The Effect of Complementary Vitamin D on Sputum Conversion in Pulmonary TB Patients Lubis, Nita Andriani

Abstract

Background: Pulmonary TB is one of the most important health problems in Indonesia. The number of pulmonary TB patients ranked second after India. Vitamin D has a complex effect in the immune system. One of the effects of vitamin D is to stimulate the production of Cathelicidin which increases macrophage phagocytosis. This study aimed to analyze the effect of complementary vitamin D on sputum conversion in pulmonary tuberculosis patients.Subjects and Method: This study was a randomized controlled trial conducted at several health centers in Deli Serdang, North Sumatera, from November 2015 to June 2016. A total sample of 80 patients with positive acid-resistant bacilli (BTA) was selected for this study by consecutive sampling. The sample was allocated randomly into two groups: (1) TB drug group + 2.5 mg vitamin D; (2) TB drug group + placebo. The dependent variable was sputum conversion. The independent variable was complementary vitamin D. Pre and post vitamin D levels were examined by ELISA technique. Sputum conversion was examined by TB laboratory worker at the health center where patients received TB drug treatment. Difference in percent of sputum conversion between the two groups was tested by chi-square test. Difference in mean of sputum conversion time between the two groups was tested by t-test.Results: The mean serum vitamin D level in the intervention group (62.05±22.08) was higher than the control group (28.56±7.48) after the intervention, and it was statistically significant (p<0.001). After 56 days, all subjects in the vitamin D group experienced sputum conversion (100%) with a median sputum conversion time of 28 days, whereas 90% of subjects in the placebo group experienced sputum conversion with a median sputum conversion time of 48 days (p= 0.002).Conclusion: Adding vitamin D to anti-TB drug treatment effectively improves and accelerates sputum conversion of patients with tuberculosis acid-fast bacilli smear positive.Keywords: Vitamin D, sputum conversion, pulmonary tuberculosisCorrespondence: Nita Andriani Lubis. Abstract

Introduction: Family empowerment might be considered asan effort to strengthen the role and knowledge of familywith TB patients. Objective: Proving the effectiveness of family empowerment with SEDAP (Sharing, Education, and Practice) method to improve the quality of life in pulmonary tuberculosis patients. Methods: Mixed methods research, qualitative research was done using FGD approach, while pre-post control group design was used for the quantitative research. Thirty-eighttrials of TB patients were divided into 2 groups: intervention group where family empowerment was introduced using the SEDAP method and control group. Qualitative data were collected using a recording device which would be transcripted, followed by keyword observation and analysis. Quantitative data were obtained using SGRQ questionnaires, both on knowledge and quality of life variables. Qualitative data were analyzed by interactive models, whereas quantitative data by independent samples t-tests and paired samples t-test. Results: Three main domains (knowledge, attitude, and behavior) were obtained from the FGD results, along with the keywords analyzed from each domain. Knowledge domain was associated with TB causes and routes of transmission, attitude was related to the negative stigma on TB patients, and lastly, behaviour was linked with patients’ forgetfulness in taking medication and the side effects occurred due to the medication. Abstract

Pulmonary tuberculosis (TB) is still a problem that must be resolved in Indonesia, especially North Sumatra. The study aims to determine the effect of vitamin D supplementation on the speed of sputum conversion in pulmonary TB patients. Randomized controlled trial study was conducted on 41 pulmonary TB patients according to inclusion criteria. Research subjects were given TB drug (DOTS program) and vitamin D treatment for 2 months. Sputum examination based on smear was carried out microscopically at 2nd week, 4nd week, 6nd week, and 8nd week after treatment. The results showed that all pulmonary TB patients who were given the intervention experienced sputum conversion. Research subjects with AFB +1 occurred the fastest conversion compared to the others. Statistical analysis using chi square obtained significant results (p = 0.0001). Sputum conversion evaluation based on the initial AFB status, there was a significant relationship between the time of sputum conversion with the initial AFB status with a very strong correlation level (r = 0.767). Abstract

Vitamin D plays a role in innate and adaptive immune functions, and its deficiency has become a risk factor for TB infection. This study aimed to describe the prevalence of vitamin D deficiency in newly diagnosed TB patients and to examine the relationship between vitamin D levels and sputum smear positivity that reflects bacterial load. This is a cross-sectional study conducted at several primary health care facilities in Medan and Deli Serdang from November 2015 to June 2016. As many as 86 subjects adult TB patients with positive smear sputum who had not received therapy or had at most 1 week on therapy were recruited. Sputum examination were interpreted using IUATLD (International Union Against Tuberculosis and Lung Disease) grading scale. Serum 25 (OH) vitamin D level was estimated using enzyme-linked immunosorbent assay (ELISA) and interpreted as deficiency (<20 ng/ml), insufficiency (20–29 ng/ml), or optimum (30–100 ng/ml). The association between vitamin D level and positivity level of AFB sputum was analyzed using Spearman correlation test. The prevalence of VDD in new pulmonary TB patients with positive smear sputum was 17.4%.  There was a weak relationship between vitamin D level and positivity level of  AFB sputum (p = 0.014), with a correlation coefficient of (-0.264). Many of newly diagnosed TB patients had vitamin D deficiency despite the fact that Indonesia is an equatorial area with sufficient sunshine throughout the year. This study shows vitamin D indeed affect bacterial load and a low 25 (OH) vitamin D level is associated with higher bacterial load. Prevalensi Defisiensi Vitamin D pada Pasien TB Paru Baru dan Hubungan dengan Derajat Kepositifan Sputum Basil Tahan AsamVitamin D berperan dalam imunitas bawaan dan adaptif. Kekurangan vitamin D merupakan faktor risiko terinfeksi TB. Penelitian ini bertujuan menggambarkan prevalensi defisiensi vitamin D pada pasien TB baru dan mengetahui hubungan kadar vitamin D dengan derajat kepositifan sputum yang mencerminkan kepadatan bakteri. Penelitian potong lintang ini dilakukan di beberapa Puskesmas di wilayah Medan dan Deli Serdang pada November 2015 hingga Juni 2016. Subjek penelitian sebanyak 86 pasien TB dewasa dengan sputum basil tahan asam (BTA) positif, yang belum atau maksimal 1 minggu mengkonsumsi obat. Pemeriksaan sputum dinilai menggunakan skala IUATLD. Kadar vitamin D serum 25 (OH) dinilai dengan metode ELISA dan diklasifikasikan menjadi defisiensi (<20 ng/mL), insuffisiensi (20–29 ng/mL), optimum (30–100 ng/mL). Hubungan kadar vitamin D dengan tingkat kepositifan sputum BTA dilakukan dengan uji korelasi Spearman. Prevalensi VDD pada pasien TB paru baru dengan sputum positif adalah 17,4%. Terdapat  hubungan antara kadar vitamin D dan tingkat kepositifan sputum BTA (p=0,014), dengan koefisien korelasi (-0,264). Banyak pasien TB paru baru yang mengalami kekurangan vitamin D, meskipun Indonesia berada di khatulistiwa dengan kecukupan sinar matahari sepanjang tahun. Penelitian ini menunjukkan bahwa vitamin D berhubungan dengan kepositifan sputum dan kadar vitamin D yang rendah berhubungan dengan kepadatan bakteri yang lebih tinggi.

Abstract

Adolscent girls phase (12-21 years) is a very important individual development segment, beginning with the maturation of physical and reproductive organs that characterize women of childbearing age. Body mass index (BMI) is one of indicator used to see women of childbearing age status related to pregnancy. Kidney damage causes an increase in urinary creatinine but high urinary creatinine does not necessarily indicate kidney damage. Our objective is to determine the relationship between BMI and urine creatinine  in adolscent girls in several Medan health schools. We performed an observasional study with cross sectional design of 56 participants   19-21 old girls. 24 hours urine creatinine concentration was measured using jaffe reaction method. The highest average BMI value (21.10 kg/m2 is the normal category (75%), mean urine creatinine concentration (1263,86 mg/24 hour) is also normal category. Data analysis with spearman correlation between BMI and 24-hour urine creatinine level was not statistically different (p = 0.140). High BMI values tend to have high 24-hour urine creatinine levels too, but statistically there was no significant relationship between 24-hour urine creatinine levels and BMI values in this study.

Abstract

Tuberculosis (TB) is a contagious infectious disease caused by the bacterium Mycobacterium Tuberculosis which can attack various organs, especially the lungs. However, before treatment, the diagnosis needs to be made through laboratory tests with the right method with good sensitivity and specificity The purpose of this study was to determine the level of sensitivity and specificity of TB diagnoses. The research method is a descriptive analytic survey research with cross sectional design research design research was conducted from May to October 2019. The place of research was conducted at Pancur Batu Health Center Deli Serdang for taking sputum specimens and blood samples of TB suspect patients. The number of samples in this study were 33 people. The results obtained by the sensitivity and specificity of BTA by 75% and 88.9%. The results obtained sensitivity and specificity of GeneXpert of 95.8% and 100%. The results obtained sensitivity and specificity of Tuberculosis Immunocromatography (ICT) of 28% and 100%. The research results obtained sensitivity and specificity of Interferon Gamma Release Assay (IGRA) of 88% and 100%. BTA is suitable for use, the GeneXpert method is used for the TB RO program and the MGIT Gold Standard for MDR TB.

Abstract

Background: Coronaviruses are a large family of viruses that are transmitted zoonically (between animals and humans) and can cause mild to severe symptoms. SARS-CoV-2 infection in humans causes symptoms of acute respiratory distress such as fever, cough, and shortness of breath. In severe cases, the disease can cause pneumonia, acute respiratory syndrome, kidney failure, and even death. Efforts to break the chain of spread of COVID-19 require a good understanding and knowledge of all elements including nurses. Objective: To find similarities, advantages, and disadvantages regarding the Description of Nurses' Knowledge and Attitudes to prevent Covid-19 disease in 2021. Methods: This study used a descriptive type with a Literature review design obtained from sources from the International Journal of Nursing Research (IJNR) and the National Library. The keywords in this study were Description, Knowledge, Attitudes, Nurses, Covid-19 Prevention. Results: From five articles on the Description of Nurses' Knowledge and Attitudes to prevent Covid-19 disease, four articles had good knowledge and attitudes and there was one article which stated that nurses' knowledge and attitudes were still not good. Conclusion: This study aims to determine the similarities, advantages, and disadvantages of using a literature review. Of the five articles, it shows that the five research journals have research similarities in this research method, the five journals use descriptive research methods, there are 4 journals that have similarities in the sample, namely nurses, the third equation there are five journals that use a cross sectional approach. The advantages of this study are that there are four complete studies in accordance with the research objectives, and the drawback of this research is that there is one incomplete article whose research is not in accordance with its objectives. Keywords: knowledge; attitude; nurse; Covid-19 prevention

Abstract

Pengabdian masyarakat merupakan bagian dari Tri Darma Perguruan Tinggi yang dilakukan dengan berbagi pengetahuan sekaligus kepedulian terhadap isu di lingkungan sekitar. Jurusan Teknologi Laboratorium Medis memiliki kewajiban dalam memberikan pelayanan kesehatan yang diimplementasikan dalam bentuk pengabdian masyarakat berupa edukasi tentang pentingnya vitamin d dan berjemur pagi hari pada ibu-ibu yang tinggal di Lingkungan 2 Kelurahan Pulo Brayan Bengkel Kecamatan Medan Timur. Pengabdian masyarakat ini merupakan aplikasi dari Hasil Penelitian Dosen Pemula yang berjudul Perbandingan Kandungan Vitamin D dari Ekstrak Minyak Ikan Beberapa Jenis Ikan. Metode pengabdian masyarakat ini dilakukan dengan memberikan edukasi dengan pemanfaatan media booklet untuk meningkatkan pengetahuan dan perilaku ibu-ibu mengenai pentingnya vitamin D dan berjemur pagi hari. Selanjutnya pemberian kuesioner mengenai pengetahuan dan perilaku ibu terhadap pentingnya vitamin D dan berjemur pagi hari dilakukan pre and post untuk melihat perbedaan pengetahuan dan perilaku ibu sebelum dan sesudah diberikan intervensi. Kuesioner tingkat pengetahuan berisi 10 pertanyaan, kuesioner tingkat perilaku berisi 10 pertanyaan. Pertanyaan di dalam kuesioner ini telah di uji validitas dan reliabilitasnya dan setiap pertanyaan mempunyai nilai tertentu. Gambaran pengetahuan sesudah edukasi diberikan, dari 50 responden, 6 orang berpengetahuan kurang, 37 orang cukup dan 7 orang baik pengetahuannya terhadap pentingnya vitamin D dan berjemur di pagi hari. Tingkat perilaku responden setelah diberikan edukasi, 13 responden berperilaku cukup dan 37 responden berperilaku baik terhadap pentingnya vitamin D dan berjemur di pagi hari.
