Abstract

Immune checkpoint inhibitors (ICIs), including PD-1, PD-L1, and CTLA-4 inhibitors, have revolutionized cancer therapy but are associated with immune-related adverse events (IRAEs). Among these, ICI-associated cardiotoxicity is an uncommon yet serious complication, often resistant to glucocorticoid therapy, which effectively manages most IRAEs. A pharmacogenomic approach might be useful in prescribing ICIs and screening for relevant clinically measurable phenotypes such as the history of autoimmune diseases and cardiovascular disorders. This review explores the impact of genetic variations on ICI-associated cardiotoxicity, the mechanistic basis behind it, potential clinical applications, and directions of the future on how pharmacogenomics can assist oncologists in reducing the risk of cardiotoxicity. Evidence-based hypotheses on how ICI-associated cardiotoxicity occurs suggest that genetic differences might play a role in ICI response, especially regarding cardiotoxic IRAEs. Pharmacogenomic studies and multi-omics profiling might provide valuable insight regarding ICI-induced cardiotoxicity. They could be implemented to make fine-tuned clinical decisions for individual patients in the future.

Abstract

Background: The shifting from the old paradigm to the new paradigm with the philosophy of pharmaceutical care demands pharmacists to enhance the quality of pharmaceutical services. However, this role has not been optimally fulfilled, especially in the community setting. Objectives: This study aimed to identify the perceptions and expectations, the gap between expectations and reality, and public confidence as well as to analyze the relationship between sociodemographic factors and the perceptions, expectations, and public confidence in Bandung City regarding the role of pharmacists in pharmaceutical services. Methods: A cross-sectional study was conducted using a validated survey instrument with 440 respondents in Bandung, West Java, from March to April 2023. The survey data were analyzed descriptively and through inferential statistics. Results: The analysis showed that 58.41% of the community had a positive perception, 56.82% had high expectations, the average gap between expectations and reality was 14.04%, and more than 51.36% had a high level of public confidence in pharmacists. Sociodemographic factors influencing perceptions were age and the highest level of education (p = 0.005; p = 0.003), factors influencing public confidence were age, the highest level of education, residence address according to ID card (KTP), and occupation (p < 0.001; p = 0.034; p = 0.001; p = 0.001), and the factor influencing expectations was the residence address according to ID card (KTP) (p = 0.043). A correlation was found between the dimensions of perceptions, expectations, and public confidence. Conclusion: Most participants held a positive perception, had high expectations, and shown high confidence in the pharmacist's position, despite the existence of a disparity between expectations and the actual provision of pharmacy services. The elements of perception, expectations, and confidence are interrelated.

Abstract

Tuberculosis (TB) patient compliance with ongoing therapy and knowledge regarding TB has an impact on the cure rate. Despite implementing various strategies, the prevalence of TB in Indonesia remains high, and the success rate of its treatment is still below the global average, potentially leading to continued transmission of TB within the community. This study aims to determine the improvement of knowledge and compliance among pulmonary TB patients following an intervention involving education and the use of a medication reminder application in two community health centers in the city of Bandung. The research design was experimental, where 29 subjects recruited were randomized into two groups: one group receiving additional education and introduction to the mobile application (Jadwal Minum Obat), and the other group receiving only counseling at the health centers. Primary data that was collected include baseline test scores and final test scores of subjects based on the KATUB-Q questionnaire. Patient compliance was assessed using the self-assessment method with the MARS-5 questionnaire and the pill count method. The subjects' experiences in the intervention group while using the mobile application were also documented. A total of 16 subjects completed the study. The education method and medication reminder application used in this study have not significantly improved the knowledge and compliance of pulmonary TB patients in taking their medication (p > 0.05). However, percentage of consumed medication compared to the prescribed medication in the intervention group higher than the control group (87.4 ± 4.8% vs 78.8 ± 8.3%). Subjects found the application useful, though there were still some shortcomings. Further research with a larger sample size using an improved medication reminder application is needed.

Abstract

Abstract

The use of long-acting contraceptive methods (LACMs) in Indonesia remains relatively low despite government efforts to increase their adoption to reduce maternal and child mortality. This study aims to examine differences in knowledge, attitudes, and practices (KAP) between LACM users and non-LACM users, as well as to identify factors influencing contraceptive method selection among women of reproductive age in a rural area of Indonesia. A cross-sectional design was employed involving 394 respondents selected through Cluster Random Sampling, all of whom completed validated and reliable questionnaires. Data were analyzed descriptively. The Mann–Whitney test was used to compare KAP between LACM and non-LACM users. Chi-square tests and logistic regression analysis were conducted to identify factors associated with contraceptive method selection. The results showed significant differences between the two groups. LACM users demonstrated higher levels of knowledge, more positive attitudes, and better contraceptive practices compared with non-LACM users. Factors associated with LACM use included high knowledge (OR = 2.415; p = 0.001), education above primary school level (OR = 7.03; p = 0.003), exposure to print media (OR = 28.331; p = 0.000), and obtaining contraceptives from community health centers (OR = 6.6; p = 0.009). In conclusion, short-acting contraceptive users exhibited lower knowledge, attitudes, and practices than LACM users. Therefore, individuals with low KAP levels should be prioritized in reproductive health education and promotion programs.
