Abstract

Background: Nutrition are the essential factor to maintain body weight, body mass index and food intake in CAPD patients. Dialysis could increase nutrients lost and had to be replaced by adequate intake. Nutrition education should be needed to increase patient’s knowledge, food intake and body mass index.Objectives: To determine the role of nutrition education towards energy-protein intake and body mass index in patient with continuous ambulatory peritoneal dialysis (CAPD)Method: A cross-sectional study design with total sampling technique to obtains thirthy CAPD outpatient at RSUPN Dr. Cipto Mangunkusumo. Energy and protein intake were assessed by 3-days-food records (weekday and weekend). Nutritional status were obtained with BMI by dry body weight. Data were analyzed with independent t-test.Results: Most of subject are male outpatients, 25 – 55 years old (76.7%), had hypertension (83.3%) and normal BMI (50%). Most of subject had been undergoing peritoneal dialysis more than a year (67%), and had less than two times education per year. Energy intake were categorized as sufficient (80%) but protein intake were categorized as insufficient. There were no differences between frequency of nutrition education with energy-protein intake and nutritional status (p0.05)Conclusion: There are no differences between energy-protein intake and nutritional status with the frequency of nutrition education. Nutritional education by nutritionist and/or dietitian according to the recommendations help to maintain normal body mass index.

Abstract

Strok saat ini menjadi salah satu penyebab utama kematian global. Hubungan antara kejadian strok dengan diabetes telahlama diketahui. Kontrol gula darah, yang dipantau melalui kadar HbA1c, telah menunjukkan hubungan dengan strok dan penyakitkardiovaskular lain. Kajian ini untuk menentukan perbedaan kadar HbA1c antara pasien diabetes melitus tipe 2 dengan dan tanpakejadian strok infark trombotik. Metode penelitian yang digunakan adalah analisis retrospektif menggunakan rekam medis pasienselama 3,5 tahun. Penelitian ini mengumpulkan data kadar HbA1c dari 443 pasien diabetes melitus tipe 2 kemudian membandingkanrerata kadar HbA1c antara pasien diabetes mellitus tipe 2 dengan kejadian strok trombotik (n=74) dan tanpa kejadian strok trombotik(n=369). Perbandingan tingkat HbA1c juga dilakukan secara terpisah antara laki-laki dan perempuan. Kajian ini menemukan reratakadar HbA1c yang tinggi di kedua kelompok sampel (10,49%±2,53% untuk kelompok dengan kejadian strok infark trombotik dan10,44%±2,8% untuk kelompok tanpa kejadian strok infark trombotik) dengan perbandingan sarana p>0,05. Perbandingan yangdilakukan secara terpisah di laki-laki dan perempuan juga menunjukkan hasil yang sama dengan p>0,05. Sebagai simpulan, kadarHbA1c di kedua kelompok penelitian sama-sama tinggi dan tidak ada perbedaan bermakna kadar HbA1c yang ditemukan di pasiendengan diabetes tipe 2 dengan dan tanpa kejadian strok trombotik.

Abstract

Coronavirus-19 (Covid-19) dinyatakan sebagai pandemi dunia oleh WHO pada tanggal 12 Maret 2020 dan berdasarkan Keputusan Menteri Kesehatan Nomor HK.01.07/Menkes/104/2020, Covid-19 telah ditetapkan sebagai penyakit yang berpotensi wabah di Indonesia. Tujuan umum dari penelitian ini adalah Pre-Testing Information, Education and Communication (IEC) Fokus Covid-19 melalui media video yang telah diedarkan Kementrian Kesehatan Republik Indonesia dengan komponen metode penelitian kualitatif trianggulasi sumber dari variabel attraction, comprehension, acceptability, self-involvement, dan persuasion. Subjek penelitian terdiri dari 9 informan dengan latar belakang mahasiswa kesehatan dan non-kesehatan. Media video yang diteliti berasal dari Kemenkes RI dengan durasi 1 menit 1 detik berjudul “Jaga Diri dan Keluarga anda Dari Virus Corona (Covid-19) dengan Germas. Hasil wawancara mendalam dengan 9 informan menunjukkan bahwa secara umum video edukasi ini sudah memenuhi aspek pretesting media komunikasi yang meliputi attraction, comprehension, acceptability, self-involvement, dan persuasion. Analisis triangulasi sumber menghasilkan bahwa video edukasi tersebut terbukti dapat mempengaruhi masyarakat dalam mengubah pengetahuan, sikap dan perilaku untuk penanggulangan dan pencegahan pandemic virus Covid-19. Pentingnya menjaga jarak atau social and physical distancing merupakan komponen yang sebaiknya ditambahkan dalam media edukasi, selain itu pentingnya cuci tangan menggunakan sabun dan penggunaan alat pelindung diri seperti masker harus lebih ditekankan sebagai bahan evaluasi komponen media edukasi.

Abstract

Pendahuluan: Spastisitas adalah gangguan motorik yang sering dijumpai dan muncul setelah stroke. Spastisitas dapat menyebabkan nyeri dan disabililitas pada bagian tubuh yang mengalaminya. Tujuan: mencari hubungan antara rasio H/M yang diukur dengan elektromiografi dengan derajad spastisitas yang terjadi setelah fase akut stroke. Metode: Penelitian ini adalah studi analisis korelatif observasional, dengan 26 sampel. Pasien diukur rasio H/M pada saat stroke akut dan diukur derajad spastisitasnya dengan menggunakan Modified Ashworth Scale setelah 3 bulan. Hasil yang didapatkan dilakukan analisa statistik dengan menggunakan tes korelatif kategorik dari Spearman. Hasil: Pasien yang mengikuti penelitian ini sebanyak 26 orang. Terdapat perbedaaan antara nilai H/M rasio antara sisi parese dengan sisi sehat dan tidak didapatkan hubungan yang bermakna antara nilai rasio H/M yang diukur saat fase akut stroke dengan derajad spastisitas yang diukur dengan Modified Ashworth Scale (MAS) setelah 3 bulan (p = 0,06 ; r = 0, 37). Kesimpulan: Rasio H/M pada pasien stroke akut meningkat pada sisi parese dibanding pada sisi sehat, namun peningkatan ini tidak memiliki hubungan yang signifikan dengan derajad spastisitas pasca stroke yang diukur dengan MAS, sehingga rasio H/M tidak dapat digunakan sebagai prediktor munculnya spastisitas pasca stroke

Abstract

Introduction. Energy and protein intake in chronic kidney disease (CKD) with continuous ambulatory peritoneal dialysis (CAPD) therapy plays an important role in balancing hypercatabolic conditions. Unbalanced intake will result in energy and protein deficits which are increasing the risk of muscle catabolism, decreasing body mass index (BMI), and malnutrition. Subjective global assessment (SGA) as the gold standard in nutritional assessment is needed in assessing nutritional status and determining malnutrition, therefore nutritional intervention can be delivered and improve CAPD patients’ quality of life. This study was conducted to determine the relationship between SGA with energy and protein intake, hand grip strength, and body mass index on CKD patients undergoing CAPD. Methods. A cross-sectional study was conducted among 30 CKD patients undergoing CAPD which were purposively selected using total sampling. Nutritional status was assessed using SGA questionnaire, energy and protein intake was assessed through food records, and hand grip strength was measured by jamar hydraulic hand dynamometer. The normality of the data was tested using the Kolmogorov-Smirnoff test and bivariate analysis was conducted using the Chi-Square test. Results. Of a total of 30 CKD patients, most of them were aged 25-55 years (76.6%), while the age group <25 years was only>6.7%. More than 73% of patients had normal SGA nutritional status and 26% were malnourished. The average energy requirement was 1,942 (SD 277) Kcal, the lowest energy intake was 921 kcal/day and the highest was 1,959 kcal/day with an average dialysate energy intake of 404 (SD 42) kcal. The average protein intake was 54 (SD 9.5) grams, while the average protein requirement was 70.7 (SD 9.7) grams/day. Based on BMI status, 20% of subjects were underweight, 60% normal, and 20% overweight. A total of 53.3% of patients had poor hands grip strength and only 46.7% were normal with an average grip strength was 26.7 (SD 9.3) kg. Analysis showed that SGA was not related to energy intake (p<0.857), grip strength (p<0.307), and BMI (p<0.829). However, there was a relation between protein intake and SGA (p<0.048, OR: 1.233, 95% CI: 1,058 – 2,389). Conclusions. Inadequate protein intake is related to the risk of malnutrition than adequate protein intake. There is no relationship between energy intake, hands grip strength, and BMI on SGA scores.

Abstract

Introduction: Septic cavernous sinus thrombosis (CST) is a rare thrombosis, but severe, causing damage to all nerves inside the cavernous sinus. The incidence rate of CST is estimated at two to four per million people per year. The symptom usually varies, gradually worsening, making diagnosis harder and potentially making permanent disability. Early detection and prompt treatment are very important to lower the morbidity rate. Case: Tn. M had complained of multiple headaches attacks since 8 months ago before being admitted followed by diplopia. Complaints had worsened 3 months before with the loss of vision in the left eye. MRI showed left thrombosis sinus cavernous. The terrible headache was scaled 8-9 with VAS score. The patient’s left eye was found proptosis palpebra, with anisocoria and mydriasis followed by no direct light reflex also indirect light reflex, visual acuity of the left eye is no light response, whereas the patient’s right eye is normal. On the patient’s left eye was found gaze palsy sinistra. The abnormal blood result was leucocytosis and thrombocytosis. The patient was given fondaparinux once a day with a dose of 6000 units subcutaneously for 5 days. After that, the headache was decreased significantly. The patient underwent 5 tooth extraction done by mouth surgeon after being discharged. The patient was given 20 mg rivaroxaban per day for three weeks, then 10mg per day for three months, and evaluated with MRV again. Conclusion: CST is still very rare but very lethal and makes permanent disability on the patient. Through this case, we demonstrate the potential of tooth infection which leads to septic CST and caused ophthalmoplegia and blindness.

Abstract

Highlights:1. The findings of this study indicate that clinicians should remain vigilant, as effective glycemic control, reflected by optimal HbA1c levels, does not necessarily correlate with favorable distal cerebrovascular resistance, as measured by pulsatility index (PI).2. This study recommends that patients with acute thrombotic stroke undergo a comprehensive examination to ensure an accurate diagnosis and appropriate treatment.   Abstract Stroke is the second leading cause of death and disability in the world. Diabetes mellitus is a risk factor for stroke with an increase in blood viscosity associated with hyperglycemia, and HbA1c is a well-established biomarker primarily used for the long-term monitoring of glycemic control in patients with diabetes mellitus. The Pulsatility index (PI) parameter of Trans Cranial Doppler (TCD) can measure blood vessel resistance. This study was to prove the correlation between HbA1c and PI in Middle Cerebral Artery (MCA) of acute thrombotic stroke patients. A cross-sectional study was conducted to thrombotic stroke patients treated at the neurological ward of Dr. Soetomo General Academic Hospital, Surabaya, Indonesia from March 2019 to April 2020 who met the inclusion and exclusion criteria. Data were obtained from medical records. Statistical analysis was performed using Spearman test for numerical data and Chi-square for categorical data with a significant value of p <0.05. There were 32 subjects who met the inclusion and exclusion criteria. A total of 24 subjects (75%) were males and 8 (25%) females with a mean age of 56.41±13.22 years. PI MCA results were abnormal in 14 subjects (43.75%) with an average of 1.8±0.97 and PI MCA was normal in 18 subjects (56.25%) with an average of 0.87±0.15. The average HbA1c was 6.55±1.9%. There was no statistically significant association observed between HbA1c and PI in MCA examination (r=0.07; p=0.73). In conclusion, the findings of this study indicate the lack of correlation between pulsatility index and HbA1c in patients with acute ischemic stroke. This underscores the importance of comprehensive cerebrovascular evaluation regardless of HbA1c status.

Abstract

Background: Malnutrition was a nutritional problem in hemodialysis with prevalence 50-70% and 2018 study at Cipto Mangunkusumo Hospital showed the prevalence was 39%.Objectives: This study was team study of Renal and Hypertension Unit RSCM, using cross sectional design, primary data collected in February and May 2018.Materials and Methods: The sample size used a hypothesis test with total sample of 120 patients. Data obtained through interviews and laboratory data obtained through medical records. Univariate, bivariate and multivariate analysis were carried out to see the determinant factors that influence the Malnutrition Inflammation Score (MIS).Results: The results showed that there were relationship between sex (p<0.022), age (p<0.031), etiology chronic kidney disease (p<0.037), duration hemodialysis (p<0.025), protein intake (p<0.030) and grip strength (p<0.010). The determinant factors significantly associated with MIS were sex p<0.0001, age p<0.014, protein intake p<0.018, and grip strength p<0.017.Conclusion: The conclusions of this study were the prevalence of inflammatory malnutrition in hemodialysis patients was 55.8%, male had risk of 4.1 malnutrition, age ≥40 years had risk of 3.1, inadequate protein intake had risk 2.8 times malnutrition and less grip strength as a protective variable had a 0.23 times lower risk.Keywords : Chronic kidney disease; hemodialysis; malnutrition inflammation; nutritional status
