Abstract

Pneumonia is a lung parenchyma disease caused by bacteria, fungi, viruses, and parasites. Irrational use of antibiotics will lead to antibiotic resistance, so the goal of therapy has yet to be achieved. This study evaluates the rationality of using community antibiotics for the clinical outcome of community pneumonia patients in RSUD. Dr. H. Abdul Moeloek.This research is descriptive-analytic with a cross-sectional design and retrospective data retrieval from medical records in 2019. The data analysis was carried out descriptively, and the correlation analysis of the rationale of antibiotic use with clinical outcomes using the Spearman test. The study's results of 144 analyzed showed the characteristics of community pneumonia patients, 78 male, and 66 female patients. The evaluation of the use of antibiotics using the ATC/DDD method showed that Ceftriaxon DDD was 46.80 DDD/100 patient-days, Azithromycin (23.91 DDD/100 patient-days), Levofloxacin (22.70 DDD/100 patient-days). With the Gyssen method, patients used antibiotics correctly (category 0) by 72.91% and incorrectly (categories I – VI) by 27.09%. Statistical analysis showed a significant relationship (p-value=0.003) between the rational use of antibiotics and patient clinical outcomes. Further research is needed regarding the incidence of Drugs Related Problems on the clinical effects of community pneumonia patients.

Abstract

Kersen (Muntingia calabura L.) and soursop (Annona muricata L.) have many properties to cure several diseases, including antihiperurisemia, which is a disorder of purine metabolism in which patients experience increased uric acid levels. This study aimed to examine the antihiperuricemia effect of a combination of kersen leaf extract and soursop in male rats orally. Extraction was carried out by maceration using 70% ethanol. This research is a laboratory experimental study using male white mice as test animals. Data were processed in the form of ANOVA and LSD (Least Significant Difference) test results to compare between groups P0 (normal control), PI (negative control), P2 (Allopurinol 9 mg/Kg BW), P3 (410 and 3.5 mg/Kg BW), P4 (410 and 7 mg/Kg BW), P5 (410 and 10.5 mg/Kg BW), P6 (610 and 3.5 mg/Kg BW), P7 (610 and 7 mg/Kg BW), P8 (610 and 10.5 mg/Kg BW), P9 (910 and 3.5 mg/Kg BW), P10 (910 and 7 mg/Kg BW) ), P11 (910 and 10.5 mg/Kg BW). Giving the test material starting on the 8th to 14th day. On the 15th day blood was taken by slaughtering to measure uric acid levels using a UV-Vis spectrophotometer. On examination of uric acid levels, the administration of beef liver can significantly increase uric acid levels in male rats. and the administration of kersen leaf extract and soursop leaves can significantly reduce uric acid levels in male rats. The administration of kersen leaf extract and soursop leaves for 7 days at doses of 910 and 10.5 mg/Kg BW most effectively reduced uric acid levels in hyperuricemic mice.   Keywords: Hyperuricemia, kersen leaves extract, soursop leaves extract

Abstract

The high mortality rate in breast cancer and with the most malignancy attacking women, therapy is needed in the treatment of breast cancer by chemotherapy. Chemotherapy works by killing rapidly dividing cancer cells. However, due to its non-selective nature, chemotherapy also affects healthy cells and causes side effects that affect the patient's quality of life. This study aims to determine the side effects of chemotherapy in breast cancer patients at Dr. H. Abdul Moeloek Hospital, Lampung Province in 2023. This type of research is a qualitative descriptive observational study with a cross-sectional design and accidental sampling data collection techniques. Data analysis was done descriptively in the form of a percentage. The sample in this study was 41 patients. The results of the study based on the characteristics of female gender were 41 patients (100%) and male patients were 0 patients (0%), the age of the diagnosed patients was in the age range of 36-45 years as many as 13 patients (31.7%), 46-55 years as many as 13 patients (31.7%), and 56-65 years as many as 13 patients (13.7%), based on the patient's occupation with the occupation of a housewife (IRT) as many as 24 patients (58.5%), treatment with the TdCar regimen (Taxane (docetaxel), Carboplatin) as many as 13 patients (26.9%) and the AC regimen (Doxorubicin, Cyclophosphamide) as many as 11 patients (21.2%). The conclusion of this study is the side effects using the Naranjo algorithm, the score obtained is between 1-4 (possible) which means that there may be an ADR. The most common side effects suffered by breast cancer chemotherapy patients at Dr. Soetomo Hospital. H. Abdul Moeloek Lampung Province in 2023 was hair loss (87.8%).
