Abstract: Coding is a classification of a patient's disease, the implementation of which there are still errors both in terms of writing a diagnosis that is unclear to and incomplete. Based on the initial survey conducted, it was found that 15 (75%) of the writing of the diagnosis were unclear, 12 (60%) of the writing of the diagnosis were incomplete, and 14 (70%) incorrect coding of the 20 medical record files. This study aims to determine the accuracy of the coding of injury cases based on ICD-10 at Ropanasuri Hospital, Padang. This research was conducted at the Hospital for Special Surgery Ropanasuri Padang,. The population was 74 medical record files, using the total sampling technique. This research is by means of observation, using a checklist table, the data is processed using a computerized and analyzed by univariate. The results showed that the clarity of writing the diagnosis of injury cases was not clear 49 (66.2%), the completeness of writing the diagnosis of injury cases was incomplete 51 (68.9%), the accuracy of coding the injury cases was not accurate 17 (22.9%). The results of the research on the implementation of coding, especially in injury cases, have been very good where the number of inaccuracies in the code of injury cases is small, but the clarity of writing the diagnosis and completeness of the diagnosis is still not good. Researchers suggest to doctors to improve diagnosis writing and coder to be more active and thorough in coding in order to produce good reporting so as to improve the quality of the hospital.Keywords: Accuracy of coding, Medical Records, ICD-10, Clarity, Completeness

Implementation of e-puskesmas in Padang City health centers is expected to contribute to health centers in improving the effectiveness and efficiency of health services. The results of observations of e-puskesmas applications are not in accordance with the needs of health center staff (from registration to reporting). To assess the benefits of implementing an e-puskesmas one of the evaluation methods that can be used is an evaluation based on the user's perception using the PIECES method. This research is a qualitative case study approach. This research was conducted at the Padang City health center from February to May 2018. The informants in this study amounted to 10 people taken by purposive sampling technique, data analysis results of Focus Group Discastion (FGD) will be carried outprocess transcription, data reduction, coding, categories, presentation data and data interpretation. Aspects of Performance input of patient data and response time is faster, in the aspect of information there are still obstacles, namely the resulting report is still limited to LB1 reports, still constrained for data briging with P-care BPJS, on theaspect the economic use of e-puskesmas still does not provide benefits which is large in terms of reporting because there are still data input activities and reporting that must be done manually. In terms ofaspects control, the implementation of e-puskesmas still faces constraints in terms of data security because all officers can access the e-puskesmas with the same user and password. For theaspects of efficiency the e-puskesmas users are still constrained by the workload of recording and reporting that is still done manually to the City Health Office, theaspect service needs to be the development of the modules in the e-puskesmas such as the LB2 to LB 4 reporting menu module. It is necessary to develop a menu / module on the e-Puskesmas system, the need for a data security system by providing login access according to the user level and the need for communication with BPJS related to briging data.

Based on the initial survey conducted by researchers, it was found that there were differences in perceptions between code verifiers and BPJS verifiers about medical action codes. In February, there were 15 disease codes with different perceptions, because the hospital had to follow the code based on the BPJS. The research objective was to analyze the perception of medical action codes by verifiers at Bunda Medical Center Hospital.This type of research is qualitative with a phenomonological approach. Research is conducted by in-depth interviews with 1 medical support person, 1 medical committee person, 1 code verifier, and 1 BPJS verifier, using a voice recorder and using interview guidelines. Data were analyzed using the Collaizzi method.The results showed that what hampered the implementation of the verification of medical action codes was that the doctor's writing was difficult to read by the verifier. There are differences in the perceptions of the verifier about the medical action code, where the verification of the medical action code is based on the number of resources used in the service, while the BPJS verifier thinks the medical action code is based on the level of severity or severity level. The conclusion in this study is that the educational qualifications between the BPJS verifier and the verification of medical action codes are not yet appropriate, then the BPJS verifier verifies the code based on the severity level it should be based on the most spent resources.

Coronavirus (CoV) adalah keluarga besar virus yang menyebabkan penyakit mulai dari gejala ringan sampai berat. Berdasarkan Surat Edaran walikota, maka semua sekolah di Kota Padang diaktifkan dengan tetap memperhatikan protokol kesehatan. Tujuan dari pengabdian ini adalah memberikan pemahaman kepada anak sekolah tentang pentingnya penggunaan masker dalam lingkungan masyarakat sesuai anjuran untuk penerapan protokol kesehatan sebagai upaya meningkatkan kesadaran tentang pentingnya pencegahan dini dari virus COVID-19. Metode kegiatan yang digunakan dalam kegiatan pengabdian masyarakat ini adalah Sosialisasi Pentingnya Penggunaan Masker Pada Masa Pandemi COVID-19 sebagai upaya penerapan protokol kesehatan demi terciptanya pencegahan terhadap virus Covid-19 pada masyarakat dalam menghadapai situasi pandemik saat ini. Pelaksanaanya adalah melakukan presentasi kesekolah dengan memberikan sosialisasi kepada siswa/anak sekolah dan juga memberikan brosur agar mahasiswa memahami pentingnya penggunaan masker selama melaksanakan pembelajaran disekolah. Pada dasarnya sebenarnya siswa/siswi memahami betul pentingnya menggunakan masker dimasa pendemi COVID-19 seperti sekarang apalagi di dilingkungan sekolah. Namun terkadang masih banyak siswa/siswa yang tidak ingin memakai masker dengan alasan susah bernafas, panas dan mereka berada dalam ruangan yang cukup lama. Dari 10 SMA yang akan dilakukan sosialisasi hanya 5 SMA yang bersedia untuk melakukan sosialisasi yaitu: SMA SIMA,PGRI 3,SMAN 6,PGAI, dan Pembagunan. Kegiatan pengabdian ini dapat meningkatkan pengetahuan serta pemahaman siswa/anak Sekolah Menegah Atas (SMA) mengenai pentingnya pengunaan masker dimasa pandemi COVID-19. Sehingga untuk selanjutnya kualitas kehidupan masyarakat akan meningkat dan penyebaran COVID-19 bisa di kurangi. Kata kunci: Masker, Pembejaran, Covid-19

Mortality reports are a reference to see the number of death in hospital due to certain diseases. The current mortality report is still experiencing problems, namely the processing of reporting data is still manual, there is no determination of the cause of death code because there is no death certificate. This study aims to determine the analysis of the implementation of the mortality report at the hospital Dr. M. Zein Painan in 2021. Type of research is qualitative with a phenomenological approach. Data analysis using collaizi method. Research was conducted by in-depth interviews with 4 informants, namely reporting officers, head of the medical record room, coder and death certificate issuance officers. Results of the study indicate that the implementation of the mortality report has experienced problems including insufficient number of employees resulting in double jobs, staff knowledge is still minimal because they have never attended training, there are no policies, inadequate facilities and infrastructure and evaluation monitoring has not been carried out, thus affecting the output results. Implementation of mortality report has not been carried out completely because officers prioritize reports that have a direct impact on services. Therefore, it is necessary to recruit reporting officers, develop SIM RS for processing mortality data, make policies on mortality reports and evaluate the performance of officers on a regular basis.

Pengelolaan dokumen dengan menggunakan sistem yang berbasis komputer/elektronik di sektor kesehatan yang sedang menjadi trend global adalah rekam medis elektronik (RME). Pihak rumah sakit, khususnya rekam medis bagian assembling melakukan pemantauan kualitas mutu berkas rekam medis dengan melaksanakan analisa kuantitatif untuk mengetahui kelengkapan suatu berkas rekam medis. Tujuan penelitian ini adalah untuk mengetahui pelaksanaan analisa kuantitatif rekam medis berbasis elektronik di RSUP Dr. M. Djamil Padang. Jenis penelitian ini adalah penelitian deskriptif cross-sectional dengan pendekatan kuantitatif. Sampel pada penelitian ini berjumlah 182 rekam medis. Berdasarkan hasil penelitian menunjukkan analisis kuantitatifdiantara keempat komponen yaitu identifikasi pasien, kelengkapan laporan/catatan yang penting, auntentifikasi penulis, dan catatan yang baik diperoleh rata-rata presentase kelengkapan sebesar 95%,kelengkapantertinggi ditemukan pada komponen identifikasi pasien sebesar99,5%, sedangkan persentasekelengkapan terendah ditemukan padakomponen kelengkapan laporan/catatan yang penting sebesar89%. Sebaiknya dilakukan evaluasi dari hasilanalisis kuantitatif guna mengurangiketidaklengkapan rekam medis agar dapat mendukung dalam pengambilansuatu keputusan.

Coronaviruses (CoV) are a large family of viruses that cause illness ranging from mild to severe. Based on the mayor's circular, all schools in the city of Padang are activated while still observing health protocols. The purpose of this service is to provide understanding to school children about the importance of using masks in the community according to the recommendations for implementing health protocols as an effort to increase awareness about the importance of early prevention of the COVID-19 virus. The method of activity used in this community service activity is the dissemination of the importance of using masks during the COVID-19 pandemic as an effort to implement health protocols in order to create prevention of the Covid-19 virus in the community in dealing with the current pandemic situation. The implementation is to do presentations at school by providing socialization to students/school children and also providing brochures so that students understand the importance of using masks while carrying out learning at school. Basically, students really understand the importance of wearing masks during the COVID-19 pandemic, especially in the school environment. But sometimes there are still many students who don't want to wear masks on the grounds that it's hard to breathe, it's hot and they are in the room for a long time. Of the 10 high schools that will be socialized, only 5 high schools are willing to do the socialization, namely: SMA SIMA, PGRI 3, SMAN 6, PGAI, and Development. This service activity can increase the knowledge and understanding of high school students/children about the importance of using masks during the COVID-19 pandemic. So that in the future the quality of people's lives will improve and the spread of COVID-19 can be reduced.

Qualitative and quantitative analysis often found the number of incomplete medical records, analytical reports based on daily recaps, manual data input, and processing of the percentage of data that did not exist. So, medical record analysis requires database Microsoft Excel Fast and accurate Excel VBA programming language. The purpose of this study was to determine the database design for qualitative and quantitative analysis of inpatients using Microsoft Excel in the RSJ. Prof. Hb Saanin Padang. The type of research used is Research and development. In May – June 2022. The number of informants is 2 people and this study uses in-depth interview guidelines. The results showed that in the qualitative analysis database component there were 6 components and 15 data, while in the quantitative there were 4 components and 30 data. The person in charge of filling out the database is one officer. The database design uses UML including, Usecase, class and activity with VBA Excel programming language. The use of a database using Microsoft Excel has been tested and is in accordance with the hospital format. Based on the results of the study, it can be concluded that the database design is in accordance with the hospital format. It is hoped that the database design can be used by hospitals and this research can be continued by further researchers.

A medical resume is a summary of all services received by patients during treatment or treatment carried out by health workers. The incompleteness of filling in inpatient medical resumes in hospitals still exists such as lack of attitude and knowledge of health workers, busyness of doctors, absence of SOPs, disagreement with the time limit for filling out medical resumes, which is 1x24 hours. The purpose of this study was to describe the attitudes and knowledge of medical officers to the completeness of filling out an inpatient medical resume. The data used is secondary data, then the data is analyzed by looking for similarities, dissimilarities, views, comparisons and summaries of several research results. The results of the study from 10 journals showed that there were still incomplete medical resumes, due to the lack of knowledge and attitude of officers regarding the importance of completing medical resumes, the unavailability of SOPs for filling out resumes, busy doctors serving patients, resulting in limited time to complete them and the discipline of the doctor in charge. which is still low. From the researcher's analysis, it was concluded that the high number of completeness of completing inpatient medical resumes in hospitals was due to undisciplined officers and the lack of knowledge of officers about filling out resumes. In order to increase the completeness of filling in inpatient medical resumes in hospitals, health workers need to cooperate with each other in filling out medical record documents, especially medical resume sheets in order to create a good, complete and accurate filling.

Internal reports are reports that are used and utilized by the hospital. Based on the initial survey that the researchers conducted, it was found that there were delays in collecting outpatient internal reports and processing was still done manually. The purpose of this study was to determine the design of an outpatient patient reporting database application based on Microsoft Excel's Visual Basic For Application (VBA) in Mental Hospitals Prof Hb Saanin Padang. The method used is Research and Development. The results of the study found that the flow was in accordance with the flow of medical record documents. The basic patient data components used are 20 basic patient data components and 7 outpatient internal report components. The person in charge of recording and reporting outpatient care is 1 officer. The database application design uses UML visual modeling with the types of use case diagrams, class diagrams, and activity diagrams and the programming language is VBA Excel. The use of database applications has been effective and speeds up report processing with usage results of 03.60 minutes. So it can be concluded that the database application design has been effective and in accordance with the needs of the hospital.
