Abstract

Background: COPD exacerbations is an acute condition with worsening symptoms and require more intensive treatment changes. Increased inflammatory exacerbations marked increase in plasma levels of IL-8, exacerbation of clinical symptoms and risk of hospitalization. Ginseng extracts have anti inflammatory so it can be used as an additional drug in COPD exacerbations. Method: This study aimed to analyze the effect of ginseng extract on plasma levels of IL-8, CAT scores, and length of hospitalization of patients with COPD exacerbations. Clinical trials of experimental with pre test and post test design was conducted on 34 patients with COPD exacerbation in Dr. Moewardi Hospital Surakarta and Dr. Ario Wirawan Lung Hospital Salatiga from August until September 2016. Samples were taken by consecutive sampling were divided into two groups. The treatment group (n = 17) received standard therapy and ginseng extracts 2x100mg/day and the control group received only standard therapy (n = 17). Clinical improvement measured in CAT score and length of stay. Plasma levels of IL-8, CAT scores, and longer hospitalization time measured at entry and discharge criteria are met. Length of stay based on the number of days of care in hospitals. Results: There were significant difference (p=0.000) towards decreased of of IL-8 plasma level between treatment group (-12.38 ± 10,35pg/ml) and control group (0.65 ± 7,92pg/ml), towards decrease of CAT score (p=0.000) between treatment group (-16.53 ± 1.28) and control group (-12.59 ± 2.87), and length of stay (p = 0.004) between treatment group (4.29 ± 1.45) and control group (5.76 ± 1.20). Conclusions: The addition of ginseng extract 2x100 mg/day during hospitalization was significantly lowered plasma levels of IL-8, CAT scores, and shortening the length of stay. Abstract

Introduction: Chronic obstructive pulmonary disease (COPD) is the leading cause of morbidity and mortality worldwide. Cigarette smoke and noxious agent result in oxidative stress and activate release of inflammatory mediators such as Interleukin-8 (IL-8). Quercetin is a flavonoid compound containing anti-inflammatory effects which can be used as an adjuvant therapy in stable COPD. Objective: To analyze the effect of quercetin on serum IL-8 levels, % VEP1, and CAT score of stable COPD patients. Methods: Experimental clinical trial with pre-test and pasca-test design was performed in 30 patients with stable COPD in Dr. Moewardi Surakarta between December 2017 and January 2018. The samples taken by using purposive sampling were divided into two groups treatment groups received standard therapy and quercetin 500mg/day for 28 days and control groups only received standard therapy. The decrease in inflammation was measured by serum IL-8 examination, improvement of obstruction measured by %FEV1 and clinical improvement measured by CAT score. Results: IL-8 serum level was significantly lower in treatment group than of in control group (p=0,001). The percentage of FEV1 was insignificant different between the two group (p=0,236). However CAT score was significantly lower in treatment group compared to that of in control group (p=0,001) Conclusions: Quercetin can decrease IL-8 serum level and decrease CAT score when given in combination with standard therapy for COPD patients. Abstract

Background: Inhaled magnesium sulphate has a bronchodilator and antiinflammatory effect by block the calcium channels and inhibiting substance P. This study aimed to analyze the effect of magnesium sulfate inhalation on bronchodilator response, substance P levels, and clinical improvement on AECOPD patients. Methods: A quasi-experimental clinical trial, pre-test and post-test design with 34 acute exacerbation of COPD patients who are hospitalized in emergency room on Dr. Moewardi Hospital, Surakarta and Ario Wirawan Hospital, Salatiga on March-April 2017 used consecutive sampling. The independent variable is inhaled magnesium sulphate dose of 150 mg 3 times every 20 minutes when the patients was admitted in emergency room, while the dependent variables are peak expiratory flow rate, the plasma levels of substance P and CAT score acute exacerbation of COPD patients. Results: There was a significant difference (P=0.009) decrease of PEFR value of treatment group (111.76±12.37) compared to control group (141.18±24.21). There was a significant difference (P=0.0001) decrease in CAT score of treatment group (-14.88±1.75) compared to control group (-9.00±1.17). There was a significant difference (P=0.0001) treatment group (-1305.92±417.91) than control group (-355.95±206.25). Conclucions: The addition of MgSO4 inhalation of 150 mg during exacerbation increased PEFR, decreased the level of P substance, and decreased the CAT score with statistically significant results. Abstract

Background: Chronic obstructive pulmonary disease (COPD) is characterized by permanent and progressive air way obstruction. In terms of morbidity, this disease is the first highest pulmonary disease in the world. DLCO is used for measurement of gas transfer from alveoli to blood capillary and Hb level is one of the main factors affecting the gas transfer. The aim of this study was to examine prognosis of patients with stable COPD using lung function, DLCO and Hb tets. Methods: Consecutive sampling was used to recruit 33 outward patients who suffered stable COPD and visited the Lung Clinic at Public Hospital dr. Moewardi Surakarta. Abstract

Background: Chronic inflammation in chronic obstructive pulmonary disease (COPD) causes respiratory muscle dysfunction and decreased respiratory muscle capacity. Incongruity of the capacity and the burden of the respiratory muscle results in increased symptoms of breathlessness, decreased inspiratory capacity, exercise capacity, and quality of life. The objectives of the study were to analyze the effect of harmonica exercise as a pulmonary rehabilitation modality on inspiratory capacity, shortness of breath symptoms, exercise capacity, and quality of life on stable COPD patient. Methods: Clinical trials with pre and post test group design were performed on 30 stable COPD patients at the respiratoy clinic at Dr. Moewardi Hospital Surakarta in August - September 2017 taken by purposive sampling. Evaluation of inspiratory capacity (IC) by spirometry, symptoms of breathlessness by mMRC, exercise capacity by 6MWT and quality of life by SGRQ were measured at baseline and after 6 weeks in the harmonic and control exercises group. Abstract

Background: Pneumonia is an infectious disease with highest morbidity and mortality in the world. The research objectives is to determine the effect of Nigella sativa cement extract in procalcitonin, TNF-α levels, and clinical improvement in pneumonia patients. Methods: This study is a clinical trial quasi experimental design with pre and post test approach in the treatment group and the comparison group. Samples were taken by consecutive sampling between April and May 2016 a total of 30 patients were divided into 2 groups:the treatment group and the comparison group. The treatment group was given additional therapy capsules Nigella sativa cement extract, the comparison group was given the standard therapy. Procalcitonin levels and TNF-α is checked when the patient was admitted and when clinical improvement is achieved. Results: Decreased levels of procalcitonin in the treatment group (8.969±13.591 ng/dl) and a comparison group (1.907±6293 ng/dl) was statistically significant (p= 0.014; 95 % CI= 0.222-1.770). Decreased levels of TNF-α was not significantly different between treatment groups (58.759(1.840-166.50) pg/dl) and a comparison group (57.485 (35.410-81.650) pg/dl), with p= 0.395. The achievement of clinical improvement in the treatment group 4.60±8.28 days and the comparison group 5.53 ±1.45 today, statistically no difference between the two groups. Conclusion: Capsule Nigella sativa cement extract provides benefit for declining levels of procalcitonin and accelerate achievement of clinically stable in pneumonia patients. Abstract

Muscular dystrophy terdiri atas beberapa tipe, perbedaan tipe muscular dystrophy menyebabkan hasil uji fungsi paru yang berbeda. Komplikasi pada sistem pernapasan merupakan penyebab utama morbiditas dan mortalitas penderita muscular dystrophy. Pemeriksaan dan analisis fungsi paru perlu dilakukan pada pasien muscular dystrophy pada awal diagnosis dan selama proses perjalanan penyakit. Pemeriksaan tersebut membantu monitoring progresifitas dan perjalanan penyakit, menilai respons terhadap terapi intervensi, dan menentukan prognosis.There are several types of muscular dystrophy with different pulmonary function characteristics. Respiratory complication is a major cause of morbidity and mortality ini muscular dystrophy patients. Lung function test and analysis ought to be performed in patients with the disease as a basic examination for diagnosis and throughout the course of the disease. The test helps to monitor disease progresivity, assess theurapeutic response, and evaluate disease prognosis.

Abstract

Background: Pneumonia is an acute inflammation of the lung parenchyma caused by microorganisms (bacteria, viruses, fungi, parasites) and causes of high morbidity and mortality. Ginsenoside in ginseng have anti-inflammatory effects, potentially as adjunctive therapy in community-acquired pneumonia (CAP) patients. As much as 43% of patients at dr. Moewardi Hospital in 2016 were hospitalized have passed away. The aim of this study was to determine the effect of ginseng extract on levels of procalcitonin, neutrophils, and length of stay of community-acquired pneumonia patients. Method: The quasi-experimental clinical trial with the pretest posttest design approach was conducted on 26 community acquired pneumonia patients who were hospitalized in Dr. Moewardi Hospital Surakarta from October until November 2019 using consecutive sampling. The control group (n=13) received standard antibiotics while the treatment group (n=13) received additional ginseng extract 2x100 mg/day for 14 days. Procalcitonin was measured at admission, third and fourteenth day; neutrophils are measured at admission and third day; length of stay is based on the number of days of treatment in hospital. Results: There was a significant difference (P=0.001) towards decreased of serum procalcitonin levels between treatment group    (-0.63±0.06) compared to control group (-0.38±0.07). There was a significant difference (P<0.001) towards decreased of neutrophil levels between treatment group (-6.68±1.34) compared to control group (-3.51±0.88). There was a significant difference (P=0.001) towards decreased of length of stay between treatment group (5.69±0.95) compared to control group (7.15±0.99). Conclusion: Ginseng extract containing ginsenoside as its main ingredient, can act as an anti-inflammatory which can be used as adjunctive therapy in community-acquired pneumonia patients who are hospitalized to accelerate clinical improvement and shorten the length of stay.

Abstract

Background: Global TB Report 2020 states that 78% of tuberculosis patients experience drug resistancy with a global treatment success rate of 57%. Drug resistance tuberculosis (DR TB) patients whom experience side effects such as arthralgia and hyperuricemia with inadequate treatment will affect the treatment result.Methods: Retrospective cohort study to DR TB patients who underwent treatment from January 2015 to August 2020 at RSUD dr. Moewardi Surakarta. Survival analysis using Kaplan Meier method and Cox regression test for the effect of risk factors on the safety and survival of TB RO patients. Quality of life analysis using Mann Whitney test.Results: From 372 patients, delayed treatment factor (OR=2.906; 95% CI=1.890-4.469; P≤0.001) and arthralgia factor (OR=1.775; 95% CI=1.148-2.744; P=0.010) were variables that had a significant effect on recovery of DR TB patients. Delayed treatment and arthralgia have risk (2,906 times and 1.775 times) for non-recovered DR TB patients. Delayed treatment factor with HR=14.772 (95% CI=13.381-16.163), arthralgia with HR=15.170 (95% CI=13.960-16.379), and anemia with HR=15.304 (95% CI=14.074-16.535; P=0.002) affect on the survival of DR TB patients. Anemia affect on the quality of life DR TB patients.Conclusion: Delayed treatment for more than 14 days and arthralgia that is not treated adequately can affect the recovery of DR TB patients. The survival and quality of life of DR TB patients can be increased by monitoring the time of taking medication, monitoring side effects of drugs such as arthralgia, and adequate nutritional intake so that anemia does not occur.

Abstract

Background: Intensity for aerobic exercise is unclear in patient with COPD. Previous systematic review comparing effects of different levels of training intensity was done in 2011 and did not reach any conclusion. We conducted this systematic review to see the differences in aerobic training intensity on various aspects of health in COPD patients with updated information. Method: We included RCTs, comparing the differences in intensity of aerobic training between groups. The primary search was done through Pubmed, Scopus, Science Direct, Proquest, and PEdro. The PEdro scale and Risk of Bias 2 tool was used to rate the studies. Lastly, we also rate the certainty of evidence using GRADE approach. The assessment was carried out by two reviewers independently. Data were extracted by one reviewer then evaluated by second reviewer. Results: We found and analysed data from four studies with total of 472 patients. The primary outcomes extracted were Disease-specific Health-Related Quality of Life (HRQoL), Activities of Daily Living (ADL), Functional exercise capacity, Dyspnea symptoms. There was a significant difference only in St George’s Respiratory Questionnaire (SGRQ) symptoms domain for HRQoL outcome (MD=5.53; 95% CI=1.08-9.97), favoured lower intensity group. No other significant results were found for any other outcomes/ outcome measures. According to GRADE, quality of the studies was very low to moderate. Conclusion: The evidence we collected is very limited and difficult to evaluate. Further research comparing higher intensity with lower intensity of aerobic training is needed.
