Abstract

Introduction: The quality of medical record documents is one of the indicators of the quality of hospital services that can be seen from the completeness of a medical record document.  The purpose of this study is to determine the relationship between management elements and the quality of outpatient medical record documents at Nashrul Ummah Lamongan Islamic Hospital. Methods: The design of this study used correlation analytics with a cross-sectional approach. The sample of this study was 60 respondents using random sampling techniques. The independent variable in this study is the management element, while the dependent variable in this study is the quality of medical record documents. Data was collected using a questionnaire, which included knowledge, SOPs, infrastructure facilities, and a checklist of completeness of medical record documents and observations. The analysis data used the chi-square test with a significance level of p<0.05. Results: The results showed that more than a part (63.9%) of well-informed men had complete medical records, and more than a proportion (79.2%) of poorly knowledgeable men had incomplete medical records. More than a part (69.0%) of those who comply with the SOP have complete medical records, while more than a part (74.2%) of those who do not comply with the SOP have incomplete medical records. More than a part (72.4%) of good infrastructure facilities have complete medical records, while more than a part (77.4%) of poor infrastructure facilities have incomplete medical records. The results of the data analysis stated that there was a relationship between the elements of man, method, and material management to the quality of outpatient medical record documents at the Nashrul Ummah Lamongan Islamic Hospital, with values of p=0.001 and p=0.000 <0.05. Conclusion: It is hoped that the results of this study can be material for improvements in human resources, SOPs, and infrastructure facilities related to medical record documents. Abstract

Introduction: A medical resume is a summary of the medical services provided by a health professional, especially a doctor, during treatment until the patient is discharged, either alive or dead. The purpose of this study is to determine the percentage and factors that cause the completeness of the medical resume of maternity patients at RSI Nashrul Ummah Lamongan. Methods: The design in this study is a combination of sequential explanatory research that combines quantitative and qualitative research methods sequentially. The number of samples is 72 medical resumes for the End of the Year 2022. Results: Almost all (97%) complete patient identification fills out and a small part (4%) of 3 incomplete medical resumes. The completeness of filling out important reporting is all (100%) fully filled and a small part (8%) of 6 medical resumes are incomplete. The completeness of filling out the authentication is almost entirely (99%) complete and a small part (3%) of 2 medical resumes are not filled out completely. The completeness of documentation based on the recording is clearly read on the medical resume sheet which is complete (100%) complete as many as 73 medical resume sheets and a small part (1%) is incomplete. Conclusion: The factor causing the delay in filling out a medical resume is time constraints caused by busyness and other work demands from the doctor in charge. The socialization of filling out the medical resume as well as Monitoring and Evaluation should be carried out to the maximum. Keywords: Health worker, Knowledge, Occupational Health and Safety (K3)

Abstract

Introduction:     Time     Wait    Registration    Treat    Roads     at the hospital     Soegiri    Lamongan    known    from     30     they respond     The     observed    ,     show    that     19     they respond    or  63,3% tidak sesuai dengan standar (≤60 menit). This study aims to determine the relationship between management elements (Man, Method, Information and Time) and waiting time in outpatient registration of Soegiri Lamongan Hospital. Methods: This study used a cross sectional method, with an observational analytical design using an accidental sampling technique obtained from 100 respondents. The independent variable is the management element (Man, Method, Information and Time), and the dependent variable is the waiting time for outpatient registration. The instrument used management elements (Man, Method, Information and Time) using a closed questionnaire, waiting time using an open questionnaire. Results: The research found that the management elements of Man were 51% competent, Method 77% registered via Offline, Information 59% informative, Time 58% fast, and more than some patients stated that the waiting time was not suitable as many as 77 respondents (77%). This study uses the SPSS 25 For Windows program using the Chi Square test with a value of α=0.05 obtained a value of Man p = 0.000, Method p = 0.001, Information p = 0.000, Time p = 0.006 meaning that there is a relationship between management elements (Man, Method, Information and Time) to the waiting time in the outpatient registration of Soegiri Lamongan Hospital. Conclusion: It is hoped that the outpatient unit will further increase the speed of waiting time in outpatient registration through improving the competence of registration officers by taking courses, participating in various trainings, and taking competency certification tests. Abstract

Introduction: Management Information System (SIM) is one of the most important assets in the era of globalization. The role of technology makes information processing easier because processing is very necessary so that the information produced can be useful for its users, especially in pharmaceutical warehouse drug inventory. The results of the initial survey through interviews showed that there were problems at dr. Suyudi like the discrepancy between the data on drug stock inventory in the computer inventory system and the drug stock in the pharmaceutical warehouse. The purpose of the research is to analyze human resources, SPO, Data, Software, Hardware of drug inventory management information systems in pharmaceutical warehouses. Methods: Descriptive qualitative research design with purposive sampling technique. Data collection instruments are in the form of interview guidelines, observation and document review. Results: The results of human resources research (can run smoothly if supported by human resources who have adequate quality and capacity). SPO (SPO is available but the applicable SPO has not been carried out optimally by pharmacists). Data (the existence of data can help officers in minimizing errors and losses). Software (can help ease the work because the supply of drug data is already stored on the computer but the obstacle is often errors and slow). Hardware (important to support the smooth flow of drug supply activities through the hospital SIM application and facilities are in accordance with standards). Conclusion: Based on the results of the study, it is expected that the management will evaluate the supply of drugs in the pharmaceutical warehouse in using the hospital driver's license.
