Abstract

Diabetes mellitus is a chronic metabolic disorder characterized by persistent hyperglycemia and associated with serious complications affecting multiple organs. The increasing global prevalence of diabetes has encouraged exploration of alternative natural therapies with antihyperglycemic potential. This study aimed to determine the effectiveness of corn silk (Zea mays L.) extract in reducing blood glucose levels in diabetic mice. An experimental study was conducted using two groups of mice, each consisting of six animals. Group I received corn silk extract, while Group II received metformin. Blood glucose levels were measured after treatment, and differences between groups were analyzed using the Mann–Whitney test. Both corn silk extract and metformin reduced blood glucose levels in diabetic mice. Statistical analysis showed no significant difference in the mean reduction of blood glucose levels between the two groups (p = 0.083). Corn silk extract demonstrated a glucose-lowering effect comparable to metformin in diabetic mice, indicating its potential as a natural antihyperglycemic agent.Keywords: corn silk; diabetes mellitus; metformin

Abstract

Bronchogenic cysts are rare congenital malformations of the respiratory tract, typically presenting as cystic structures filled with fluid or air. Although generally detected in childhood, some cases remain asymptomatic and are discovered incidentally in adulthood. This case report aims to describe the clinical findings, diagnostic process, and management of a giant bronchogenic cyst detected incidentally in an adult patient. This study used a case report approach. A 56-year-old female patient underwent routine health screening in which a thoracic abnormality was detected. Diagnostic evaluation included chest radiography, thoracic computed tomography (CT) scan, and CT-guided fine needle aspiration biopsy with cytological examination. Chest radiography revealed a large mass in the paratracheal region without mediastinal shift. Further evaluation with thoracic CT scan confirmed a massive hypodense cystic lesion measuring 14.8 × 9.0 × 17.0 cm in the left hemithorax, extending to the left paracardial region and causing partial collapse of the left lung. CT-guided fine needle aspiration produced clear fluid, and cytological analysis showed no evidence of malignancy, supporting the diagnosis of a benign bronchogenic cyst. The patient subsequently underwent surgical resection. In conclusion, Giant bronchogenic cysts may remain asymptomatic and be detected incidentally. Imaging modalities combined with histopathological confirmation play a crucial role in establishing the diagnosis. Surgical resection is the definitive treatment to prevent potential complications and possible malignant transformation, even in asymptomatic cases.Keywords: bronchogenic cyst; congenital lung cyst; case report

Abstract

Type 2 diabetes mellitus is a chronic metabolic disorder with a rising global prevalence and is frequently accompanied by hypertension. Adequate glycemic control plays a crucial role in preventing elevated blood pressure among patients with T2DM. This study aimed to compare systolic and diastolic blood pressure between patients with controlled and uncontrolled glycemic status. An analytical cross-sectional design was employed involving 71 patients with T2DM from three primary healthcare centers. Data were collected through measurement of glycated hemoglobin (HbA1c) levels to determine glycemic control status, along with blood pressure measurements. The data were analyzed using the Independent Samples t-test in JASP software. The statistical analysis revealed a significant difference in systolic blood pressure (p = 0.020) and diastolic blood pressure (p = 0.039) between the controlled and uncontrolled groups. Patients with uncontrolled HbA1c levels exhibited higher systolic and diastolic blood pressure compared to those with controlled HbA1c levels. These findings highlight the importance of routine HbA1c monitoring in primary healthcare settings to prevent hypertension among patients with T2DM. In conclusion, glycemic control status may serve as a significant predictor of blood pressure levels in patients with type 2 diabetes mellitus.Keywords: blood pressure; type 2 diabetes mellitus; glycated hemoglobin; hypertension; primary healthcare centers

Abstract

Postoperative wound care for Open Reduction Internal Fixation (ORIF) and Open Reduction External Fixation (OREF) is a core competency in medical–surgical nursing. However, access to task‑specific and affordable simulators remains limited, particularly in low‑resource educational settings. Low‑cost simulation models have been shown to enhance clinical skill acquisition and self‑efficacy among nursing students, yet challenges related to anatomical fidelity and wound realism persist in many existing models. This study aimed to develop and evaluate ORISIM, a low‑cost, context‑specific wound‑care simulator designed for ORIF and OREF postoperative conditions, and to assess its feasibility, usability, and educational value in nursing skills training. This research employed a Research and Development (R&D) design consisting of five phases: needs identification, prototype design and construction, expert validation, model revision, and preliminary testing. Three experts participated in the validation process: one orthotics–prosthetics specialist and two wound‑care nursing practitioners. Preliminary testing involved 20 undergraduate nursing students who had completed theoretical and demonstrative instruction on wound care. Quantitative data were collected using a modified Michigan Standard Simulation Experience Scale (MiSSES), while qualitative feedback was analyzed thematically. Expert validation indicated that ORISIM was feasible for educational use, with recommendations for improving anatomical fidelity and wound realism. Quantitative findings showed high levels of student acceptance across all MiSSES domains, with mean scores of 4.17 for self‑efficacy, 4.23 for educational value, and 4.10 for simulation realism. Students reported increased confidence in performing wound‑care procedures and highlighted the simulator’s usability and durability. Qualitative analysis produced three themes: wound realism, material characteristics, and educational value with ease of use. Although some students noted limitations in anatomical detail and material elasticity, ORISIM was widely regarded as beneficial for structured and repetitive skills practice. In conclusion, ORISIM demonstrates strong potential as a cost‑effective and educationally relevant simulator for ORIF and OREF wound‑care training. While improvements in anatomical fidelity are warranted, the model effectively enhances students’ self‑efficacy and supports foundational clinical skill development. Future research should incorporate experimental or quasi‑experimental designs to further evaluate ORISIM’s effectiveness and compare its performance with commercial simulators.Keywords: wound‑care simulation; nursing education; low‑cost simulator; clinical skills training; self‑efficacy

Abstract

Adolescent pregnancy remains a major reproductive health problem worldwide and is associated with substantial health and social consequences for both mother and child. Globally, a considerable proportion of girls give birth before the age of 18, and Indonesia continues to report relatively high rates. This study aimed to analyze the determinants and consequences of adolescent pregnancy based on evidence from recent studies. This study used a literature review design. Articles were retrieved from PubMed, ScienceDirect, and Google Scholar using Boolean search strategies. Publications from 2021–2025 were screened using predetermined inclusion and exclusion criteria, resulting in eligible studies for synthesis. The findings indicate that adolescent pregnancy is influenced by multiple interacting factors, including low education, limited reproductive health knowledge, poor socioeconomic conditions, early marriage, family and peer influence, media exposure, and risky behaviors such as substance use. Adolescent pregnancy is also associated with adverse outcomes, including maternal complications, low birth weight, prematurity, and increased risk of stunting, as well as social consequences such as school dropout.In conclusion, adolescent pregnancy is shaped by complex structural and behavioral determinants. Comprehensive prevention strategies involving early reproductive health education, youth-friendly health services, family and school involvement, and policies addressing education and social protection are essential to reduce its incidence.Keywords: adolescent pregnancy; determinants; reproductive health; adolescent

Abstract

Dental caries is one of the most common oral health problems experienced by school-age children, particularly those aged 6–14 years, who are considered a vulnerable and critical age group for the development of dental caries. This study aimed to determine the difference in knowledge about dental caries before and after counseling using a modified snakes and ladders game as an educational media among students of class 3A and 3B at Wachid Hasyim 2 Elementary School, Surabaya. This study employed a quasi-experimental design with a one group pretest–posttest approach. The population consisted of all students in class 3A and 3B at Wachid Hasyim 2 Elementary School, Surabaya. A total of 44 students were selected using simple random sampling. Data were collected using a questionnaire and analyzed using the Wilcoxon test. The analysis showed a p-value < 0.05, indicating a significant difference in knowledge about dental caries before and after counseling using the modified snakes and ladders game as educational media. In conclusion, counseling using a modified snakes and ladders game is effective in improving knowledge about dental caries among elementary school students.Keywords: dental caries knowledge; snakes and ladders game media; elementary school students

Abstract

Congestive heart failure (CHF) secondary to three‑vessel coronary artery disease (CAD 3VD) is characterized by high morbidity and markedly reduced functional capacity, highlighting the importance of effective and individualized rehabilitation strategies. Exercise‑based cardiac rehabilitation, particularly during Phase II, plays a critical role in restoring functional performance, improving cardiovascular efficiency, and enhancing quality of life in high‑risk cardiac patients. This study aimed to evaluate the safety and clinical effectiveness of a Phase II cardiac rehabilitation program incorporating structured walking exercises in a patient with CHF and CAD 3VD. A single‑case design was applied to a 54‑year‑old male diagnosed with CHF and CAD 3VD. The intervention consisted of ten supervised sessions of moderate‑intensity aerobic exercise using track and treadmill walking. Clinical outcomes were assessed using the Six‑Minute Walk Test (6MWT), metabolic equivalents (METs), heart rate–walking speed index (HRWSI), vital signs, and perceived exertion. Hemodynamic responses were monitored throughout all sessions to ensure safety and tolerance. Following completion of the program, the patient demonstrated clinically meaningful improvements in functional and aerobic capacity. The 6MWT distance increased from 428 to 535 meters, METs improved from 4.8 to 5.7, and HRWSI increased from 1.21 to 1.50. These gains were accompanied by stable hemodynamic parameters and reduced perceived exertion, indicating enhanced cardiovascular efficiency and improved exercise tolerance. In conclusion, supervised, progressive walking exercise within a Phase II cardiac rehabilitation program is safe, feasible, and effective for improving functional capacity in high‑risk patients with CHF and CAD 3VD. The observed improvements support the integration of structured walking modalities as a core component of individualized cardiac rehabilitation.Keywords: congestive heart failure; cardiac rehabilitation phase II; walking exercise; aerobic capacity; physiotherapy

Abstract

Pulp disease is one of the most common oral health problems among the elderly. In recent years, cases of pulp disease among older adults have shown a consistent increase. This condition indicates the need to explore contributing factors, particularly the level of knowledge regarding oral and dental health, which may influence the occurrence and progression of pulp disease in this population. This study aimed to determine the relationship between knowledge of oral and dental health and the incidence of pulp disease among the elderly at Wonosalam Primary Health Center, Jombang Regency. This study employed a cross-sectional design conducted at Wonosalam Primary Health Center, Jombang Regency. The study population consisted of elderly participants selected as research respondents. Data were collected using a structured questionnaire to assess the level of knowledge regarding oral and dental health. The association between knowledge and pulp disease was analyzed using the Chi-Square test. The analysis demonstrated a statistically significant relationship between knowledge of oral and dental health and the occurrence of pulp disease among the elderly (p = 0.03). Participants with lower levels of knowledge were more likely to experience pulp disease compared to those with better knowledge. As conclusion, knowledge of oral and dental health is significantly associated with pulp disease among the elderly. Improving educational and preventive efforts targeting older adults is essential to enhance awareness and reduce the risk of pulp disease.Keywords: knowledge; elderly; pulp disease; oral and dental health

Abstract

Malignant melanoma is a highly aggressive tumor in which immunohistochemical (IHC) evaluation is often hindered by endogenous melanin, whose brown pigment mimics DAB staining and may lead to false‑positive interpretation. An effective melanin bleaching method is therefore essential to ensure accurate IHC assessment. This study aimed to determine the optimal melanin bleaching protocol for melanoma, particularly for IHC analysis. This preliminary laboratory study used eighteen malignant melanoma samples. Initial bleaching with 0.5% H₂O₂ in PBS or Tris-HCl at 80°C for 4–12 minutes caused marked tissue damage. A second protocol using 5% H₂O₂ in PBS at 50°C for 60, 90, and 120 minutes was tested, followed by a 24‑hour room‑temperature protocol. Tissue integrity, bleaching efficacy, and subsequent HMB45 IHC staining were evaluated. High‑temperature bleaching (protocol 1) produced significant morphological deterioration. Bleaching with 5% H₂O₂ at 50°C effectively removed melanin while preserving tissue structure at 60 and 90 minutes, whereas 120 minutes caused tissue detachment. Room‑temperature bleaching for 24 hours also reduced melanin but resulted in partial tissue damage. HMB45 staining remained strong and interpretable after bleaching at 50°C for 60 and 90 minutes. In conclusion, the most favorable balance between melanin removal, tissue preservation, and IHC quality was achieved using 5% H₂O₂ in PBS at 50°C for 60 minutes. This protocol is recommended as the optimal melanin bleaching method for malignant melanoma tissue.Keywords: melanin bleaching; immunohistochemistry; malignant melanoma
