Abstract

Diabetes mellitus (DM) is one of the non-communicable diseases that cause serious health problems. This chronic disease requires lifelong treatment to prevent complications and death. In the pandemic era, DM became comorbid that contributed to the death rate in Covid-19 cases, so that patients with type 2 DM become important to comply with the treatment underway. Aim: This study aimed to evaluate the treatment compliance in patients with DM type 2 at the Purwodiningratan primary health care, Surakarta. Method: This study was conducted in 2020 used an analytical method with crosssectional design. Populations were all of the patients who experience DM type 2, sampling techniques used total sampling with 108 respondents. Univariate, bivariate analysis, and logistic regression were used in this study. Compliance levels measured used the MMAS-8 questionnaire. Result:  The results of this study were obtained from several independent variables that related to compliance with DM type 2 treatment. The descriptive analysis result of patient compliance showed that 70.4% of patients were obedient. The significant variable was the side effects on DM drugs with (OR = -0.44; 95% CI = -0.74 - 0.15; p = 0.003). Conclusion: Most of the patients were adherent to the treatment and the most influencing factor was the side effect of the drug.

Abstract

One of the characteristics of metabolic syndrome (MetS) is the low high-density lipoprotein (HDL) levels. Jamblang leaves (Syzygium cumini L.) contain various phytochemicals, which have antihyperglycemic, antihyperlipidemic, and antioxidant properties. This study aims to demonstrate the effectiveness of ethanol extract of jamblang leaves in increasing HDL levels in the Wistar rats model of MetS. A pretest-posttest with a control group design was conducted using 30 male Wistar rats, divided into five groups randomly, with six rats for each group.  Here, KN means normal control group, K- negative control group, and P1, P2, and P3: treatment groups that were administered the 100 mg/kg/day, 150 mg/kg/day, and 200 mg/kg/day of ethanol extract of jamblang leaves for 28 days, respectively. The rat MetS model was developed by administering the high-fat fructose diet and streptozotocin-nicotinamide. The data were analyzed using paired t-test and one-way ANOVA (α=0.05). This study found that extract administration in P2 and P3 increased HDL levels significantly, but not with the P1 group. However, HDL levels in P1 were significantly higher compared to the K- group. Moreover, increasing the dose was significantly followed by higher HDL levels, although the highest dose did not reach the HDL level as in the KN group.

Abstract

BACKGROUND: Oxidative stress and inflammation contribute to pancreatic cell dysfunction that promote insulin resistance in type 2 diabetes (T2D). Red rice bran contains bioactive substances with anti-inflammatory and antioxidant properties which improved insulin resistance in obese mice. However, no studies have explored the potential of ethanol extract of red rice bran (EERRB) to prevent T2D progression, particularly pancreatic fibrosis complications. This study was conducted to investigate the effect of EERRB on inflammation measured with interleukin (IL)-1β and fibrosis of pancreatic tissue in a rat model of T2D. METHODS: Rats were induced with streptozotocin and nicotinamide to induce diabetes, and then separated into five groups. One group received no treatment, while the other four received 9 mg/kg/day acarbose, 165, 330, or 660 mg/kg/day EERRB orally for 21 days. Immunohistochemistry was conducted on pancreas tissues to measure the expression of IL-1β, while pancreatic fibrosis was assessed with Masson’s Trichrome staining.RESULTS: EERRB reduced the expression of pro-inflammatory cytokine, IL-1β, in pancreas tissue in a dose dependent manner. Significantly lower IL-1β expression were found in group receiving 660 mg/kg/day EERRB (10%) compared to diabetic with no treatment group (50%) (p<0.0001). Additionally, the IL-1β expression in the highest dose of EERRB group was comparable to the group receiving acarbose (10%). CONCLUSION: This finding suggests the beneficial effect of EERRB in the hyperglycemic condition that causes oxidative stress through blocking the IL-1β expression, hence alleviating the inflammation in pancreas tissue, and have a tendency in preventing pancreatic fibrosis progression, a process implicated in T2D pathogenesis. KEYWORDS: diabetes, inflammation, pancreatic fibrosis, red rice bran

Abstract

Introduction: Metabolic syndrome (MetS) is a cluster of metabolic factors, including low high-density lipoprotein cholesterol (HDL-C) levels. Vernonia amygdalina (VA) leaves have been documented for their anti-cholesterol properties. The purpose of this study to evaluate ethanol extract of VA leaves in rising HDL-C levels in the MetS model. Methods: An experimental study using pre-and post-test design was conducted using 30 male Wistar rats divided into five groups randomly. KN group served as the control group. KP group was the MetS model without ethanol extract administration. Meanwhile, P1, P2, and P3 were MetS models and were administered ethanol extract of VA leaves administration (50mg/kg, 100mg/kg, and 150 mg/kg) for 28 days. The data were analysed using paired t-test and one-way ANOVA (α=0,05).Results: This study discovered that administering the extract in P1, P2, and P3 significantly improved HDL levels. Furthermore, increasing the dose resulted in significantly greater HDL levels. VA leaves contain phytochemicals that have impacts on cholesterol levels in the blood. Flavonoids and tannin inhibit ester cholesterol formation. Terpenoid and flavonoid hinder HMG-CoA. Saponin contributes to free cholesterol transporter, reducing oxidative stress and increasing LDL receptors. Lastly, vitamin C raises catabolism levels and clearance LDL.Conclusion: There are increases in HDL levels in the MetS model group receiving ethanol extract VA leaves.
