Abstract

Background: Obesity cause various physiological changes in the body, one of which is insulin resistance causes high blood glucose levels. Chewing is a stimulus of cephalic phase responses and sensory stimulation that can increase hormones releasing such as insulin, ghrelin, cholecystokinin (CCK) and glucagon like peptide-1 (GLP-1). Chewing plays important role in determining postprandial plasma glucose concentration.Objective: Investigate the effect of chewing on postprandial blood glucose in obese adults.Method: This was true experimental research. Research subjects were treated in the form of chewing 22 times and 40 times each mouthful. Blood glucose levels were measured using glucometer on fasting blood glucose and postprandial blood glucose 15 minutes, 30 minutes, 60 minutes, and 120 minutes. Statistical test using Independent t-test.Results: The mean postprandial glucose levels in the 22 chews group at 15 minutes, 30 minutes, 60 minutes, and 120 minutes were 112.11 ± 14.3328, 126.11 ± 15.667, 116.94 ± 15.539, and 89.67 ± 11.668 . While the mean postprandial blood glucose levels in the 40 chews group at 15 minutes, 30 minutes, 60 minutes, and 120 minutes were 122.22 ± 14.381, 129.61 ± 15.112, 109.50 ± 14.995, and 85.83 ± 13.963. There were statistically significant differences between chewing groups 22 times and chewing 40 times on fasting blood glucose and 15 minutes postprandial blood glucose (p = 0.041 and p = 0.042), while on 30 minutes postprandial glucose testing, 60 minutes , and 120 minutes there was no significant difference (p> 0.05).Conclusion: There was significant differences in 15 minutes postprandial blood glucose level between group 22 times chewing and 40 times chewing each mouthful.

Abstract

BACKGROUND : Type 2 diabetes mellitus patients have risk to suffer from dry eyes syndrome (DES), caused by peripheral and autonomic neuropathy, oxidative stress, and metabolic dysfunction. DES can reduce the patient’s quality of life then it could induce blindness if the patient didn’t get any therapy for DES. It is important to prevent DES by controlling its risk factors. OBJECTIVE : To investigate risk factors of DES in patients with type 2 diabetes mellitus. METHOD : . This study was an analytical observational study with cross-sectional design and purposive sampling. Subjects were examined by using questionnaire and slit lamp, then followed by deep interview in some subjects. This study was also using subject’s medical record to complete the required data. Subjects were diagnosed to suffer DES if OSDI score >12 and schirmer score <10mm. Data was analyzed by using statistic software. RESULT : . From 42 subjects, 19 subjects (45,2%) suffered from DES whereas another 23 subjects (54,8%) didn’t. There was significant relation between diabetic retinopathy (p=0.009) and DES in patients with type 2 diabetes mellitus showed by p value under 0,05 (OR 5.700, 95% CI:1.364-23.821). However, age (p=0.516), gender (p=1.000), level of knowledge (p=0.555), accessibility to health provider (p=1.000), occupation (p=0.644), economic level (p=0.105), smoke exposure (p=0.432), duration of diabetic (p=0.707), diabetic controlling (p=0.305), and history of dyslipidemia (p=0.155) had no significant relations with DES in patients with type 2 diabetes mellitus. CONCLUSION : There is a significant relation between diabetic retinopathy and DES in patients with type 2 diabetes mellitus. Diabetic retinopathy is the most dominant risk factor among the other factors in this study.

Abstract

BACKGROUND: More than 75% of people with type 2 DM will anguish diabetic retinopathy 20 years after being diagnosed. However, the current changing of people’s lifestyle contributes to the tendency in type 2 DM that diagnosed at a younger age. This causes diabetic retinopathy can be occurred earlier and blindness will occur more quickly. It is important to prevent diabetic retinopathy by controlling some associated factors. OBJECTIVE: To investigate associated factors of diabetic retinopathy in people with type 2 DM. METHOD: Analytic observational study with cross-sectional design. Subjects were determined by purposive sampling. The subjects were interviewed using a questionnaire meanwhile, funduscopic examination is aimed to assess diabetic retinopathy and followed by deep interview. This study also used the subject’s medical record to complete the required data. The results were analyzed by using statistic software. RESULT: From 43 subjects, there wes no significant relation among age (p=0.965), gender (p=0.437), level of knowledge (p=0.640), accessibility (p=1.000), economic level (p=0.680), duration of DM (p=0.477), history of hypertension (p=0.708), and history of dyslipidemia (p=0.244) with diabetic retinopathy in people with type 2 DM. However, there was significant relation among controlling blood glucose (p=0.014) with diabetic retinopathy in people with type 2 DM. Uncontrolled blood glucose was 13 times more affected in diabetic retinopathy than controlled blood glucose (OR 13.417, 95% CI:1.474-122.117). CONCLUSION: There was a significant relation between controlling blood glucose and diabetic retinopathy in people with type 2 DM. Controlling blood glucose is the most dominant risk factor among the other factors in this study. Keywords: diabetic retinopathy; risk factor; type 2 diabetes mellitus

Abstract

Background: The COVID -19 pandemic that has occurred since 2020 has changed and affected all aspects of life. In health services, the pandemic has limited patients from coming to health facilities. Recording of ophthalmology cases in health facilities has an important role in providing information about eye health problems that often occur in the community to identify what are the main challenges faced in the field of eye care.Objective: This study aims to identify the characteristics of eye diseases at the Merpati Eye Polyclinic Dr Kariadi Hospital Semarang in 2020-2022.Methods: This study was a retrospective descriptive study. The clinical characteristic data (gender, age, diagnosis, and Clinic) are retrieved from the medical records of patients suffering from eye diseases who underwent treatment at the Merpati Eye Clinic, Dr. Kariadi Hospital Semarang and was diagnosed in 2020-2022. The data obtained were processed and analyzed using the google sheets program.Results: There were 2961 newly diagnosed patients with the 10 most diagnosed diseases are Refractive disorders (12,87%), Cataracts (11,21%), Glaucoma (10,40%), Diabetic Retinopathy (6,75%), Pseudophakia (5,30%), Papile Atrophy (5,10%), Retinal Vein Occlusion (4,22%), Corneal Ulcer (3,92%), Retinal Detachment (3,71%) and Uveitis (3,51%). Based on age category, the most patients were 51-60 years old (22,29%). Based on gender category, female gender (50,35%) is more than male (49,65%).Conclusion: Of all patients, the 10 most diagnosed diseases were Refractive Disorders, Cataracts, Glaucoma, Diabetic Retinopathy, Pseudophakia, Papillary Atrophy, Retinal Vein Occlusion, Corneal Ulcers, Retinal detachment and Uveitis.

Abstract

Background: Elevated intraocular pressure (IOP) induces inflammation in the trabecular meshwork (TM) through increases in tumor necrosis factor-alpha (TNF-α) expressions by activating nuclear factor kappa B (NF-κB). Mechanical injury to the TM, such as increased IOP, can cause extracellular matrix (ECM) remodeling, impacting the permanent outflow of aqueous humor. Moringa oleifera (MO) leaf extract is an anti- inflammatory agent that inhibits NF-κB activation and has been studied to have antioxidant effects.Objective: to analyze the effects of MO leaf extract on TNF- α expression and the thickness of the trabecular meshwork ECM in a Wistar rat glaucoma model. Methods: Fourteen male Wistar rats were divided into two groups (n=7): the glaucoma group received oral MO leaf extract at 300 mg/kg body weight for four weeks, and the control group. TNF-α expressions were measured by immunohistochemical stain, and the thickness of the ECM was examined with hematoxylin eosin staining. P-values of less than 0.05 were considered to represent statistical significance.Results: The mean of TNF-α expressions in the treatment group was 4.29 ± 0.76, and the control group was 5.29 ± 0.49. There was a significant difference in TNF-α expressions between the two groups (p=0.015). The mean thickness of the ECM TM experimental group and control group were 155.65 ± 28.72μm and 218.63 ± 40.85μm respectively. The extracellular matrix TM experimental group was statistically significantly thinner than the control group (p=0.006).Conclusion: MO could protect TM from damage caused by high IOP by suppressed TNF- α expression and ECM thickness in the TM Wistar glaucoma model.

Abstract

Objective: Age-related macular degeneration (AMD) is a common ocular disease that affects millions of people worldwide. Cataract surgery is one of the most performed surgical procedures in the world and is effective to improve visual function and quality of life (QOL) in individuals with cataracts. Both cataract and AMD are conditions that occur in late age. However, the presence of AMD may complicate the postoperative outcomes and quality of life (QOL) of these patients. This review aims to examine the impact of cataract surgery on the QOL of patients with AMD. Methods: A search of the literature was conducted using electronic databases. Studies were included if they evaluated QOL outcomes in patients with AMD who underwent cataract surgery. Result: The results of the review suggest that cataract surgery can improve visual acuity and QOL in patients with AMD. However, the degree of improvement is influenced by the severity of AMD. Patients with mild to moderate AMD tended to have greater improvements in QOL and visual acuity compared to those with advanced AMD. Incidentally, it is still cost effective to perform cataract surgery in patients with advanced AMD. Conclusion: Overall, cataract surgery is found to be able to improve QOL and visual acuity in patients with AMD. Nevertheless, individual characteristics including severity of AMD must be considered to optimize the outcomes. Further research, particularly with longer follow up times, is needed to determine the long-term effects of cataract surgery on QOL in patients with AMD.

Myopia is one of the most common visual disturbances in the world. It is believed that the use of spectacle can reduce the progressivity rate of myopia. Spectacle can help light fall on the retina and prevent axial elongation of the eye. This theory needs to be investigated further so that it can be used as a reference. This study aimed to know the correlation between compliance of spectacle wear and myopia progressivity rate in medical students of Diponegoro University (UNDIP). Methods This research was an observational analytical study with cross-sectional design on 54 subjects ranging from 18 to 22 years old. Data are primary data obtained from questionnaires and objective eye examinations using an autorefractometer. The research was conducted from October to November 2021. Data were analyzed using Spearman Rank test which p<0,05 was considered significant. Results The mean of compliance of spectacle wear and myopia progressivity rate in medical students of UNDIP are (1,57±0,108) and (0,259±0,515) dioptres respectively. The correlation between compliance of spectacle wear and myopia progressivity rate are p=0,082 and r=0,239. Conclusion There are no correlation between compliance of spectacle wear and myopia progressivity rate in medical students of UNDIP. Abstract

Introduction & ObjectivesMacular hole (MH) is a vitreomacular interface disorder that could cause visual impairment. Clinicalcharacteristics of MH are important for predicting MH treatment outcomes. This study purpose wasto describe MH patients’ characteristics and postoperative outcomes in Dr. Kariadi Hospital. MethodsA retrospective study held on January 2021–December 2022 utilizing patients’ medical records.Preoperative data: sex, age, symptoms, duration of symptoms, origin, preoperative visual acuity(VA), modified Gass classification stage, macular hole index (MHI), and hole forming factor (HFF)were obtained. Postoperative outcomes data: postoperative VA and anatomical closure were alsocollected. Results20(20 eyes) patients were obtained, with 14(70%) female, 12(60%) aged >60 years old, 13(65%)metamorphopsia, 12(60%) central visual loss, 10(50%) preoperative VA 6/24-6/60, 10(50%) durationof symptoms <3 months, and 16(80%) idiopathic origin. OCT (Ocular Coherence Tomography)findings revealed base diameter mean was 780,7 um(+ SD 312). 13 patients (65%) stage 3 of MH,12(60%) MHI <0,5, 12(60%) HFF > 0,9. All patients underwent MH surgery procedures.Postoperative examination showed 10 patients(50%) with VA 6/6-6/20. 11(100%) with preoperativeVA>6/60, 10(100%) with duration of symptoms <3 months, 8(100%) with MHI > 0,5, and 11(100%)with HFF > 0,9 had improved visual acuity and anatomical closure. It also revealed 17(85%) hadanatomical closure. ConclusionThe most common characteristics found in MH patients were female sex, age >60 years old, anddisorder of idiopathic origin. Preoperative VA, duration of symptoms <3 months before surgery, MHI> 0,5, and HFF > 0,9 were positive prognostic factors for better postoperative functional andanatomical outcomes.

Abstract

Introduction : Malignant glaucoma most commonly occurs after glaucoma filtration surgery. Most cases require surgical intervention such as vitrectomy. However, even after successful treatment, it can reoccur in some cases. We present two cases of recurrent malignant glaucoma. Case Illustration : The first case involved a 48-year-old female with primary angle closure glaucoma who developed malignant glaucoma 1-week after undergoing combined glaucoma surgery. The second case involved a 70-year-old man with primary angle closure glaucoma and pseudophakia who developed malignant glaucoma 4-month after trabeculectomy. They received total vitrectomy treatment. We observed anterior chamber deepening in these 2 cases, but intraocular pressure remained increased 2-week after intervention. Discussion : We found improvement in anatomy by deepening the anterior chamber, but the intraocular pressure increased in the 2 weeks. Although the problem of releasing aqueous accumulations from the vitreous has been resolved after vitrectomy, but there is no permanent passage between the anterior chamber and the vitreous cavity. So, recurrences can occur. It is possible to manage this condition by restoring aqueous flow from the anterior vitreous to the anterior chamber. YAG laser capsulo- hyaloidotomy or vitrectomy combined with hyaloido-zonula-iridectomy are frequently effective in treating underlying causes. Conclusion : Vitrectomy can help to improve the deepening anterior chamber, but intraocular pressure may remain increased after 2-week follow-up. The recurrence of malignant glaucoma may be related to incomplete resolution of the underlying condition.
