Abstract

The severity of diseases, the complexity of treatment, and the use of medical devices in the intensive care unit (ICU) may change the pharmacokinetics (PK) profile of antibiotics among critically ill patients.This narrative review aims to explain the PK profile of critically ill patients compared to other group of patients and to describe the pharmacokinetic-pharmacidynamic (PK-PD) target attainment among this group of patients. Only articles published less than 10 years ago were included in this narrative review. Evidences have indicated that critically ill patients have relatively larger volume distribution (Vd) of hydrophilic antibiotics compared to patients with stable conditions. The fluid shifting to interstitial space, hypoalbuminemia, and aggressive fluid treatment may contribute to the increase value of Vd in critically ill patients. The clearance (CL) of hydrophilic antibiotics in critically ill patients is highly determined by dynamic changing of renal function compared to patients in other wards. The phenomenon of augmented renal clearance and the use of high intensity of renal replacement therapy can increase the CL of hydrophilic antibiotics. The different PK profile of antibiotics may lead to the failure of attaining the PK-PD target if the dose of antibiotics is not adjusted according to such differences

Abstract

Insulin resistance plays a crucial role in maternal metabolic adaptation during pregnancy. Underweight pregnant women have metabolic conditions potentially affecting insulin sensitivity and glucose homeostasis. The Homeostatic Model Assessment of Insulin Resistance (HOMA-IR) quantitatively measures fasting glucose and insulin levels, providing insights into metabolic health during pregnancy. This study aims to determine the differences in HOMA-IR between underweight and normal-weight pregnant women. The research employs a case design with a quantitative approach, using non-probability sampling techniques including quota and purposive sampling. The total sample comprises 24 participants, divided into 12 underweight pregnant women and 12 normal-weight pregnant women. Statistical analysis shows significant differences between groups: glucose levels (p <0.001), insulin levels (p = 0.018), and HOMA-IR values (p <0.001), all indicating significant findings (p <0.05). The study found differences in glucose and insulin levels between the two groups. Insulin and glucose levels tended to be higher in underweight pregnant women, indicating insulin resistance involvement with higher HOMA-IR levels compared to normal-weight pregnant women. This study can serve as a basis for understanding the patho-mechanism in underweight pregnant women.

Abstract

Menurut data dari Kementerian Kesehatan Indonesia (2020), dari total 4.656.382 ibu hamil di seluruh provinsi Indonesia, sebanyak 451.350 di antaranya memiliki risiko Kekurangan Energi Kronis (KEK), mewakili persentase sebesar 9,7%. Di antara ibu hamil, 59,5% merupakan primigravida yang mengalami KEK, 56% merupakan ibu hamil yang bekerja, dan 88% merupakan ibu hamil dengan usia berisiko (20-35 tahun) yang mengalami KEK. Tujuan: untuk menganalisis antara kadar Hb dan jumlah eritrosit antara ibu hamil normal dan ibu hamil yang mengalami KEK dan korelasi antar variabel. Metode: eksperimental dengan desain penelitian epidemiologi analitik dengan pendekatan cross-sectional, menggunakan teknik non-probability sampling dengan quota dan purposive sampling, dengan jumlah sampel sebanyak 30 orang. Hasil: Analisis penelitian menunjukkan bahwa variabel gravida memiliki distribusi normal pada kedua kelompok ibu hamil normal dan yang mengalami KEK. Data usia dan gravida menunjukkan homogenitas. Terdapat perbedaan yang signifikan dalam kadar Hb dan jumlah eritrosit antara ibu hamil normal dan ibu hamil yang mengalami KEK. Selain itu, terdapat korelasi yang signifikan antara kadar Hb dan jumlah eritrosit. Kesimpulan: terdapat perbedaan yang signifikan dalam kadar Hb dan jumlah eritrosit antara ibu hamil normal dan ibu hamil yang mengalami KEK (nilai p 0,001).Kata Kunci: KEK, Hb, Eritrosit
