Abstract

Safety culture is an aspect of organizational culture that refers to how health workers see andtreat safety in a health service, which has implications for patient satisfaction. This study aimedto identify relationship between patient safety culture and the level of patient satisfaction atPublic Health Center Ajung Jember Regency.The research design used was cross-sectional,involving inpatients at Public Health Center Ajung Jember Regency as many as 45 respondents.The sampling technique uses simple random sampling. Data was collected using the Indonesianversion of the Hospital Survey of Patient Safety Culture and Service Quality questionnaire.Statistical analysis using Spearman rho test with significance ≤0.05. Most patients expressed ahigh category of patient safety culture (57.8%), with more than half (51.1%) expressingsatisfaction with services. There was a relationship between patient safety culture and patientsatisfaction (p-value = 0.000; α = 0.05; r = 0.694). Discussion: Patient satisfaction will alsoincrease by implementing a good patient safety culture. Therefore, training on theimplementation of patient safety culture is very necessary to be held for nurses at public healthcenter; in this case, the policy stakeholders who are obliged to hold the training.

Abstract

Introduction: Diabetes mellitus is a long-term illness that presents threats to one’s physical health as well, but also has a significant psychological impact on sufferers, as a result of which the ability to increase self-efficacy decreases. Psychoeducation can be chosen as an alternative to help the elderly to form positive coping by increasing their self-efficacy. This study aims to analyze the effect of psychoeducation on self-efficacy in elderly patients with diabetes mellitus in the Jenggawah Community Health Center area. Methods: The method used in this research was a quasi-experimental design using an intervention group and a control group. The sample in this study was 60 elderly people with diabetes mellitus. This research uses a purposive sampling technique. The measuring tool in this research is the Diabetes Management Self-efficacy Scale questionnaire which consists of 18 statements that are valid and reliable. Data analysis used the Wilcoxon and Mann Whitney tests. Results: The results of the Wilcoxon Test statistical analysis were obtained in the pre-post test of the intervention and control groups, p-value 0.000 < 0.05. The Mann Whitney test showed a p-value of 0.000 < 0.05 indicating that there was an influence of providing psychoeducation on Self Efficacy in elderly people with diabetes mellitus at the Jenggawah health center. Conclusions: Psychoeducational activities can be an alternative intervention used by Public Health Center to increase motivation and also information to patients about the importance of Self Efficacy in empowering diabetes mellitus.

Abstract

Diabetes mellitus is not treated immediately can cause ineffective peripheral tissue perfusion. Ineffective peripheral tissue perfusion can lead to lower extremity amputation. Buerger allen exercise can improve ineffective peripheral tissue perfusion with an indicator of increasing the value of the ankle brachial index (ABI). This literature study aims to explore the intervention of Buerger Allen exercise on the ineffectiveness of peripheral tissue perfusion in patients with diabetes mellitus. This study uses a literature review method with 3 databases, namely Proquest, Google Scholar, and PubMed with a total of 6 articles reviewed with the PICOS framework criteria. The criteria for the articles used are those published from 2016 to 2021 amounting to 2,445 articles, then a selection is made based on duplicate titles, PICOS and critical appraisal to 6 articles. The administration of Buerger Allen exercise greatly affects the increase in the value of the ankle brachial index (ABI) as an indicator of the effectiveness of peripheral tissue perfusion in patients with diabetes mellitus. All reviewed articles show the same results. Buerger allen exercise can increase oxygen in the blood vessels so that it can be an alternative therapy that can be given to people with diabetes mellitus to improve ineffective peripheral tissue perfusion.

Abstract

The behavior of hypertension self-care management in the elderly is very important to be applied to reduce the prevalence of hypertension in rural areas. This study aims to analyze the factors associated with hypertension self-care management behavior in the elderly in rural areas based on the Protection Motivation Theory (PMT). The research design uses analytic observational with a cross-sectional approach. The sample in this study was 150 elderly people with hypertension who lived in rural areas. The independent variables are perceived vulnerability, perceived severity, perceived reward, response efficacy, self-efficacy, and intention. The dependent variable is hypertension self-care management behavior. Data collection uses the PMT and HSMBQ questionnaires. Bivariate analysis used the Spearman correlation test and multivariate analysis used linear regression with a significance level = 0.05. Bivariate test results found that perceived vulnerability (r=0.615 and p<0.05), perceived severity (r=0.497 and p<0.05), perceived reward (r=0.598 and p<0.05), efficacy response (r=0.510 and p<0.05), self-efficacy (r=0.477 and p<0.05), and intention (r=0.513 and p<0.05) related to hypertension self-care management behavior. Multivariate test results found that the PMT model associated with hypertension self-care management behavior includes perceived vulnerability (p<0.05), perceived severity (p<0.05), perceived reward (p<0.05), and self-efficacy (p<0.05). The value of R square = 0.519 indicates that the PMT model can predict 51.9% of self-care behavior. The dominant factor that can increase hypertension self-care management behavior is the perception of vulnerability. Nurses should be able to provide health education to improve hypertension self-care management behavior.

Abstract

People with Diabetes Mellitus need long-term care and treatment to improve their quality of life. Family support and self-efficacy are believed to have an influence on the quality of life of elderly people with diabetes mellitus. The objective of the study was to identify family support, self-efficacy and quality of life for elderly people with diabetes mellitus. This study is a descriptive analytic study with a cross-sectional approach.  The sample in this study were elderly people with diabetes mellitus who were selected using a purposive sampling technique with a total of 54 respondents. Furthermore, univariate data analysis was carried out using the frequency distribution and statistical tests using the Spearman Rho Test. The results of the Rho Spearman analysis showed that family support was positively correlated with the quality of life of elderly people with diabetes mellitus (rho=0.579; p<0.01). Meanwhile, the results of the Rho Spearman analysis showed that self-efficacy correlated positively with the quality of life of elderly people with diabetes mellitus (rho=0.537; p<0.01). The recommendation from the results of this study, is that nurses can increase family support and self-efficacy for elderly people with diabetes mellitus by forming a support group consisting of families who have elderly family members with diabetes mellitus so that they can share information related to how to provide family support to provide positive reinforcement and motivation to elderly people with hypertension so that their quality of life increases.

Abstract

Introduction: Self-care management is needed to improve the quality of life of elderly people with hypertension, however, the self-care management behavior displayed by elderly people with hypertension tends to be poor. The family is an important figure to take into account in fulfilling optimal self-care management for hypertensive elderly people. Objectives: This research aims to analyze the relationship between family affective function and self-care management behavior of elderly people with hypertension. Methods:  This research used a cross-sectional design with a sample of 150 respondents taken using simple random sampling. Data were analyzed using the gamma correlation test.  Results:  There is a relationship between family affective function and self-care management behavior in elderly people with hypertension with p-value = 0.000 ≤ 0.05. Discussion: Good family affective function tends to increase hypertension self-care management behavior. Elderly people with hypertension should always maintain maximum self-care management behavior. Families should maintain closeness to elderly people who suffer from hypertension in their family. Families should always respond well to the existing needs of elderly people with hypertension in their families.

Abstract

Introduction: Anxiety is a common psychological response experienced by patients before undergoing surgery. This condition can affect physiological stability, the healing process, as well as the success of surgery. Factors that affect a patient's anxiety level can come from internal aspects such as age, gender, education level, and knowledge; as well as external factors such as family support, information from health workers, and previous surgery experience. This study aims to analyze internal and external factors related to the patient's anxiety level before surgery. Methods: This study uses an analytical descriptive design with a cross sectional approach. The population obtained was obtained from the average number of surgery patients each month of 100 people. The number of samples calculated using the slovin formula was obtained by 80 respondents who were taken using the Random Sampling technique. Data collection was carried out using the Hamilton Anxiety Rating Scale questionnaire to measure anxiety levels as well as questionnaires of related factors. Data analysis uses chi-square tests to look at the relationships between variables. Results: The results of the study showed that the factors related to the level of anxiety in pre-operative patients through statistical tests were age (pvalue = 0.04), education level (pvalue = 0.02), surgical experience (pvalue = 0.001) and family support (pvalue = 0.001). Meanwhile, another factor that was not related to the anxiety level of pre-operative patients was gender (pvalue = 0.216). Conclusions: Both internal and external factors have an influence on the patient's anxiety level before surgery. Efforts are needed to improve pre-operative education and psychological support for patients to reduce anxiety levels and increase readiness for surgery.

Abstract

Introduction: The ability of the self and the implementation of family functions of the elderly with diabetes in the management of diabetes mellitus is essential to know because it can influence the elderly to take appropriate actions to get effective interventions and prevent complications from the disease. This study aims to determine the validity and reliability of a comprehensive nursing assessment instrument for the management of diabetes mellitus in the elderly.   Method: This study's design is descriptive, with a crossectional approach conducted on 30 elderly respondents with diabetes mellitus who meet the inclusion criteria. This research instrument was developed based on the theory of family-centered nursing and the functional consequences model, which was then carried out with construct validity and reliability tests.   Result: The results of the validity test of the comprehensive nursing assessment instrument in the management of diabetes mellitus in the elderly living with family for all statement items based on family-centered nursing and functional consequences. The results were valid, all statement items had an r value calculated > the r table, which was all more than 0.361. Meanwhile, the reliability test of the instrument found that all instruments were declared reliable, namely the Cronbach alpha value of the entire instrument was less than 0.7.   Conclusion: The comprehensive nursing assessment instrument developed can be used to see how efforts to handle diabetes mellitus in the order of education, research, and health services at the Health Center.   Keywords: Diabetes mellitus; disease management; elderly; family; self-ability.

Abstract

Malnutrisi pada anak masih merupakan masalah kesehatan masyarakat yang krusial secara global. Di Indonesia, stunting masih menjadi permasalahan kesehatan utama, di mana Kabupaten Jember tahun 2022 memiliki angka stunting tertinggi di Jawa Timur. Kejadian stunting di Desa Sukorambi menduduki peringkat 1 Kabupaten Jember yaitu 19,10%. Kegiatan pengabdian ini bertujuan untuk meningkatkan kemampuan kader dan masyarakat Desa Sukorambi tentang pijat bayi, serta meningkatkan pengetahuan tentang pemanfaatan pangan fungsional berbasis protein pada balita menjadi yogurt melalui pemberdayaan perempuan masyarakat. Metode yang digunakan adalah presentasi/ceramah dan demonstrasi yang ditujukan kepada kader Posyandu. Tahap tahap yang digunakan dalam metode yang pertama yaitu pendekatan lahan, pelaksanaan Pelatihan Pijat Bayi dan Pembuatan yogurt skala rumah tangga, evaluasi pelatihan dan penerapan program yogurt dan pijat bayi di posyandu dan yang terakhir adalah evaluasi program dengan masyarakat dan kader.  Hasilnya menunjukkan bahwa pelatihan ini mampu memberikan dampak yang baik terhadap pengetahuan dan penerapan pijat bayi pada balita untuk menstimulasi perkembangan. Selain itu, pelatihan pengolahan susu menjadi Yogurt berhasil diterapkan sebagai makanan tambahan pada balita di Posyandu Desa Sukorambi. Pengolahan susu menjadi Yogurt juga memberikan pengetahuan dan perubahan perilaku balita dalam mengonsumsi pangan berprotein tinggi. Kendala yang dihadapi adalah kurangnya kepercayaan diri kader dalam mempraktikkan pijat bayi, sehingga diberikan tutorial video dan konsultasi.

Abstract

Keadaan gizi balita sangat penting dalam menilai kualitas kesehatan komunitas masyarakat, terutama di wilayah pedesaan yang mengalami hambatan keterbatasan layanan kesehatan dan informasi. Faktor pengasuhan dan praktik ASI eksklusif diduga memiliki keterkaitan penting terhadap status gizi balita, khususnya indikator IMT/U. Penelitian ini bertujuan untuk melihat bagaimana hubungan riwayat ASI eksklusif dan pengasuh balita dengan (IMT/U) sebagai cerminan status gizi pada balita yang tinggal di wilayah pedesaan. Penelitian ini dirancang untuk menganalisis studi korelasional dan pendekatan cross-sectional. Teknik sammpling menggunakan Random sampling untuk memilih responden dari balita dan orang tua. Analsisis statistik menggunakan Uji Spearman Rank untuk melihat hubungan antar variabel; variabel independen adalah status gizi yang didasarkan pada indeks IMT/U, dan variabel dependen adalah riwayat pemberian ASI eksklusif dan pengasuh balita. Hasil penelitian menunjukkan adanya hubungan signifikan antara riwayat ASI eksklusif dengan status gizi balita (p = 0,048), di mana balita yang mendapatkan ASI eksklusif lebih banyak berada pada kategori gizi normal. Selain itu, terdapat hubungan signifikan antara pengasuh balita dengan status gizi (p = 0,030). Balita yang diasuh oleh ibu memiliki proporsi terbesar dalam kategori gizi normal dibandingkan dengan balita yang diasuh keluarga lain. Pengsuh balita dan riwayat ASI eksklusif berperan penting dalam menentukan status gizi balita di daerah pedesaan. Intervensi kesehatan masyarakat perlu memperkuat edukasi ASI eksklusif serta meningkatkan kompetensi pengasuh dalam mendukung tumbuh kembang dan pemenuhan nutrisi pada balita.
