Abstract

Disorders that are often found in the lower extremities are vascular disorders. Varicose veins are very common, especially in women due to venous insufficiency due to poor blood circulation such as reflux and obstructive and can be inherited. The chronic phase of varicose veins can manifest as venous ulcers which is the development of wound below the knee. The lesions appear irregular in shape with clear boundaries, these ulcers can be prevented as early as possible by treating the trigger factor first before referring to the chronic direction. Physical examination is a medical process that must be undertaken when diagnosing a disease. The results are recorded in the medical record which is used to make a diagnosis and plan further treatment. Physical examination will be carried out systematically, starting from head to toe which is carried out in four ways, namely inspection, palpation, auscultation, and percussion. Varicose ulcers are a manifestation of chronic venous insufficiency. Patients with varicose ulcers have a history of varicose veins which, if treated, prevent the occurrence of varicose ulcers.

Abstract

Herbal-based complementary therapy approaches have recently gained attention in pediatric surgery due to their potential to support wound healing, reduce inflammation, decrease oxidative stress, and accelerate tissue regeneration. Indonesia possesses rich biodiversity of medicinal plants that have the potential to be developed as adjuvant therapies based on evidence-based medicine. This article aims to review the potential of Indonesian herbal medicine in the context of pediatric surgery through a molecular biomedical approach, focusing on wound healing, anti-inflammatory, antimicrobial, and immunomodulatory mechanisms. The method used in this study was a narrative literature review by searching scientific articles from PubMed, Scopus, ScienceDirect, and Google Scholar databases published between 2015 and 2025. The review findings indicate that several Indonesian herbs, including turmeric (Curcuma longa), moringa leaves (Moringa oleifera), ginger (Zingiber officinale), aloe vera (Aloe vera), and temulawak (Curcuma xanthorrhiza), exhibit biological activities through the regulation of NF-κB, TGF-β, VEGF, and PI3K/Akt pathways, as well as modulation of inflammatory cytokines and oxidative stress. The bioactive compounds of these herbs have the potential to accelerate angiogenesis, fibroblast proliferation, collagen synthesis, and wound epithelialization. Molecular approaches suggest that Indonesian herbal medicine has promising potential as a complementary therapy in pediatric surgical patients, particularly in the management of postoperative wounds and tissue inflammation.

Abstract

Dyslipidemia and obesity are major metabolic disorders characterized by abnormal lipid metabolism, chronic inflammation, and oxidative stress, all of which contribute to progressive renal injury. Excessive accumulation of oxidized low-density lipoprotein (ox-LDL) promotes macrophage infiltration and foam cell formation within renal tissue, accelerating glomerular damage and increasing the risk of chronic kidney disease. Aloin, the principal anthraquinone glycoside isolated from Aloe vera, possesses potent antioxidant and anti-inflammatory properties that may attenuate oxidative injury and inhibit foam cell formation. This study aimed to investigate the effect of oral aloin administration on renal foam cell formation in male Mus musculus with dyslipidemia-induced obesity. A true experimental study employing a posttest-only control group design was conducted using male mice with experimentally induced dyslipidemia and obesity. Animals were randomly allocated into control and treatment groups. The treatment group received oral aloin for 28 consecutive days following dyslipidemia induction. Kidney tissues were collected for histopathological examination using Hematoxylin–Eosin staining. Foam cell formation was evaluated microscopically, and differences between groups were analyzed using the Independent Samples t-test with a significance level of p < 0.05. Histopathological analysis demonstrated that aloin administration reduced renal foam cell formation compared with the untreated control group. The treatment group exhibited approximately 25% foam cell formation, whereas the control group demonstrated approximately 93.8%. These findings suggest that aloin suppresses lipid accumulation and inflammatory responses within renal tissue through its antioxidant activity. Nevertheless, the therapeutic effect remained lower than that reported for atorvastatin, indicating that aloin is more appropriately considered a complementary rather than a replacement therapy. Oral administration of aloin effectively reduced foam cell formation in the kidneys of dyslipidemia-induced obese male mice. The protective effect is likely mediated through antioxidant activity, inhibition of lipid peroxidation, and attenuation of inflammatory processes.

Abstract

Infanticide accompanied by mutilation represents an extreme form of fatal violence against neonates and poses significant challenges in forensic andmedicolegal investigations. In criminal proceedings, the absence of witnesses and limited non-medical evidence often complicate the determination of the cause and mechanism of death, particularly when mutilation involving sharp force trauma is present. Forensic medicine, through comprehensive postmortem examination and the preparation of visum et repertum, plays a crucial role in providing objective and scientifically valid evidence. This article aims to analyze the mechanism of death in a case of infanticide with mutilation based on external and internal examinations of a neonatal body. This study employed a forensic case report design using data derived from a visum et repertum Pro Justitia. External examination revealed dismemberment of body parts, multiple abrasions, lacerations, and irregular open wounds consistent with sharp force trauma associated with mutilation. Internal examination demonstrated hemorrhagic infiltration of the cranial bones and dilated cerebral blood vessels, indicating vital reactions due to blunt force trauma. These findings indicate that the cause of death was blunt force head trauma sustained while the victim was alive, whereas the mutilation caused by sharp force trauma occurred during the peri-mortem or post-mortem period. It is concluded that visum et repertum provides strong evidentiary value and plays a central role in determining the mechanism of death and supporting legal proceedings in cases of infanticide with mutilation.
