Abstract

Background: Pancreatic β-cells compensate for the increased insulin demand due to increasing glucose levels during pregnancy or in gestational diabetes mellitus (GDM). The enzyme that regulates the rate of gluconeogenesis production is PEPCK. Circulatory and metabolic disturbances in the brain caused by hyperglycemia could affect brain-derived neurotrophic factor (BDNF) expression as an agent for neurodevelopment. Objective:  The purpose of this work is to ascertain the impact of combining nanoemulsions of Rosmarinus officinalis and Centella asiatica on the expression of BDNF and PEPCK. Methods: This study was a true experimental laboratory using zebrafish larvae. The larvae were exposed to 3% glucose and a combination of nanoemulsion extracts. The study sample of zebrafish larvae at 72 hpf (hour post fertilization) was divided into control, glucose, treatment 1, 2, and 3, with the combination of Rosmarinus officinalis and Centella asiatica nanoemulsions at 2.5 μg/ml, 5 μg/ml, and 10 μg/ml, respectively. PEPCK and BDNF expression were measured by the real-time PCR method. Results: The expression of PEPCK increased significantly in the glucose group (3.05 ± 0.27). The combination of Rosmarinus officinalis and Centella asiatica nanoemulsions significantly decreased PEPCK (p-value 0,001 < 0.05). The BDNF level did not significantly differ (p-value 0.253 >0.05) among groups, but the highest level (1.06 ± 0.60) was gained in the T2 group, which received an embryonic medium (EM), 3% glucose, and the combination of Rosmarinus officinalis and Centella asiatica nanoemulsions at 5μg/ml. Conclusion: The combination of Rosmarinus officinalis and Centella asiatica nanoemulsions significantly reduces PEPCK expression but does not affect BDNF expression.

Abstract

Hyperbilirubinemia is a common neonatal condition requiring prompt treatment to prevent complications. While phototherapy is the standard approach, it has limitations and potential side effects, such as diarrhea, dehydration, skin rashes, retinal damage, hyperthermia, extended hospital stays, maternal-infant separation, and disruptions in breastfeeding. This study aimed to evaluate the effectiveness of Field massage as a complementary therapy to phototherapy in reducing serum bilirubin levels in neonates. Field massage is a gentle manipulation technique performed on the face, chest, abdomen, limbs, and neck to stimulate the vagus nerve and enhance metabolic processes, which is believed to help lower serum bilirubin levels and has shown no negative side effects. This research method used a quasi-experimental study conducted at Abdoel Wahab Sjahranie Hospital from March to June 2024 involving 20 neonates divided into two groups: a treatment group that received Field massage and phototherapy (n = 10) and a control group that only received phototherapy (n = 10). Field massage was administered twice daily at 07:00 and 16:00 for 15–20 minutes, at least one hour after feeding. Statistical analysis was conducted using the Paired T-Test to assess changes in bilirubin levels before and after treatment, while the Independent T-Test was used to compare outcomes between the treatment and control groups. The results of the study showed serum bilirubin levels decreased significantly in both groups, with a greater reduction observed in the treatment group (15.28 ± 3.01 mg/dL to 7.09 ± 1.60 mg/dL, p = 0.000) compared to the control group (15.94 ± 2.88 mg/dL to 8.63 ± 1.57 mg/dL, p = 0.000). Post-intervention analysis confirmed the superior efficacy of the combined therapy (p = 0.043). These results suggest that combining field massage with phototherapy is more effective in reducing bilirubin levels than is phototherapy alone. It was concluded that Field massage enhances phototherapy’s effectiveness in managing neonatal hyperbilirubinemia. Future research should include larger samples and explore additional therapies to reduce bilirubin. Examining phototherapy duration, hospitalization length, and bowel movement frequency could provide deeper insights.

Abstract

Adolescent girls are a vulnerable group to nutritional problems, particularly Chronic Energy Deficiency (CED), which can affect reproductive health and the quality of future generations. One of the main contributing factors to CED is the lack of knowledge and attitude toward nutrition. Video media is considered an effective educational tool as it combines engaging visual and audio elements. This study aimed to determine the effect of video media on improving the knowledge and attitudes of adolescent girls regarding CED at SMAN 4 Samarinda. The study employed a pre-experimental one-group pre-test post-test design with a sample of 39 eleventh-grade students selected through purposive sampling. The intervention consisted of a ±10-minute educational video on CED shown twice a week. Data were collected using questionnaires and analyzed using univariate and bivariate analysis with the Wilcoxon test. Results showed that most respondents were 16 years old (64.1%) and had a below-normal Body Mass Index (84.6%), indicating a risk of CED. The most common physical activity was participation in scouting (35.9%), followed by dance and volleyball. After the intervention, there was a significant increase in knowledge, from a median score of 10.00 to 13.00 (p = 0.002), and in attitude, from a median score of 31.00 to 33.00 (p = 0.001). These findings indicate that video media is effective in improving adolescents' understanding of CED. However, its effectiveness may also be influenced by age and prior access to information. Therefore, video media is highly recommended as a nutritional education tool in adolescent health programs.
