Abstract

Objective. This study investigated the effect of shared decision-making (SDM) approaches on medication adherence among individuals with chronic diseases receiving primary healthcare from studies conducted between 2012 and 2023. Method. A literature review was conducted in PubMed, Scopus, ProQuest, and EBSCO databases, resulting in 220 articles. Only randomized controlled trials (RCTs) were included; the two studies that matched the inclusion criteria were examined. Results. Two RCTs in primary healthcare settings examined SDM interventions. The study on hypertension patients found no significant effect of SDM training on medication adherence, while the study on depression patients using a decision aid also found no significant impact on medication adherence. Discussion & Conclusion. SDM interventions did not significantly improve medication adherence in chronic diseases such as hypertension and depression. Factors identified include short training duration, the complexity of Adherence, and a need for consistent evidence. The conclusion suggests that SDM may not significantly affect chronic disease patients receiving primary healthcare medication adherence. However, given the limited number of eligible studies, more research is needed in diverse settings and populations.

Abstract

Background: Clinical education in radiology often faces challenges due to time constraints and the passive nature of conventional teaching methods, which hinder the development of students' diagnostic reasoning. The One Minute Preceptor (OMP) model offers a structured, learner-centered teaching approach that may address these gaps, particularly in time-limited settings. Objective: This study aimed to evaluate the effectiveness of the OMP model compared to conventional teaching methods in improving the knowledge of radiographic interpretation and diagnosis among medical students during their clinical rotation in the Radiology Department at Universitas Cenderawasih (FK Uncen). Methods: A quasi-experimental pre-test post-test nonequivalent control group design was used. Sixty professional medical students were purposively assigned to either the OMP group or the conventional group (lectures and observation), with 30 students in each. Group allocation was based on comparable cumulative GPA and clinical activeness to ensure balanced academic performance between groups. Over a one-week intervention, both groups were exposed to the same thoracic radiology cases. The OMP group received instruction following the five microskills framework, eliciting commitment, probing for evidence, teaching general rules, reinforcing correct responses, and correcting mistakes, while the control group received conventional lectures followed by tutor-led discussions without structured feedback. Knowledge improvement was measured using validated multiple-choice questions administered before and after the teaching session. Data were analyzed using independent sample t-tests and normalized gain (N-Gain) scores. Results: Both groups showed improvement in post-test scores. The OMP group had a higher mean post-test score (71.30 vs. 63.73) and mean N-Gain (29.06 vs. 23.70), although the differences were not statistically significant (p = 0.363). These findings are consistent with trends reported in international literature, which support the use of OMP in enhancing diagnostic learning outcomes. Conclusion: While not statistically significant, the OMP method showed a trend of greater improvement in radiographic diagnostic knowledge compared to conventional teaching. This pilot study supports the feasibility and potential of OMP as an effective instructional model in radiology education within the Indonesian clinical teaching context, warranting further research with larger samples and extended durations.

Abstract

Abstract

Introduction: The quality of health center services is influenced by various factors, including the proficiency of analysts in performing fundamental tests such as Gram staining. This skill is of critical importance for ensuring the accurate interpretation of results, ensuring that therapy and treatment is in accordance with standard practices. However, the competence of health analysts in the health center's working area varies considerably, which has the potential to affect the interpretation of test results. The objective of this activity was to enhance the knowledge and skills of health analysts at Puskesmas in Jayapura City in performing Gram staining. Methods: This study used a pre-experimental one-group pretest–posttest design (without a control group). The activity was carried out in Jayapura in June 2025, during a one-day training session. The research population consisted of all laboratory analysts who participated, with 11 participants who met the inclusion criteria included as samples using total sampling techniques. Knowledge assessment was conducted using a multiple-choice questionnaire. The data were analyzed descriptively to describe the characteristics of the respondents and the distribution of scores, as well as inferentially using a paired t-test to compare the mean pre-test and post-test scores, with a significance level of 0.05.Results: The implementation of gram staining training resulted in a significant enhancement in the average knowledge score, which increased from 4.45 to 6.73 (an increase of 2.27 points). A statistically significant difference was identified between the pre-test and post-test scores (t(10) = 2.46; p = 0.034), exhibiting a medium-to-large effect size (Cohen's dz = 0.74).Conclusion: The impact of gram staining training on learning outcomes was significant, with a moderate to large effect size (Cohen's dz = 0.74). However, it is important to note that the assessment of skills not included in this study is also necessary.

Abstract

Background: Despite the availability of effective antimalarial medications, poor adherence remains a significant obstacle in malaria control, particularly in endemic areas. Numerous interventions have sought to enhance adherence; however, a clear understanding of their effectiveness is lacking. Objective: This systematic review aimed to evaluate the effectiveness of intervention strategies to improve adherence to antimalarial treatments and identify the contextual factors that influence them. Method: Following the PRISMA guidelines, studies published between 2020 and 2025 were systematically retrieved from PubMed, ScienceDirect, and Google Scholar databases. Ten studies that met the predefined inclusion criteria were analyzed using narrative thematic synthesis. The interventions were categorized into five groups: health education, digital reminders (mHealth), directly observed therapy (DOT), home visits, and family/community involvement. Results: Structured health education and mHealth strategies (e.g., SMS reminders) consistently improve medication adherence across diverse contexts. Directly observed therapy, particularly involving family supervision, significantly enhances adherence in community settings. Home visits and active community involvement promote adherence by addressing the sociocultural barriers. Multicomponent strategies that integrated educational, technological, and social elements had the highest and most sustained adherence rate. Key challenges include adverse drug reactions (ADRs), medication supply issues, complex treatment regimens, and limited health literacy. Discussion and conclusion: Improving antimalarial treatment adherence requires context-specific, multifaceted interventions that integrate education, technology, and community engagement while addressing systemic health challenges in the region. These findings provide valuable insights into the design of culturally sensitive and sustainable interventions for eliminating malaria.
