Abstract

This study aims to examine the procedures for the release of medical records information to third parties at Wava Husada Hospital, ensuring compliance with applicable regulations to maintain the security and confidentiality of patient data. The research method used is a qualitative approach, including direct observation, in-depth interviews, and analysis of relevant documents. The results show that the procedure for releasing medical record information involves several important steps, including filling out an application form, submitting photocopies of official identification, and, if necessary, a notarized power of attorney. This process involves several related parties, such as the Medical Records Department, doctors or medical personnel, and ward staff working in inpatient or outpatient units. The main purposes for third-party requests for medical records information include insurance claims, transfer of treatment to ensure continuity of care, and administrative or consultation needs.

Abstract

Ketidaklengkapan catatan medis dapat berdampak negatif pada keselamatan dan kualitas perawatan pasien, khususnya dalam kasus bedah. Di RS Wava Husada, indikasi ketidaklengkapan catatan medis menimbulkan kekhawatiran tentang kualitas dokumentasi medis. Penelitian ini menggunakan metode deskriptif dengan pendekatan kuantitatif deskriptif. Sebanyak 25 berkas catatan medis pasien bedah dievaluasi menggunakan checklist. Data kuantitatif dianalisis dengan statistik deskriptif, sementara data kualitatif diperoleh melaui observasi. Hasil audit menunjukkan bahwa untuk identitas pasien di setiap formulir rekam medis sudah terisi 100% lengkap, sedangkan ketidaklengkapan berkas tertinggi hingga terendah terdapat pada komponen autentifikasi (20%), komponen identifikasi penting(16%), komponen pendokumentasian (16%), komponen pelaporan penting 8%.Dengan temuan ini perlunya perbaikan dalam sistem pencatatan, termasuk program pelatihan berkelanjutan dan penyediaan waktu yang memadai bagi petugas (Ardiyanto,2024).

Abstract

This study analyzed the efficiency of bed use using the Barber Johnson graph as an effort to reduce the length of waiting time for inpatients in the emergency room of Wava Husada Kepanjen Hospital. The research method used a mixed design with quantitative data collection to measure efficiency indicators such as BOR, LOS, TOI, and BTO, as well as qualitative interviews to explore the factors that cause long waits. The results showed high bed occupancy rates and bed turnover, which contributed to the length of patient wait times. This study suggests optimizing bed management and improving organizational processes to accelerate patient services. These findings provide practical implications for hospital management to improve patient flow and emergency service efficiency.

Abstract

The completeness of diagnosis is something that medical recorders must pay attention to. The completeness of diagnosis is very important in the fields of clinical data management, cost reimbursement, and health care and services. INA-CBG's (Indonesia Case Groups) is an application used by all hospitals, one of which is Wava Husada Kepanjen Hospital, in submitting claims using a package system based on the illness suffered. The method of implementing this activity involves explaining the necessary updates to the diagnosis coding system, which will be automatically bridged to the INA-CBGs application. The activity was conducted at Wava Husada Hospital, specifically in the CASE MIX unit. The method of implementing this activity was to explain the parts that needed to be updated or corrected, conduct hands-on practice with each user, and hold a question and answer session regarding obstacles in its use. The socialization activity proceeded smoothly, and staff members felt assisted by the changes made to the system, which had been adapted to user needs. However, during implementation, staff members still need to perform double checks to anticipate errors related to the assignment of diagnosis codes. This is because the system lacks prompts to remind staff members about the completeness or accuracy of diagnosis code entry, resulting in frequent errors made by CASE MIX staff members.
