Abstract

Cancer is a disease with complex side effects that can interfere with the body functions, impacting on increasing patient dependence on caregivers. This study objectives was to prove the duration of treatment hours and the level of dependence of cancer patients affect caregiver’s stress level. This was a cross-sectional study. Population 50 caregivers who live at the Indonesian Cancer Foundation East Java Branch Surabaya Indonesia and only 30 samples were chosen based on inclusion criteria. Instruments used duration of treatment hours questionnaire, KATZ index and caregiver stress self-assessment. All instruments had been tested validity and reliability. Ethical procedures were done before the research starts. After the data collected it was analyzed used a multinomial logistic regression test with p < 0.05. The majority of participants taking care of cancer patients for 4 – 6 hours/day (56.7%), most of the cancer patients who treated by caregivers, experienced moderate levels of dependence (50%) and the majority of caregivers experienced stress at moderate levels (56.7%). There was a significant effect between the duration of treatment hours and the level of dependence of cancer patients on the caregiver’s stress level. The duration of treatment hours affect 34.3% on caregiver’s stress level (p = 0.03) and level of patient’s dependence affect 41.9% (p = 0.01). The longer hours of care lead to less caregiver’s free time for their personal needs, so that most of the time is used for patient care. Likewise, the higher the level of patient dependence on caregivers, the more time is given for care and can further trigger stress on caregivers.

Abstract

Background: Cancer is a serious stressor that induces spiritual distress, loss of optimism, and dissatisfaction with subjective well-being. This study aims to determine that spiritual well-being (SWB) and optimism as factors that contribute to influencing the subjective well-being of cancer patients. Methods: This study had a cross-sectional design. A total of 88 cancer patients from public health centers in Surabaya and the Indonesian Cancer Foundation of East Java were selected to be sampled by consecutive sampling in 4 months. The Instruments used were valid and reliable throughout the test, comprising of the spiritual well-being scale, the 9-item version of the personal optimism scale, self-efficacy optimism scale, and satisfaction with life scale. The ethical feasibility was declared ethical. Data were analyzed using Pearson Product Moment Correlation Test and Linear Regression Test with p < 0.05. Results: The demographic data show that the mean age of 52.5 years, female gender (84%), being married (78.4%), having cervical cancer (52.3%), having stage-III cancer (55.7%), having cancer therapy (surgery, chemotherapy, and radiotherapy) (37.5%), and patients newly diagnosed from 6 months to 1 year (33%). The mean score of SWB was 99.67 (high SWB), optimism 20.03 (high optimism), and subjective well-being 24.18 (good subjective well-being). Pearson Product Moment Correlation Test between SWB and optimism showed p < 0.001, indicating that SWB had a significant relationship with optimism; patients with high SWB will have high optimism. From the Linear Regression Test, there was a significant effect of SWB on subjective well-being (r2 = 0.982 and p < 0.001) and optimism on subjective well-being as well (r2 = 0.988 and p < 0.001), meaning that patients with high SWB and high optimism tended to have good subjective well-being.  Conclusions: Cancer patients who have high scores of SWB become more optimistic, which has a positive effect on the high score of subjective well-being as well.

The instrument was Beck Anxiety Inventory that it was valid and reliable based on the test. Ethical feasibility tests has been carried out. Acupressure therapy is given on acupoint St36, Li4 and Li11. It conducted 2 times per week for 4 weeks. Wilcoxon Signed Rank Test was applied to analyze this result with p < 0.05.Results: There was a significant effect of acupressure for improving anxiety levels in both groups with p value in the chemotherapy group 0.001 and in the chemoradiotherapy group 0.002. But a greater influence occurred in the chemotherapy group compared to chemoradiotherapy group. Conclusions: Acupressure therapy in three acupoints can stimulate relaxation condition, it can decrease the anxiety level for cancer patients with all types of cancer therapy.

Abstract

Background and purpose: The duration of care taking of cancer patients can trigger stress which impacts on the emergence of a burden which further affects the physical health of the family caregivers. This study aims to determine the effect of caring duration on the burden and health status of cancer patient’s family caregivers. Methods: This was a cross-sectional study. Samples were 56 cancer patient’s family caregivers who were selected by a purposive sampling technique based on specified inclusion criteria from two public health centers (PHCs) in Surabaya Indonesia. Participants completed the questionnaire which comprised of questions on caring duration, caregiver burden scale and physical health status. Statistical tests using simple linear regression and multiple linear regression were performed. Results: The average age of family caregivers was 49.91 (SD=12.24), mostly female (66.7%), while half of the patients (51.8%) were a stage III cancer patients and the most frequent type of therapy (35%) received was chemotherapy. The average of caring duration was 4.44 (SD=1.92) years, score of burden was 51.07 (SD=18.45) and score health status was 60.32 (SD=10.81). After adjusted for age, gender, cancer stage and type of therapy, caring duration was significantly associated with increased burden of family caregivers (b=9.186, 95%CI: 7.85-10.10, p<0.001) and with reduced health status (b=5.282, 95%CI: 4.50-5.85, p< 0.001) Conclusion: The longer caring duration of cancer patients causes a higher burden and also a lower health status of family caregivers. Health workers and community have an important role in reducing the burden and improving the health status of caregivers by providing supports and appropriate prevention measures.

Abstract

Background: Cancer therapy during the pandemic causes physical and psychological problems. It impacts happiness in their lives. Laughter yoga is an intervention that can increase happiness. The purpose of this study is to prove that laughter yoga can increase the achievement of individual and peer happiness in cancer patients undergoing therapy in a pandemic situation. Methods: This was a quasi-experimental with a non-equivalent control group design. The total sample was 40 cancer patients at the Indonesia Cancer Foundation East Java selected through the inclusion and exclusion criteria. In the implementation, 20 people belong to the intervention group (IG) and 20 other people belong to the control group (CG). The IG provided laughter yoga 2x/a week for 4 weeks with 14 steps, while CG only through daily activities. Both groups conducted pre-test and post-tests using The Subjective Happiness Scale. The scale consists of two indicators: individual happiness and peers’ happiness. In addition, the calculation of the two indicators is done separately. The instrument has been tested for its validity and reliability. The data were normally distributed (Shapiro-Wilk test p > 0.05). Statistical test was conducted using paired t-test (p < 0.05) to test the pre-post data in both groups and independent sample t-test (p < 0.05) to test the effectiveness of the intervention compared to the control group. Results: Demographic data of respondents was homogenous. The mean score of individual and peer happiness before and after intervention in the IG increased. It means that respondents had better happiness. Meanwhile, in the CG there was a stagnation score of individual and peer happiness. Paired t-test in the IG showed that individual and peers’ happiness got the same p-value < 0.001 each. In the CG, individual happiness records p = 1.000 and peers’ happiness p = 0.083. Statistical tests conducted by Independent t-test to compare IG and CG had the p = 0.012 for individual happiness and p = < 0.001 for peers’ happiness. Conclusions: Laughter yoga has a significant effect to enhance individual and peer happiness compared to respondents who only carry out normal daily activities, especially for cancer patients undergoing therapy during the COVID-19 pandemic.
