Abstract

The advanced dose of COVID-19 vaccination aims to maintain the level of immunity and extend the period of protection against SARS-CoV-2 virus infection, given especially to vulnerable groups of people, one of which is the elderly. Currently, the number of elderly people receiving COVID-19 vaccination is still inadequate, partly due to public misinformation regarding the side effects of post-vaccination in the elderly. It has been known that vaccination in the elderly must be carried out with extra care because of the frailty of the elderly.  Therefore, to ensure that vaccination for the elderly is effective and optimal, the feasibility of receiving a COVID-19 vaccination needs to be determined through screening for frailty syndromes. The advanced-dose COVID-19 Vaccination Center at Tarumanagara University, in partnership with the Grogol Petamburan District Health Center, West Jakarta, was held on February 2-4, 2022. Besides the general criteria for vaccination, which included history of diseases and the results of measuring body temperature and blood pressure, the medical team from the Faculty of Medicine Tarumanagara University applied the criteria for frailty syndrome to the elderly, namely difficulty climbing 10 stairs or walking 100-200 meters, often feeling tired, have at least 5 of 11 chronic diseases and experienced significant weight loss in the past year. Elderly was not eligible to receive COVID-19 vaccine if there were >2 conditions of vulnerability were found.  Of the 45 elderly who took part in the activity, none had a frailty condition >2, so a further dose of COVID-19 vaccination could be given. ABSTRAK: Vaksinasi COVID-19 dosis lanjutan bertujuan untuk mempertahankan tingkat kekebalan serta memperpanjang masa perlindungan terhadap infeksi virus SARS-CoV-2, diberikan terutama pada kelompok masyarakat rentan, salah satunya adalah lansia. Saat ini jumlah lansia yang menerima vaksinasi COVID-19 belum memadai, diantaranya disebabkan media informasi publik mengenai efek samping pasca vaksinasi pada lansia.  Telah diketahui bahwa vaksinasi pada lansia harus dilakukan dengan ekstra hati-hati karena adanya kondisi kerentaan (frailty) pada lansia.  Sebab itu, untuk memastikan pemberian vaksinasi pada lansia efektif dan optimal, kelayakan menerima vaksinasi COVID-19 perlu ditentukan melalui penapisan sindrom kerentaan.  Sentra Vaksinasi   COVID-19 dosis lanjutan di Universitas Tarumanagara, bermitra dengan Puskesmas Kecamatan Grogol Petamburan Jakarta Barat, diselenggarakan pada tanggal 2-4 Februari 2022. Selain kriteria umum layak vaksinasi yang mencakup hasil pengukuran suhu tubuh dan tekanan darah serta riwayat penyakit, tim medis dari Fakultas Kedokteran Universitas Tarumanagara menerapkan kriteria sindrom kerentaan pada sasaran lansia, yaitu kesulitan naik 10 anak tangga, kesulitan berjalan 100-200 meter, sering merasa kelelahan, memiliki minimal 5 dari 11 penyakit kronik dan mengalami penurunan berat badan yang bermakna dalam setahun terakhir.  Jika didapatkan >2 kondisi kerentaan, maka lansia belum layak divaksinasi COVID-19. Dari 45 lansia yang menghadiri kegiatan tersebut, tidak didapatkan lansia dengan kondisi renta >2 sehingga dinyatakan layak diberikan vaksinasi COVID-19 dosis lanjutan.

Abstract

Tuberculosis is a serious health problem and is one of the 10 leading causes of death globally. Tuberculosis can be influenced by medical and non-medical risk factors. Medical risk factors that play a role are patients with HIV positive or other immunocompromised diseases such as diabetes mellitus. Therefore, this activity aims to conduct blood sugar screening so that risk factors for tuberculosis can be identified. The method used is by taking blood to determine glucose levels and counseling to increase respondent knowledge. The results obtained were that all respondents had undergone GDS (Random Blood Sugar) examinations and there was an increase in respondent knowledge after counseling. It is hoped that this activity can take place routinely so that risk factors for tuberculosis can be reduced

Abstract

Tuberculosis (TB) is an infectious disease caused by the bacteria Mycobacterium tuberculosis. Indonesia is ranked 2nd in the country with the highest burden of pulmonary TB. In February 2024, the Ministry of Health recorded that TB cases in Indonesia reached 1,060,000 cases and became the highest number ever before. In April 2024 until now, TB ranked 2nd out of 10 most visits to the Health Center, and 95 new cases of pulmonary TB were found throughout January - June 2024. The purpose of this activity is to increase public understanding of TB, risk factors and prevention of TB, cough etiquette, hand washing steps, and proper use of masks in the Health Center work area. Based on the results of initial observations, lifestyle factors play a role in the high number of TB. The results of the intervention showed that 22 participants (88%) obtained a post-test score. ≥70 points and there was an increase in the average pre-test and post-test scores of 52.9%, and 3 random participants were able to practice all three demonstrations correctly. It can be concluded that there is an increase in knowledge about TB, risk factors and prevention of TB, cough etiquette, hand washing steps, and proper use of masks so that it is expected to reduce TB cases in the work area of ​​the Health Center.

Abstract

Hypertension is an increase in blood pressure above normal values ​​due to various factors. Normal hypertension values ​​are in the range of 90/60 mmHg to 120/80 mmHg. Systolic pressure values ​​of more than 139 mmHg and diastolic pressure of more than 89 mmHg are categorized as hypertension. With increasing prevalence, hypertension is becoming a global health problem. In the world, an estimated 1.13 billion people aged 30 to 70 years suffer from hypertension, and this disease is a major risk factor for cardiovascular disease and stroke. In Indonesia, the prevalence of hypertension is estimated to reach more than 40% in the adult population in 2019 (WHO, 2019). The purpose of this activity is to prevent an increase in the prevalence of hypertension. The method used is to provide counseling and screening for hypertension risk factors. The results obtained are blood pressure data, anthropometry and hypertension exercises. It can be concluded that this activity was successful because there was an increase in the pre-test to post-test value of 25.21 points so that this activity can be sustainable to prevent hypertension patients.

Abstract

Stunting is a condition where a toddler has a length or height that is less than their age. This is a chronic nutritional problem experienced by toddlers, especially in the first 1000 HPK (First Days of Life). WHO (World Health Organization) data in 2020 shows that 22.2% or 149.2 million toddlers worldwide suffer from stunting. Based on data from the SKI (Indonesian Health Survey) in 2023, the prevalence of stunting in Indonesia is 21.6%. Based on data from the Health Center as of July 2024, there were 51 cases of stunting. In the same period in the previous year, there were 44 cases of stunting at the end of July 2023. The purpose of this activity is to increase knowledge about compiling a balanced nutritional menu at the Teratai Posyandu. Counseling as a method for explaining the preparation of a balanced nutritional menu. The results obtained were that all participants were able to work together in designing a balanced nutritional menu. It can be concluded that this activity was successful because participants were able to repeat the explanation given and this activity could be carried out periodically so that there was a decrease in stunting cases in the health center's work area.

Abstract

Tuberculosis (TB) is an infectious disease caused by infection with bacteria from the Mycobacterium tuberculosis complex group. TB prevention efforts are crucial, especially for individuals at high risk of exposure, to disrupt the chain of transmission of this disease. The intervention aimed to increase community understanding of TOSS TB (Find, Treat, Treat Until Cured) so that it can be applied in daily life. It is expected that after the intervention, participants' knowledge will increase, as will their ability to demonstrate the practices taught appropriately. Data collection was conducted through a mini-survey of the community in the local Community Health Center (Puskesmas) working area. The data obtained were then analyzed using the Blum Paradigm to identify factors contributing to the problem. Problem prioritization was carried out using the non-scoring Delphi method, while root cause analysis was conducted using a fishbone diagram. During the intervention process, pre- and post-tests were conducted to evaluate changes in the level of knowledge of villagers before and after the intervention. For program monitoring and evaluation, the PDCA (Plan-Do-Check-Act) cycle approach and a systems approach were used. The analysis using the Blum Paradigm showed that lifestyle was the primary contributing factor to the problem. After the intervention, three participants were able to repeat the material effectively, demonstrating a better understanding of TB prevention. Based on these results, it can be concluded that the intervention was successful and is expected to encourage improvements in public health in the village. Furthermore, this program is also expected to serve as a model for other public health interventions, thereby expanding the positive impact of TB control efforts in other areas. Thus, public awareness and knowledge about TB can continue to increase, which in turn will contribute to reducing TB incidence in Indonesia and improving the overall quality of life of the community.

Abstract

Latar Belakang: Vitamin D adalah vitamin larut lemak yang mengatur berbagai proses biologis, termasuk metabolisme glukosa dan profil lipid, yang penting untuk kesehatan metabolisme. Tujuan: Penelitian ini bertujuan untuk menilai hubungan antara defisiensi vitamin D dan dampaknya terhadap profil lipid serta kontrol glikemik pada populasi lanjut usia. Metode: Studi potong lintang ini melibatkan 78 lansia, dengan data yang diperoleh dari pengukuran HbA1c, kolesterol total, trigliserida, dan serum vitamin D. Analisis korelasi Spearman digunakan untuk menilai hubungan antara variabel-variabel tersebut. Hasil dan Pembahasan: Hasil penelitian menunjukkan bahwa terdapat korelasi positif signifikan antara HbA1c dengan kolesterol total dan trigliserida, serta korelasi negatif signifikan antara HbA1c dan vitamin D. Korelasi antara kolesterol total dengan trigliserida dan vitamin D tidak menunjukkan signifikansi. Kesimpulan: Temuan ini menunjukkan bahwa vitamin D memiliki peran penting dalam manajemen profil lipid dan kontrol glikemik pada lansia diabetes tipe 2. Penelitian lebih lanjut diperlukan untuk memahami manfaat jangka panjang dari suplementasi vitamin D dalam populasi lanjut usia.

Abstract

Tuberculosis (TB) is an infectious disease caused by the bacterium Mycobacterium tuberculosis. It is transmitted through the air, specifically through droplets expelled by sufferers when coughing or sneezing. TB is divided into two main types: pulmonary TB, which attacks the lungs, and extrapulmonary TB, which can affect other organs such as the pleura, lymph nodes, skin, bones, and meninges. In the Community Health Center (Puskesmas) work area, 77 new cases of pulmonary TB were recorded from January to April 2025. Meanwhile, coverage of Tuberculosis Prevention Therapy (TPT) only reached 44.7%, still below the national target of at least 50%. This condition indicates the need for increased health promotion activities and community diagnostic approaches as promotive and preventive measures, in order to increase TPT coverage and reduce the incidence of TB. The results of the respondent questionnaire showed that the level of public knowledge about TB reached 64.6%, positive attitudes at 94%, and preventive behavior at 72%. Based on these findings, it is recommended that outreach activities be conducted regularly to improve public understanding, which can ultimately encourage behavioral changes and reduce TB cases.

Abstract

Insomnia is the most common sleep disorder experienced by the elderly, which has a significant negative effect on morbidity and mortality in the elderly group. A study from the Penn State Adult Cohort found that people who are sleep deprived with objectively short sleep duration have a higher risk of developing hypertension. This cross-sectional study aims to determine differences in systolic and diastolic blood pressure in the elderly group (³60 years) with insomnia which were selected consecutively at the Tarumanagara University COVID-19 Vaccine Center for the period March – April 2021. The Insomnia Severity Index (ISI) questionnaire was used to evaluate the severity of insomnia. Blood pressure is measured with a digital tensimeter according to standard procedures. Statistical analysis using the Mann Whitney test. Of the 119 respondents, the average age was 69.84 years and 58% of the respondents were women. The mean systolic blood pressure was 150.18 and the average diastolic blood pressure was 83.23. From all respondents, it was found that 17.6% of respondents experienced insomnia. The results of the Mann Whitney statistical test explained that there was no difference in the mean systolic and diastolic blood pressure in the group with insomnia and without insomnia (p-value > 0.05). Further clinical studies revealed that the group of elderly who had insomnia tended to have higher blood pressure than the group of elderly who did not experience insomnia (SBP: 160 vs 151; DBP: 85 vs 84). Dealing with insomnia can be an effort to lower blood pressure.

Abstract

Banyaknya kejadian dermatitis di Puskemas Legok, Kabupaten Tangerang selama periode Januari 2022 – Februari 2023 sebesar 1145 kasus menjadi pilihan untuk dilakukan penyuluhan di Puskesmas Legok guna meningkatkan upaya promotif dan preventif sehingga diharapkan meningkatkan pengetahuan dan kesadaran masyarakat akan penyakit dermatitis. Berdasarkan hasil diskusi, perlu dilakukannya beberapa alternatif pemecahan masalah mengenai peningkatan kunjungan kasus dermatitis di Puskesmas Legok diantaranya: Melakukan penyuluhan mengenai dermatitis mulai dari definisi, penyebab, gejala, pencegahan, dan terapi. “Ayo Tolak Dermatitis” merupakan suatu slogan untuk Tahu, Obati, dan Laksanakan segala sesuatu tentang Dermatitis. Penyuluhan dilakukan kepada 30 orang. Hasil pre test didapatkan sebanyak 3 peserta (10%) yang mendapatkan nilai pre-test > 70 dan setelah dilakukan penyuluhan didapatkan 27 peserta (90%) yang mendapatkan nilai post-test > 70. Dari hasil tersebut penyuluhan dikatakan berhasil, sehingga diharapkan perilaku dari masyarakat dapat berubah sehingga penyakit dermatitis bisa menurun.
