Abstract

Interaction of Cysteine-rich Interdomain Region (CIDR)α-Plasmodium falciparum Erythrocyte Membrane Protein (PfEMP1) and Endothelial Cell Receptors especially CD36 on host cells is main malaria pathogenesis, makes this domain as a malaria vaccine candidate. Recently, the development of the malaria vaccine is conducted by recombinant technology, and the purification of the CIDRα-PfEMP1 recombinant protein is a pivotal step. This study aimed to determine an optimal condition to purify the CIDRα-PfEMP1 recombinant protein by affinity chromatography through imidazole and NaCl concentration. The purified recombinant protein was visualized using SDS-PAGE and its concentration was measured using Image J software and Bradford Assay. The data were analyzed using SPSS 26 software, and the Paired T-Test analysis was conducted to compare the concentration of purified recombinant protein from two different methods. The result showed that thetarget band of purified recombinant protein was 27 kDa. The thickest target protein band was observed in purified recombinant protein using 140 mM imidazole and 300 mM NaCl. The recombinant protein concentration using Image J software was 0.025 µg/µL, while the Bradford Assay was 0.56 µg/µL. The Paired T-Test analysis has a significance value of 0.010 (p<0.05), meaning there was a significant difference between the concentration measurement using Image J software and Bradford Assay. In conclusion, the optimized condition to purify the CIDRα-PfEMP1 recombinant protein by affinity chromatography was using 140 mM imidazole and 300 mM NaCl. It is suggested to measure the purified CIDRα-PfEMP1 recombinant protein concentration using the Bradford Assay method due to its convenience and sensitivity.

Abstract

Indonesia is a country with the highest Maternal Mortality Rate (MMR) in Southeast Asia. Ngawi Regency itself still had high maternal mortality rate that recorder six pregnant women died in 2023, so this is still a concern for the local Ngawi Regency Government. Knowledge is one of many factors that influences the occurrence of maternal mortality in Indonesia. This study is descriptive and aims to evaluating level of knowledge about danger sign in pregnancy on maternal in Ngrawe district, Ngawi. The data used were primary data obtained from pretest and posttest using a questionnaire. The results of pretest were in the sufficient category on 18 samples (45%), 5 samples (12.5%) in poor category, and 17 others samples (42 .5%) in good category. And the postetst showed good result that all samples were able to answer all research questionnaire questions with correct answers The conclusion of this study is that knowledge is main factor to improve good labour in maternal and decrease MMR.

Abstract

Salah satu kemungkinan komplikasi meningensefalitis adalah hidrosefalus. Drainase ekstraventrikular secara dini membantu meningkatkan luaran klinis pasien dan dapat digunakan untuk mengevaluasi cairan serebrospinal berulang dan mengendalikan tekanan intrakranial. Seorang laki laki muda berusia 24 tahun datang ke unit gawat darurat dengan keluhan nyeri kepala yang memberat sejak 2 hari lalu. Keluhan disertai dengan leher kaku dan demam sejak 4 hari lalu. Pasien memiliki riwayat sakit telinga kiri sejak 1 minggu lalu. Pasien juga sulit fokus ketika diajak berkomunikasi dan gelisah. Pemeriksaan fisik menunjukkan tekanan darah 139/80 mmHg, nadi 91x/menit, suhu 39,5°C, dan saturasi oksigen (SpO2) 97%. Status lokalis auricula sinistra terdapat pus berwarna kuning jernih disertai nyeri tarik tragus.  Pemeriksaan neurologis menunjukkan GCS 335, tanda meningeal (+), VAS 8-9. Pemeriksaan laboratorium awal menunjukkan Leukosit 21.610 sel/uL; Monosit 2.060 sel/uL; neutrofil 17.390 sel/uL, gula darah acak 193 mg/dL, HIV (-), dan DNA MTB (-). Pemeriksaan CT Scan Kepala dengan kontras menunjukkan kesan encephalitis/meningoencephalitis dengan komplikasi hydrocephalus comunican. Pasien didiagnosis dengan meningensefalitis dengan hydrocephalus dan direncakaan Extraventricular Drainage (<24 jam) serta pemeriksaan LCS. Dari hasil pemeriksaan LCS didapatkan warna jernih, Nonne apelt (+), dan pandy (+). Pasien diberikan terapi steroid, antivirus, antibitoik spektrum luas, analgesik, dan antipsikosis. Pasien menunjukkan perbaikan kondisi klinis setelah 6 hari perawatan. Kasus ini relatif jarang, terutama pada komplikasi otitis media yang menyebabkan meningoensefalitis dan hidrosefalus komunikans pada pasien dewasa muda. EVD dini dan pengobatan spesifik tampaknya meningkatkan luaran klinis.
