Abstract

Strategi eliminasi tuberkulosis yang disebut dengan End TB Strategy mempunyai misi mencapai Zero TB pada tahun 2050. Untuk mencapai hal ini, diperlukan manajemen TB yang baik. Pendekatan kolaboratif interprofesi mulai dari dokter, perawat, apoteker, kader kesehatan dan masyarakat diharapkan dapat membantu, tidak hanya dalam hal pengawasan, pengobatan, tetapi juga dalam deteksi dini infeksi tuberkulosis. Pelaksanaan kegiatan pengabdian ini menggunakan metode ceramah dan pada akhir kegiatan dilakukan evaluasi. Metode awal adalah ceramah berupa penyuluhan secara interaktif yang dibawakan berbagai profesi yaitu dokter, perawat, dan apoteker. Materi mengenai pengetahuan tentang TB serta program pemerintah dalam menjaring hingga pengobatan kasus TB diberikan. Selain itu dalam penyuluhannya ditegaskan pula peran masing-masing profesi agar dapat berjalan sinergis dalam manajemen TB di layanan perifer. Tahap terakhir adalah evaluasi kegiatan dilakukan dengan mengisi lembar tes yang dibagikan kepada peserta penyuluhan yang berisi pertanyaan terkait materi yang telah diberikan. Kegiatan ini diikuti oleh 50 orang yang dimana peserta merupakan perangkat puskesmas mulai dari dokter, perawat dan apoteker serta melibatkan kader puskesmas sebagai perwakilan masyarakat di wilayah kerja Puskesmas Tabaringan. Setelah dilakukan penyuluhan, peserta yang mampu menjawab dengan benar diatas 70 poin sebanyak 80%. Banyaknya peserta yang menjawab 70 benar dari 100 poin menggambarkan tingkat pengetahuan peserta secara umum memahami topik yang diberikan dengan baik. Manajemen tuberkulosis dapat dilakukan dengan pendekatan kolaborasi interprofesi yang melibatkan peran aktif dari berbagai pihak, tidak hanya tenaga kesehatan namun juga peran aktif dari masyarakat.

Abstract

Latar belakang: Derajat keparahan Covid-19 dikaitkan dengan koagulasi dan kerusakan endotel yang dipicu oleh reaksi inflamasi akibat dari pelepasan sitokin proinflamasi yang berlebihan oleh karena infeksi SARS CoV-2. Metode: Penelitian deskriptif analitik yang dilakukan dengan pendekatan kohor retrospektif dilakukan di Rumah Sakit Umum Pemerintah Wahidin Sudirohusodo Makassar dengan mengumpulkan data rekam medis pasien Covid-19 pada bulan Juni 2020 – Juni 2021 dengan menggunakan total sampling yang memenuhi kriteria inklusi dan eksklusi. Hasil: Pada penelitian ini didapatkan rerata trombosit lebih rendah (265.442) pada kasus berat, sedangkan rerata fibrinogen (405,54) dan D-dimer (9,84) lebih tinggi pada derajat keparahan lebih berat, serta Prothrombin time (PT) lebih panjang (10,92) pada derajat keparahan lebih berat. Kesimpulan: Profil koagulasi penderita Covid-19 didatakan rerata rombosit lebih rendah pada derajat keparahan lebih berat, sedangkan fibrinogen dan D-dimer lebih tinggi serta Prothrombin time (PT) lebih panjang pada derajat keparahan lebih berat.

Abstract

Introduction: Laryngeal tuberculosis (LTB) is usually accompanied by pulmonary tuberculosis (TB) involvement. Misdiagnosis often occurs because the symptoms are non-specific and resemble laryngeal malignancy. We reported a rare condition of pulmonary TB masquerading as larynx abnormalities such as malignant manifestations. Case: A 59-year-old man presented with a main complaint of prolonged hoarseness without clinical respiratory complaints. The patient additionally reported coughing as a symptom while receiving medical treatment in the hospital. A computed tomography (CT) scan of the neck without contrast revealed a glottic tumor invading the vocal cord – T3N2cMx. Laryngoscopy examination revealed T1-T1 tonsils. The pharyngeal mucosa was slightly hyperemic. The arytenoids and epiglottis were less hyperemic. There was no edema, the left vocal fold was paralyzed, could not be adducted, and there was a lump. The glottis rima gap was narrow, size 3–4 mm, and the mass could not be evaluated. Chest X-ray examination was normal, but rapid molecular tests of TB detected very low Mycobacterium tuberculosis (MTB) levels. The patient was treated with anti-TB treatment (ATT) according to body weight. An evaluation was performed after 2 weeks, and the patient's clinical and physical condition improved. Conclusion: Pulmonary TB patients in endemic countries can have clinical manifestations such as LTB or laryngeal malignancy in 1% of cases. The prognosis for LTB with pulmonary T involvement is quite good with ATT administration.

Abstract

Stigma often accompanies people with multidrug-resistant tuberculosis (MDR-TB) and potentially affects their health-related quality of life (HRQoL). The aim of this study was to investigate the stigma faced by patients with MDR-TB, both from the patients' and community's perspective, and its relationship with HRQoL. Data was gathered at the provincial hospital in Makassar, South Sulawesi, Indonesia. The instrument employed in this research was the Indonesian version of the tuberculosis (TB) stigma instrument to assess MDR-TB stigma from the patient and community perspectives. The patient perspective represents how individuals with TB perceive and experience stigma, including the fear of disclosure, isolation, and guilt (feeling responsible for the burden on their family or their own risky behaviors). Meanwhile, the community perspective reflects how individuals with TB perceive societal attitudes towards them, such as social distancing, avoidance, and reluctance to interact. HRQoL was measured using the European quality of life-5 dimensions-5 level version (EQ-5D-5L) instrument. Notably, the evaluation of anxiety and depression is centered on the fifth dimension of the EQ-5D-5L instrument. A total of 210 patients with MDR-TB were included in the study, all of whom reported experiencing stigma. Most participants perceived stigma at a moderate level, with 76% from the patient perspective and 71% from the community perspective. The average EQ-5D-5L index score was 0.72 (95% confidence interval (95%CI): 0.68–0.76). Measurements from both perspectives show similar scores. There is a substantial negative association between the level of stigma and HRQoL, both from the patient's perspective (R2=-0.33; F=102.52; p<0.001) and the community's (R2=-0.32; F=96.76; p<0.001). The study highlights that the stigma of MDR-TB significantly affects the HRQoL from the patient and community perspective.

Abstract

Backgrounds: Active hexose correlated compound (AHCC), an α-glucan extracted from Lentinula edodes, is known for its immunomodulatory properties. It is suspected that the strong nutrients contained in AHCC are very safe and without side effects, including in tuberculosis patients. This study evaluated the effect of AHCC on clinical and hematologic responses in pulmonary tuberculosis (TB) patients Methods: A double-blind randomized controlled trial was conducted involving 60 pulmonary TB patients receiving standard anti-TB therapy. Participants were randomly assigned to receive either AHCC (3 g/day) or a placebo for six months. Clinical and laboratory parameters were assessed before and after treatment. Results: Patients in the AHCC group showed greater improvement in clinical symptoms (reduced cough, improved appetite, and weight gain) and significant enhancement of hematological and immunological markers compared with placebo (P<0.05). AHCC supplementation significantly increased CD4⁺ and CD8⁺ lymphocyte counts, suggesting improved immune recovery and modulation of host defense against Mycobacterium tuberculosis infection. Conclusion: AHCC supplementation alongside standard anti-TB therapy improves clinical outcomes and immune responses in patients with pulmonary tuberculosis.
