Abstract

Depression and diabetes are often occur together and their morbidity has a significant impact of health outcomes. Antidepressants dan antipsychotics may cause metabolic abnormalities, as well as neurologic abnormalities. 52 years old diabetic woman presented to Immanuel Teaching Hospital with slurred speech. Since ten years ago, she had bipolar  mood  disorder, which was recently treated with clozapine. As patient consumed higher dose of clozapine (beyond her psychiatrist’s supervision), she had uncontrolled blood glucose level, fever, and slurred speech. The side effects of clozapine (weight gain, increased insulin resistance, dysarthria, fever) were discussed in this case report. During hospitalization, with strict supervision of clozapine use, the symptoms resolved. In diabetic patients with comorbid psychiatric disorders, a good collaboration between internist and psychiatrist is required to manage the patients effectively and comprehensively. Keywords: bipolar mood disorder, clozapine, diabetes mellitus, depression

Abstract

accelerates wound healing. We examined the effect of Ethanol Extract of Neem Leaf (EENL) inaccelerating wound healing and compared its potential with povidone iodine. This was ananalytic experimental study, using Complete Randomized Design (CRD) with a comparativemethod. We used 30 male Swiss Webster mice that were divided into five groups (n=6). Eachgroup was given the Ethanol Extract of Neem Leaf (EENL) with a concentration of 25%, 12.5%and 25%, aquades (negative control) and povidone iodine 10% (comparison). The averagewound healing time of EENL 25%, EENL 12.5% , EENL 6.25% , povidone iodine and aquadeswere 11.2; 12.2;13.2;13.2;and 1.3 days respectively. The data was analized with one wayANOVA and showed a highly significant difference of p=0.000. Tukey HSD test showed thatEENL 25% and 12.5% had better potential than povidone iodine 10% and EENL 6.25% had thesame potential with povidone iodine 10%. This study concludes that ethanol extract of neem leafhas an effect in accelerating the duration of wound healing, with EENL 25% as the bestconcentration in accelerating the duration of wound healing. Keywords: neem leaf, Azadirachta indica A. Abstract

Ulcerative colitis is a chronic inflammatory disease mainly affects sigmoid colon and rectum. The inflammation process will activate NF-κB and leads to proinflamatory cytokine release such as Interleukin-6 (IL-6). Fig leaves contain a high level of flavonoid which can prevent NF-κB activation, and further inhibits IL-6 secretion. This research aims to see the preventive effect of methanolic extract of fig leaves towards colon histopathological feature and IL-6 serum level on ulcerative colitis induced mice. Balb/C male mice were randomly assigned into 5 groups (n=5). The treatment groups were dextran suphate sodium (DSS) control group (group I), methanolic extract of Fig leaves dose 28 mg/ day control group (group II), and methanolic extract of Fig leaves dose 7 mg/ day (group III), 14 mg/ day (group IV), and 28 mg/ day (group V) for 14 days, which at the 8th till 14th day were given DSS to induce colitis. The results showed that both of colon mucosal damage and IL-6 serum level of group I were significantly different from other groups (p=0,029). In  conclusion, the methanolic extract of Fig leaves can improve colon mucous damage and decrease IL-6 serum level on ulcerative colitis-induced mice. Keywords: ulcerative colitis, fig leaves, colon mucousal damage, interleukin-6

Abstract

Domestic chicken eggshells contain calcium and protein that could be useful in wound healing process. This research was conducted to determine whether domestic chicken eggshells can accelerate wound healing. Twenty one male Swiss-Webster mice  with 2 cm incision were divided into 3 groups. Negative control group were given Carboxymethyl cellulose 1% (CMC), positive control group were given feracrylum 1% (FER), and the eggshells group given domestic chicken eggshells powder (KT). Treatment and data mesurement was measured every day until  the 7th day. On the 7th  day the average length of the wound were analyzed and the skin tissue is taken for microscopic examination. Data analyzed using one-way ANOVA followed by Post Hoc Least Significant Difference (LSD) with p=0,05. On macroscopic examination, there were  highly significant differences between KT compared to CMC and highly significant differences between KT compared to FER. the average length of wound on the 7th day on KT 0,63 cm shorter than CMC 1,07 (p=0,000) and FER 0,81 cm (p=0,003). On microscopic examination, the degree of reepithelization on KT groups are severe, moderate for collagen density, fibroblasts density, subepithelial neutrophil density, degree of edema, and mild for angiogenesis. We conclude domestic chicken eggshells powder can accelerate wound healing. Key words: domestic chicken eggshells, incision wound, wound healing

Abstract

Snake-bite is an important medical emergency case and caused of many hospitaladmission especially in the rural area, forests, plantations and swamps. Despite its importance, there have been fewer proper data of snake-bite incidence in Indonesia. World HealthOrganization estimate that at least 421,000 envenomings and 20,000 deaths from snakebitesoccur each year, especially in South and South East Asia and sub-Saharan Africa. The authorsreport a case of a 76-year-old man came to Hasan Sadikin Hospital with chief complaint woundin his right hand and right forearm from snake-bite. Snake-bites can cause DIC because thevenom activates the coagulation system and cause fibrinolysis which occurs in less than 24hours. Laboratory results, we found abnormalities such as anemia, thrombocytopenia, hypofibrinogenemia, and increased levels of D-dimer. Patients were treated for 8 days and thenallowed to go home. Snake-bite is an occupational disease of farmers, plantation workers, herdsmen, fishermen, other. Snake bite cases require prompt and comprehensive managementso as to minimize the possibility of disability and death. Keywords: snake bite, DIC, hypertension
