Abstract

Background: Clinical leadership among nurse managers plays a vital role in ensuring the quality and effectiveness of hospital services. Nurse managers are responsible for integrating professional competence, managerial skills, and interpersonal collaboration to enhance patient outcomes and team performance. However, in Indonesia, there remains a lack of standardized and contextually relevant instruments to measure clinical leadership competencies. Objectives: This study aimed to develop and validate a clinical leadership competency assessment instrument specifically tailored to the Indonesian hospital context and to evaluate its feasibility for practical application in nursing management. Methods: The study employed a research and development design conducted in Tasikmalaya City. The sample consisted of 38 nurse managers selected through accidental sampling. The research process consisted of several stages: instrument design, expert validation, revision, and limited testing. Content validity was examined using the Content Validity Index to ensure conceptual and contextual relevance, while item validity and reliability were analyzed through appropriate statistical methods to ensure measurement accuracy and internal consistency. Results: The developed instrument included 49 items across five core dimensions: communication, collaboration, decision-making, team empowerment, and change management. Validation results showed a CVI of 0.86 (>0.80), and high reliability (Cronbach’s Alpha 0.950). Non-valid items were removed, resulting in a final instrument that is valid, reliable, and contextually appropriate for use by nurse managers. Conclusions: The instrument is valid, reliable, and feasible for assessing nurse managers’ clinical leadership competencies. Beyond its psychometric soundness, it provides a strategic framework to strengthen leadership accountability, improve team coordination, and enhance hospital service quality. The instrument can also serve as a valuable tool in hospital management and nursing education to guide professional development and promote evidence-based leadership practices

Abstract

A stroke is a pretty serious health disorder when not treated immediately. A common problem is hemiparesis, where the patient will have difficulty performing daily activities, such as walking, working, eating, drinking, and so on. Mirror Therapy is one of the rehabilitation therapies with the help of mirrors that can be done in patients with hemiparesis. This therapy can be claimed to increase muscle strength in hemiparesis. The purpose of this writing is to analyze the application of mirror therapy to increased muscle strength in stroke patients. The method used is systematic review with search engines elsivier, PubMed, and Proquest. The keywords used are mirror therapy, stroke, and muscle strength. The SLR process step uses PRISMA and produces 8 articles for analysis. The results of the study have been obtained that, the effect of the application of mirror therapy to increase muscle strength, results is a significant difference in motor skills before and after the therapy, this application is effective when performed for 15–30 minutes every 3 times in a week, for 1-2 months or 12-20x exercise in 1-2 months. Mirror therapy can be applied to all stroke patients whether ischemic or hemorrhagic without visual impairment. Therefore, it is expected that mirror therapy can be performed in hospitals or home care as a patient rehabilitation program.

Abstract

Background: Nurse managers play a crucial role in shaping the quality of nursing care and organizational culture. However, their leadership characteristics often remain implicit and deeply embedded within personal experiences and professional narratives. Archetypal patterns provide a lens to uncover symbolic dimensions of leadership that influence managerial practices in clinical settings. Objective: This study aimed to investigate the leadership characteristics of nurse managers through archetypal patterns using a narrative inquiry approach in clinical practice. Methods: A qualitative narrative inquiry design was employed. Data were collected from [X] nurse managers working in [hospital/clinical setting] through in-depth interviews, reflective journals, and field observations. Narrative data were analyzed thematically, guided by archetypal frameworks (e.g., caregiver, ruler, warrior, sage), to identify underlying leadership patterns. Results: Findings revealed four dominant archetypal patterns shaping nurse managers’ leadership: the Caregiver (emphasizing compassion and protection), the Ruler (prioritizing structure, authority, and decision-making), the Warrior (focusing on resilience and advocacy), and the Sage (highlighting wisdom and reflective practice). These archetypes manifested differently depending on situational demands, organizational context, and personal values. Conclusion: Nurse managers’ leadership is characterized by dynamic interplay between multiple archetypal patterns. Recognizing these patterns provides new insights into leadership development, mentoring, and strengthening clinical governance. Future research should explore archetypal leadership across diverse healthcare contexts to support transformative nursing leadership.

Abstract

Family members of patients receiving mechanical ventilation in the Intensive Care Unit (ICU) often experience significant emotional and psychological distress. The highly technological environment, limited patient access, and uncertainty regarding prognosis can profoundly affect family experiences and coping processes. This study aimed to explore family experiences during ICU care for patients receiving ventilator support. A qualitative descriptive study was conducted using Focus Group Discussions (FGDs) involving six family members of mechanically ventilated patients in the ICU of Welas Asih Regional General Hospital. Participants were selected using purposive sampling. Data were analyzed using thematic analysis to identify recurring patterns and meanings in family experiences. Five major themes emerged: (1) emotional shock at the initiation of ventilator use, characterized by fear, anxiety, sadness, and trauma; (2) uncertainty and psychological burden related to unclear diagnosis, limited patient interaction, and lack of regular information; (3) generally positive communication with healthcare professionals, though families desired more structured and routine updates; (4) firm hopes for patient recovery, rapid clinical response, and humane care; and (5) the need for comprehensive support, including emotional, informational, spiritual, social, and financial support. Family experiences during ventilator-assisted ICU care are complex and multidimensional. Implementing a comprehensive family-centred care approach, particularly through structured communication and holistic family support, is essential to improve family well-being and the quality of critical care services.
