Abstract

Patient safety incidents, one of which is the unexpected event of patient injury, should not occur in hospitals, so risk management programs are very important to implement, but in the implementation of the care process, patient safety incidents still occur. Implementing effective risk management is an important strategy to identify, analyze, and mitigate risks that may compromise patient safety. The purpose of this study was to review risk management strategies used in hospitals to improve patient safety. The method used in this literature review utilizes a comprehensive strategy, such as searching for articles in research journal databases, searching through the internet, reviewing articles. The database search used includes database searches used including Science Direct getting 29,162 articles, Pubmed getting 527 articles, Google Scholar getting 9370 articles, and Scopus getting 10 articles. The keywords used in the article search were risk management and patient safety. After going through the identification, screening and eligibility stages, 10 articles were obtained that were suitable through analysis of objectives, suitability of topics, research methods used, sample size, research ethics, results of each article, and limitations that occurred. The results showed that implementing an incident reporting system, regular staff training, improving standard operating procedures (SOP) and implementing health information technology are important steps to reduce patient safety incidents. Integrated and sustainable risk management has been shown to improve the quality of care and provide better protection to patients. This study recommends that hospitals continue to strengthen their safety practices and increase collaboration in risk management.

Abstract

Spirituality helps the elderly adapt to changes in themselves. The process of healing chronic diseases in the elderly requires good spirituality. However, in reality, many elderly people still have low spirituality. This study considers the factors of spiritual needs of elderly people with chronic illnesses in Indonesia. Descriptive correlation research design with a cross-sectional approach. The sample was 210 elderly people taken by purposive sampling. The independent variables studied include age, gender, religion, ethnic group, education, work, history of illness, ADL, and cognitive function. The dependent variable is spiritual needs. The research instrument in the form of the Spiritual Needs Questionnaire (SpNQ). A linear regression test to simultaneously correlate several dependent variables with independent variables by following several modeling assumptions. There is a significant relationship between religious factors (0.01), ethnicity (0.003), cognitive function (0.04), and the spiritual needs of elderly people with chronic diseases (<0.05). The equation obtained from modeling can predict the value of the spirituality needs of elderly people with chronic illnesses. Respondents who have sufficient spiritual needs because they are good at connecting with other people, nature/the environment, and God.

Abstract

Diabetes is not solely an adult health issue—it has also become a growing concern among school-age children. The rise in childhood obesity has contributed to an increasing incidence of type 2 diabetes in children and adolescents, which now parallels and sometimes surpasses that of type 1 diabetes. Parents need to take proactive steps to prevent this disease by providing accurate information to their children. Children can also gain knowledge through educational approaches aimed at enhancing their cognitive abilities, including through games. This study aims to analyze the effectiveness of the Diabetamon game in improving cognitive skills related to diabetes mellitus prevention among school-age children. This research used a quasi-experimental design with a control group. A total sample of 99 school-age children was divided into an intervention group (47 students) and a control group (52 students), selected using proportional random sampling. The study was conducted over a one-month period, with the intervention delivered twice a week for 60 minutes over three weeks. Data collection instruments included a questionnaire and an observation checklist to assess children’s snack habits. Statistical analysis was performed using paired t-tests to examine within-group changes and independent samples t-tests to compare between-group differences, with a significance level of p<0.05. The results showed that the cognitive abilities of the intervention group improved significantly compared to the control group. The Diabetamon game was proven effective in enhancing school-aged children's knowledge about the importance of consuming fruits and vegetables, in line with the pillars of balanced nutrition and physical activity.

Abstract

Proses manajemen risiko mencakup penilaian, identifikasi, dan pengaturan prioritas risiko yang dilakukan secara proaktif, bertujuan untuk meminimalkan kemungkinan terjadinya Kejadian Tidak Diinginkan (KTD) dan risiko lain yang dapat mengancam keselamatan pasien dan tenaga kerja. Tujuan: Studi literatur ini bertujuan mengidentifikasi penerapan manajemen risiko pada pelayanan kesehatan rumah sakit . Metode: Pencarian artikel jurnal dilakukan pada 2 database jurnal, yaitu Google Scholar, PubMed. Terdapat 236 artikel yang didapatkan dalam proses pencarian jurnal dan 6 artikel yang memenuhi kriteria inklusi untuk dilakukan review lebih lanjut. Hasil: hasil review artikel menunjukan bahwa Manajemen risiko harus diterapkan dengan baik agar dapat meminimalkan potensi insiden yang berisiko terhadap keselamatan pasien. Elemen penting dalam implementasi manajemen risiko di rumah sakit meliputi pemahaman staf, kepemimpinan yang efektif, kebijakan strategis yang diterapkan oleh rumah sakit, serta keberadaan posisi yang bertanggung jawab atas program untuk menjamin kelancaran pelaksanaan manajemen risiko. Kesimpulan: Peningkatan efektivitas program manajemen risiko dapat dicapai melalui pengembangan kebijakan strategis dalam pelayanan kesehatan dan penunjukan individu yang bertanggung jawab atas program, sehingga program dapat dilaksanakan dengan efektif.

Abstract

Rekam medis elektronik merupakan suatu sistem yang memungkinkan penyimpanan data pasien secara digital, menggantikan metode tradisional berbasis kertas. Pelaksanaan dokumentasi keperawatan yang tidak sesuai standar menurunkan komunikasi antar tim dan berdampak negatif pada kualitas asuhan keperawatan. penggunaan EMR dengan dukungan faktor sistem informasi, sarana dan prasarana, serta kompetensi pengguna dalam melaksanakan dokumentasi elektronik sehingga menjadi strategi dalam mengurangi dampak merugikan dari penggunaan catatan keperawatan terhadap keselamatan pasien. Penelitian ini bertujuan untuk mengetahui efektivitas penggunaan rekam medis elektronik terhadap keselamatan pasien di rumah sakit menjadi sangat relevan dan penting. Penelitian ini menggunakan metode literature review. Pencarian literatur menggunakan panduan alur dari diagram prisma. Database online yang digunakan ada 2, yaitu PubMed dan Google Scholar. Kajian artikel dan jurnal penelitian yang dicari melalui penelusuran database online yang terbit tahun 2019-2024. Kriteria inklusi yang ditetapkan pada penelitian ini antara lain: 1) literature berbentuk jurnal ilmiah 2) sumber jurnal ilmiah pubMed dan google scholar, 3) jurnal ilmiah memiliki akses terbuka, 4) tahun publikasi jurnal ilmiah tahun 2019-2024, 5) pembahasan dalam jurnal meliputi rekam medis elektronik dalam meningkatkan keselamatan perawatan pasien. Sementara kriteria ekslusi pada penelitian ini yaitu semua hal yang tidak termasuk kriteria inklusi. Hasil penelitian menunjukkan bahwa sebanyak 5 artikel digunakan dalam penelitian ini. Hasil penelitian menunjukan studi kasus ini bahwa pendokumentasian berbasis elektronik berimplikasi terhadap peningkatan keselamatan pasien. Kajian ini menemukan wawasan baru yang komprehensif terkait dampak penggunaan dokumentasi keperawatan elektronik baik yang menguntungkan maupun merugikan dalam upaya menjaga keselamatan pasien.

Abstract

Adverse events, particularly falls and medication errors, remain a significant challenge in elderly patient care at home. The Peer to Peer Safety Companion (P2PSC) intervention leverages peer support to enhance patient engagement and safety vigilance. This study aimed to evaluate the effectiveness of P2PSC in reducing fall risk and improving medication adherence among elderly patients at home. A quasi-experimental one-group pretest-posttest design involved 56 elderly participants receiving P2PSC support for 4 days over 2 weeks. Data analysis revealed significant reductions in fall incidents (p=0.001) and improvements in medication adherence (p=0.001). Mean fall risk scores decreased from 12.5 to 9.2, while medication adherence scores increased from 72.5 to 81.3. The P2PSC intervention effectively enhances safety management for elderly patients and is recommended as a supplementary standard in geriatric care management at home.

Abstract

Introduction: Higher education institutions operate in complex and uncertain environments shaped by digital transformation, financial pressures, governance challenges, and increasing stakeholder demands. These conditions expose institutions to strategic, operational, academic, and reputational risks that influence managerial decision-making and institutional sustainability. Objectives: This study aimed to conduct a literature review and synthesize the literature on risk management practices and their implications for managerial decision-making in higher education institutions, with particular attention to governance and performance outcomes. Methods: A literature review was conducted using Scopus, Web of Science, and Google Scholar. Articles published between 2020 and 2025 were screened using predefined inclusion and exclusion criteria. Titles and abstracts were reviewed, followed by full-text assessment. Data were extracted on study design, thematic focus, and managerial implications. A thematic synthesis approach was applied to identify recurring patterns across studies. Results: The findings indicated that risk management improved the effectiveness of quality assurance, institutional governance, service quality, and organizational performance. Institutions with higher risk management maturity demonstrated stronger integration of governance structures, strategic planning, and performance monitoring systems. Risk-based approaches enhanced evidence-based managerial decision-making, institutional accountability, and academic competitiveness. However, fragmented implementation and limited governance capacity remained significant challenges. Conclusions: Risk management functions as a strategic governance instrument that strengthens managerial decision-making and institutional resilience in higher education. Integrating risk-based frameworks into governance and performance systems is essential for sustainable institutional development, particularly in complex and rapidly changing educational environments.

Abstract

Introduction: Primary health care plays a vital role in ensuring equitable access to essential health services; however, quality and patient safety gaps remain significant, particularly in resource-limited settings. Risk management is recognized as a governance strategy for reducing preventable harm and improving service quality, yet its implementation at the facility level is often inconsistent. Objectives: This study aimed to examine how risk management was implemented and its contribution to improving service quality at Siriwo Primary Health Care in Indonesia. Methods: A qualitative single-case study design was conducted. Participants included the health center director, quality improvement officers, health professionals, and administrative staff with at least 1 year of experience. Data were collected through in-depth semi-structured interviews, non-participant observations, and document reviews. All interviews were audio-recorded and transcribed verbatim. Data were analyzed thematically using coding, categorization, and theme development. Triangulation across data sources was applied to enhance credibility and rigor. Results: Risk management was implemented progressively through risk identification, assessment, and control integrated into routine service activities. Key risks included patient safety incidents, service delays, procedural errors, supply limitations, and human resource constraints. The implementation strengthened adherence to standard operating procedures, improved internal monitoring, enhanced staff awareness of patient safety, reduced the recurrence of complaints, and increased perceived community satisfaction. Conclusions: Integrating structured risk management into daily operations improved service quality and fostered a stronger patient safety culture. Context-adapted risk governance may be an effective strategy for enhancing primary health care performance in resource-limited settings.
