Abstract

Objective: To present care with lemon aromatherapy intervention as an effort to overcome nausea and vomiting in gastritis patients. Methods: The method used is descriptive and uses nursing care that refers to the IDHS and SIKI books. The subject of this study was 1 patient with a nursing diagnosis of nausea and vomiting in gastritis patients who were treated in the Dahlia Room, Banjar City Hospital. Data collection includes interviews, observations, physical examinations and documentation. Results: The results of the case studies conducted showed a change after the non-pharmacological technique of lemon aromatherapy was proven by the final results, namely the evaluation carried out by the patient not feeling nauseated, the INVR scale 3. Conclusion: The conclusion from the results of the lemon aromatherapy intervention was considered effective for reducing nausea and vomiting with a decreased INVR scale. with the results of the final evaluation of the INVR 3 scale, the client said that nausea was reduced.

Abstract

Objective: To present nursing care with slow deep braething intervention in an effort to reduce the intensity of heartburn in gastritis clients. Methods: The subject of this case study was a client who experienced pain associated with an injury to a biological agent. Data collection techniques are interviews, observation, physical examination, and paper research. Results: The results of the case study at the assessment stage showed that the client experienced heartburn, felt nauseous, and dizzy. The client looks in pain with a smirk on his face in pain. The established diagnosis is acute pain associated with an injury biological agent which refers to the SDKI book. The intervention and implementation used on clients is the application of non-pharmacological slow deep breathing therapy. Conclusion: After implementing slow deep breathing therapy the client says the pain has decreased slightly, while the subjective data from the history of the client looks fresh and can move as usual.

Abstract

Introduction: : Diabetes Mellitus (DM) is a non-communicable disease that continues to show an increase globally, including in Indonesia. According to WHO data in 2016, as many as 422 million adults in the world have diabetes. This disease is caused by insulin deficiency, either relatively or absolutely, and often causes complications such as peripheral neuropathy which causes decreased sensation in the feet. One of the steps to prevent these complications is through foot exercises. Foot exercises are special physical exercises for diabetics that aim to prevent wounds, improve blood flow, strengthen small muscles in the feet, and prevent deformities due to nerve disorders and muscle weakness. Objective: This case study aims to implement foot exercises in patients with impaired physical mobility with diabetes mellitus. Method: This research uses a descriptive method in the form of a case study with a nursing approach which includes assessment, nursing diagnosis, nursing care plan, nursing implementation, and nursing evaluation. Data collection was done by interview, observation, physical examination, and documentation. Result: After the assessment, it is known that Mrs.D obtained a nursing diagnosis of impaired physical mobility associated with decreased muscle strength with diagnosis number D.0054, after being given implementation in the form of foot exercises for 3 days with a duration of 5-7 minutes a day, the nursing problem of impaired physical mobility of the client is partially resolved which is characterized by increased muscle strength with muscle strength values on the right lower extremity 4 and the left 5. Conclusion: The implementation of foot exercises has proven to be useful for reducing movement limitations and increasing muscle strength in all extremities, the benefits of this foot exercise will be maximized if done gradually and periodically.

Abstract

Background: Diabetes mellitus is a chronic metabolic disease that can cause various complications, one of which is diabetic ulcers. These ulcers occur due to impaired circulation and peripheral neuropathy, which causes wounds to be difficult to heal and increases the risk of amputation. One method of wound management that has proven effective in accelerating the healing of diabetic ulcers is the use of modern hydrogel dressings. Hydrogel functions as a moist dressing that is able to maintain optimal moisture in the wound area, accelerate the process of autolysis of necrotic tissue, and stimulate the growth of granulation tissue. With a moist and protected wound environment, the process of tissue regeneration is more effective and infection can be minimized. Objective: This case study is to evaluate the effectiveness of using modern hydrogel dressing as a moist dressing in accelerating wound healing in patients with diabetic ulcers through a nursing care approach. Methods: This research is a case study with a descriptive method, using a nursing care approach that includes assessment, nursing diagnosis, intervention, implementation, and evaluation. The patient's house is located in the Sinar Mawar Neighborhood Hamlet, RT.001 RW.008, Cigembor Village, Ciamis District. Nursing diagnoses refer to (SDKI), interventions based on (SIKI), and evaluation based on (SLKI), and all documentation is compiled using the SOAPIER method. Results: Interventions were carried out on one patient with grade 4 diabetic ulcers. Wound care using hydrogel as a moist dressing was performed for 3 visits. Wound evaluation was carried out using the Bates-Jensen Wound Assessment Tool as well as real-time blood sugar (GDS) checks. Results showed a decrease in wound score from 45 to 38, and a decrease in GDS from 518 mg/dl to 269 mg/dl. There were significant changes in the wound tissue, such as the reduction of slough tissue and the formation of healthy granulation tissue. Conclusion: The use of modern hydrogel dressing as a moist dressing in accelerating diabetic ulcer wound healing. This implementation can be an option for modern non-pharmacological wound care therapy that can be applied at home with a holistic nursing approach.
