Abstract

Neonatal sepsis remains a major cause of morbidity and mortality among preterm infants, particularly in low- and middle-income countries. Early identification through clinical scoring systems, such as the Singh Score, Rosenberg Score, and Bekhof Score, may improve diagnostic accuracy and support timely management. This study aimed to systematically evaluate the diagnostic performance of these three clinical scoring instruments in detecting neonatal sepsis. A systematic search was conducted across seven electronic databases from inception to August 2025. Studies assessing the diagnostic accuracy of the selected scoring systems were included, critically appraised, and analyzed. Data on sensitivity, specificity, and other diagnostic accuracy measures were extracted and synthesized. Five eligible studies were identified, most of which were conducted in low- and middle-income countries, with sample sizes ranging from 80 to 658 neonates. The pooled sensitivity of the three instruments ranged from 0.56 to 0.76, with the Singh Score demonstrating the highest sensitivity (76%), followed by the Bekhof (63%) and Rosenberg (56%) scores. The pooled specificity ranged from 0.33 to 0.69, indicating low to moderate ability to exclude non-sepsis cases. Overall, these instruments demonstrate moderate sensitivity but limited specificity and are more suitable for ruling out sepsis rather than definitively confirming the diagnosis. Their use should be integrated with comprehensive clinical assessment and additional laboratory investigations. These scoring systems may aid in risk stratification and help reduce unnecessary treatment by identifying neonates at low risk of sepsis. Keywords: neonatal sepsis; preterm infants; clinical scoring system; diagnostic accuracy

Abstract

Stunting is a serious public health problem in Indonesia and reflects chronic malnutrition as well as poor environmental conditions, particularly inadequate sanitation. In the working area of Mejuwet Health Center, Bojonegoro Regency, the prevalence of stunting in 2023 remained above the WHO threshold for high prevalence. This study aimed to analyze the relationship between environmental sanitation conditions and the incidence of stunting among toddlers aged 24–59 months in the area. This research employed a quantitative method with an observational analytic approach and a cross-sectional design. The independent variables consisted of environmental sanitation indicators, while the dependent variable was stunting incidence. The sample included 180 toddlers divided into two groups: 90 cases (stunted toddlers) and 90 controls (non-stunted toddlers). Data were collected through questionnaires, direct observation, and secondary data from the health center. Statistical analysis was conducted using chi-square tests and logistic regression. The Chi-square results demonstrated significant associations between stunting and all environmental sanitation indicators: ownership of healthy latrines (p=0.000), availability of clean water facilities (p=0.001), solid waste disposal facilities (p=0.000), and wastewater disposal facilities (p=0.007). Overall environmental sanitation conditions were significantly associated with stunting (p=0.005). The most dominant variables influencing stunting were healthy latrine ownership and solid waste disposal facilities. In conclusion, all environmental sanitation indicators show significant relationships with stunting among toddlers. Integrated water, sanitation, and hygiene (WASH) interventions involving improvements in basic infrastructure and promotion of hygienic behavior are necessary to reduce stunting prevalence and create a healthy environment that supports optimal child growth and development. Keywords: stunting; environmental sanitation; toddlers

Abstract

Overweight among adolescents has become an important public health concern because it increases the risk of various degenerative diseases and long-term health complications. Poor dietary patterns, particularly frequent consumption of fast food and junk food, are often associated with limited nutritional literacy among adolescents. This study aimed to examine the effect of nutritional literacy on fast food and junk food consumption behavior among overweight adolescents. This study employed a quasi-experimental design with a pre-test and post-test control group approach. The sample consisted of 46 overweight adolescents from SMA Negeri 7 Bengkulu City, divided into an intervention group (23 participants) and a control group (23 participants), selected using purposive sampling. The intervention group received nutritional literacy education using a flipchart accompanied by explanations, while the control group received education through leaflets without explanation. The study variables included nutritional literacy, fast food and junk food consumption habits, body weight, and height. Data were analyzed using independent t-test, paired t-test, Mann–Whitney test, and Wilcoxon test. The results showed a significant increase in knowledge scores before and after the nutritional literacy intervention (p = 0.014). There was a significant difference in the mean change in attitude scores between the intervention and control groups (p = 0.0001), with a higher improvement observed in the intervention group. In addition, there was a significant difference in the mean change in fast food and junk food consumption habits between the two groups (p = 0.043). In conclusion, nutritional literacy education is effective in improving knowledge, attitudes, and reducing fast food and junk food consumption behavior among overweight adolescents. Keywords: nutritional literacy; consumption behavior; overweight

Abstract

Type 2 diabetes mellitus is a prevalent non-communicable disease associated with serious complications. Physical activity is a modifiable risk factor that plays an important role in glycemic control. This study aimed to analyze differences in fasting blood glucose and glycated hemoglobin (HbA1c) levels based on physical activity levels among patients with type 2 diabetes mellitus at primary health care centers in Kupang City. An analytic observational study with a cross-sectional design was conducted involving 71 respondents selected through consecutive sampling. Physical activity levels were assessed using the Global Physical Activity Questionnaire (GPAQ). fasting blood glucose was measured from capillary blood samples, while HbA1c was obtained from venous blood samples. Data were analyzed using univariate and bivariate methods, and mean differences across PA categories were tested using one-way ANOVA. The results showed no statistically significant differences in fasting blood glucose (p = 0.610) or HbA1c (p = 0.283) based on physical activity levels. In conclusion, physical activity level was not significantly associated with fasting blood glucose or HbA1c among patients with type 2 diabetes mellitus at primary health care centers in Kupang City. These findings suggest that optimal glycemic control requires a holistic approach, including physical activity, treatment adherence, and dietary management. Keywords: physical activity; fasting blood glucose; glycated hemoglobin; type 2 diabetes mellitus; primary health care centers

Abstract

Household waste is predominantly organic, consisting of food scraps, vegetables, and fruits that naturally decompose. However, the decomposition process may lead to environmental and health problems if not properly managed; therefore, appropriate waste management is essential. One method for managing household waste is composting. This study aimed to determine the potential of the bioinoculant BioTP to enhance macronutrient content in household organic waste compost. This research employed an experimental pretest–posttest with control design. Macronutrient levels (N, P, and K) in household organic waste were measured before and after the decomposition process in both the treatment group (with BioTP) and the control group. The data were analyzed using a t-test. The results showed that after decomposition, all macronutrients (N, P, and K) in the treatment group increased. The macronutrient content in all treatments met the standards for organic fertilizer. Statistical analysis before and after decomposition indicated a significant difference in nitrogen (N) levels in both the treatment and control groups (p < 0.05), whereas phosphorus (P) and potassium (K) levels showed no significant differences (p > 0.05). Comparison between the treatment and control groups also revealed no significant differences in N, P, and K levels (p > 0.05). In conclusion, BioTP has the potential to enhance macronutrient content in household organic compost; however, its effect has not yet reached an optimal level. Keywords: household organic waste; decomposition; bioinoculant; macronutrients
