Abstract

Background: Prevalence of gastroesophageal reflux disease (GERD) is escalating in Asia. The present study aimed toinvestigate the prevalence of GERD symptoms as well as potential risk factors associated with GERD in residents in a rural area in Jakarta. Method: The study population consisted of subjects who aged 20 years or more and lived in rural area Kebon Bawang-Tanjung Priok, Jakarta. A self-administered questionnaire based on earlier validated questionnaire (GERDQ) was given. The questionnaire consisted of demographic characteristic, self and environment sanitary, social and past illness history, and the symptoms score for GERD. Total score for GERD’s symptoms score was 18, with a score of at least 8 was considered to have symptomatic GERD.Results: The questionnaire was completed in 90 subjects. Of the 90 subjects, 12 (13.3%) had GERDQ score ≥ 8 which was the cut-off for definingpresence of GERD. On both univariate and multivariate analysis, age 50 years old (OR = 6.33; 95% CI 1.1-35.6), handwashing habit before eating (OR = 6.93; 95% CI 1.1-43.8), and raw/tap water (OR = 10.28; 95% CI 0.8-130) were significantly associated with the presence of GERD.Conclusion: Prevalence of GERD was high (13.3%) in rural area in Jakarta. Age 50 years old, handwashing habit before eating, and raw/tap waterwere risk factors for GERD in this population.

Abstract

Pendahuluan. Terdapat peningkatan mortalitas akibat penyakit kardiovaskular (PKV) sebesar 50% pada pasien artritis reumatoid (AR). Trigliserida pascaprandial (TGPP) saat ini dikaitkan dengan risiko penyakit jantung iskemik, infark miokard, stroke iskemik, kematian, serta peningkatan kadar molekul adhesi. Kadar molekul adhesi yang meningkat merupakan tanda terjadinya aktivasi endotel, proses awal pada terbentuknya lesi aterosklerosis. Belum ada penelitian tentang peran TGPP dalam risiko kardiovaskular pada pasien AR. Penelitian ini ingin mengetahui hubungan antara TGPP dengan penanda biologis aktivasi endotel. Metode. Penelitian ini adalah studi potong lintang, yang menggunakan analisis korelasi dengan analisis multivariat. Sampel diambil dengan metode consecutive sampling. Pada subjek penelitian dilakukan pemeriksaan profil lipid dan penanda aktivasi endotel. Sebagai penanda biologis aktivasi endotel digunakan sICAM-1 dan sE-selectin. Dilakukan uji korelasi antara TGPP dengan sE-selectin dan sICAM-1. Hasil. Tidak terdapat korelasi antara TGPP dengan kadar sE-selectin dan sICAM-1 pada analisis multivariat. HDL mempengaruhi kadar sICAM-1 (R2=0,087). Sementara itu kadar sE-selectin dipengaruhi oleh DAS-28 (R2=0,174), indeks massa tubuh (R2=0,125), dan gula darah pascaprandial (R2=0,138). Simpulan. Tidak ditemukan kaitan antara TGPP dengan kadar sE-selectin dan sICAM-1 pada pasien AR.

Abstract

Background: Malnutrition is a very common problem in hospitalized patients. Based on studies conducted in various countries, the prevalence of hospital malnutrition ranges from 20-45%. Hospital malnutrition can occur as a result of insufficient nutrient intake, impaired absorption of nutrients in the gastrointestinal (GI) tract, loss of nutrients due to disease, or increased metabolic requirement during illness. Nutrition is the basis for the healing process as it requires good nutrients through various enzymatic reactions. Some studies have shown that hospital malnutrition increases morbidity, mortality, prolongs the duration of hospitalization, and delays recovery. To improve nutritional status, a high-protein diet can be given. Liquid diet is one of the alternative forms of nutrition because it is easier to swallow.Method: This study was a double-blind randomized controlled trial conducted to assess the effectiveness of high protein liquid dietary supplementation in malnourished hospitalized patients. The patients were assessed for their nutritional status based on ESPEN 2015 criteria. Patients who experienced malnutrition will be divided into 2 groups, namely the control group which was given normal protein liquid diet, while the intervention group was given high protein liquid diet supplementation as much as 2 bottles (200 mL) per day for 7-10 days. Furthermore, the nutritional status of the patient was assessed.Results: By administering high-protein liquid diet supplementation, there was a tendency to increase the strength of the right handgrip in 3 subjects (50%), the tendency to increase body mass index (BMI) in 3 subjects (50%), Subjective Global Assessment (SGA) score improved in 2 subjects with mild/moderate malnutrition (33.3%) and 1 subject with normal nutrition (16.7%), and the tendency to increase the fat-free mass in 3 subjects (50%).Conclusion: The administration of high protein liquid dietary supplementation tended to increase the strength of the right handgrip, BMI, SGA score, and body fat-free mass index.
