Abstract

Periodic paralysis hypokalemia is a muscle weakness that can be caused by hyperthyroidism due to Graves’ disease. The prevalence reaches 10% of patients with periodic paralysis hypokalemia with symptoms of thyrotoxicosis. Graves’ disease is an immune system disorder that can cause other organ dysfunction, such as autoimmune hepatitis. Currently, the diagnosis of autoimmune hepatitis is still a challenge for clinicians due to limited supporting examination facilities, such as SMA, AMA, anti-LKM-1. This article discusses a case of a 34-year-old woman with limb weakness for 3 months, losing 4 kg of weight in 1 month, often feeling hungry, restless, having trouble sleeping, unable to stand the heat, and palpitation. Her urine was looked like a tea color for the last 10 days. Laboratory examination results showed an increase in alanine transaminase (ALT) levels 256 U/I and aspartate aminotransferase (AST) 142 U/I, accompanied by an increase in total bilirubin levels of 12.09 mg/dl, direct bilirubin 10.5 mg/ dl. Markers of hepatitis B (HbsAg) and anti-HCV were negative. The potassium level was 2.29 mmol/L and the antinuclear antibody test was 1.3 IU/ml. Thyroid-stimulating hormone (TSH) <0.01 uIU/ml, and FT4 5.46 mg/dl. Abdominal ultrasound showed chronic liver disease. Histopathology of liver tissue showed chronic hepatitis with inflammatory infiltration. The patient had been treated with thiamazole, potassium supplements, propranolol, and prednisone. As a result, the patient’s condition and liver enzyme tests improved

Abstract

Background: Osteoarthritis (OA) is the most common form arthritis in the world, and its prevalence is predicted to rise higher in the future due to increasing life expectancy and growing number of elderly population. With the emergence of new treatment options in the last several years, a better understanding of OA diagnosis and management is required by every physician in Indonesia. Methods: A panel of eight selected rheumatologists from the Indonesian Rheumatologist Association (IRA) developed recommendations based on key questions formulated by a steering committee from IRA. These recommendation materials were taken from several online databases such as PubMed, Science Direct, and Google Scholar. Level of evidence and grades of recommendation were then assigned, and each member of the panelist team will assign a score to express their level of agreement. Results: A total of 25 recommendations discussing the diagnosis, pharmacological and non-pharmacological therapies, as well as monitoring for OA were formulated. Conclusion: These recommendations can be used to help clinicians in accurately diagnosing OA and choosing the most suitable therapy for their patients. All recommendation statements were tailored to the clinical setting, facility, and drug availability in Indonesia.

Abstract

Introduction. COVID-19 is considered a new emerging infection due to SARS-CoV-2. CoronaVac refers to an inactivated SARS-CoV-2 vaccine obtaining EUL (Emergency use listing) from BPOM RI and WHO. This vaccine was previously reported to exhibit good efficacy in generating protective antibodies against SARS-CoV-2. Nevertheless, health workers in Indonesia might pose a high risk of COVID-19 exposure, leading to mortality. This study aims to identify the level of SARS-CoV-2 exposure to the CoronaVac vaccination in health workers at Dr. Saiful Anwar General Hospital, Malang. Methods. This study implemented analytical observational research with a prospective cohort approach on health workers at Dr. Saiful Anwar General Hospital, Malang, who received the 2nd dose of vaccination dose with a 14-day interval. All study subjects were observed for 6 months by examining the level of exposure to SARS-CoV-2 obtained from the results of nasopharyngeal and oropharyngeal swab examinations of SARS-CoV-2 RT PCR examination. Categorical data were tested by using Chi-square or Fisher exact. Results. From 184 samples, the SARS-CoV-2 exposure indicated several outcomes: positive 57 (31 %) and negative 127 (69%). Severity degrees of COVID-19 disease were asymptomatic 17 (29.8%), mild 37 (64.9), moderate 3 (5.3%), severe 0 (0%), and there were no mortality cases. The time (duration) of exposure to SARS-CoV-2 post-CoronaVac vaccination indicated that the highest number of participants with positive RT-PCR swab results post-vaccination was in the fifth month of the CoronaVac vaccination with 41 people. Clinical symptoms of SARS-CoV-2 exposure experienced by participants post the CoronaVac vaccination included pneumonia (3 people), cough and cold (27 people), fever (22 people), myalgia (17 people), and sore throat (11 people). There was no significant relationship p value > 0.05 between gender, age, comorbidities, and Ct value on SARS-CoV-2 exposure post CoronaVac vaccination in health workers at Dr. Saiful Anwar General Hospital, Malang. Conclusion. There was SARS-CoV-2 exposure of 31% post the CoronaVac vaccination in health workers at Dr. Saiful Anwar General Hospital, Malang.

Abstract

Lupus patients and their families require adequate support to manage the disease's progression. This community service aims to help the Kupu Parahita Indonesia Foundation provide strengthened moral and material support for lupus patients and their families at the Kupu Parahita Indonesia Foundation. Results: The cinematherapy planned by the PKNM and Community Service Team received much positive feedback from patients and families. The sharing session expressed that patients and their families are grateful for the opportunity to enjoy free recreation together. Enthusiasm comes with enthusiasm to meet people with fellow lupus. . In conclusion, cinematherapy can help the Kupu Parahita Indonesia Foundation provide lupus patients and their families with more substantial moral and material support.

Abstract

Selama pandemi Covid-19, pasien lupus dan keluarga dihadapkan dengan berbagai stresor dan tantangan yang dapat mempengaruhi dukungan sosial. Kegiatan pengabdian pada masyarakat ini bertujuan untuk mengoptimalkan dukungan sosial bagi pasien lupus dan keluarga pasca pandemi Covid-19 di Yayasan Kupu Parahita Indonesia. Metode yang digunakan dalam kegiatan pengabdian pada masyarakat ini adalah dengan melakukan Forum Group Discussion (FGD) untuk menggali kebutuhan pasien lupus dan keluarga akan dukungan sosial; menyelenggarakan pertemuan offline antara pasien lupus, keluarga, dan tenaga kesehatan pemerhati lupus; serta memberikan edukasi intens secara online. Hasil kegiatan menunjukkan bahwa dari kegiatan FGD, didapatkan permasalahan bahwa belum semua pasien lupus dan keluarga di Yayasan Kupu Parahita Indonesia mengenal dan memahami lupus dengan baik. Melalui kegiatan yang diselenggarakan secara offline maupun online, masalah yang didapat saat FGD teratasi, di mana pasien lupus dan keluarga lebih memahami lupus dan dengan sharing sesama pejuang lupus, kebutuhan dukungan sosial pasien lupus dan keluarga terpenuhi. Dapat disimpulkan bahwa pendekatan secara intens baik secara offline maupun online sangat diperlukan untuk mengoptimalkan dukungan sosial bagi pasien lupus dan keluarga.

Abstract

Kurangnya pengetahuan masyarakat tentang COVID-19, lupus, reumatik, dan alergi mempengaruhi keberhasilan terapi, penurunan angka kejadian, dan pencegahan kekambuhan. Tujuan pengabdian masyarakat adalah membangun desa-desa binaan tanggap COVID-19, lupus, reumatik, dan alergi di Malang oleh Tim Kelompok Kajian Lupus, Autoimun, Reumatik, dan Alergi (LAURA) Universitas Brawijaya. Warga desa binaan diberikan penyuluhan berupa seminar awam dan pelatihan tentang pertolongan awal pada penyakit COVID-19, lupus, reumatik, dan alergi, kemudian diminta mengisi kuesioner posttest untuk mengukur pemahaman. Desa-desa binaan diberikan thermo-gun dan wastafel untuk menerapkan protokol kesehatan. Satu bulan kemudian dievaluasi adanya kejadian COVID-19, lupus, reumatik, dan alergi di desa binaan. Hasil evaluasi menunjukkan rata-rata warga desa binaan memahami 78,3% materi yang diberikan dan menerapkan protokol kesehatan sesuai yang diajarkan saat penyuluhan. Dilaporkan tidak ada kejadian COVID-19 serta kekambuhan lupus, reumatik, dan alergi dalam satu bulan terakhir kegiatan. Kesimpulan: pembangunan desa-desa binaan di Malang meningkatkan tanggap warga terhadap COVID-19, lupus, reumatik, dan alergi.

Abstract

Understanding knowledge gaps is essential for developing more effective educational interventions that can bridge theory and practice. This study aimed to evaluate knowledge gaps in Systemic Lupus Erythematosus between pre-clinical students and junior doctors at a medical faculty. A cross-sectional study was conducted with 250 participants, divided equally between pre-clinical students and junior doctors (N=125 each). Variables such as gender, educational year, GPA, and study time were analyzed. Knowledge was assessed in five domains: pathophysiology, clinical manifestations, diagnosis, treatment, and complications. Linear regression analysis was used to identify factors associated with mean knowledge scores. No significant differences in gender distribution or study time (p = 0.899, p = 0.633, respectively). However, GPA scores were significantly higher among junior doctors (p < 0.001). Pre-clinical students scored higher than junior doctors in all SLE-related domains. GPA and study time were the strongest predictors of higher scores (Beta = 0.917, p < 0.001; Beta = 0.261, p < 0.001). GPA and study time significantly contributed to SLE knowledge scores, while gender and educational year did not. These findings suggest that academic performance and consistent study habits play critical roles in mastering SLE concepts among medical trainees.

Abstract

Background: Systemic Lupus Erythematosus (SLE) is a complex autoimmune disease requiring comprehensive patient knowledge for effective management. However, gaps in understanding and educational challenges persist among patients and caregivers. Aim: This study aimed to explore the knowledge, perceptions, and educational needs of SLE patients and their caregivers through a qualitative approach. Methods: Semi-structured interviews and focus group discussions were conducted with 15 patients and 10 caregivers from rheumatology clinics at Saiful Anwar General Hospital. Participants were selected using purposive sampling. Data were analyzed using thematic analysis, identifying key themes related to understanding of SLE, sources of information, knowledge gaps, and educational needs. Results: Participants demonstrated a basic understanding of SLE, but significant misconceptions were identified, particularly regarding the disease's multi-systemic impact. Healthcare providers were the primary source of information, though time constraints limited the depth of understanding. Patients and caregivers frequently turned to online sources, encountering challenges in identifying reliable information. Complex medical terminology and the emotional burden of SLE management were significant barriers. There was a clear preference for simplified, visual educational materials and a need for ongoing, continuous education and support. Conclusion: The study highlights persistent knowledge gaps and the need for enhanced educational strategies tailored to the needs of SLE patients and caregivers. Improving communication, providing reliable resources, and offering continuous education could significantly improve disease management and patient outcomes.

Abstract

Introduction: Caregivers of patients with Systemic Lupus Erythematosus (SLE) face significant emotional and psychological challenges due to the chronic and unpredictable nature of the disease. This study aimed to explore the anxiety experienced by caregivers in their daily caregiving roles. Methods: A qualitative, phenomenological approach was used to gain insights into the lived experiences of 15 caregivers providing daily care to SLE patients. Data were collected through in-depth, semi-structured interviews, and thematic analysis was conducted to identify key themes related to caregiver anxiety. Results: Five main themes emerged: (1) Emotional Burden and Anxiety—Caregivers reported constant worry and stress due to the unpredictability of SLE; (2) Impact on Personal Life and Relationships—Caregiving responsibilities led to feelings of isolation and strained relationships; (3) Coping Mechanisms and Resilience—Caregivers utilized social support, religious practices, and routines to manage their anxiety; (4) Perceived Lack of Professional Support—Caregivers felt overlooked by healthcare providers and expressed a need for more comprehensive support services; (5) Long-Term Outlook and Future Uncertainty—Anxiety about the future and their ability to continue caregiving as they aged was a significant concern. Conclusion: The study highlights the multifaceted nature of caregiver anxiety in the context of SLE. There is a critical need for healthcare systems to provide holistic support that addresses both the emotional and practical needs of caregivers. By enhancing caregiver support, the overall well-being of both caregivers and patients can be improved.
