Abstract

Diabetic foot ulcers (DFUs) are a major health problem as the number of patients continues to increase, are difficult to heal, require enormous management costs, and deteriorate the quality of life of patients, their families and societies. The pathogenesis of DFUs is complex. Most important factors that increase the risk of DFUs are peripheral neuropathy, foot deformities, frequent minor trauma, and peripheral arterial disease. Neuropeptides derangement, hypoxia, hyperglycemia, and infection act as the cause of chronicity of DFUs. Therefore, during the initial evaluation of DFU, patients need to be checked for their metabolic status, presence or absence of peripheral neuropathy, vascular insufficiency, foot deformities, and infection of the ulcer and its underlying bone. Then, DFUs are classified by the severity of vascular insufficiency, the depth of the wound, and the severity of the infection. This classification system helps clinicians to determine whether the patient needs to be hospitalized or amputated and helps to establish DFU management strategies. In the management of DFUs, adequate blood flow to the wound area should be achieved. Glycemic control and standard wound care should be encouraged. Standard wound care includes debridement, offloading, wound moisture balance with suitable dressing, edema control, and infection control. Education about preventive foot care should be taught to the patients and their families. As the pathogenesis and management of DFUs are complex, a multidisciplinary team consists of expert individuals in their respective fields should be involved.

Abstract

Wound healing disturbances that occur in chronic diabetic wounds result in various complications and cause a significant burden. With the increasing prevalence of diabetes mellitus, it is estimated that the numbers of chronic diabetic wounds and their complications will also continue to increase. One of the latest managements using growth factors for chronic diabetic wounds gives satisfactory results, but their use is still limited because of the high costs. This study aims to explore the effectiveness of using topical insulin as a cheaper and effective alternative therapy in chronic diabetic wounds. The method of this study is a literature review sourced from Google Scholar and ProQuest search engine. Eight articles were obtained that are relevant to be discussed in this study. According to studies conducted so far, topical insulin can improve wound healing through the modulation of inflammation and insulin signaling pathways. In conclusion, there is improved wound healing in diabetic chronic wounds patients that were given topical insulin. Therefore topical insulin should be considered as part of chronic diabetic wounds management. Keywords: wound healing; chronic diabetic wounds; growth factors; topical insulin

Abstract

Pada praktik kedokteran sehari-hari pemeriksaan kuku sering terlewatkan, padahal cukup banyak penyakit yang dapat dilihat melalui kuku. Pemeriksaan kuku dapat dijadikan sebagai salah satu modalitas deteksi dini dan skrining penyakit selain kelainan lokal struktur kuku sendiri. Kemampuan mendeteksi kelainan kuku dan memberi penanganan yang sesuai selain berdampak medis, juga dapat mengurangi dampak psikologis akibat kelainan kuku, yang dapat mengurangi percaya diri dan kemampuan bersosialisasi. Tulisan ini bertujuan memberikan gambaran umum beberapa kelainan bentuk kuku yang sering ditemui sehari-hari.The examination of nails is often overlooked and poorly understood in daily clinical practice; in fact, several illnesses can be detected by the appearance of nails. Examination of nails can be used for early detection and screening of internal illnesses, in addition to detect local nail abnormalities. Appropriate management results in medical and psychological improvement on patients. This review aims to provide a general overview of nail shape abnormalities often found in daily life.

Abstract

Perubahan warna kuku (kromonikia) dapat merupakan variasi normal atau menunjukkan kelainan; dapat disebabkan suatu infeksi, keganasan, pengaruh obat, atau penyakit sistemik lainnya. Pemeriksaan warna kuku perlu mendapat perhatian dalam praktik kedokteran sehari-hari agar dapat membantu mengenali suatu penyakit.Nails discoloration (chromonychia) may indicate normal variations or abnormality; can be caused by infection, malignancy, drugs, or other systemic diseases. Examination of the nail color in daily clinical practice to aid diagnosis deserves more attention.

Abstract

Background: Primary hyperhidrosis is a condition of excessive sweating on certain parts of the body with unknown cause. The severity and location of primary hyperhidrosis vary and are thought to affect the quality of life.Purpose: The study aims to determine the association between severity and locations of primary hyperhidrosis and quality of life in medical students.Methods: This study was conducted on 77 medical students at Atma Jaya Catholic University of Indonesia. Hyperhidrosis Disease Severity Scale (HDSS) questionnaire and Hyperhidrosis Quality of Life Index (HidroQoL) questionnaire were used. Data analysis was performed using Chi-Square.Results: The prevalence of primary hyperhidrosis was 15.33%. Mild-moderate hyperhidrosis was found in 77.9% respondents, while severe-very severe hyperhidrosis was found in 22.1% respondents. The location of hyperhidrosis was found in palmar (66.1%), axillary (28.6%), plantar (1.0%), and other locations such as the face, thigh, and back (9.1%). The score of the daily activity domain (29.65 ± 21.96) was higher than the psychosocial score (27.92 ± 20.46). Data showed that 33.3% of respondents with mild-moderate hyperhidrosis and 82.4% of respondents with severe-very severe hyperhidrosis’ quality of life were affected by their excessive sweating (p=0,000). As many as 34.0% of respondents with palmar hyperhidrosis and 54.5% with axillary hyperhidrosis’ quality of life were all affected by their conditions (p=0,106).Conclusion: We found a significant association between the severity of primary hyperhidrosis and the quality of life, and no significant association between the location of primary hyperhidrosis and quality of life among medical students.

Abstract

Pruritus is a common complaint of diabetic patients with a substantial impact on financial and health status, but the pathophysiology is unclear and treatment with antihistamines has mostly been unsuccessful. To date, we still do not have guidelines to help us treat pruritus in diabetes mellitus, so we felt the need to review the existing literature to explore possible ways to treat these patients. We collected 85 pieces of literature from various sources such as PubMed and Google Scholar, and independently extracted these data to make this review. While the pathophysiology behind pruritus in diabetes mellitus remains largely unknown, some trials have found a few pharmacological treatments to be effective in alleviating itch in these patients.

Abstract

Background: Dandruff is an understated hair and scalp problem that causes striking social and concentration issues among medical students. Haircare practice among them may or may not be related to the dandruff incident. Methods: A cross-sectional study was conducted on 74 pre-clinical medical students at Atma Jaya Catholic University of Indonesia. Trained health staff assessed dandruff prevalence by physical examination, and the haircare practice was measured using a questionnaire. Bivariate analysis was utilized to assess the association between haircare practice and dandruff prevalence’s. Result: Among respondents, 85.1% of them have dandruff. Respondents who shampooed less than five times a week, combed their hair more than seven times a week, did not use hair care and styling products were associated with higher dandruff prevalence (p Conclusion: There was a significant association between haircare practice and dandruff in medical students.

Abstract

Penuaan kulit adalah perubahan struktur pada kulit yang merupakan konsekuensi dari bertambahnya usia. Paparan sinar ultraviolet (UV), yang dapat menginduksi pembentukan reactive oxygen species (ROS) dan reactive nitrogen species (RNS) sehingga merusak sel, dapat mempercepat timbulnya penuaan. Munculnya tanda-tanda penuaan kulit tersebut dapat mengganggu penampilan sehingga banyak orang mulai menggunakan produk perawatan kulit. Melatonin diperkirakan memiliki efek antipenuaan kulit, namun penggunaannya sebagai antipenuaan kulit akibat sinar UV belum banyak dipelajari. Sel kulit manusia diketahui dapat mensintesis serta memiliki beberapa jenis reseptor melatonin. Melatonin dan metabolitnya merupakan antioksidan poten yang dapat menekan formasi ROS dan RNS serta menurunkan respons inflamasi akibat sinar UV pada kulit. Pada beberapa penelitian, melatonin terbukti dapat menurunkan kerusakan dan jumlah sel yang mengalami apoptosis akibat paparan sinar UV. Disimpulkan bahwa melatonin dapat digunakan sebagai antipenuaan kulit akibat sinar UV, namun masih dibutuhkan penelitian lebih lanjut mengenai bentuk sediaan, dosis, dan efek samping penggunaannya.

Abstract

Pendahuluan: Dermatitis atopik (DA) adalah penyakit inflamasi kronis yang sering dikaitkan dengan alergen tungau debu rumah (TDR), terutama Dermatophagoides pteronyssinus. Penatalaksanaan saat ini belum memadai untuk memberikan perlindungan jangka panjang. Tujuan: Tinjauan ini bertujuan untuk mengeksplorasi potensi vaksin TDR sebagai terobosan baru dalam pengelolaan DA. Metode: Artikel ini disusun melalui telaah literatur dari jurnal, buku, dan dokumen relevan lainnya yang membahas DA, imunoterapi, dan vaksinasi terkait. Data dianalisis secara komprehensif untuk mendukung kesimpulan. Diskusi: Disfungsi sawar kulit dan respons imun yang dimediasi IgE adalah faktor utama dalam pathogenesis DA. Vaksinasi berbasis epitope TDR menawarkan peluang untuk pencegahan dengan efektivitas tinggi. Studi in sillico menunjukkan hasil menjanjikan, namun validasi lebih lanjut secara in vitro dan in vivo diperlukan. Pengembangan vaksin TDR multi-epitop dapat menjadi alternatif jangka panjang untuk DA. Simpulan: Vaksinasi TDR memiliki potensi besar sebagai strategi pencegahan primer untuk DA, namun penelitian lebih lanjut masih dibutuhkan untuk penerapan klinis.
