Abstract

Coronavirus Disease 2019 (COVID-19) is a disease which caused by Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2). In December 2019, this disease was encountered in Wuhan, China. Covid-19 is still a global pandemic to this day. Obesity is one of the comorbidities which has been shown to increase the severity of COVID-19 cases. There is research which stated that invasive mechanical ventilation may contribute to mortality rate in severe case of COVID-19. Other than that, the increase rate of severe COVID-19 cases and the limited availability of facilities and infrastructure makes the usage of high flow nasal cannula (HFNC) begins to be considered. Aim of this case report is to represent the outcome of obese COVID-19 patient whom treated with HFNC, furthermore could evoke readers to take further research of the effectiveness of HFC usage as an oxygen therapy in severe COVID-19 cases. Case summary: 21 years old woman came to Emergency Department in Wangaya Regional Hospital with shortness of breath, unproductive cough, sore throat, and inability to smell. Patient was treated and diagnosed with confirmed severe COVID-19. She got obesity as comorbidity (weight 105 kg; height 160 cm; Body Mass Index (BMI) 41.02 kg/m2). Patient was treated for 15 days, which include 10 days of treatment in negative pressure ward in Intensive Care Unit Department (ICU). During the treatment course, patient received pharmacologic and non-pharmacologic treatment, including the administration of HFNC. After five days of HFNC usage, oxygen supplementation was downgraded to conventional oxygen supplementation (non-rebreather mask and nasal cannula). Patient then discharge without any complain and proceed self-quarantine at home. HFNC could be considered as one of non-invasive oxygen supplementation treatment in patient with severe COVID-19 disease. The initiation of usage of HFNC could be started with 30 L/minute flow, with 40% fraction of inspired oxygen (FiO2) in accordance to patient comfort with Sp02 target 92-96%. If there is an increased breathing effort, high respiratory rate, and unachieved Sp02 target, flow and inspiration fraction titrated gradually. ROX index could be used as a treatment failure indication and the need of invasive ventilation.

Abstract

CA-125 is the most commonly used tumor marker in ovarian cancer, its known as the "Gold Standard" for the diagnosis of ovarian cancer. CA-125 allegely related to the inflammatory mechanisms which associated with leukocytes and lymphocytes, as well as Lymphocyte Platelet Ratio. This study is to prove above hypothesis. The objective is to investigate CA-125 correlation as tumor marker of inflammatory hematology examination which consisting of Hemoglobin, leucocytes and PLR as indicator of inflammatory examination in ovarian cancer patient. The method is an observational analytic study with cross sectional retrospective descriptive. The sample is 42 patients medical record who are treated in Oncology Polyclinic RSUD ULIN in October 2014 - October 2017 period. Samples selected by time limitation method. Sampling technique using nonpropability method followed by purposive sampling method. After the data is taken and selected then processed into SPSS for normality test and continued by multiple linear regression test. The results showed that correlation analysis of CA-125 tumor marker on Hb, leukocyte and PLR levels was P = 858b; P value <0.05. The CA-125 relationship to Hb, leucocytes and PLR is a negative relationship (r Hb = -3,463, r leukocytes = -6,117, r PLR = -2.281), which means that more high the Hb, leukocyte and PLR value, conversely CA125 value become more low. The conclusions of this study is there no correlation of CA125 tumor marker against Hb, leucocytes and PLR in ovarian cancer patients. The results of this study indicate that the increasing of CA125 value is negative correlation because it is not followed by decreasing of Hb, increasing of leukocyte and PLR.

Abstract

Vitiligo is a acquired skin disease characterized by a well-defined depigmented macules caused by progressive loss of functional melanocytes. Vitiligo usually appears first during childhood or young adults. Management of vitiligo in children are more difficult because the preference of therapy is limited when compared with adult. Targeted 308 nm UVB phototherapy is used for local vitiligo in children as it can be focused on the lesion and reduce the side effects. A 4-year-old girl, came to dermato-venereology polyclinic of Indera Hospital Denpasar with complaints of white patch on her lips for 3 weeks. Based on the history and physical examination, she was diagnosed with focal vitiligo. She was treated with targeted 308 nm excimer light phototherapy three times a week. Five sessions of phototeraphy resulted good response characterized by repigmentation of the lesion without redness and blisters.The therapy aims to achieve the best results with minimal side effects. Phototherapy targets are recommended for local vitiligo and especially for new small lesions as well as pediatric vitiligo. The mechanisms of this phototherapy is to trigger repigmentation, stabilization, immunosuppression and immunomodulation. The conclusion is targeted 308 nm phototherapy is quite useful for treating vitiligo even though more therapy sessions are needed.

Abstract

Diabetes is a disease characterized by increasing blood sugar. People with diabetes continue to increasing number each year, so it needs better prevention of diabetes. Chromium is a cofactor in improving insulin action in the transfer of glucose into cells. In addition to chromium, vitamin C and vitamin E also plays a role in the prevention of diabetes, which is an antioxidant that can capture free radicals and prevent a chain reaction result in no cell damage. The aim o thi study was to analyzed the effect of chromium, vitamin C, and vitamin E on blood sugar rats that induced by alloxan. This research is an experimental laboratory. Sample were 20 male Wistar rats during 6 weeks. The independent variables consist of 5 treatment groups namely normal diet; normal diet + 1 g / day chromium; normal diet + 2 mg / day of vitamin C; the normal diet + 0.5 mg / day of vitamin ; a normal diet + 1 g / day chromium + 2 mg / day of vitamin C + 0.5 mg / day of vitamin E. The dependent variable is blood sugar levels. To know the differences of each treatment used statistical tests One Way ANOVA, followed by Tukey HSD Post Hoc test with p <0.05. There were significant differences in blood glucose (p = 0.000) between the control group and group 1, 2, 4, but between control group and  groups 3 showed no significant difference in blood glucose (p = 0.718). Chromium, vitamin C, and a mixture of chromium, vitamin C, vitamin E showed an effect on blood sugar, while vitamin E alone had no effect on blood sugar.

Abstract

Increasing levels of blood cholesterol is one of the risk factors for atherosclerosis. Currently, experts have been trying used herbal medicine as an alternative treatment of atherosclerosis. One of the highlights is the use of celery stalks. Celery is generally used as a flavouring dish. In addition, celery stalks are believed able to reduce cholesterol levels because they contain flavonoids, tannins, and saponins. This study was aim to determine the effect of celery stalk ethanol extract to reduce LDL an increase of HDL level in rats after fed high-fat diet. Randomized Post Test Only Control Group Design was used to measure the HDL and LDL blood serum levels of male Wistar (Rattus norvegicus) rats fed a high-fat diet. This study was used 27 male Wistar rats (Rattus norvegicus) divided into 3 groups; K1 was only given regular feed and water, K2 was given a high-fat diet and water, K3 was given a high-fat diet and celery stalk extract at a dose of 75mg/kg BW/day for 14 days. The data were analysed by ANOVA test and Tukey HSD test. The results of the ANOVA test showed a significance value of p-value of LDL = 0.000 and HDL p-value = 0.536. Celery stalk extract (Apium graviolens L) had an effect on decreasing LDL levels but not on increasing HDL blood serum levels of white Wistar rats (Rattus norvegicus) given high-fat diet.

Abstract

Several studies have reported the Mesenchymal Stem Cell (MSC) triggered and inhibited tumor growth. We therefore performed a systematic review and meta-analysis of experimental studies evaluating the effects of MSC on tumors. Method: We collected the relevant studies by searching ProQuest, PubMed Publisher, and GoogleScholar. A meta-analysis was performed for the outcome measure: tumor volume and tumor weight. The search resulted in 440 references, of which 22 could be included in the meta-analysis. Of these 22 experiments, 18 (40%) showed a significant decrease of tumor growth and 27 (60%) experiments showed a significant increase of tumor growth. The engineered MSC and the late phase of MSC injection have a significant contribution to tumor inhibition effect. The meta-analysis showed that there are no significant effect of mesenchymal stem cell therapy to the tumor animal volume (SMD-0.23[-0.55, 0.09]; n=15), and to the tumor animal weight (SMD -0.19 [-0.77,0.39]; n=7). Conclusion: MSC appears to have a relative dependent effect in animal models.

Abstract

Trauma menjadi masalah kesehatan masyarakat yang utama di seluruh dunia karena dikaitkan dengan morbiditas dan mortalitas yang tinggi baik di negara maju dan berkembang. Trauma toraks terjadi hampir pada 50% dari semua kecelakaan dan menjadi penyebab penting kematian. Penelitian ini bertujuan untuk mendapatkan data tentang trauma toraks di ruang rawat inap bedah RSUD Gambiran Kediri periode Maret 2017 – Maret 2018. Jenis penelitian ini adalah deskriptif retrospektif dengan menggunakan semua jumlah kasus trauma torak sebanyak 31 pasien dari 545 pasien trauma bedah yang membutuhkan rawat inap. Hasil penelitian mendapatkan kasus trauma toraks paling banyak ialah fraktur iga (29%) yang terjadi pada jenis kelamin laki-laki (93,5%) dan kelompok usia 46-60 tahun (35,5%). Penyebab tersering adalah kecelakaan lalu lintas (80,6%) sedangkan penanganan paling banyak dilakukan ialah penanganan konservatif (100%). Durasi rawat inap pasien trauma toraks paling sering sekitar 1-3 hari (58,1%) dengan hasil akhir trauma toraks yang didapat umumnya sembuh (93,6%).
