Abstract

Plasmodium vivax malaria remains a major health problem in tropical countries due to its high relapse rate and various hematological manifestations that may worsen the patient’s condition. This case report aimed to describe the clinical manifestations, laboratory findings, and evaluation of therapeutic rationality in a patient with Plasmodium vivax malaria during hospitalization. The study was conducted using a case report method involving a 22-year-old male patient who presented with periodic fever accompanied by chills for one week after traveling from a malaria-endemic area. Laboratory examination revealed microcytic hypochromic anemia, leukopenia, mild thrombocytopenia, and malaria parasites identified on peripheral blood smear examination. The patient received supportive therapy including intravenous Ringer Lactate fluid, paracetamol, omeprazole, domperidone, and ferrous sulfate, as well as definitive therapy consisting of dihydroartemisinin-piperaquine (DHP) for three days and primaquine for 14 days. The evaluation showed that most of the therapies administered were in accordance with national guidelines and WHO recommendations for uncomplicated P. vivax malaria. However, the absence of glucose-6-phosphate dehydrogenase (G6PD) enzyme activity testing prior to primaquine administration was considered one of the limitations in patient management. During hospitalization, the patient showed clinical improvement without severe complications. The management of P. vivax malaria requires a comprehensive approach including early diagnosis, rational therapy, clinical monitoring, and patient education to prevent relapse and improve therapeutic outcomes.

Abstract

Stunting is a disruption in the growth and development of children within 1000 days of life due to chronic malnutrition and recurrent infections which is characterized by their length or height being below standard. Calcium and vitamin D are nutrients that influence bone mineralization. Good bone mineralization during growth allows optimal linear growth. This study aims to determine the relationship between providing food containing calcium and Vitamin D with the incidence of stunting in children under five in Sijunjung Regency. This research is a quantitative research with an analytical observational design with a cross sectional approach method. The number of samples in this study was 36 people with a distribution of 13 men (35.71%) and 23 women (64.29%). There were 5 (38.46%) male toddlers and 9 (39.13%) female toddlers who experienced stunting. Based on data analysis carried out using chi-aquare (X2), it was found that p=0.021 and p=0.018 for the relationship between calcium and vitamin D intake and the incidence of stunting. Based on this, it shows that p < 0.05. Based on the research results, it can be concluded that there is a significant relationship between providing food containing calcium and vitamin D and the incidence of stunting in toddlers in Sijunjung Regency.

Pneumonia is inflammation or infection of the respiratory tract which causes pain and makes it difficult for sufferers to breathe because there is fluid in the form of pus in the alveoli sacs. Pneumonia is the highest cause of death in the world, especially in children under 5 years of age. Pneumonia caused by bacterial infections is treated using antibiotics. Antibiotics that are used inappropriately can cause bacterial resistance, resulting in worsening infections, an increase in infections, longer hospital stays, and increased health service costs. The aim of this research is to determine the quantitative evaluation of antibiotic use in pediatric patients with pneumonia at RSI Siti Rahmah Padang. This research is observational analytic with a retrospective design. The samples used in this study were pediatric pneumonia patients aged ≤18 years who cmet the inclusion criteria. Quantitative analysis was carried out by calculating the quantity of antibiotics used by pediatric pneumonia patients based on ATC/DDD criteria. The research results showed that the total DDD was 51.69 DDD/100 patient-days with the highest DDD value for the antibiotic ceftriaxone with a value of 19.44 DDD/100 patient-days.
