Abstract

Puskesmas merupakan unit pelaksana teknis Dinas Kesehatan yang memiliki tanggungjawab untuk menyelenggarakan pembanguan kesehatan di wilayah kerjanya. Sebagaipusat penggerak pembangunan bidang kesehatan, puskesmas juga didorong untuk selalumeningkatkan kinerjanya. Salah satu tolak ukur yang dapat digunakan puskesmas untukpeningkatan kinerjanya adalah harapan masyarakat terhadap pelayanan yangdiselenggarakan puskesmas. Dengan mengetahui seberapa besar harapan masyarakat terhadap puskesmas, maka akan lebih mudah bagi pemerintah dalam mewujudkankepuasan masyarakat khususnya di bidang kesehatan. Tujuan penelitian ini adalahmengukur tingkat harapan masyarakat terhadap kinerja puskesmas berdasarkanreliability, responsiveness, assurance, emphathy, tangibles, assesibility, availability,continuity, keuangan, humanitas, ketersediaan informasi, dan kenyamanan lingkungan,serta kompetensi petugas. Penelitian ini menggunakan jenis penelitian survey denganpendekatan kuantitatif. Harapan masyarakat pada penelitian ini dibagi menjadi 5 kategoriyakni: Sangat rendah, rendah, biasa saja, tinggi, dan sangat tinggi. Berdasarkan hasilpenelitian diketahui bahwa harapan masyarakat terhadap pelayanan puskesmas beradapada level tinggi. Hal ini dapat disimpulkan dari hasil survey dimana sebagian besarmasyarakat menilai 11 dari 13 variabel penilaian puskesmas merupakan suatu hal yangpenting. Variabel tersebut diantaranya : reliability (42,69%), responsiveness (43,8%),assurance (43,93%), emphathy (53,07%), tangibles (48,6%), assesibility (36,27%),availability (52,84%), continuity (35,4%), humanitas (46,13%), dan kenyamananlingkungan (48,27%), dan kompetensi petugas (49,75%), sedangkan 2 variabel sisanyayakni: keuangan (35,6%) dan ketersediaan informasi (41,47%) dinilai sebagai hal yangbiasa saja. Abstract

Setiap peserta Jaminan Kesehatan Nasional (JKN) berhak untuk memperoleh pelayanankesehatan secara menyeluruh, berjenjang, efektif, dan efisien sehingga Fasilitas KesehatanTingkat Pertama (FKTP) dan Fasilitas Kesehatan Rujukan Tingkat Lanjut (FKRTL) wajibmenerapkan sistem rujukan berjenjang. Nilai rasio rujukan pasien peserta JKN di UPT.Pelayanan Kesehatan Universitas Jember pada bulan Januari-November 2015 yaitu37,22% dan meningkat menjadi 39,2% hingga bulan Desember 2015 atau melebihi nilairasio rujukan optimal sebesar 10%. Penelitian ini bertujuan untuk mengkaji pelaksanaanSistem Rujukan Berjenjang dalam Program JKN di UPT. Pelayanan Kesehatan UniversitasJember. Jenis penelitian ini adalah deskriptif dengan pendekatan kualitatif. Hasil penelitian menjelaskan bahwa, ketersediaan dokter belum mencukupi apabila dilihat dari statusdokter yang bukan dokter tetap karena apabila semua dokter tidak bisa hadir untukmemberikan pelayanan kesehatan maka poli umum terpaksa akan ditutup. Ketersediaanobat dan alat kesehatan kurang mencukupi karena tidak selengkap pada FormulariumNasional dan Kompendium Alat Kesehatan. Diagnosis penyakit yang banyak dirujukterutama Diabetes Melitus tanpa komplikasi maupun dengan komplikasi dan hipertensiesensial yang merupakan kompetensi FKTP. Pasien peserta JKN yang dirujuk tidaksemuanya membutuhkan pelayanan kesehatan spesialistik/subspesialistik serta masihterdapat pasien yang meminta dirujuk dengan alasan terutama sugesti pasien yang lebihmempercayai kompetensi dokter spesialis. Abstract

UPT. UNEJ Medical Center (UMC) is one of the health service provider institutions in Jember. Patient satisfaction is one indicator of the quality of services provided by health providers. Patient satisfaction is also an investment to get more patients and have a high loyalty. Therefore UPT. UMC requires the satisfaction index to identify the extent service user satisfaction levels of services. Community satisfaction index consisting of 14 essential elements which valid and reliable as the minimum elements that must exist. The purpose of this study was to describe the user community satisfaction index of health services in UPT. UMC. Design research using observational studies because they did not give a treatment of the respondent. While time-based research using cross-sectional approach. Respondents research were patients of UPT. UMC as many as 94 people. Results obtained research data and information that the community satisfaction index of UPT. UMC was 68.72 which included in a good performance category. Keywords : Community Satisfaction Index, Health Services, UPT. UMC.

Abstract

Jember as a regional center for agriculture and plantations in East Java has a population with considerable professional farmers. Health insurance for farmers is indispensable as by the implementation of a national health insurance program as of 1 January 2014. The problem that arises was that the number of informal sector workers (including farmers not PBI) which register was still very little. Therefore, it was necessary to study on theeffectiveness of the program socialization JKN on knowledge and attitudes of family farmers. This type of research wasa descriptive observational where researchers conducted interviews and questionnaires as well as performing measurements on several variables being studied without the intervention of the research object. This study was conducted in three (3)districts which have a number of farming households highest in Jember, in August 2015. The variables in the research was the respondents' knowledge, attitude, and information source.The results showed that only 40 respondents (45.4%) had knowledge of the health insurance program. Of the 40 respondents, only 20% were aware of how the mechanism ofregistration, 22.5% know the mechanism of payment, and 20% know how to take advantage of health services in the health insurance program. Sources of information that most (52.5%) are in the vicinity (word of mouth). The attitude of the majority of respondents rejected thehealth insurance program participants (68.9%).the socialization of the health insurance program as part of a policy of running less effective communication, especially in the aspect of the transmission, clarity, and consistency.Keywords: socialization, participation, health insurance.

Abstract

One of the Alert Village activities is implementation of surveillance based on community namely surveillance of maternal and child health through activities which are conducted at the Integrated Health Services Post.  Surveillance activities are conducted by a cadre who records and reports using information system of the integrated health services. The resulted information is weight,  child’s nutritional status, immunization, risk level of pregnant women, immunization of Tetanus Toxoid, list of mothers and children who are either life or lifeless, and presence of health workers during providing services. Based on the previous study, resulted information was difficult to reaccess and had not resulted the complete, accurate, and clear information. The objective of this research was to develop information system of the integrated health services post for supporting surveillance of maternal and child health based on community at the alert village. Design of this research was pre-experimental (one group pretest-posttest). Subject consisted of the end user  (cadres of the integrated health services post). Data were analyzed using considered average of resulted information between before and after developing the information system.The score of the considered average on the new system (3.07) was higher than the score of the considered average on the old system (2.54). It means that the new system could overcome problems related with quality of information namely accessibility, completeness, accurateness, and clarity. Abstract

Antibiotics are becoming more prevalent as environmental pollutants, raising public health concerns. Antibiotics are routinely employed as growth promoters and therapeutic agents in poultry feeds. Because antibiotics are not completely metabolized in chicken body tissues, they are deposited in meat as parent compounds and eventually extruded into the environment via fowl droppings. Organic fertilizers based on chicken litter are typically recycled into the soil to improve the structure and fertility of agricultural land. Chicken litter, a vital source of nutrients for crop productivity, may also carry a variety of human infections that might endanger humans who consume contaminated food or water. A convenience sampling of five places was carried out. Litter samples were collected aseptically. CLSI standards were used to isolate E. coli and Proteus spp. The disc diffusion method was used to determine antibiotic susceptibility. E. coli and Proteus spp. were found in 80% and 60% of the samples. All E. coli and Proteus spp isolates derived from laying hen manure were resistant to sulfamethoxazole, ciprofloxacin, cefixime, tetracycline, amoxiclav, ceftriaxone, and chloramphenicol. However, only 50% and 33% were resistant to azithromycin. All E. coli and Proteus spp. Isolates were multidrug resistant. There was no resistance reported to cefepime or imipenem. Contamination of chicken litter with multidrug-resistant E. coli and Proteus spp. may be an underappreciated source of antimicrobial resistance (AMR) transmission to animals, people, and the environment. This demonstrates the importance of a One Health strategy to AMR surveillance and control in Jember. Monitoring AMR hazards and trends in the chicken litter would be possible with ongoing surveillance.

Abstract

The COVID-19 pandemic condition presents its own difficulties in achieving this competency for students. Learning from home has several limitations so that the Government of Indonesia through the Indonesian Ministry of Education allows the school learning process offline or face-to-face. This requires readiness from schools in implementing health protocols in a disciplined manner to reduce the risk of transmission of covid 19. Supervision of the enforcement of health protocols will be very difficult if you only rely on health workers. The Hope Integrated Islamic Elementary School (SDIT Harum) Jember will now start limited face-to-face learning in order to fulfill the competence of students. The Faculty of Medicine and the Faculty of Public Health, University of Jember have a moral responsibility to provide education and health promotion activities to the surrounding community, one of which is basic education institutions. One solution that can be done is to form cadres who are tasked with enforcing the implementation of health protocols in a disciplined manner. The hope of this program is that this health protocol discipline enforcer cadre can become an extension of health workers in promoting the importance of health protocol discipline as the main pillar of preventing COVID 19 in limited face-to-face learning.

Abstract

Digital transformation in the healthcare sector offers many benefits, but achieving it requires good digital maturity. This study aimed to identify the role of digital maturity in innovative healthcare, challenges, opportunities, and recommendations that could be applied in an effort to achieve digital maturity. This study used a systematic literature review method by extracting articles from the Scopus, PubMed, and Google Scholar databases in the last 10 years using the keywords "digital maturity", "healthcare", "hospital", and "health." The selected articles were in English and available open-access, which were then synthesized theoretically and presented in narratives. The results of the study showed significant benefits of digital maturity, including improved diagnostic accuracy, resource optimization, and patient engagement. However, obstacles such as limited funding, diverse digital literacy, and regulatory challenges were still found. The study provided recommendations to address barriers and emphasized the need for tailored digital solutions and supportive regulations for health practitioners and policymakers in an effort to increase digital maturity in the healthcare sector.

Abstract

Puskesmas Silo II merupakan salah satu puskesmas di Kabupaten Jember yang telah melaksanakan pelayanan ANC dan pada pelaksanaannya Puskesmas Silo II sudah melakukan berbagai upaya untuk meningkatkan motivasi ibu hamil, namun capaian K6 di Puskesmas Silo II mencapai target RPJMN tahun 2020-2024 yaitu sebesar 60%. Kurangnya capaian K6 di Puskesmas Silo II menunjukkan bahwa masih banyak ibu hamil yang tidak memanfaatkan pelayanan ANC secara lengkap. Penelitian ini bertujuan untuk menganalisis faktor yang berhubungan dengan pemanfaatan pelayanan kunjungan ANC lengkap di Wilayah Kerja Puskesmas Silo II Kabupaten Jember. Jenis penelitian ini adalah analitik dengan menggunakan desian cross sectional. Sampel Penelitian sebanyak 76 ibu nifas yang pernah mendapatkan pelayanan ANC dengan teknik pengambilan sampel menggunakan simple random sampling. Penelitian ini menggunakan data primer yang diperoleh dari hasil wawancara dengan ibu nifas menggunakan kuesioner. Analisis data menggunakan aplikasi SPSS dengan analisis univariat untuk mendeskripsikan terkait budaya, pengetahuan, ketersediaan fasilitas kesehatan dan aksesibilitas, serta analisis bivariat dengan menggunakan uji chi square untuk mengetahui hubungan antara variabel independent dengan dependen. Hasil penelitian menunjukkan bahwa beberapa faktor yang berhubungan dengan pemanfaatan pelayanan kunjungan ANC lengkap di Wilayah Kerja Puskesmas Silo II Kabupaten Jember adalah budaya, pengetahuan, ketersediaan fasilitas kesehatan dan aksesibilitas.

Abstract

The increasing number of Social Security Agency of Health (BPJS Kesehatan) participants who are inactive or drop out be a threat to participation in the health insurance program in Indonesia. Inactive participants in August 2022 recorded 45,987,956. The most common cause is not paying premiums according to applicable regulations. The purpose of the research is to analyze the factors that influence the willingness to pay for BPJS Kesehatan participants using the contingent valuation method approach. This research used quantitative methods with an analytic observational approach through surveys. The unit of analysis in research was the individual. The reachable population for the study were BPJS Kesehatan participants in the category of non-Recipients of Contribution Assistance (non-PBI) in the BPJS Kesehatan Jember working area (Jember and Lumajang Regencies). The sample size was 167 respondents taken using the quota sampling technique. Data analysis with multiple linear regression. The results showed that the Perceived Benefit Package, Perceived Sacrifice, and Free Mentality contributed to the formation of willingness to pay premiums regularly. Only the Conformity Effect does not significantly contribute of willingness to pay premiums regularly. Perceived Sacrifice and Free Mentality contribute in opposite directions. This means that if the respondent's Perceived Sacrifice and Free Mentality are high, it will affect the low willingness to pay premiums. It is necessary to develop a program that can provide added value for Non-PBI participants who so far rarely or have never used health services. This aims to maintain the motivation of participants to remain willing to pay premiums regularly.
