Abstract

National Social Security System, Act Number 40 Year 2004  mandated that National Social Security System as National Health Insurance starting January 1, 2014. As a private hospital that follow the program, Muhammadiyah Gamping Hospital has to prepare strategies for it. This study aims to analyze the strategy of Muhammadiyah Gamping Hospital facing National Health Insurance. The analysis used a quantitative of Strenght, Weakness, Opportunities, Threats (SWOT) analysis to determine the actual position of the hospital so it can choose the right strategies. Data collection using in-depth interview method, quesioner, and document review. The sample of the research are 4 people who are the directors of Muhammadiyah Gamping Hospital. Quantitative SWOT analysis puts the hospital in quadrant III, which means that the hospital is in a position of having many weaknesses but has many opportunities, then they have to make a strategy changes. In this research there are many strategies that can be done.Some strategies are making Clinical Pathway, recalculate unit cost and utilization Hospital Information Technology and bridging with BPJS system.  This study show that Muhammadiyah Gamping Hospital has to make a strategy changes according to the position of hospital in quadrant III. Keywords: SWOT analysis; Hospital strategy; National Social Security.

Abstract

Pada karsinoma terjadi peningkatan proliferasi sel dibanding dengan apoptosis sel, sehingga sel akan tumbuh lebih banyak dari jumlah normalnya. Pada kondisi homeostasis, untuk dapat mengatur jumlah sel pada tubuh manusia, mekanisme utama tubuh adalah dengan menghasilkan p53. Protein p53 sebagai protein penghambat tumor mengaktifkan pembentukan p21 yang berperan untuk aktivasi beberapa komplek kinase tergantung siklin dan memutus siklus pembelahan sel. Tujuan penelitian ini untuk analisis hubungan ekspresi p53 terhadap prognosis radioterapi pada karsinoma nasofaring dengan metode retrospektif. Ekspresi p53 telah dicat imunohistokimia dan dianggap positif bila jumlah sel tumor positif lebih dari 10% dan dianalisis hubungan ekspresi p53 dengan prognosis pada pasien karsinoma nasofaring dengan radioterapi. Hasil penelitian menunjukkan dari 43 Pasien karsinoma nasofaring dengan terapi radiasi terdapat 8 orang (18,6%) meninggal selama perawatan. Berdasarkan jenis kelamin, 74,42% pasien adalah laki-laki dan 25,58% wanita. Kelompok usia 51 sd 60 tahun adalah yang terbanyak yaitu sebesar 14 kasus (32,56%). Terdapat hubungan yang bermakna antara ekspresi p53 dengan prognosis hasil terapi radiasi pada karsinoma nasofaring p = 0,000 (p0,05). Disimpulkan bahwa ekspresi p53 yang positif akan memberikan prognosis yang lebih baik terhadap hasil terapi radiasi pada karsinoma nasofaring. In the cancer condition, there is increasing of cell proliferation than apoptosis, so cell will grow rapidly than normal. In normal homeostatic, to set the amount of the cells in human body, the primary body mechanism is producing gene p53. The p53 as protein cancer suppressor activate p21 that role to activate some of complex kinase  based on cyclin and cut mitosis cycle. The aims of this research to analyze relation p53 expression with prognosis of radiotherapy in nasofaringeal carcinoma using retrospective method. P53 expression strained by imunohistochemistry and considered positive if we find greater than 10% positif cell tumor then analyzed relation p53 expression with prognosis of radiotherapy in nasofaringeal carcinoma. The result shows from 43 patient nasofaringeal carcinoma with radiotherapy there are 8 patient (18,6%) died. 74,42% patient are male and 25,58% are female. 51-60 years old group is 14 cases (32,56%). There was significant correlation between prognosis and p53 expression p= 0,000 (p0,05). The conclusion is that positive p53 expression will give a better prognostic for nasofaringeal carcinoma with radiotherapy.

Abstract

Physical inactivity contributed to 7.2% of all-cause mortality. Many research demonstrates that the integration of physical activity and exercise into daily routine prevents chronic disease and lowers mortality. This study aims to assess the physical fitness of members of the Indonesian Medical Association in the Bantul region using a descriptive from secondary data. The subjects were 38 members of the Indonesian Medical Association. Physical fitness data was obtained using the Rockport One Mile Walking Test. Body Mass Index (BMI) is also calculated to describe excessive body fat. The majority of participants are female (52.63%) and participants dominant age ranged between 19 and 44 years old (50%). The participants aged 18 years old showed good physical fitness. The results in participants aged 19 to 44 years were more varied, there were 8 participants, 7 participants, 3 participants 1 participant had fair, good, poor, and very poor physical fitness, respectively. Physical fitness in participants aged 45 to 59 years were 9 and 5 participants showed good and fair physical fitness, respectively. The subjects aged >59 years old have 2 people with good physical fitness and 1 person with fair physical fitness. None of the participants with BMI status underweight, normal weight, or overweight showed poor physical fitness, while participants with obesity had poor and very poor physical fitness. Physical fitness is a multidimensional concept and is related to regular activity.

Abstract

Tahajud prayer is a night prayer that is performed when most people are fast asleep, in the last third of the night. Physiologically, human life has a diurnal circadian rhythm pattern. If this pattern is accompanied by the obligation to perform tahajud prayer, the circadian rhythm pattern can become nocturnal. As a result, the central nervous system undergoes behavioral changes aimed at adjusting to changes in circadian rhythms. Stress is a condition where a person's physical and psychological balance is disturbed due to a trigger and can hinder a person in carrying out their tasks. Through prayer, a person will feel close to the Creator, all tiredness, problems, burden of thoughts, and emotions, all of which are abandoned during prayer in a solemn manner. The purpose of this study is to analyze whether there is a relationship between the intensity of tahajud prayer and stress levels in students of the Faculty of Medicine, Ahmad Dahlan University Yogyakarta. The research method used in this study is an observational analytic quantitative method using a cross-sectional design. The research sample amounted to 153 students of the Faculty of Medicine, Ahmad Dahlan University. The research instruments were the Perceived Stress Scale (PSS-10) questionnaire and the Tahajud Prayer Practice questionnaire. Statistical tests using the Rank-Spearman test found a p-value of 0.000 < α (0.05) with a correlation coefficient of -0.342 and the Kendall's Tau test found a p-value of 0.000 < α (0.05) with a correlation coefficient of -0.328 indicating moderate correlation strength. The conclusion of this study is that there is an inversely proportional relationship between the intensity of tahajud prayer and stress levels in Ahmad Dahlan University Faculty of Medicine students which means that the higher the intensity of tahajud prayer, the lower the stress level tends to be, and vice versa, if the intensity of tahajud prayer is lower, stress levels tend to be higher. Therefore, it is expected that students can increase the intensity of tahajud prayer in their daily lives to reduce stress levels.

Abstract

Aging is associated with various factors identified in the hallmark of aging. Numerous studies have been conducted to identify genes involved in the aging process, allowing for a comprehensive understanding of this phenomenon. This study aims to provide an overview of research on genes associated with the aging process. Data for this study was obtained and analyzed from the SCOPUS database covering the period from 2014 to 2024, with a focus on medicine, neuroscience, and nursing. The keywords used for the search were "gene," "longevity," and "aging." The search results were filtered to match the study's purpose and were limited to articles in English. The data obtained was analyzed using VOSviewer version 1.6.20. A total of 1,603 articles were analyzed. The highest number of publications occurred in 2021. The most frequent sources for these articles were the journals Biogerontology and The Journals of Gerontology Series A: Biological Sciences and Medical Sciences. The United States was the leading contributor in publications (n=488), with Harvard Medical School producing the highest number (n=45). The most-cited article was by Loboda in 2016 (1,863 citations). Research on genes associated with aging has shifted from identifying genes (2017-2019) to focusing on the functions of these genes in physiological processes and other risk factors of aging. In conclusion, the aging research has evolved from focusing solely on gene identification to exploring the physiological functions and risk factors related to aging.

Abstract

Paracetamol is an antipyretic and analgesic drug that is relatively safe to consume orally with the appropriate dose and can be obtained without a doctor's prescription. The antioxidant quersetin contained in red spinach leaves can bind free radicals and strengthen the antioxidant defense system in the body. This study was conducted to determine the histopathological view of liver and kidney Wistar rats induced by toxic dose of Paracetamol after administration of ethanol extract of red spinach leaves. This research method used 24 Wistar rat which were divided into 4 groups. Kontrol  (aquadest), KN (Paracetamol 0.5 g/kg BW), P1 (ethanol extract of red spinach leaves 0.4 g/kg BW and Paracetamol 0.5 g/kg BW) and P2 (ethanol extract of red spinach leaves 0.8 g/kg BW and Paracetamol 0.5 g/kg BW). The administration of the extract was carried out for 15 days with Paracetamol on the last 5 days. Euthanasia was performed on the 16th day and then made preparations of liver and kidney with paraffin method (Hematoxylin eosin). Data observed included hepatosomatic index, total and area of hepatocytes, total of liver and kidney necrosis cells, area of inflammatory cells, total and area of glomeruli, and area of hemorrhage. Observation data were analyzed using One-Way ANOVA and DMRT further test (P<0.05). The results showed that the dose of ethanol extract of red spinach leaves 0.4 g/kg BW was able to reduce hepatocyte area, the total of necrosis cells, the area of inflammation and protect the glomerulus (P < 0.05). The conclusion of this study is that ethanol extract of red spinach leaves at a dose of 0.4 g/kg BW can protect the liver and kidneys of Wistar rats induced by Paracetamol at a dose of 0.5 g/kg BW
