Abstract

Type 2 diabetes mellitus (T2DM) is associated with a risk of microvascular complications, including diabetic nephropathy. Glycemic status is determined through glycated hemoglobin (HbA1C) measurements, where levels below 7% classify diabetes as controlled, and levels of 7% or higher indicate uncontrolled diabetes. Measuring blood urea nitrogen (BUN) and creatinine is essential for determining kidney function. This study aims to analyze the differences in BUN and creatinine levels between controlled and uncontrolled T2DM patients. A cross-sectional study was conducted using secondary data from the medical records of outpatient T2DM patients at the Internal Medicine Clinic of Pasar Rebo General Hospital, East Jakarta, from January to December 2024. Subjects were divided into controlled and uncontrolled diabetes groups based on HbA1C values, with inclusion criteria consisting of patients aged 50–59 years who had been diagnosed with T2DM within the past five years. BUN data were obtained by calculating urea x 2.14 (mg/dL). Normality was tested using the Kolmogorov-Smirnov test. Comparative analysis of BUN and creatinine levels between the two groups was performed, with p<0.05 statistically significant. Among the 100 T2DM subjects evaluated at Pasar Rebo General Hospital, 64% were female, and the average age was 56 years. The mean BUN and creatinine levels in the controlled group were 13.5 and 1 mg/dL, while in the uncontrolled group were 13.59 and 1.04 mg/dL. Analysis using an independent comparison test showed that BUN and creatinine levels were not significantly different in the controlled group (p = 0.93) nor in the uncontrolled group (p = 0.38). The research revealed that there was no meaningful variation in BUN and creatinine levels between T2DM patients with controlled and uncontrolled glycemic status at Pasar Rebo General Hospital, East Jakarta.

Abstract

Hypertension in the elderly often manifests as Isolated Systolic Hypertension, a condition characterized by a surge in systolic pressure while diastolic pressure conversely decreases. According to statistics from the Koto Baru Public Health Center in Dharmasraya Regency in 2025, there were 672 elderly residents diagnosed with this condition. Uncontrolled blood pressure in the elderly carries a high risk of triggering cardiac rhythm disturbances. This study aims to determine the relationship between hypertension categories and cardiac rhythm disturbances among the elderly in Nagari Koto Baru. This research is an analytical observational study with a cross-sectional approach. The study population consisted of 212 elderly individuals, with a purposive sampling technique resulting in a sample of 68 respondents. Bivariate data analysis was conducted using the Chi-Square test. The findings indicated that most of the elderly experienced moderate hypertension, totaling 34 individuals (50%), and 37 elderly participants (54.4%) experienced tachycardia heart rhythms. Based on the Chi-Square statistical test, a significant relationship was found between hypertension categories and heart rhythm in the elderly, with a p-value of 0.000 (< 0.05). There is a significant correlation between the severity of hypertension and the incidence of cardiac rhythm disturbances (tachycardia) in the elderly. Health professionals at the Koto Baru Public Health Center are advised to integrate heart rhythm screenings into the Elderly Integrated Service Post (Posyandu Lansia) program and to enhance education regarding medication adherence to control blood pressure.

Abstract

Type 2 Diabetes Mellitus (T2DM) is a metabolic disease that can lead to complications such as diabetic nephropathy, characterized by a decline in renal filtration function. Glycated hemoglobin (HbA1c) is used to classify patients into controlled (HbA1c <7%) and uncontrolled (HbA1c ≥7%) groups. Kidney function is assessed by measuring the estimated Glomerular Filtration Rate (eGFR). This study aims to determine the differences in eGFR values between controlled and uncontrolled T2DM patients. This cross-sectional study used secondary data from T2DM patients visiting the internal medicine clinic at Pasar Rebo Regional General Hospital, Jakarta, between January and December 2024. The subjects were categorized into controlled and uncontrolled groups based on HbA1c values. Inclusion criteria included patients diagnosed with diabetes in the last 5 years, aged 50-59 years, and having complete baseline data, including HbA1c and serum creatinine values. eGFR was estimated using the 2021 CKD-EPI formula. A normality test was performed on eGFR values between the two subgroups, with p>0.05 indicating a significant difference. The study of 100 T2DM subjects at Pasar Rebo Regional General Hospital found that the average eGFR value in the controlled group was 73.50, while in the uncontrolled group it was 56.92 mg/dL. An independent sample T-test showed a significant difference in eGFR values between the controlled and uncontrolled groups (p<0.001). Conclusion: There was a significant difference in eGFR values between controlled and uncontrolled T2DM patients at Pasar Rebo Regional General Hospital, Jakarta.
