Abstract

ABSTRAK Latar belakang Penelusuran 10 tahun pemeriksan HE pada penderita karsinoma payudara dengan T1N0M0 ditemukan angka rekurensi 33%, karena adanya metastasis yang tidak terdeteksi. Karena itu dipandang perlu dilakukan pemeriksaan dini yang dapat memprediksi potensi metastasis tumor dengan mengunakan faktor prediktif biologik seperti CXCR4 dan VEGF A yang belum dilakukan. Penelitian ini bertujuan untuk mengetahui hubungan antara CXCR4 dan VEGF A pada karsinoma payudara dengan invasi pembuluh darah dan metastasis jauh secara klinis. Metode Penelitian potong lintang dilaksanakan di Bagian/SMF Patologi Anatomik Fakultas Kedokteran Universitas Udayana/RSUP Sanglah Denpasar mulai 20 September 2011 sampai 30 Januari 2012. Karsinoma payudara sebanyak 75 sediaan yang memenuhi kriteria inklusi dan eksklusi dipulas dengan pewarnaan imunohistokimia VEGF A dan CXCR4 lalu dikorelasikan dengan invasi ke pembuluh darah dan metastasis jauh. Penilaian ekspresi CXCR4 dan VEGF A dibuat berdasarkan analisis persentase sel tumor yang positif dan intensitas pewarnaan (Imuno score). Hasil Berdasarkan analisis dengan analisis statistik non parametrik Spearman’s rho dengan SPSS 19.0 for window terlihat hubungan yang bermakna dari beberapa variabel yang dikorelasikan. Penelitian ini menunjukkan ada hubungan yang bermakna antara tingkat ekspresi CXCR4 dengan VEGF A (r=0,572; p=0,000). Ditemukan pula adanya hubungan yang bermakna antara CXCR4 dengan invasi ke pembuluh darah (r=0,538; p=0,000) dan CXCR4 dengan metastasis jauh (r=0,526; p=0,000) serta antara VEGF A dengan invasi ke pembuluh darah (r=0,568; p=0,000) dan VEGF A dengan metastasis jauh berdasarkan klinis (r=0,518; p=0,000). Kesimpulan CXCR4 dan VEGF A dapat dipertimbangkan sebagai petanda biologi untuk memprediksi metastasis pada karsinoma payudara. Methods This study is a cross sectional study, conducted in Anatomical Pathology Department, Faculty of Medicine, University of Udayana, Sanglah general hospital since September 20th 2011 until January 30th 2012. 75 breast carcinoma speciment was steined by CXCR4 and VEGF A and correlated with evidance of vascular invasion and distant metastasis. The expression of CXCR4 and VEGF was scoring using immuno score system (analysis of percentage of positive stained tumor cells and intensity of the staining). Results Statistical analysis was done using non parametric Spearman’s rho with SPSS 19.0 for windows program, found correlation between degree of CXCR4 and VEGF A expression (r=0.572; p=0.000). There was also correlation between degree of CXCR4 expression with vascular invasion (r=0.538; p=0.000) and with distant metastasis (r=0.526; p=0.000). There was correlation between VEGF A expression with vascular invasion (r=0.568; p=0.000) and VEGF A with distant metastasis (r=0.518; p=0.000). Conclusion CXCR4 and VEGF A could be considerated as a predictive factor for metastasis in breast carcinoma. Key words : breast carcinoma, CXCR4, VEGF A, vascular invasion, distant metastasis.

Abstract

In 2021 there were 10 breast cases from the Baturiti sub-district which carried out examinations in the PA laboratory at Tabanan Hospital. 6 of them (60%) turned out to be cases of malignancy, and 4 of them (66.66%) stage III and IV which are the final stages of breast cancer. Perean Kangin Village which is one of the villages in the Baturiti sub-district with the most cases of breast tumors. The long-lasting pandemic has contributed to the decreased activity of cadres and increased delays in breast cancer screening. If the patient can be detected early and get treatment as soon as possible, it will give hope for recovery and a better life expectancy. One of the early detection of breast cancer is breast self-examination (BSE). It is hoped that re-activating midwives and village cadres will motivate the community to do BSE. The result of this PKM was an increase in the knowledge and attitude of cadres towards BSE. We also provide investment goods in the form of digital baby and adult weight measuring devices, stunting mats and height measuring devices for early detection of health problems.

Abstract

Cervical cancer is one of the most common types of cancer suffered by women in developing countries, including Indonesia. This condition occurs due to lack of knowledge about prevention and detecting early symptoms of cervical cancer, especially in areas far from public health service centers. The purpose of this community partnership program is to help government programs to reduce the incidence of cervical cancer through counseling and interactive dialogue to increase the knowledge of partners and their families about cervical cancer and pap smears as screening for cervical cancer, which was carried out in the village of Gali Ukir Pupuan Tabanan, then carried out assisting partner family members in providing information about cervical cancer and pap smears to the surrounding community, as well as visits to monitor family members who are willing to carry out pap smears, and providing assistance in the form of pap smear examinations, giving antiseptics for the vagina, pocket books that can be used as guidelines for giving information about cervical cancer and its prevention, as well as providing a blender that can be used to make anti-septic from nearby plants and to make nutritious drinks to increase immunity. The results obtained are in the form of increased knowledge and changes in thinking patterns about cervical cancer and there are family members who do pap smear examinations.

Abstract

Background: Several studies have proven the relationship between PD-L1, which is known as an immunotherapy target, and poor prognosis. The presence of a tumor budding (TB) group tumor outside the main tumor is also associated with a poor prognosis. The purpose of this study was to determine the relationship between PD-L1 and the grade of TB in cervical carcinoma. Methods: The design of this research is a case-control study with a retrospective approach. The sample is from a paraffin block of the primary cervical cancer tumor from the results of surgery in Balimed Denpasar Hospital between 2020 and 2023. From the statistical formula for the case-control study, a minimum sample of 15 is obtained for the case group with the criteria of high-grade TB, while 15 people were in the control group with low-grade TB. Immunohistochemistry (IHC) PD-L1 is positive if the membrane or cytoplasm is brown, and is negative if there is no brown color on the membrane or cytoplasm. Inferential statistics were used to evaluate the relationship between PD-L1 overexpression and low- and high-grade TB in cervical carcinoma using a chi-square test. Results: The results of this research revealed (p = 0.273) that there was no correlation between PD-L1 expression and high- and low-grade TB. Overexpression of PD-L1 was associated with a 2.250-fold increased risk of high-grade TB (OR = 2.250 with a 95% confidence interval of 0.103–1.915). Conclusions: The severity of TB does not correlate with PD-L1 expression. The functions of PD-L1 and other variables in the carcinogenesis of TB development are the cause of this weak association.

Abstract

Kanker serviks merupakan jenis kanker tertinggi ke-2 di Bali dan Indonesia, dengan infeksi virus HPV high risk sebagai faktor risiko utama. Meskipun vaksinasi HPV efektif menurunkan insiden kanker serviks, implementasinya di Indonesia, khususnya Bali masih baru dimulai dan tingkat pengetahuan remaja tentang kanker serviks masih rendah. Kegiatan pengabdian kepada masyarakat (PKM) ini bertujuan untuk meningkatkan pengetahuan, sikap, dan perilaku siswi SMP, khususnya pengurus OSIS terkait pencegahan kanker serviks dan vaksinasi HPV, serta mendorong peran aktif OSIS sebagai agen edukasi di lingkungan sekolah. Program dilakukan melalui penyuluhan aktif, pelatihan, dan pendampingan kepada pengurus OSIS di tiga SMP di Bali. Evaluasi dilakukan menggunakan pre-test dan post-test untuk mengukur perubahan pengetahuan, sikap, dan perilaku, serta pemantauan capaian vaksinasi HPV. Terdapat peningkatan signifikan pada nilai pengetahuan (dari rata-rata 3,47 menjadi 8,74), sikap (4,12 menjadi 9,32), dan perilaku (3,54 menjadi 9,01) setelah intervensi (p < 0,05). Cakupan vaksinasi HPV mencapai lebih dari 95%. Sebanyak 75% siswa mampu melakukan edukasi di kelompok kecil, dan 25% mampu melakukan penyuluhan di kelas besar. Intervensi berbasis OSIS efektif meningkatkan pengetahuan, sikap, perilaku, dan cakupan vaksinasi HPV pada siswa SMP. Peran OSIS sebagai duta kesehatan perlu terus didukung untuk memperluas dampak edukasi pencegahan kanker serviks di lingkungan sekolah.

Abstract

Background: Dalam 5 tahun ini terdapat 15/6.324 orang atau 30/100.000 penduduk yang menderita kanker kolon dan rektum di kecamatan Baturiti, Tabanan. Tingginya angka kematian disebabkan sebagian besar 11/15 (73,33%) datang pada stadium lanjut yaitu stadium III dan IV. Tujuan kegiatan ini untuk meningkatkan peran kepala desa dan tokoh masyarakat untuk mensosialisasikan faktor resiko, gejala klinis dan deteksi dini kanker kolorektal di masyarakat. Metode: Mitra kegiatan yaitu kepala desa dan tokoh adat Desa Candikuning, Bali beserta 40 penduduk setempat. Metode yang dilakukan, pertemuan dengan masyarakat, sistem belajar bersama dengan cara praktik langsung, melakukan sosialisasi dan penyuluhan, dan evaluasi kemampuan Mitra serta evaluasi hasil pada masyarakat. Pengambilan data dilakukan dengan observasi berupa ceklis, kuesioner berupa pretest dan posttest.  Hasil: Kegiatan ini menunjukkan bahwa terjadi peningkatan pengetahuan para tokoh dan masyarakat terhadap pengetahuan serta gejala klinis sebesar 99,1%, peningkatan sikap dalam pencegahan kanker kolorektal sebesar 90,8%. Kesimpulan: Disimpulkan bahwa peran kepala desa dan tokoh masyarakat dapat meningkatkan pengetahuan dan sikap terhadap gejala klinis dan pencegahan kanker kolorektal pada masyarakat.

Abstract

Background: Penile cancer cases are rarely found, but in the Bali area, the case is higher than in other regions in Indonesia. The death rate of penile cancer is still high, and 80% of cancer deaths are related to metastasis. As immunotherapy in cancer develops, PDL-1 is becoming one of the targets for immune therapies that promise to replace conventional therapies or serve as adjunct therapies. This study aims to see the correlation between the overexpression of PDL-1 and lymph node metastasis, which is one indicator of the poor prognosis of a cancer.Methods: This study used a case-control study design with a retrospective approach. The sample was 70 primary squamous type penile cancer formalin-fixed paraffin-embedded (FFPE) specimens with complete clinical data from several Anatomic Pathology laboratories in Bali, 2015-2023. FFPE sections that were damaged and did not have complete data were excluded. The case is 35 with lymph node metastasis, and 35 without lymph node metastasis as a control. In this study, the correlation between PDL-1 overexpression and tumor grading and lymph node metastasis in penile carcinoma will be explored. Data analysis with Chi-square using SPSS. The p-value for the significance test was less than 0.05. The accuracy of the data is determined by a 95% confidence interval (CI). The research was conducted from February 2024 to August 2024 Results: This study showed a significant correlation between PDL-1 overexpression and lymph node metastasis, p < 0.05. There was a significant correlation of PDL-1 overexpression with lymph node metastasis (p = 0.001). There was a correlation between overexpression of PDL-1 and tumor size (p = 0.035), tumor grading (p = 0.027), and lymphatic invasion (p = 0.038). There was no correlation between PDL-1 overexpression with age (P = 0.424)Conclusions: This study demonstrates a significant correlation between PDL-1 overexpression and lymph node metastasis in penile carcinoma patients. Furthermore, PDL-1 overexpression is significantly associated with larger tumor size, higher tumor grade, and lymphatic invasion. These results suggest that PDL-1 serves as a critical prognostic biomarker for predicting disease aggressiveness and metastatic potential.
