Abstract

The Quick Sequential Organ Failure Assessment (qSOFA) is a simple parameter, however, its sensitivity as a mortality predictor is low. This study aimed to improve the predictor performance of qSOFA for in-hospital mortality. This study was a retrospective single-centered cohort using medical record data. This study included 150 patients aged 18-80 years old, who experienced sepsis and received ICU care between September 2021 and August 2022. qSOFA and modified ROX index (mROX) were calculated based on the most severe condition recorded in the emergency department (ED). Each variable's area under the receiver operating characteristic (AUROC) curve, sensitivity, and specificity were compared to predict in-hospital mortality. qSOFA scores ≥2 and mROX values ≤3.20 were independent factors that increased the risk of in-hospital mortality (OR 3.69 and 21.50; p 0.004 and 0.005, respectively). The combination of qSOFA scores ≥2 and mROX value ≤3.20 as in-hospital mortality predictors resulted in AUROC 0.791 with a sensitivity of 71.7% and specificity of 75.7%. The AUROC, sensitivity, and specificity of the combination of qSOFA and mROX were higher than qSOFA (0.766, 70.8%, 70.3%) or mROX (0.760, 68.1%, 67.6%) alone. In conclusion, the combination of qSOFA scores ≥2 and mROX values ≤3.20 increase the sensitivity and specificity for predicting in-hospital mortality in sepsis patients.

Abstract

Menurut data dari Kementerian Kesehatan Indonesia (2020), dari total 4.656.382 ibu hamil di seluruh provinsi Indonesia, sebanyak 451.350 di antaranya memiliki risiko Kekurangan Energi Kronis (KEK), mewakili persentase sebesar 9,7%. Di antara ibu hamil, 59,5% merupakan primigravida yang mengalami KEK, 56% merupakan ibu hamil yang bekerja, dan 88% merupakan ibu hamil dengan usia berisiko (20-35 tahun) yang mengalami KEK. Tujuan: untuk menganalisis antara kadar Hb dan jumlah eritrosit antara ibu hamil normal dan ibu hamil yang mengalami KEK dan korelasi antar variabel. Metode: eksperimental dengan desain penelitian epidemiologi analitik dengan pendekatan cross-sectional, menggunakan teknik non-probability sampling dengan quota dan purposive sampling, dengan jumlah sampel sebanyak 30 orang. Hasil: Analisis penelitian menunjukkan bahwa variabel gravida memiliki distribusi normal pada kedua kelompok ibu hamil normal dan yang mengalami KEK. Data usia dan gravida menunjukkan homogenitas. Terdapat perbedaan yang signifikan dalam kadar Hb dan jumlah eritrosit antara ibu hamil normal dan ibu hamil yang mengalami KEK. Selain itu, terdapat korelasi yang signifikan antara kadar Hb dan jumlah eritrosit. Kesimpulan: terdapat perbedaan yang signifikan dalam kadar Hb dan jumlah eritrosit antara ibu hamil normal dan ibu hamil yang mengalami KEK (nilai p 0,001).Kata Kunci: KEK, Hb, Eritrosit

Abstract

Breastfeeding is essential for infant health and development, yet Indonesia faces challenges in meeting global breastfeeding targets. This review examines the potential of tele-counseling (telelactation) as a solution to improve breastfeeding support in the digital era, focusing on overcoming geographical and service delivery barriers. A systematic literature search covering PubMed, Scopus, Google Scholar, and local Indonesian journals identified 25 articles from 2019 to 2023, with 14 unique studies meeting inclusion criteria. Findings indicate that telelactation primarily utilizes WhatsApp groups, Androidbased apps, and online counseling sessions, resulting in improved breastfeeding techniques, increased exclusive breastfeeding rates, and enhanced maternal knowledge. However, privacy concerns and technological barriers persist, though these are addressed through specialized training and ongoing support for healthcare providers. Telecounseling demonstrates promise in supporting breastfeeding in Indonesia, though further research is needed to address geographical limitations and assess longterm impacts. Integrating telecounseling into national breastfeeding policies could significantly advance maternal and child health outcomes in Indonesia.

Abstract

One strategic approach to supporting maternal, child, and environmental health is enhancing breastfeeding literacy among adolescents. However, education efforts are often limited to pregnant and breastfeeding mothers. During World Breastfeeding Week (WBW) 2025 at SMA Muhammadiyah 2 Surabaya, an intervention was conducted to raise awareness among female adolescents about the importance of breastfeeding. More than 100 students participated in the “Tree of Hope” activity, which included presentations, discussions, and breastfeeding simulations. Evaluation results showed a 46% increase in understanding and strengthened commitment among participants to support breastfeeding practices. Teachers and students also received educational leaflets and a DIGIbook as learning resources. Aligned with the WBW 2025 theme, “Prioritize Breastfeeding: Create Sustainable Support Systems,” this program reinforced the role of adolescents as part of a long-term breastfeeding support ecosystem and fostered early preventive values and social empathy through a multimodal approach combining education, practice, and reflective participation.

Abstract

BackgroundGiant congenital melanocytic nevus (GCMN) is a rare disease with an extremely low incidence, that is present from or develops at birth and typically affects the dermis but may also affect other skin layers. Its incidence is estimated at <1 in 20,000 newborns. Despite its rarity, this lesion is important because it may be associated with severe complications such as malignant melanoma. A thorough follow-up is crucial since the probability of malignancy can vary depending on the clinical course. As such, careful observation is necessary to support possible management plans. Case DescriptionIn this case report, we present a three-day-old newborn male with abrasions on a black patch on his back. He presented with fever, jaundice, and black patches on more than 50% of his trunk down to the sacral area. The black raised patches resembling nodules had wounds on the lower back near the gluteus. A histopathology examination of specimens taken from 3 nodules on the back revealed hypocellular tissues with lymphocytes, histiocytes, neutrophils, fat droplets, and mature fat cells interspersed with some erythrocytes. The lesion was, therefore, diagnosed as a giant congenital melanocytic nevus (GCMN). Parents were counseled regarding the possible future course and were asked to come for regular follow-ups. Conclusions In this instance, we document a rare occurrence of GCMN that warrants recognition and appropriate treatment. Abstract

Stunting merupakan permasalahan tumbuh kembang anak akibat asupan nutrisi yang tidak terpenuhi. Dampak stunting pada anak adalah berkurangnya fungsi kognitif, motorik dan daya tahan tubuh. Pemenuhan nutrisi pada 1000 hari pertama kehidupan merupakan langkah utama dalam mencegah stunting pada anak. Kemudahan akses informasi dan layanan kesehatan tidak menjadikan kota Surabaya terbebas dari stunting, terdapat 4.8% anak mengalami stunting. Pemberian ASI eksklusif pada anak dengan rentang usia 0-6 bulan dapat mencegah stunting. Nutrisi yang dibutuhkan anak terkandung dalam ASI diantaranya, zat besi, vitamin B6, kalsium, vitamin A, vitamin D. Setelah melakukan pengumpulan data diketahui terdapat ibu yang belum memahami pentingnya dan manfaat ASI dalam tumbuh kembang anak. Selain itu permasalahan menyusui seperti payudara yang bengkak, puting yang lecet dan ibu yang sibuk bekerja menyebabkan anak tidak mendapat ASI eksklusif. Tujuan pengabdian ini membantu meningkatkan pemahaman terkait manfaat ASI dan memberikan pelatihan cara menyusui yang benar. Metode pelaksanaan meliputi analisis faktor penyebab permasalahan, perumusan solusi yang sesuai dengan kebutuhan masyarakat, serta edukasi melalui ceramah dan diskusi interaktif. Selain itu, dilakukan pula pelatihan praktik menyusui yang benar serta simulasi melalui metode role-play. Peserta yang mengikuti kegiatan ini adalah kader dan kelompok ibu menyusui dan memiliki balita. Hasil pelaksanaan pengabdian ini adalah pemahaman ibu yang meningkat terkait ASI dan keterampilan ibu yang semakin baik dalam menyusui sehingga diharapkan dapat meningkatkan cakupan ASI eksklusif dan terhindar dari stunting.
