Midwifery care through Continuity of Care (COC) is a continuous service approach that covers the entire reproductive cycle of women, from pregnancy, childbirth, postpartum, newborns, to family planning. The method of midwifery care at PKD Keji Village is through home visits by providing counseling according to the mother's needs. Midwifery care provided to Mrs. C lasts from pregnancy, childbirth, postpartum, neonates, to family planning with a frequency of 3 pregnancy visits, 4 postpartum visits, and 3 neonates. The method in this study uses data collection methods, namely using interviews, physical examinations, observations with primary and secondary data through the KIA Book, and this study started from November 2024 - May 2025, the research instrument used SOAP documentation. Based on the results of a Comprehensive case study (Continuity Of Care) on Mrs. C from the second trimester of pregnancy, childbirth, postpartum, newborns and neonates. It was found that Mrs. C, 22 years old, G1P0A0, 19 weeks gestation. Mrs. C's delivery took place at the regional hospital, the postpartum period was normal, there was no abnormal bleeding, uterine contractions were good, and complementary therapy of oxytocin massage was given. The newborn baby had normal anthropometric examination results, and Mrs. C decided to use a 3-month injection of contraception. It is expected that the midwife profession in providing continuous midwifery care (continuity of care) will continue to apply midwifery management, maintain and improve competence in providing care in accordance with midwifery service standards. Abstrak Asuhan kebidanan secara Continuity of Care (COC) merupakan pendekatan pelayanan berkesinambungan yang mencakup seluruh siklus reproduksi wanita, mulai dari kehamilan, persalinan, masa nifas, bayi baru lahir, hingga keluarga berencana. Metode asuhan kebidanan di PKD Desa Keji melalui kunjungan rumah dengan memberikan konseling sesuai dengan kebutuhan ibu. Asuhan kebidanan yang diberikan kepada Ny. C berlangsung dari masa kehamilan, bersalin, nifas, neonatus, sampai KB dengan frekuensi kunjungan hamil sebanyak 3 kali, nifas 4 kali, dan neonatus 3 kali. Metode dalam penelitian ini menggunakan metode pengumpulan data yaitu menggunakan Interview, pemeriksaan fisik, observasi dengan data primer dan sekunder melalui Buku KIA, serta penelitian ini dimulai dari bulan November 2024 - Mei 2025 instrumen penelitian menggunakan dokumentasi SOAP. Berdasarkan hasil studi kasus secara Komprehensif (Continuity Of Care) pada Ny. C dari kehamilan trimester II, persalinan, masa nifas, bayi baru lahir dan neonates. Didapatkan Ny. C umur 22 Tahun G1P0A0 usia kehamilan 19 minggu. Persalinan pada Ny. C berlangsung di RSUD, masa nifas berlangsung normal tidak ada perdarahan yang abnormal, kontraksi uterus baik, dan diberikan terapi komplementer pijat oksitosin. Pada bayi baru lahir hasil pemeriksaan antropometri normal, dan Ny. C memutuskan untuk menggunakan KB suntik 3 bulan. Diharapkan profesi bidan dalam memberikan asuhan kebidanan berkelanjutan (continuity of care) selanjutnya selalu menerapkan manajemen kebidanan, mempertahankan dan meningkatkan kompetensi dalam memberikan asuhan sesuai dengan standar pelayanan kebidanan.
