Abstract

Ventilator-associated pneumonia (VAP) is reported as one of the highest nosocomial infection conditions in several countries. Risk factors can be classified into modifiable and non-modifiable factor. This study aims to analyze modifiable factors related to the incidence of VAP in patients using mechanical ventilation. This study used a prospective cohort observational analytic design with a quantitative involving 40 patients with mechanical ventilation in the ICU and selected using purposive sampling. Observation sheets and Clinical Pulmonary Infection Score (CPIS) were used as data collection tools. Hypothesis testing in this study utilized Somers’s d, contingency coefficient, and Kendall’s tau. The analysis revealed significant associations between the duration of mechanical ventilation (p=0.009), and level of consciousness (p=0.021) with the incidence of VAP. However, no significant associations were found between antibiotic use (p=0.075), ICU length of stay (p=0.135), and reintubation (p=0.253) with the incidence of VAP. The study showed that the length of time mechanical ventilation was used and the patient's level of consciousness were factors associated with the incidence of VAP in the ICU. Thus, special attention to modifiable factors can be one of the effective strategies in reducing the incidence of VAP in the ICU.

Abstract

Maternal mortality and morbidity in hospitals mostly occur in preventable cases. The early warning system, as one of the prevention efforts based on clinical findings, has not been implemented comprehensively. The large number of incomplete data and the inconsistency of the early warning system escalation are the basis for the development of electronic technology solutions. With an electronic system, it is expected that management can be carried out earlier, preventing life-threatening conditions because patients will be treated faster so that the expected output is better. Objective: This study aims to analyze the differences in the completeness of data entry and the appropriateness of the early warning system escalation before and after the digitalization of EWS and MEWS in the Alamanda Room of Dr. Hasan Sadikin Hospital, Bandung. Method: This study is a quantitative study using a pre-experimental design with a one-group pre-post test approach. The sampling technique with a total sampling of 648 datas, both before and after digitalization. Secondary data was taken from all data from the results of the filling responses that had been collected. Data analysis used univariate frequency distribution analysis and bivariate analysis using the Chi-Square and Fisher's Exact tests. Results: Based on the results of the study, it was found that according to the hospital's Standard Operating Procedure, the completeness of EWS data filling after the intervention increased to 97.2%, the appropriate EWS data escalation after the intervention became 56.61%. MEWS data also increased after the intervention to 91.2%, the appropriate MEWS data escalation after the intervention became 63.6%. The results of the bivariate data analysis showed a significant difference in the completeness of data filling and the appropriateness of escalation before and after digitalization with a p-value of 0.000. Conclusion: Digitalization can improve the completeness of data filling based on SOP and the appropriateness of the hospital's early warning system escalation. The appropriateness of escalation and rapid clinical response has an impact on the quality of maternal care, patient safety, and the quality of hospital services.

Abstract

Abstract

Success in providing basic life support to cardiac arrest patients is influenced by self-efficacy related to readiness and confidence in giving first aid. Students as prospective health workers are required to be able to provide basic life support in emergencies. Limited training and direct experience in providing basic life support causes students' skills and self-efficacy to be less than optimal. This study aimed to identify the relationship between self-efficacy and the ability of cardiopulmonary resuscitation performed by students. A quantitative cross-sectional study was conducted on 107 diploma III nursing students selected using the proportional stratified random sampling technique. Primary data were collected using the General Self-Efficacy Scale questionnaire and basic cardiopulmonary resuscitation skills observation sheets. Hypothesis testing in the form of Spearman correlation was conducted to identify the general objectives of the study. The results of the study showed a mean value of self-efficacy of 36.06 and a mean value of basic life support skills of 89.00. The results of the Spearman test obtained a p-value of 0.0001 with a relationship strength of 0.813. It is concluded that there is a very strong positive relationship between self-efficacy and basic life support skills, which means that higher self-efficacy in performing cardiopulmonary resuscitation is directly proportional to the higher success of implementing basic life support. Researchers hope that nursing students can improve their self-efficacy related to cardiopulmonary resuscitation through regular knowledge refresher activities and basic life support skills training.
