Abstract

Healthcare providers’ perspectives on barriers and needs in delivering perinatal mental health services are crucial, as they play a central role in identifying and managing perinatal mental health problems and are often the first point of contact for women experiencing such conditions. This review aimed to describe healthcare providers’ views on the challenges and requirements in providing perinatal mental health services. A scoping review design was employed, drawing data from PubMed, ScienceDirect, EBSCO, and Google Scholar. The included articles were original research studies published in English or Indonesian within the last five years (2019–2024). The search terms included healthcare worker, healthcare professional, healthcare provider, barrier, need, challenge, perinatal mental, health service, and maternal mental health care. The findings from 15 relevant articles indicated that barriers to perinatal mental health services include limited awareness, insufficient knowledge and skills, cultural beliefs and stigma, inadequate resources and infrastructure, and poor communication. Identified needs encompassed enhanced training and resource allocation, public awareness campaigns, and the development of clear policies and guidelines. In conclusion, perinatal mental health services across various countries highlight substantial barriers and needs from the perspective of healthcare providers. Integrating mental health into routine maternal care and strengthening interdisciplinary collaboration may improve outcomes for both mothers and infants.Keywords: barriers; needs; healthcare providers; mental health; perinatal

Abstract

Premature aging reduces collagen production and activates MAPK and MMPs pathways. Continuous use of retinol may cause adverse effects; therefore, natural ingredients are recommended as alternative therapies. Noni fruit (Morinda citrifolia L.) has potential anti-aging properties; however, its polyphenolic compounds are sensitive and exhibit limited skin absorption. The application of nanotechnology may address these bioavailability challenges. This review aimed to identify, analyze, and provide evidence regarding the enhancement of bioavailability of Indonesian herbal plants through nanotechnology-based topical formulations for managing skin aging. A systematic literature review was conducted by searching accredited national and international journals indexed in Google Scholar using the keywords “Aging,” “Morinda citrifolia L.,” and “Nano” from 2014 to 2024. From 2,950 identified articles, 80 met the inclusion criteria. The findings indicate that noni fruit contains diverse bioactive compounds with significant anti-aging potential. It is rich in vitamin C, flavonoids, and phenolic compounds. Nanoparticle formulations of noni fruit enhance its anti-aging efficacy through multiple mechanisms and improve the permeation of active compounds into the skin. In conclusion, noni fruit demonstrates strong potential as a topical anti-aging agent, and its limitations can be overcome through nanoparticle-based technology, thereby increasing its therapeutic, functional, and economic value.Keywords: aging; Morinda citrifolia L.; nanotechnology

Abstract

Tuberculosis remains a major public health challenge in Indonesia, with a substantial disease burden particularly affecting the productive-age population. Beyond biological susceptibility, socio-economic disparities are recognized as key contributors to the risk of pulmonary tuberculosis. However, comprehensive analyses using nationally representative data remain limited. This study aimed to examine the association between socio-economic determinants and the incidence of pulmonary tuberculosis among adults in Indonesia. An observational analytic study with a cross-sectional design was conducted using combined secondary data from the 2023 Indonesian Health Survey and the National Socio-Economic Survey, comprising a total of 174,062 respondents. Socio-economic variables included educational level, income, housing density, housing environmental quality, and access to health services, while age, sex, place of residence, and occupation were treated as confounding variables. Data analysis consisted of univariate procedures, bivariate tests using Chi-square and Mann–Whitney, and multivariate analysis using multiple logistic regression. The findings indicated that low educational level (p<0.001; OR=1.8), low income (p=0.042; OR=0.8), high housing density (p=0.009; OR=1.4), male sex (p<0.001; OR=1.8), urban residence (p=0.029; OR=1.4), age (p<0.001; OR=1.02), and occupation (p=0.009) were significantly associated with pulmonary TB incidence. In conclusion, these results demonstrate that socio-economic and demographic factors play a substantial role in the occurrence of pulmonary tuberculosis in Indonesia. Strengthening TB control efforts through socio-economic improvement, enhanced educational attainment, and targeted promotive and preventive interventions for high-risk groups is essential to reduce disease transmission.Keywords: tuberculosis; socio-economic factors; adult

Abstract

Periodontitis is a progressive inflammatory condition affecting the supporting structures of the teeth, often originating from untreated gingival inflammation and potentially leading to tooth loss. This condition represents a significant public health concern, particularly among older adults, due to its impact on oral function, quality of life, and overall health. Limited knowledge regarding oral health is considered a critical contributing factor, as it may hinder preventive behaviors and timely treatment-seeking, thereby increasing the risk of disease progression. Therefore, understanding the role of knowledge in the occurrence of periodontitis is essential. This study aimed to examine the relationship between elderly knowledge of oral health and the incidence of periodontitis. A cross-sectional design was employed, involving 48 elderly participants aged 60–80 years selected using the Slovin formula. Data were collected through questionnaires to assess knowledge and clinical observation to determine the presence of periodontitis. Statistical analysis was conducted using the Chi-square test. The results showed that the majority of respondents had low levels of oral health knowledge, and more than half of the participants experienced periodontitis. Bivariate analysis revealed a statistically significant association between knowledge and the incidence of periodontitis (p < 0.05), indicating that lower knowledge levels were correlated with a higher occurrence of the disease. This finding suggests that knowledge plays an important role in influencing oral health status among the elderly. In conclusion, knowledge is a determinant factor in the occurrence of periodontitis among older adults, where inadequate knowledge is associated with an increased risk of the disease.Keywords: periodontitis; knowledge; elderly

Abstract

Improving the health of mothers, newborns, and children through various maternal and child health (MCH) initiatives is an essential priority. One important strategy to enhance the quality of MCH services is improving the quality of recording and reporting data through the Local Area Monitoring of Maternal and Child Health. Midwives’ performance plays a crucial role in ensuring the accuracy and completeness of these recording and reporting activities. This study aimed to analyze factors associated with midwives’ performance in recording and reporting PWS KIA data in the working area of Panarung Primary Health Center. This study employed a cross-sectional design. The sample consisted of 20 midwives working in the Panarung Primary Health Center area, selected using a total sampling technique. Data were collected using a structured questionnaire. The relationships between variables were analyzed using the Fisher’s exact test. The analysis showed that supervision (p = 0.03) and facility availability (p = 0.02) were significantly associated with midwives’ performance. In contrast, age (p = 1.00), education level (p = 0.45), length of service (p = 1.00), perception (p = 0.65), motivation (p = 1.00), and workload (p = 1.00) were not significantly associated with midwives’ performance. In, conclusion, midwives’ performance in recording and reporting data of the Local Area Monitoring of Maternal and Child Health is significantly associated with supervision and the availability of facilities.Keywords: supervision; facilities; midwives’ performance; maternal and child health

Abstract

Timely return of outpatient medical records is an important component of effective health information management in primary healthcare facilities. Proper management of medical records ensures the continuity of patient care and supports administrative and reporting functions. This study aimed to analyze the factors causing delays in the return of outpatient medical records at Community Health Center (Puskesmas) X based on predisposing, enabling, and reinforcing factors. This study employed a qualitative research design involving nine participants, consisting of one operational coordinator of the medical record unit, one medical record officer, and seven polyclinic staff members. Data were collected through in-depth interviews, observations, documentation review, and brainstorming sessions. The data were then analyzed to identify key factors contributing to delayed medical record returns. The results revealed several contributing factors to the delay. Predisposing factors included the presence of medical record staff who did not meet the minimum educational qualification of a diploma in medical record management and limited compliance among nursing staff regarding the required return time. Enabling factors involved the suboptimal use of expedition logbooks and the absence of tracer tools in the medical record borrowing process. Reinforcing factors included the lack of standard operating procedures (SOPs) regulating the return of medical records. Several improvement strategies were identified, including proposing medical record management training, assigning monitoring responsibilities to polyclinic coordinators, developing and implementing tracer systems, applying administrative sanctions such as additional tasks or point-based accountability mechanisms, and establishing formal SOPs for medical record return procedures. In conclusion, strengthening staff capacity, monitoring mechanisms, record tracking systems, and institutional regulations is essential to improve compliance with medical record return timelines and reduce delays in outpatient medical record management.Keywords: delay; medical records; primary healthcare center; health information management; outpatient services

Abstract

Hospitals, as part of the public service sector, are expected to implement strategies to improve service quality through the evaluation of Balanced Scorecard implementation. This study aims to identify and analyze the implementation of the Balanced Scorecard in improving organizational performance at Prof. Dr. W. Z. Johannes Regional General Hospital, Kupang. This study employed a qualitative research approach involving 16 participants and was supported by secondary data from the Financial and Development Supervisory Agency audit on hospital performance in 2022. The variables examined included financial perspective, customer perspective, internal business process perspective, and learning and growth perspective. The thematic analysis showed that the financial perspective covered hospital performance measurement, the benefits of performance assessment, constraints in financial performance management, and its impact. The customer perspective included patient satisfaction, complaint handling, and comparison of services received. The internal business process perspective highlighted the achievement of minimum service standards and service quality. The learning and growth perspective addressed human resource management, including medical, nursing, midwifery, and other health personnel, as well as hospital support for capacity building. Based on the Financial and Development Supervisory Agency audit data, the organizational performance of Prof. Dr. W. Z. Johannes Regional General Hospital, Kupang, measured using the Balanced Scorecard, was categorized as very good.Keywords: balanced scorecard; organizational performance; regional public service agency; regional general hospital

Abstract

Leprosy is a chronic infectious disease caused by Mycobacterium leprae that affects peripheral nerves, skin, the upper respiratory tract, eyes, and nasal mucosa. Despite global progress, it remains a public health problem in Indonesia. This study aimed to analyze the risk factors associated with leprosy among adults at UPT RSUD RAA Soewondo Pati in 2025. An analytical observational study with a case–control design was conducted involving 70 cases and 70 controls (1:1 ratio). Data were analyzed using Chi‑square tests and multivariate logistic regression. In results, age (p=0.007), sex (p=0.033), close contact with leprosy patients (p=0.005), personal hygiene (p=0.011), and immunity status (p=0.000) were significantly associated with leprosy. Multivariate analysis identified close contact with leprosy patients (OR=8.3; 95% CI: 1.897–36.310) and immunity status (OR=6.2; 95% CI: 2.798–13.570) as the strongest predictors. In conclusion, close contact with leprosy patients and compromised immunity status are the major risk factors for leprosy among adults. Strengthening early detection, monitoring close contacts, improving personal hygiene, and enhancing community immunity are essential strategies to reduce transmission and prevent new cases.Keywords: leprosy; risk factors; close contact

Abstract

Complete outpatient medical records are essential to ensure service quality, patient safety, and administrative efficiency in primary healthcare settings. However, incomplete documentation remains a persistent issue. This study aimed to analyze the factors contributing to incomplete outpatient medical record documentation at a primary healthcare center based on Gibson’s performance theory, encompassing individual, organizational, and psychological factors, and to identify priority problems and improvement strategies. A qualitative study design was employed, with data collected through interviews, observations, documentation review, and brainstorming. Problem prioritization was conducted using the Urgency, Seriousness, Growth (USG) method. The findings revealed that incomplete documentation was influenced by multiple factors. At the individual level, limited work experience and lack of training in medical records were identified. Organizational factors included the absence of control cards, constraints in computer network systems, suboptimal implementation of Standard Operating Procedures (SOPs), and excessive workload beyond core job descriptions. Psychological factors involved the absence of sanctions for incomplete documentation and insufficient attention to detail among staff. Several improvement strategies were proposed, including routine rechecking of record completeness before submission, staff training and supervision, optimization of facility usage, budgeting for electrical system improvements, and regular dissemination and provision of SOPs in each service unit. In conclusion, incomplete outpatient medical record documentation is driven by interconnected individual, organizational, and psychological factors; therefore, comprehensive and targeted interventions are required to improve documentation completeness in primary healthcare settings.Keywords: incompleteness; primary healthcare; medical records; urgency, seriousness, growth (USG)
