Abstract

Introduction: Children and adolescents with non-syndromic cleft lip and palate (CLP) experience a higher prevalence of caries and periodontal disease compared to those without CLP. This increased prevalence is often attributed to greater plaque accumulation, which can lead to gingival inflammation and periodontal disease. This study aims to analyze the correlation between oral health knowledge and awareness and the periodontal health status of orthodontic patients with cleft lip and palate (CLP).Method: This study used a cross-sectional approach. The study population consisted of orthodontic patients with non-syndromic CLP who met the inclusion criteria. The research was conducted at the UNPAD Dental Teaching Hospital (RSGM Unpad) from March to May 2024. Twenty-seven participants with non-syndromic CLP were included in the study. A non-probability, purposive sampling technique was employed. oral health knowledge and awareness were assessed using questionnaires, while periodontal health examinations were evaluated using the Community Periodontal Index (CPI). Correlation analyses were performed using Kendall's Concordance Correlation (W). Additionally, Spearman Rank Correlation Analysis was conducted to identify the dominant variables. Results: The analysis revealed a Kendall's Concordance Correlation (W) of 0.544, which is statistically significant (p < 0.0001), indicating a 54.4% relationship between the variables. The correlation between knowledge and periodontal health status in orthodontic patients with non-syndromic CLP was 11.16%, with a p-value of 0.0443 (p < 0.05) and an r-value of 0.33. The correlation between awareness of oral health and periodontal health status in these patients was 19.85%, with a p-value of 0.0099 (p<0.05) and an r-value of 0.45. Conclusion: There is a weak correlation between knowledge and awareness of oral health and the periodontal health status in non-syndromic CLP patients undergoing fixed orthodontic treatment.

Abstract

ABSTRACTIntroduction: External apical root resorption (EARR) is one of the undesirable iatrogenic effects of orthodontic treatment. EARR is a non-infectious and asymptomatic damage that cause apical shortening of the root structure due to permanent loss of cementum and / or dentin. The etiology is multifactorial, which is a combination of individual biological variability and mechanical factors of treatment such as extraction. The aim of this study was to evaluate EARR of maxillary incisors after class I dentoskeletal malocclusion orthodontic treatment in extraction and non-extraction cases. Methods: This research was using descriptive comparative analytic method. Fifty two panoramic radiograph of patients who have done orthodontic treatment were used in this study. The sample were divided into two groups, twenty for extraction cases and thirty two for non-extraction cases. EARR was measured from the difference in the root-crown ratio of the maxillary incisors before and after orthodontic treatment using ImageJ software. Data analysis was tested using t-test. Results: The change in the root-crown ratio of the maxillary incisors after orthodontic treatment in both non-extraction and extraction groups showed a significant difference (p-value <0.05). The difference in the root-crown ratio of maxillary incisors after orthodontic treatment in the non-extraction group compared to the extraction group showed no significant difference (p-value> 0.05). Conclusion: There was differences between EARR of maxillary incisors in both extraction and non-extraction cases. There was no differences in EARR of maxillary incisors after class I dentoskeletal malocclusion orthodontic treatment between extraction and non-extraction cases.KEY WORDS: Root, resorption, maxillary, incisors, malocclusion, angle class IResorpsi akar apikal eksternal gigi insisif rahang atas sesudah perawatan ortodonti pada kasus ekstraksi dan non ekstraksi  maloklusi dentoskeletal kelas IABSTRAKPendahuluan: Resorpsi akar apikal eksternal merupakan salah satu efek iatrogenik yang tidak diinginkan. Resorpsi ini merupakan kerusakan non infeksius dan asimtomatik dimana terjadi pemendekan apikal akar gigi karena kehilangan permanen sementum dan atau dentin. Etiologi kondisi ini bersifat multifaktorial, merupakan kombinasi dari variabilitas biologi individu dan faktor mekanis perawatan, salah satunya adalah tindakan ekstraksi. Tujuan penelitian menganalisis resorpsi akar apikal eksternal pada gigi insisif rahang atas sesudah perawatan ortodonti kasus ekstraksi dan non ekstraksi maloklusi dentoskeletal kelas I. Metode: Jenis penelitian deskriptif analitik komparatif. Sampel penelitian adalah radiografi panoramik pasien sebelum dan sesudah perawatan ortodonti sebanyak 52 sampel, dibagi dalam kelompok kasus ekstraksi 20 sampel dan non ekstraksi 32 sampel. Resorpsi akar apikal eksternal diukur dari selisih rasio panjang akar mahkota gigi insisif rahang atas sebelum dan sesudah perawatan ortodonti menggunakan software ImageJ. Analisis data diuji dengan t-test. Hasil: Perubahan rasio panjang akar mahkota gigi insisif rahang atas sesudah perawatan ortodonti pada kelompok ekstraksi maupun non ekstraksi menunjukkan perbedaan yang bermakna (p-value < 0,05). Perbedaan selisih rasio panjang akar mahkota gigi insisif rahang atas antara kelompok ekstraksi dan non ekstraksi, sebelum dan sesudah perawatan ortodonti menunjukkan hasil yang tidak bermakna (p-value > 0,05). Simpulan: Terdapat perbedaan resorpsi akar apikal eksternal pada gigi insisif rahang atas baik pada kasus non ekstraksi maupun ekstraksi. Abstract

Introduction: Removable retainers are one of the devices used after active orthodontic treatment. Their use requires patient compliance with the prescribed wearing time; however, many patients remain non-compliant due to dissatisfaction with the removable retainer. This study aims to analyze the correlation between patient satisfaction with the use of a removable retainer and compliance. Methods: The research method used is correlational analysis. This study uses primary data collected through online questionnaires via Google Forms to patients of the Universitas Padjadjaran Dental Hospital, aged 18-45 years, who have undergone orthodontic treatment with fixed appliances followed by the use of removable retainers for 1-24 months. The total sample consists of 38 subjects. The seven questions assessing satisfaction were used to calculate each respondent’s total satisfaction score. The data were analyzed using the Spearman correlation test. Results: The data show that the majority of research subjects were satisfied (44.7%), and more than half were classified as compliant (57.9%). The correlation between patient satisfaction while using a removable retainer and compliance with its use resulted in a correlation coefficient of 0.447, with a significance value of 0.002 (p value<0.05). Conclusion: There is a moderately strong positive correlation between patient satisfaction with a removable retainer and patient compliance in wearing it.

Abstract

Orthodontic treatment results are potentially unstable that has a tendency to return to the former position. This is the so called relapses and therefore retention is necessary because the gingival and periodontal tissues are affected by orthodontic tooth movement and required time for reorganization when the appliances are removed. Retention is the prolonged dental detention that is done at the end of an orthodontic treatment with use of appliances designed for dental stabilization. The purpose of this paper is to explain the cause of relapse and a wide range to prevent relapse. The method used is based on literature. The conclusion is stabilizing treatment results with the retention procedure is an important part in orthodontic treatment. The patient must be given information about the implications of relapse and how to use the retainer, so that the patient can take responsibility for retention phase of treatment.

Abstract

Hypersensitivity towards the metals used in fixed orthodontic is sometimes seen in patients who receive orthodontic treatment with clinical symptoms found intraorally. Nickel is the metal that may cause sensitivity and often causes slow hypersensitivity reaction (immune response type IV) because nickel is the main component of orthodontic alloy. The allergic reaction is found in the form of oral inflammation triggered by the corrosion of orthodontic alloy. The allergic reaction is seen in the form of oral inflammation triggered by corrosion of orthodontic materials and continuous nickel particle release. Young patients, especially female patients, with allergic reaction show a predisposition of NiACS (Nickel Induced Allergic Contact Stomatitis) clinical symptoms. Therefore, dentists and orthodontists should be more careful and cautious in doing a clinical examination to patients through data collecting data on history of abnormalities and health history in the effort of making diagnosis and plan appropriate treatment.

Abstract

Maloklusi diperkirakan sebagai faktor yang berkontribusi terhadap perubahan posisi sendi temporomandibular. Perawatan ortodonti berisiko menyebabkan resorpsi kondilus sebagai bentuk adaptasi terhadap perubahan bentuk jaringan akibat kekuatan mekanik pada TMJ. Penelitian ini bertujuan untuk mengetahui perbandingan tinggi dan lebar kondilus serta kesimetrisan vertikal mandibula pada pasien sebelum dan sesudah perawatan ortodonti cekat. Penelitian analitik komparatif ini menggunakan data sekunder dengan teknik purposive sampling. Sampel penelitian berjumlah 35 pasang radiograf  panoramik digital dari pasien dengan maloklusi kelas I dentoskeletal yang telah selesai dilakukan perawatan ortodonti cekat di RSGM Universitas Padjadjaran pada tahun 2015-2019 dengan kelompok tanpa pencabutan sebanyak 27 pasang serta kelompok dengan pencabutan 4 premolar pertama sebanyak 8 pasang sesuai dengan kriteria inklusi dan eksklusi. Pengukuran tinggi dan lebar kondilus serta kesimetrisan vertikal mandibula menggunakan software Image J win 64 dan dilakukan uji signifikansi Paired t test dengan nilai signifikan p < 0,05. Hasil penelitian menunjukan terdapat perbedaan signifikan pada tinggi kondilus kiri setelah perawatan ortodonti cekat pada kelompok tanpa pencabutan (p<0,05). Selain itu, hasil penelitian menunjukan bahwa lebar kondilus kiri maupun kanan pada semua kelompok menunjukkan perbedaan yang tidak signifikan (p>0,05), serta ketidaksimetrisan vertikal mandibula sebelum perawatan sama dengan sesudah perawatan pada semua kelompok (p>0,05). Dari hasil penelitian dapat disimpulkan bahwa perawatan ortodontik memiliki pengaruh yang signifikan terhadap perubahan tinggi kondilus, namun tidak ada pengaruh yang signifikan terhadap lebar kondilus dan simetri vertikal mandibula.

Abstract

Dental malocclusion Class III is a challenging condition characterized by a Class III molar relationship, and it has a specific anterior condition according to Dewey classification. In some cases, Class III dental malocclusion with skeletal Class I may present with a convex rather than a concave profile. Several methods are available to correct a convex profile, including extraction, full arch distalization using TADs, intermaxillary elastic Class II, anterior segment retraction, and others. Combining more than one of these methods can be effective in correcting a convex profile. An 18 year-old woman presented to RSGM Universitas Padjadjaran with a diagnosis of Angle Class III dental malocclusion, skeletal Class I with a convex profile, normal maxillary incisor inclination, asymmetric profile, severe anterior crowding in both arches, minimal overjet, anterior crossbite, upper midline shift, clockwise rotation and high-angle mandible, and lingual inclination of the mandibular incisors. The patient was treated with a Roth prescription fixed orthodontic appliance. Maxillary and mandibular first premolars were extracted to create space, and lacebacks were applied to the canines to correct severe anterior crowding]. Bilateral intermaxillary Class III elastics were used to maintain the facial profile and prevent it from becoming concave. Treatment was completed in 18 months, achieving a Class I molar relationship, midline correction, optimal overbite and overjet, and preservation of the facial profile. This case report presents the orthodontic management of Angle Class III malocclusion with skeletal Class I and severe anterior crowding, successfully treated with first premolar extractions while maintaining a convex profile.

Abstract

Introduction: Temporary Anchorage Devices (TADs), absolute anchorage systems with minimal side effects on orthodontic mechanical movements, can be alternatives to wider non-surgical, non-extraction, and non-compliance treatments. The placement of extra-radicular mini-screws at the infra zygomatic crest (IZC) is one of the most commonly used approaches for skeletal anchorage. Maxillary arch distalization with IZC can be effectively performed in Class II malocclusion patients with large overjet using non-extraction treatment, especially due to increasing patient demands to avoid extractions. The aim of this case report is to demonstrate the effectiveness of infrazygomatic crest (IZC) extra-radicular TADs for maxillary distalization in a Class II malocclusion cases treated without extractions. Case report: A 16-year-old female patient came to the Orthodontic Clinic with the complaint of forward upper teeth, Class II Angle malocclusion and Class I skeletal with a convex facial profile, deep overbite, overjet 6 mm, posterior scissor bite, diastema between teeth 32-33, and deep curve of Spee. The patient was treated with non-extraction fixed orthodontics using a self-ligating system prescription, utilizing extra radicular Temporary Anchorage Devices (TADs) at the infra zygomatic crest measuring 2 x 12 mm for maxillary retraction. Following the maxillary retraction treatment, successful outcomes were achieved as the overjet decreased from 6 mm to 3 mm; Class I molar and canine relationships were established; and the distance from I to NA was reduced from 11 mm to 5 mm. Conclusion: The clinical use of infra-zygomatic crest (IZC) bone screw in non-extraction treatment of Class II malocclusion case was successfully conducted with non-extraction fixed orthodontic treatment.

Abstract

Background: Class III dentoskeletal malocclusion presents a significant clinical challenge, particularly in adult patients, due to limited skeletal growth potential and complex occlusal discrepancies. Non-surgical management often relies on dental compensation strategies, including selective extractions and transverse expansion to achieve functional and aesthetic outcomes. Objectives: This report aims to demonstrate the successful management of a mild skeletal Class III malocclusion using first mandibular molar extractions and maxillary expansion via quad helix appliance, as an alternative to orthognathic surgery. Case: A 20-year-old female patient presented with ectopic canines, chewing difficulty, and facial aesthetic concerns. Clinical and radiographic evaluations revealed a skeletal Class III relationship, bilateral anterior and posterior crossbites, and crowding. Notably, the patient had large composite restorations and overfilled endodontic treatments on her lower first molars. Case Management: A non-surgical orthodontic treatment was planned and executed. This included bilateral extraction of the first lower molars, maxillary expansion using a quad helix appliance, and fixed appliance therapy with a Roth-prescription straight-wire system. The treatment was followed by midline correction, retraction, and use of Class III elastics to achieve Class I molar and canine relationships. The retention phase involved clear retainers for both arches. Conclusion: The combination of lower first molar extraction and quad helix expansion provided an effective camouflage for a mild skeletal Class III malocclusion, improving facial aesthetics, correcting crossbites, and achieving stable functional occlusion. This case supports the viability of non-surgical orthodontic compensation in carefully selected adult Class III patients. Keywords: class III dentoskeletal malocclusion, first lower molar extraction, quad helix
