Abstract

Abstract

Type II Diabetes Mellitus is a chronic disease characterized by an increase in blood sugar levels at times equal to or more than 200 mg/dL, and fasting blood sugar levels above or equal to 126 mg/dl. Diabetes Mellitus as a non-communicable disease is the number one cause of death in the world. It is estimated that 422 million adults lived with diabetes mellitus in 2014. Based on the 2018 Riskesdas results, the prevalence of type 2 DM based on doctor's diagnosis in Indonesia was 2.0%, whereas in 2013 it was only 1.5%. The aim of this research is the application of cinnamon bark decoction to reduce blood sugar levels in diabetes mellitus sufferers. This research was conducted in the working area of the Lubuk Basung Community Health Center with a Quasi-Experimental research type with the research design being a one group pretest-posttest design. This research was conducted for 3 days on all clients suffering from Diabetes Mellitus who met the inclusion criteria. The research results showed that the average pretest blood sugar with cinnamon bark decoction in respondents was 215.88 mg/dL. The posttest blood sugar results for respondents with cinnamon bark were 163.38 mg/dL.The difference in reducing blood sugar levels in respondents with cinnamon bark was 52.5 mg/dl.So it can be concluded that there is an influence on sugar levels in diabetes mellitus patients by giving cinnamon decoction.

Abstract

Inconsistencies in diagnosis writing and coding accuracy for infection cases remain common. Initial observation of 10 records at Dr. Rasidin Padang Hospital showed 60% inconsistency in diagnoses and 60% in coding. This study aimed to assess the consistency between medical summaries and discharge plans in relation to coding accuracy. Conducted from February to March 2025 using a descriptive quantitative method, the study involved 53 samples from a population of 114, selected via simple random sampling. Data were collected using a checklist and analyzed with univariate analysis via Computerisasi. Results showed 45.3% inconsistent diagnoses and 39.6% inaccurate codes. The issues were linked to suboptimal medical record completion by physicians and coders, affecting data quality and coding accuracy. Hospitals are encouraged to enhance supervision, guidance, and regular training to improve understanding of documentation and coding standards. Keywords: Consistency, Accuracy, Medical Summary, Discharge Planning

Abstract

Congenital hypothyroidism is one of the causes of mental retardation in children that can be prevented if recognized and treated early. Thyroid hormones play a role in the development of the central nervous system. It is known that 95% of CH do not show typical clinical signs and symptoms at birth and the duration of early intervention to prevent mental retardation is short. Therefore, most developed countries have conducted neonatal screening programs for early detection of CH. The purpose of this study was to determine the diagnosis and management of congenital hypoteroid based on medical record analysis. This research method is a cross-sectional descriptive study with a quantitative approach to patient medical records. Data were analyzed in accordance with the standard diagnosis and management of congenital hypoteroid issued by the Pediatric Association in 2017. The clinical practice guidelines explain that congenital hypothyroidism can be transient or permanent and is classified according to the location of the disorder: primary (in the thyroid gland) or secondary (in the pituitary and/or hypothalamus); severity of hypothyroidism: (serum TSH levels > 100 mIU/L are considered severe; and age of onset of hypothyroidism. The most common form is permanent primary CH (high serum TSH levels) due to thyroid dysgenesis. Permanent CH requires lifelong treatment while transient CH does not. The results of the study have met clinical guidelines, namely CH screening in newborns is positive if the TSH level is ≥ 20 mU/L. Infants with positive screening results must be confirmed by re-checking serum TSH and FT4. The diagnosis of CH is made when TSH levels are high and FT4 is low. In non-screened infants, the diagnosis is made through clinical symptoms and serum TSH and FT4 examination.
