Abstract

ABSTRACT  Shoulder pain is a major complaint of patients with clavicle fractures as a result of trauma. At present most clavicle fracture treatments are performed surgery, although conservative therapy can produce the same results. This report aimed to show whether there were complaints of shoulder pain or disability in patients who have suffered clavicle fractures treated conservatively. There were fifteen patients with clavicle fractures at Dr. Soetomo Surabaya from January to July 2015, consisting of 14 male and one female patient. Data was taken from medical records, patient clinical records, home visits, physical examinations, SPADI questionnaires, and Quick DASH for patients. The results of SPADI questionnaire were varied; there were eight patients (53%) with an output of 0%, one patient (7%) each with a score of 0.63%, 1.25%, 5.90%, 7.13%, 8.50%, 9.90%, and a score of 10.50%. The results of calculating the Quick DASH questionnaire score also varied. There were ten patients (67%) with a score of 0, three patients (20%) with a score of 2.27, one patient (7%) with a score of 13.6, and one patient (6%) with a score of 6.81. In conclusion: (1) From the evaluation of 15 patients, there were no complaints of pain and disability in the shoulder joint. (2) From the evaluation of the results of the calculation of the SPADI questionnaire and Quick DASH, there were no significant complaints of shoulder joint pain and disability. Abstract

Introduction: Osteoarthritis is a degenerative joint disease which affects joint cartilage. As many as 80% patients movement are limited and 25% of them can not even perform daily activities. Osteoarthritis is one of the ten most disabling diseases in developed countries. Due to its chronic and progressive nature, socio-economic impacts in many developed and developing countries are enormous. The purpose of this study is to determine the profile of patients with osteoarthritis in the Orthopedic Department of RSUD Dr. Soetomo Surabaya (Dr. Soetomo General Hospital)Method: This is descriptive observasional research. Questionnaires data were obtained from all patients in the Department of Trauma and Orthopedic of RSUD Dr. Soetomo Surabaya during September 1, 2016 until October 31, 2016. Thirty five subjects with osteoarthritis were analyzed for the study. The variables observed in the patient profile include sex, age, body mass index, type of osteoarthritis, Kellgren-Lawrence scoring system, and knee osteoarthritis severity index.Result: The result shows that 43% of the samples were aged 60-69 years, 83% of the samples were female, 37% of the samples had an I BMI obesity, 60% of samples had bilateral osteoarthritis, and 77% of samples from primary osteoarthritis. A total of 15 of 53 knee samples had class III, and 15 of 53 knee samples also had class I, and as many as 21 of the 35 samples had a "very severe" clinical gradation. These results may be due to patients who have low grade gradation clinics and have received medical attention at the level I health facility or II.Conclusion: From the results of this study, it can be concluded that age, sex, BMI, Kellgren-Lawrence assessment system, index severity of patients Lequesne osteoarthritis in RSUD Dr. Soetomo Surabaya is in conformity with research that has been done in many places and theories that exist.Keywords: Knee osteoarthritis, age, gender, Kellgren-Lawrnce scoring system, knee osteoarthritis severity index

Abstract

Osteoarthritis is a degenerative disorder, which is characterized by the occurrence of thinning of the cartilage in the joints and causes bones to rub against each other so that stiffness, pain, and movement disorders arise. The condition of obesity can worsen osteoarthritis and cause complications of knee deformity. Objection of this study was to knowing the profile of knee osteoarthritis patients with obesity in the physical medicine and rehabilitation department of Airlangga University Hospital Surabaya. This research is a retrospective descriptive study by taking data from secondary data in the form of medical records for osteoarthritis patients in 2017 at the physical medicine and rehabilitation department of Airlangga University Hospital Surabaya. The results obtained was from a total sample of 36 obese knee osteoarthritis patients, the highest age group was 50-69 years (66,7%), dominated by female (69,4%), patients was grade 1 obese (77,8%), patients had bilateral knee osteoarthritis (47,2%), and the highest severity of osteoarthritis was grade 2 (61,1%). Then obtained osteoarthritis patients with obesity accompanied by knee deformity amounted to 9 people, with the most common deformity was varus deformity (81,25%) from total patients with deformity. Knee osteoarthritis was mostly found in elderly and female. Most patients were grade 1 obesity. More patients with bilateral osteoarthritis with grade 2-3. The knee deformity that more commonly found in patients was varus deformity.

Abstract

Background: The high number of knee osteoarthritis cases could lead arthroplasty more frequent. Total Knee Replacement (TKR) surgery procedure is at risk of massive bleeding. Bleeding can affect albumin levels and the onset of intra-compartment pressure on the nerves. The use of tranexamic acid could be done to reduce bleeding.Objective: Proving the effect of intravenous tranexamic acid administration on patients who will undergo TKR surgery.Methods: The experiment was conducted experimentally using primary laboratory data obtained before and after surgery in patient which will be performed TKR operation starting from April 2017.Result: There are 64 patients who had undergone total knee replacement surgery. The mean preoperative hemoglobin in the tranexamic acid group and without tranexamic acid was 13.06±1.46g/dl and 12.44±0.87g/dl. The mean hemoglobin after surgery was 10.29 ±1.14 g/dl and 9.16±0.97 g/dl. In the group receiving tranexamic acid, 32 (100%) reported low postoperative VAS scores, did not require blood transfusions and did not require albumin transfusion. Groups not receiving tranexamic acid, 32 (100%) moderate postoperative VAS scores, 4 people (12.5%) required the blood transfusion, and 3 (9.4%) required transfusion of albumin.Conclusion: The results of this study showed that the group given tranexamic acid had higher hemoglobin levels after surgery than the untreated group of tranexamic acid; there was no difference in albumin levels after surgery in both groups; the administration of tranexamic acid will decrease the amount of blood loss, decrease the degree of pain, and decrease the need for blood transfusions after total knee replacement surgery.

Abstract

Free functional muscle transfer for brachial plexus injury is one of the alternatives to repair the upper extremity function, where usually one type of muscle is used, the gracillis. The method of using two donor muscles to restore the motor function is worth considering for a better improvement of the quality. A 36 years old man with a complete left brachial plexus injury from C5-T1 due to motorcycle accident. A double free functional muscle transfer was performed using the gracillis muscle and the adductor longus muscle with the arterial source from the thoraco-acromial artery with the phrenic and accessory nerves. The gracillis muscle is used to flex the fingers while the adductor is used for flexing the elbow. A one year post-operative evaluation showed the patient was already capable of flexing his elbow with the muscle score of 3 and also the fingers with the muscle score of 1. The double free functional muscle transfer procedure is effective in achieving the moving function of both the elbow and the hand.

Abstract

Infected non-union is still a challenging orthopedic case to treat. Up until present day, there is no any established protocol to treat infected non-union of femur. Infection at subtrochanter femur specifically poses extra challenge as aforementioned location is affected by great tension and compression force. In this report, we present a 20-yo woman with infected subtrochanter femur following open reduction internal fixation (ORIF)with angle blade plate (ABP) 95o. One stage surgery was performed involving implant removal, debridement, definite external fixation, and antibiotic delivery using modified Lautenbach technique. Four years after trauma, the patient was capable of independent mobilization without aid. No infection recurrence was found, and Haris Hip Score was 91 (excellent). Based on the result of this case, definite external fixation and modified Lautenbach technique in one stage surgery can be a viable option in treating infected non-union of subtrochanter femur.

Abstract

Background: Drop hand is a clinical manifestation of radial nerve lesion that cause hand dysfunction. Jones tendon transfer is one of the therapeutic modalities in overcoming it with many modifications, but there is still no mutual agreement for which technique is the best. One of them is the Erwin Ramawan technique. The purpose of this study was to determine the differences in results before and after tendon transfer surgery with the Erwin Ramawan technique in patients with radial nerve lesions.Methods: Retrospectively, we evaluated patients with high-type lesions of radial nerve carried out by tendon transfer surgery using Erwin Ramawan, to reroute PL to EPL, so the thumb can do abduction and extension, then the FCR tendon is positioned parallel to the ECRB and EDC, then connected 45° direction. The outcome was evaluated using the DASH score and with the Robert G. Chouinard method.Results: The average DASH score in patients with high-type lesions of  radial nerve before surgery 56.83 ± 16.69 had a significant improvement (p <0.05) after tendon transfer surgery with Erwin Ramawan technique to 32.00 ± 11.88. Functionally, dorsiflexion from wrist postoperative is 75.00 ± 10.00 (excellent), palmarflexion 52.00 ± 24.65 (excellent). For finger, the metacarpophalangeal extension reaches 166.00 ± 19.49 (good). The distance from the fingertip to metacarpophalangeal crease is 0.10 ± 0.22 (good). On thumb, abduction reaches 40.00 ± 15.81 (excellent), interphalangeal extension 166.00 ± 11.44 (good).Conclusion: Based on these results, we found significant clinical differences before and after tendon transfer with the Erwin Ramawan technique.

Abstract

The management of distal Achilles tendon rupture is still controversial, especially in elderly patients, due to poor bone quality and tendon stump integrity. One of the proposed repair techniques is by using an anchoring screw. Here we report the mid-term follow-up of two patients with distal Achilles tendon rupture treated with an anchoring screw. Two elderly males in their sixties came to the outpatient clinic with similar complaints of intermittent pain, reddening, and ankle swelling after falling. Both patients were diagnosed with distal Achilles tendon rupture and were treated with open repair using a Krakow suture and anchoring screws. Functional outcome evaluation postoperatively found a good outcome. Previous studies have shown that repair with the anchoring technique is better biomechanically than with ordinary sutures. Repairs with this technique can withstand greater forces and loads before they fail. An anchoring screw repair technique for total distal Achilles tendon rupture repair provides good results in elderly with osteopenia.

Abstract

Background: Clavicle fractures are common, with treatment trends shifting from conservative to operative. While malunion often has minimal functional impact, nonunion rates and associated complications remain a concern. This study evaluates functional outcomes between plating and conservative approaches for midshaft clavicle fractures.Methods: A total of 531 cases with a midshaft clavicle fracture presented to the emergency room between January 1, 2014 and December 31, 2018 at Dr. Soetomo General Academic Hospital, Surabaya, Indonesia. Patients with a head injury, multiple traumas, re-fracture, malunion, open fracture, and pathological fracture were excluded. 161 patients remained. A conservative group (84 patients) was treated using an arm sling, and an operative group (77 patients) with Open Reduction and Internal Fixation (ORIF) and S-plate. Clinical and functional scores were evaluated retrospectively a minimum of 6 months after treatment. Shoulder function was evaluated using the Constant Shoulder Score and Manual Muscle Test.Results: 117 (72.7%) patients were male, with a mean age of 35.4 ± 12.33 years old. The right side was dominantly injured. The Manual Muscle Test score in the operative group was five, and in the conservative group, four. The Constant Shoulder score in the operative group was 93.38 ± 7.529, and in the conservative group, 86.60 ± 7.560 (p < 0.001). The DASH score in the operative group was 10.05±6.98 and in the conservative group, 23.67±3.49 (p < 0.001).Conclusion: In our study, surgery on clavicle midshaft fractures showed significant improvement and satisfaction compared to conservative treatment. Patients gained better function.

Abstract

Despite much-advanced therapy developed for bone cancer, prevention of the disease remains the most favorable and low-cost management approach. Bone cancer prevention and education should be initiated with adolescents.  Adolescents are the right group to receive bone cancer education, as they are in an active learning phase and have the potential to develop risky behaviors that can increase vulnerability to cancer, as they are in a stage of active learning but also face the development of risky and unhealthy behaviors. This study aims to assess the knowledge of bone cancer among adolescents in Indonesia. It is a cross-sectional study that uses a questionnaire to evaluate the participants' knowledge of bone cancer. A total of 39 questions regarding family history and general knowledge of cancer are given to the students in a pre-test and post-test format. Between the tests, a group of orthopedic specialists provided a short lecture to the students about the basic knowledge and prevention of bone cancer. The study involved 564 students from junior and senior high schools in Indonesia, with an overall mean age of 14.79 years. Bone cancer was mentioned in the family history of only 0.4% of the participants, but 21.5% to 23.5% were unsure about their own family cancer status. The study found a significant statistical result, indicating that the short lectures given by the orthopedic specialists were effective in enhancing the students' knowledge about bone cancer. The findings suggest that adolescents in Indonesia are still uncertain about their family cancer status and often misinformed about the symptoms of bone cancer. However, an educational lecture or discussion led by an expert in the field has proven to be an effective way to improve the students' knowledge about bone cancer.
