrisi juga dapat terjadi setelah dirawat di rumah sakit. Hasil studi pendahuluan menunjukkan bahwa 75% pasien mengalami penurunan status gizi selama dirawat di rumah sakit. Terapi gizi merupakan bagian penting dari proses pengobatan dan pemulihan serta dapat mempersingkat lama rawat inap.Tujuan: Untuk mengidentifikasi efek perubahan asupan zat gizi terhadap status gizi dan lama rawat inap pada pasien dewasa.Metode: Penelitian ini merupakan penelitian observasional dengan desain kohort prospektif. Subjek dibagi menjadi 2 kelompok, yaitu kelompok yang memiliki asupan zat gizi tidak adekuat (<80%) pada awal masa rawat inap di rumah sakit sebagai kelompok terpapar dan kelompok yang memiliki asupan zat gizi adekuat (≥80%) pada awal masa rawat inap di rumah sakit sebagai kelompok tidak terpapar. Pengukuran asupan makan dilakukan setelah pasien tiga hari dirawat di rumah sakit untuk selanjutnya dibandingkan dengan rata–rata pengukuran asupan pasien sebelum pulang.Hasil: Perubahan pada asupan dan status gizi tidak berpengaruh secara signifikan terhadap lama rawat inap pasien dewasa di bangsal penyakit dalam dan saraf RSUP Dr.Sardjito. Terapi gizi secara signifikan mempengaruhi perubahan status gizi pasien dengan RR = 2,24.Kesimpulan: Perubahan pada asupan dan status gizi tidak berpengaruh secara signifikan terhadap lama rawat inap. Abstract

Background: The decline in nutritional status of hospitalized patients was reported to be assossiated with longer length of stay and higher hospital charges. However, the effect of changes in nutritional status on hospital outcomes in Indonesia is still unknown.Objective: To determine the effect of changes in nutritional status on length of stay and hospital charge among adult hospitalized patients.Method: A total subjects of 262 adult patients who were admitted to internal and neurology departments of Dr. Sardjito, Dr.M.Jamil, and Sanglah hospitals were included in this study. Nutritional status of each patient was assessed using Subjective Global Assessment (SGA) method. Information on length of stay and hospital charge was collected based on medical records.Results: Subjects with nutritional status declined from normally to moderately, normally to severely, and moderately to severely malnourished were 6,3 (OR=6.32, 95% CI=1,3-29,8); 11,9 (OR=11.94, 95% CI=1,02-139,1); and 6,90 (OR=6.9, 95%CI=1,5-32,0 )times more likely to stay longer than those with nutritional status stayed normal during hospitalitation. They also had 3,3; unlimited; and 1,76 times risk on higher hospital charges than reference group (95% CI=1,123-9,529; unlimited; and 0,590-5,245).Conclusions: The declines of nutritional status from normally to moderately, normally to severely, and moderately to severely malnourished in hospitalized patients influenced to longer length of stay. Normally to moderately and normally to severely malnourished in hospitalized patients influenced to higher hospital charges.

Abstract

Background: Malnutrition in hospitalized patients is associated with longer stay, higher risk of complication and mortality.Subjective Global Nutrition Assessment (SGNA) is a recently discovered method to assess nutritional status.Objective: To assess the association between nutritional statuses of under-fi ve year old patients measured with SGNA and length of inpatient stay, discharge status and occurrence of malnutrition.Methods: Under-fi ve year old patients hospitalized in Pediatric Ward of Pekanbaru Hospital had their nutritional status measured with SGNA on admission. Three groups were identifi ed: exposed to bad nutritional status, i.e. SGNA C (worst) and B (moderate), and not exposed to bad nutritional status (SGNA A). Length of hospital stay, discharge status and occurrence of malnutrition in exposed groups (SGNA B and C) were independently compared to non-exposed group (SGNA A) as the reference. Other variables measured were types of diseaseand intake of energy and protein.Results: Children with SGNA C and B had higher risk to stay longer in hospital, i.e. RR (95%CI) of 3.2 (1.7-6.0) and 2.5 (1.3 – 4.9), respectively. They had also higher risk for discharged without complete recovery, i.e. RR (95%CI) of 3.3 (1.8-6.2) and 2.2 (1.1 – 4.3), respectively. Children with SGNA C had higher risk for hospital malnutrition, i.e. RR (95%CI) of 2.8 (1.5-5.2), but not children with SGNA B. Types of disease, energy and protein intake were not important effect modifi ers for this association.Conclusion: Nutritional status on admission was signifi cantly associated with length of hospital stay, discharge status and occurrence of hospital malnutrition.

Abstract

Background : Numerous research have been made to find out contributive factors of the exclusive breastfeeding.Objective: The objective of this study was to find out whether mothers’ nutritional status had any influence on the exclusivity of breastfeeding and  on the growth of their babies up to 4 months.Methods: This observational study was carried out with a prospective cohort design and lasted from November 2004 to May 2005. The subjects were 80 pairs of mother and baby treated at the Obstetric and Gynecology Unit (UPF) of Local Public Hospital (RSUD) Prof. Dr. W.Z. Johannes at Kupang – NTT. The subjects were divided into two groups namely Undernutrition status group marked by <23,5 cm mid arm-circumference, and control group comprised 46 mothers of well- nutrition  status who had bigger mid arm-circumference >23,5 cm. Observed babies were those of normal or spontaneous delivery, non twin with >2500 g birth weight. Follow up observations were made at family home and the data on breastfeeding characteristics were collected by interview, babies weight were measured by Secca baby scale with 0,01 kg level of accuracy mid arm-circumference was measured by means of LILA tape, and body length was measured using length-board. Tools measure with 0,1 cm level of accuracy. Some statistical tests were used namely Chi-Square; student’s t-test; and while Kaplan Meier log-rank test as needed.Results: More than 50% of under-nutrition mothers were under 25 years old. Chi-Square test shows that significant difference between mother’s age was (p=0,04). More than 50% of the KEK group gave birth for the first time or ‘primipara’ (p=0,25). Kaplan Meier analysis showed that  there was no difference between survival median of breastfeeding be- tween the both group (p=0,53). t-test showed that there were significant differences of the babies body weight of the both KEK and non KEK mothers, namely pd”0,01, p=0.01, and p=0.03 in the first, second, third, and the fourth month respectively. In terms of the average babies body length, the t-test also showed significant differences between both groups with p=0.02 and p=0.01 in the third and the fourth month respectively.Conclusion: I can be concluded that mother’ nutrition status did not have any influence on the exclusivity of breastfeeding, however, it did significantly influence babies growth from 0- 4 month.

Abstract

Background: Hypertension is happened generally at someone who has age more than 40 year. The prevalence of hypertension was tend to increase along with the increasing of amount of the elderly. Changing of nutrition status and unfavorable nutrition intakes like is high fat, sodium and low micronutrients (potassium, calcium, magnesium) have an effect to incidence of essential hypertension. Retreating of biologic accompanying aging process sometimes make the elderly become resistance to get quality of intakes nutrition.Objective: The study was conducted to know relations between nutrition intakes and nutrition status with occurrence on essential hypertension of the elderly in the health center Curup and Perumnas, Rejang Lebong District.Method: The study was observational analytic with case control matched study. Subject of the study was divided into two group i.e. case and control with comparison case and control 1:1 which matched to age and sex. Total subject were 150, taken by simple random method. The cases were essential hypertension patients who had recently diagnosed by doctors. Blood pressure measurements were three times in supine position. Hypertension was defined as average of three times supine systolic/diastolic blood pressures at p >140/ 90 mmHg. Nutrition status was determined by Body Mass Index (BMI). Intake nutrients were collected using Food Frequency Questionnaire (FFQ) in the last 3 months by using food model. χ 2 McNemar were used to bivariate analytic and condition logistic regression to identify risk factors associated with essential hypertension.Result: This study showed that there were 4 variables significantly related with essential hypertension, i.e. intakes of sodium (OR: 3.43; CI 95%:1.09-10.77), potassium (OR: 0.24; CI 95%:0.07-0.84), stress (OR: 3.79; CI 95%:1.18-12.12), and obesity (OR: 4.57; CI 95%:1.49-13.95). The no significant correlation (p>0.05) identified for intakes of fat, calcium, magnesium, coffee consumption, smoking status and family history of essential hypertension.Conclusion: High intake of sodium, stress and obesity were the risk factors on occurrence of essential hypertension. In the other hand, high intake of potassium were reduce the risk on occurrence of essential hypertension (protective factor) in the health center Curup and Perumnas, Rejang Lebong District.

Abstract

Background: The main problem of children’s oral cavity are caries and gingivitis. One cause of caries and gingivitis is plaque of which its formation and composition are affected by nutritional factor. Nutrition can be cariogenic or non-cariogenic both of which can bring direct or indirect impact to oral cavity. In the oral cavity both protein and fat are non-cariogenic.Objective: To identify the relationship between protein and fat intake with oral health status (level of caries seriousness, oral cleanliness, gingiva health and saliva acidity) of preschool children in Jetis subdistrict, Bantul District, Yogyakarta Special Territory.Methods: This was an observational study which used a cross sectional design. Subject of study were 100 children of 4-6 years old taken by using propotional random sampling. Data collected consisted of eating pattern, respondent characteristics, level of caries seriousness, oral cleanliness, gingiva health and saliva acidity. Data analysis used Person correlation statistic and linier regression. Data collected consisted of eating pattern, respondent characteristics, level of caries seriousness, oral cleanliness, gingival health status and saliva acidity. Data analysis used Pearson correlation statistic and linear regression tests.Results: Result of the study showed that there was relationship between protein and fat intake with oral cleanliness and saliva acidity (p<0.05) and there was no relationship between protein and fat intake with level of caries seriousness and gingiva health (p>0.05).Conclusion: There was relationship between protein and fat intake with oral health status (oral cleanliness and saliva acidity), but not with oral health status (level of caries serious- ness and gingiva health).

Abstract

Background: Indonesia is a tropical country rich in various vegetables and fruits which are necessary for health because they contain lots of vitamins, minerals and fibers. However, Indonesian children and teenagers do not often pay attention to balanced nutrition, particularly vegetables and fruits. They even tend to choose fast food which may lead to occurrence of obesity. Prevalence of obesity among teenagers becomes an important issue because it persists to adulthood. Impact of obesity is a threat to public health and needs serious attention. Considering the important role of vegetables and fruits for health especially among obese teenagers, awareness on the necessity of consuming those needs to be made and built through nutrition counseling.Objective: The objective of the study was to identify the effects of counseling on the advantages of vegetables and fruits consumption to nutrient intake (energy, protein, fat, carbohydrate and fiber) among obese teenage students of junior high school at Palembang Municipality.Method: The study was quantitative with quasi experimental design. Subject of the study were obese teenage students of junior high school at Palembang Municipality aged 12 – 15 years old. There were 72 respondents of intervention group and 72 respondents of control group. Samples were taken using non random sampling technique. T-test was used to find out effects of counseling to consumption of vegetables and fruits and nutrient intake before and after intervention.Result: Delta average value of fruit consumption and nutrient intake (energy, fat, carbohydrate, and fiber) based on t-test analysis showed significant difference in both groups with p<0.05. Meanwhile, delta average value of vegetable consumption and nutrient intake (protein) did not show significant relationship (p>0.05).Conclusion: Counseling on the advantages of vegetables and fruits affect consumption of fruits and nutrient intake (energy, fat, carbohydrate and fiber).
