Abstract

Abstract: M1 maxillary tooth has a close relationship with the maxillary sinus floor. It is a potential source of infection for the maxillary sinus because the M1 root has a higher risk of perforation than other posterior teeth. Panoramic radiographs can identify the position of the posterior maxillary teeth against the maxillary sinus floor with specific criteria or classifications, one of which, according to Jung and Cho. This study aimed to determine the relationship between the maxillary sinus base and the apical root of the maxillary M1 tooth based on gender and age as viewed from panoramic radiographs. This was an observational and analytical study using the cross-sectional design. Samples were derived from panoramic radiographs taken in 2021 of patients aged 20-50 years at RSGM Unjani using the purposive sampling method. Data were analyzed using the Mann Whitney test and the Kruskal Wallis test. The results obtained 44 panoramic radiographs of 18 males and 26 females. There were more apical M1 roots protruding into the sinus cavity (type 3). There was no significant relationship between the maxillary sinus floor and the apical root of the maxillary M1 tooth in the right and left regions based on gender and age (p>0.05). In conclusion, type 3 is the most common found, and no significant relationship between the maxillary sinus floor and the apical root of the maxillary M1 tooth in the right and left regions based on gender and age Keywords: first molar; maxillary sinus floor; panoramic radiograph; root apical    Abstrak: Gigi M1 memiliki hubungan erat dengan dasar sinus maksilaris dan banyak menjadi sumber infeksi terhadap sinus maksilaris karena akar M1 maksila memiliki risiko perforasi yang lebih tinggi daripada gigi posterior lainnya. Abstract

Objectives: The maxillary first molar has a close relationship with the base of the maxillary sinus floor. Cone-beam Computed Tomography (CBCT) provides coronal, sagittal, occlusal, and 3D sectional images of maxillofacial structures without causing distortion. Thus, CBCT allows for a comprehensive analysis of the position of the maxillary first molar about the maxillary sinus. This study aims to determine the correlation between age and the classification of vertical relationship between the maxillary sinus and the roots of the maxillary first molar using CBCT. Materials and Methods: The research design was the analytical observational research used a cross-sectional design. The study population includes all CBCT radiographs from patients aged 20-50 years who used CBCT at RSGMP Universitas Jenderal Achmad Yani. The total sampling technique was used to include all CBCT radiograph data comforms to the inclusion and exclusion criteria. Results: The study resulted in 60 CBCT radiographs, with 54 data for the right maxillary first molar and 49 data for the left maxillary first molar. Data analysis using Spearman correlation test showed r = -0.191 with a p-value of 0.166 for the right maxillary first molar and r = -0.167 with a p-value of 0.252 for the left maxillary first molar. Conclusion: There was no correlation between age and the classification of vertical relationship between the maxillary sinus and the maxillary first molar tooth root (p > 0.05). This is because the volume of the maxillary sinus decreases with age, leading to an increased distance between the maxillary sinus and the tooth roots.

Abstract

24-year-old male patient came to the Unjani Teaching Dental and Oral Hospital complaining of a disturbing residual root of the upper right front tooth and wanted to be extracted. Still, when a periapical radiograph was taken, it was found that the residual root of the tooth was a primary tooth that had not yet fallen out because there was a radiopaque picture resembling the impaction of a permanent canine tooth at the apex of the tooth. The patient never felt pain, and the complaint had never been treated before, so the patient wanted treatment to eliminate the complaint. Further supporting examination is needed, such as an occlusal radiography examination, which plays an important role in seeing the position of impaction of permanent canines in the upper and lower jaws and serves as a basis for determining the treatment plan to be carried out. This case report aims to describe the eruption disorder in the form of impaction of maxillary permanent canines due to the persistence of residual roots of primary teeth and to see in detail the position of the impaction from radiographic images of occlusal photos in the upper jaw. This case shows that the persistence of primary teeth can cause abnormalities in the direction of growth of permanent teeth into tooth impaction abnormalities found from radiographic images. Abstract
