Abstract

Background Penile cancer was a health problem in Bali. Clinicopathology and biological marker COX-2 were important predictive factor used for inguinal lymph node metastasis. The aim of this study was to find out relationship between clinicopathological prognostic factors (tumor size, grade, urethral invasion, intra vasa invasion, perineural invasion) and COX-2 exprerssion in metastases of SCC penile to the inguinal lymph nodes. Methods This observational crossectional study included 47 samples of SCC penis preparation from penectomy with inguinal lymphadenectomy since January 1st 2008-Desember 31st 2013 at Department of Anatomical Pathology Faculty of Medicine University of Udayana/Sanglah Hospital Denpasar. Sample of this study was divided into 2 groups: 16 cases SCC positive metastasis and 31 cases negative metastasis. Rediagnosis clinico-pathological prognostic factor was done, immunohistochemistry COX-2 staining was performed. The data were analized using chi-square (α=0.05). Results This study show significant positive correlation between tumor size ≥5 cm (p=0.002), grade (p=0.002), urethra invasion (p=0.014), intra vasa invasion (p=0.00), perineural invasion (p=0.00) with metastasis. COX-2 expression (+2) and (+3) have higher risk on occurrence of metastasis than (0) and (+1) (p=0.00). Conclusion Clinico-pathologic factors and COX-2 expression could be used to predict metastases of SCC penile to the inguinal lymph nodes. Abstract

Cervical cancer is the most common type of cancer in Balinese women, and most of the cervical cancer patients came in advanced condition. The health survey conducted in the village of Puhu, Payangan, Gianyar, found that the coverage of cervical cancer screening is 12%, which is considered as low. Payangan’s sub-district health centers (Puskesmas) have made an effort to give free PAP smear screenings to the local community, but it still has not received a good response. This is related to the community’s lack of knowledge on cervical cancer, low awareness of cancer screening, and false stigma about PAP smear examination. To overcome this problem, an effort is needed to increase awareness about the need for PAP smear screening by shaping it into a cervix cancer awareness movement "Gardavi", involving village midwives and housewife members of Empowerment and Family Welfare Group (PKK) in Puhu Village as cadres. Methods that are used in this community service include training for midwives, activation of tiered information system involving midwives-cadres and the community, knowledge provision as well as motivating cadres. This event is done to increase the role of village midwives and cadres to actively participate in PAP smear screenings in their region, therefore, it may increase screening outcomes and lower mother morbidity and mortality numbers due to cervical cancer. This event results in increased midwives’ knowledge and skill on the technique of sampling PAP smear independently, increased visits to the Community Health Sub-Center (Pustu) for PAP smear examination, creation of a midwives-cadres-community coordination system, cadres independently playing an active role in motivating the village community to do screening, as well as increased cadres’ optimism.

Abstract

BACKGROUND: Statin is an anti-cholesterol drug that is widely prescribed throughout the world. Statins are mainly used to treat and prevent cardiovascular disease. Several studies have found the pleiotropic effect of statin. However, related effect of statin in kidney failure is still unclear. Ischemic-reperfusion (I/R) injury is a major cause of acute kidney failure. This study aims to determine the effect of simvastatin on kidney function and glomerular conditions by periodic acid-schiff staining in I/R injury.METHODS: Eighteen male Swiss mice were grouped into sham operation group (GSO), I/R injury group (GIRI), and simvastatin group (GSIM). The GSO group was performed by sham operation and pretreatment of 1% carboxymethylcellulose (CMC) for 3 days. The GIRI group was performed by I/R procedure and pretreatment of 1% CMC for 3 days and the GSIM group was performed by I/R procedure and pretreatment of 10 mg/kg BW simvastatin for 3 days. Blood urea nitrogen (BUN) and creatinine serum were assessed to determine kidney function. Histopathological analysis of glomerulosclerosis was assessed by the extent of glomerular damage (sclerosis), capillary loops, and synechia. The data were analyzed by one-way ANOVA followed by post hoc Tukey’s test (p<0.05).RESULTS: The creatinine and BUN levels in the GIRI group were the highest (0.97±0.48) compared with the other groups. The glomerulosclerosis index in the GSO group was 0.75±0.56, the GIRI group was 3.55±0.61, and the GSIM group was 2.08±1.37. There was a significant difference in the glomerulosclerosis index between the GSO and GIRI groups, but there was no significant difference between the GIRI and GSIM groups. These differences include the formation of sclerosis in the glomerulus, capillary loop, and synechiae.CONCLUSION: Simvastatin improves kidney function and glomerulosclerosis in I/R injury.KEYWORDS: ischemic-reperfusion injury, simvastatin, glomerulosclerosis

Abstract

Adolescent are a transition period and highly sensitive to the influence of new values. The problems they face are also quite complex, one of which concerns reproductive health. Some studies suggest that peers are the most dominant factor in influencing the adolescent reproductive health behaviors. This dedication activity aims to increase adolescent knowledge and activate the role of peers and counseling teachers in disseminating correct information about reproductive health. The method of implementing activities in the form of providing material on adolescent reproductive health by a talk from the expert, training and mentoring to peer groups in carrying out mini counseling, as well as training and counseling for counseling teachers. After the implementation of this program obtained increased peer group knowledge and counseling teachers on adolescent reproductive health respectively as much as 50% and 30.8%. In addition, all peer groups are able to conduct mini-counseling as well as to do better counseling. Through this activity it is hoped that peer group partners and counseling is able to play an active role in continuously spreading information about adolescent reproductive health so that it can produce other peers who help keep the healthy atmosphere of the adolescents.

Abstract

Cervical cancer is the one of leading cause of death for women worldwide. It occurs due to infection with the human papilloma virus (HPV) which attacks the cervix in the female reproductive organs. The detecting cervical cancer method is by using pap smears and liquid based cytology (LBC) and vaccinations. However, both methods are limited in detecting cervical cancer quickly and accurately. This study aims to find potential biomarkers that will be used to quickly detect the development of cervical cancer. Large-scale studies have shown that microRNA-205 (miRNA) regulation has a very important role in various types of cancer including cervical cancer. The extraction and synthesis of cDNA from 26 preparations of cervical cancer paraffin blocks were measured by quantitative real time PCR to evaluate the expression of miRNA-205 as a biomarker of cervical cancer in cervical cancer samples. Statistical results showed that there was a significant difference in the expression of miRNA-205 which was relatively higher in cervical cancer patients (p<0.05) miRNA-205. In addition, the increased expression of miRNA-205 is associated with an increase in the severity of cervical cancer. We conclude that miRNA-205 is a potential candidate for cervical cancer molecular biomarker.

Abstract

Nevus kongenital multipel pada wajah memberikan dampak kosmetik yang buruk dan merupakan tantangan bagi ahli bedah plastik untuk mencapai hasil kosmetik terbaik. Hal ini memberikan dampak yang signifikan terhadap efek psikososial negatif pada pasien anak bahkan setelah pengobatan. Sebuah kasus anak laki-laki berusia 5 tahun datang dengan nevus kongenital multipel di pipi kanannya dalam dimensi yang bervariasi. Eksisi dilakukan untuk menghilangkan banyak nevus, kemudian menggunakan flap transposisi pedikel distal untuk menutup defek. Kemudian nevus dikirim ke laboratorium untuk pemeriksaan histopatologi menggunakan pewarnaan heamtoxylin dan eosin. Hasil patologi menunjukkan spilus nevus. Eksisi nevus kongenital multipel wajah, diikuti oleh flap transposisi, adalah metode rekonstruksi yang sangat baik dan cepat dengan hasil estetika yang menjanjikan. Flap lokal memberikan hasil terbaik untuk rekonstruksi wajah karena kesesuaian yang baik dengan kulit dalam hal warna, tekstur, dan ketebalan serta mematuhi salah satu prinsip dasar dalam operasi plastik "ganti seperti dengan" untuk menyamarkan bekas luka operasi, margin garis flap didesain agar berada di sepanjang batas anatomis. Selain itu, kesimetrian wajah perlu diperhatikan dalam rekonstruksi ini. Flap transposisi untuk rekonstruksi pipi merupakan pilihan terbaik untuk menutup defek tentunya dengan mempertimbangkan unit fungsional dan aspek estetika pasien.

Abstract

In 2021 there were 10 breast cases from the Baturiti sub-district which carried out examinations in the PA laboratory at Tabanan Hospital. 6 of them (60%) turned out to be cases of malignancy, and 4 of them (66.66%) stage III and IV which are the final stages of breast cancer. Perean Kangin Village which is one of the villages in the Baturiti sub-district with the most cases of breast tumors. The long-lasting pandemic has contributed to the decreased activity of cadres and increased delays in breast cancer screening. If the patient can be detected early and get treatment as soon as possible, it will give hope for recovery and a better life expectancy. One of the early detection of breast cancer is breast self-examination (BSE). It is hoped that re-activating midwives and village cadres will motivate the community to do BSE. The result of this PKM was an increase in the knowledge and attitude of cadres towards BSE. We also provide investment goods in the form of digital baby and adult weight measuring devices, stunting mats and height measuring devices for early detection of health problems.

Abstract

Background: Several studies have proven the relationship between PD-L1, which is known as an immunotherapy target, and poor prognosis. The presence of a tumor budding (TB) group tumor outside the main tumor is also associated with a poor prognosis. The purpose of this study was to determine the relationship between PD-L1 and the grade of TB in cervical carcinoma. Methods: The design of this research is a case-control study with a retrospective approach. The sample is from a paraffin block of the primary cervical cancer tumor from the results of surgery in Balimed Denpasar Hospital between 2020 and 2023. From the statistical formula for the case-control study, a minimum sample of 15 is obtained for the case group with the criteria of high-grade TB, while 15 people were in the control group with low-grade TB. Immunohistochemistry (IHC) PD-L1 is positive if the membrane or cytoplasm is brown, and is negative if there is no brown color on the membrane or cytoplasm. Inferential statistics were used to evaluate the relationship between PD-L1 overexpression and low- and high-grade TB in cervical carcinoma using a chi-square test. Results: The results of this research revealed (p = 0.273) that there was no correlation between PD-L1 expression and high- and low-grade TB. Overexpression of PD-L1 was associated with a 2.250-fold increased risk of high-grade TB (OR = 2.250 with a 95% confidence interval of 0.103–1.915). Conclusions: The severity of TB does not correlate with PD-L1 expression. The functions of PD-L1 and other variables in the carcinogenesis of TB development are the cause of this weak association.

Abstract

[Bahasa]: Kanker merupakan salah satu penyebab utama kematian di seluruh dunia dengan jumlah kasus yang terus meningkat. Pemahaman tentang faktor risiko, gaya hidup sehat, dan pentingnya deteksi dini dapat memainkan peran yang signifikan dalam menurunkan angka kasus kanker. Pengabdian masyarakat ini bertujuan untuk memberdayakan guru dan komite di Raudhatul Athfal (RA) Al Amin, Tabanan, dalam upaya pencegahan kanker sejak dini. Peningkatan kesadaran dan praktik hidup sehat dilakukan melalui pelatihan dan sosialisasi Gerakan B2SA (Beragam, Bergizi, Seimbang, dan Aman) serta konsep Isi Piringku. Program ini dirancang untuk membekali para pendidik dengan pengetahuan dan keterampilan dalam mengajarkan pola makan sehat serta gaya hidup yang dapat mencegah faktor risiko kanker kepada anak-anak usia dini. Kegiatan pengabdian ini dilakukan dengan metoda penyuluhan aktif dan praktik yang melibatkan mitra yaitu guru, komite sekolah dan wali murid. Program dilakukan pada sekolah (RA) yang berada di tengah kota yang di banjiri oleh kuliner modern. Penyuluhan dan praktik dilakukan dengan cara menjadikan guru RA sebagai pendidik sekaligus motivator, dan komite merupakan ujung tombak gerakan ini. Hasil evaluasi menunjukkan adanya peningkatan pengetahuan mencapai 91,3%, sedangkan peningkatan sikap mencapai 89,2%. Disimpulkan bahwa terjadi peningkatan pengetahuan dan sikap pada semua responden tentang pencegahan kanker melalui gerakan B2SA. Kata Kunci: pencegahan kanker, B2SA, isi piringku, raudhatul atfal [English]: Cancer is one of the leading causes of death worldwide, with increasing numbers of cases. Early understanding of cancer causes, risk factors, healthy lifestyles, and early detection can play a vital role in reducing the number of cancer cases. This community service aims to empower teachers and the committee at Raudhatul Athfal (RA) Al Amin, Tabanan, to prevent cancer from an early age. Increased awareness and practice of healthy living are achieved through training and socialization of the B2SA Movement (Beragam, Bergizi, Seimbang, Aman) and the concept of "Isi Piringku". This program is designed to equip educators with the knowledge and skills to teach healthy eating patterns and lifestyles that can prevent cancer risk factors in young children. This community service activity was conducted using active counseling and practical methods involving partners such as teachers, school committees, and parents. The program was conducted at a kindergarten (RA) in the city center, which is flooded with modern culinary offerings. Counseling and practice are carried out by making RA teachers both educators and motivators, and the committee is the spearhead of this movement. The results show an increase in knowledge reaching 91.3%, while the increase in attitude reached 89.2%. It is concluded that there was an increase in knowledge and attitude among all respondents regarding cancer prevention through the B2SA movement. Abstract

The Assisted Village Empowerment Program activities in Pengjaran Village, Kintamani, aim to optimize ecotourism through socializing the impact aof microplastics and innovation in processing oranges into wine. This village has enormous potential in the production of Siamese oranges but faces the challenges of overproduction and a lack of value-added processed products. This program involves training for Family Planning Program mothers regarding the dangers of microplastics and techniques for making orange wine, using outreach and hands-on methods. The results showed an increase in participants' knowledge about the impact of microplastics and their ability to produce orange wine. By reducing plastic use and promoting sustainable use of local resources, we hope to improve economic welfare and health in society. This program also supports the tourism industry by offering processed products that are attractive to tourists, thereby making a positive contribution to the local economy.
