Abstract

The first step of clinical management of the temporomandibular disorders (TMD)is to distinguish the origin ofproblem, to classify into myogeneous and joint problems, which have been regarded to be an important process.Although the precise mechanisms for the symptoms or pathology of these conditions have not yet been clarified,muscular parafunction or hyperfunction has been regarded to play an important role in the etiology of TMD. Excessmechanical loading to the temporomandibular joint (TMJ) has been reported to be one of the possible causes for thedevelopment of osteoarthritis. These forces might be exerted through mandibular parafunctions like sleep bruxism ortooth contacting habit. For clinical control of sleep bruxism, a thick palatal appliance which fills the palate and doesnot cover the occlusal surface have been developed. In this review article, clinical implication of the relationshipbetween occlusion, mastication and bruxism have also been discussed.

Abstract

Osteoporosis incidence in Indonesia increase each year, 1 of 3 menopause women suspected have osteoporosis, then earlier detection is needed. Factors that influence the successful analysis is the choice of region of interset (ROI) and extract feature method. The purpose of this research is to determine the best method to define the bone quality based on trabecular of condylus analysis. Data were obtained from Dentistry Hospital, Padjadjaran University Bandung. Research were conducted cross-section to 79 samples which measured in dual energy X-ray absorbsimetry (DEXA) as a base standard then taken its panoramic radiograph. Trabecular analysis was conducted in ROI of condylus using panoramic radiograph then cursor was clicked in cortical endorsal following the condylus head shape. To reduce the noise, we conducted pre-processing by compensational method, it is a finding of the lowest means of variant number around condylus as a reduce factor then affect the radiograph of condylus become darker. Background sets in zero (0) meanwhile trabecular stay at gray scale. Feature extraction applied 3 analytical methods, they are: gray level co accurance matrix (GLCM), histogram and fraction. Statistical analysis shows T-score DEXA correlation with 3 methods, proofed that fraction method performed the best correlation which r value is 0.377and GLCM (contrast r=0.233, correlation =0.342, energy -0.147, homogenity= r =-0.107), meanwhile histogram (max histogram r=0.253, range histogram r=0.06). As a conclusion, fraction method with ROI of condylus head shape is the best method to determine osteoporosis in post menopause women.

Abstract

The selection of cervical edge form of porcelain fused to metal (PFM)crown is very important because it is associatedwith the incidence of gingivitis. Improper types of cervical edge may cause a gap at the edge; which is a good place forthe growth and development of dental plaque bacteria that play a major role in the occurrence of inflammation. Thisstudy aims to determine the effect of the cervical edge preparation shape of PFM crown against bacterial plaqueadhesion and tissue inflammation supporting abutments, as well as to determine the best shape of the cervical edgepreparation for PFM crown restoration. This Clinical experimental research with time series design is performed to 3groups of cervical edge preparation design, i.e. shoulder, bevel shoulder and deep chamfer. Sample of 15 peopledivided equally into the 3 groups. Examination of the clinical signs of gingivitis (gingival discoloration, bleeding onprobing, and pocket depth) and the number of bacterial colonies were performed before tooth preparation; 1, 7, and 21days after insertion. The data were processed statistically by ANOVA. The results showed the form shoulder bevel edgegives the most noticeable impact to gingivitis than other types.

Abstract

Management of certain prosthetic cases can be established with optimal result when prior orthodontic treatment isundertaken, such as example cases of missing teeth with diastema or abnormal arch form. A 24-year-old femalepatient came with the complaint of diastema as a result of tooth loss and need a denture. After the examination, itwas found that the teeth were unarranged and the arch form was abnormal. Orthodontic treatment was firstlyundertaken to arrange the teeth, establish the space for denture and fix the arch form. The denture was made afterfinishing the orthodontic treatment.

Abstract

Nanohybrid resin composite is one of the restorative dental materials that has goodaesthetic value and smooth surface. However, resin composite can be discolored due toconsuming foods and beverages, one of which is a carbonated beverage. To determine the difference of discoloration of polished and unpolished nanohybrid resin compositeafter the immersion in carbonated beverage. An experimental laboratories research withthe post test only control group design. In the group of polished composite, polishing wasdone on one side only, while the unpolished composit group, any treatment wasn't done.The total were 32 samples, divided into 4 groups which were immersed in carbonatedbeverages and artificial saliva for 7 days. The shredded material was replaced every 24hours. The instrument used for testing the discoloration was a spectrophotometer. Datawas analyzed by parametric statistical test One-way ANOVA and LSD (Least SignificantDifferent). There were significant differences between the unpolished composite resin thatsoaked in carbonated beverages and the other groups (p <0.05). Unpolished resincomposite are prone to discolorate compared to the polished resin composite

Abstract

A continuous temporary filling inside the mouth would expose acid produced by microorganism during fermentation ofcarbohydrate that would lower the pH of saliva. The aim of this research is to determine the variation on solubilitylevels of Cavit temporary filling immersed in artificial saliva with pH 4, 6, and 8. Samples were placed in tubecontainers with 2 mm in height and 7 mm in diameter. Samples were immersed in four different groups of artificialsaliva based on the pH, in which the first group was immersed in artificial saliva pH 4, the second in pH 6, the third inpH 8, and the fourth group was immersed in distilled water (pH 7). The results showed that after 7 day of immersion,there was a significant difference on solubility levels between the four groups of treatment (p<0.05). The data weretabulated and analyzed using one-way ANOVA and least significant difference (LSD). Therefore, it can be concludedthat pH can affect the solubility levels of Cavit temporary filling.

Abstract

Varicella is the fastest spreading infection. This virus classified as DNA virus with primer infection that causedchicken pox mainly seen in children. Reactivation of this infection causes herpes zoster or shingle. Painfull of areainvolved proceed the unilateral and segmental nature of disease. If facial involved segmentally, it occurs painespecially at the facial and dentition that sometimes suspect as pulpitis. This case report is aimed to discussdiagnosis and management of herpes zoster with headache. Patient, male, 66 years old came with chief complaint ofheadache and ulcers on right facial with eat disturbance due to oral cavity pain. The diagnosis was herpes zoster.Patient was prescribed with acyclovir 1600 mg 4 times per day, analgesic, antibiotic mouth wash, and vitamins.Oral pain disappeared in 3 days, dan after 3 weeks healed completely. Diagnosis of herpes zoster could be get fromdetail anamnesis and examination.

Abstract

A fixed restoration should be removed because it hasdistortions at the restoration or its abutment. It's not an easyprocess, especially when it has been cemented permanently. Todo so, it is necessary to know the restoration history, cement,abutment, "' and the method. Some methods for removing thefixed restoration intactly are follow: straight chisel and mallet,excavator, reverse mallet, ATD, peerless crown-a-matic,adhesive method, ligature, Reynold technique andforcep.Altough physical and psychological traumas during the removingprocess have been prevented, dentist must give adequateinformed concent, take care the abutment health and prevent thepatient from swallowing the restoration.

Abstract

Palm wine from the palm is a typical drink of Maiwa, Enrekang. Bitter palm wine taken from fermented fruit of sugarplant intercepts (Arenga pinnata) which is called sap. Excessive drinking palm wine can cause pathological effects inthe oral cavity, because the Palm wine have acidic pH that can affect the occurrence of dental erosion. The purpose ofthis study was to determine the effect of palm wine consumption on the occurrence of dental erosion. Thisobservational analytic with cross sectional approach showed that from 100 samples, 75 people had suffered dentalerosion due to bitter palm wine consumption and 25 people did not suffer dental erosion because they consumed sweetpalm wine only. So, it was concluded that palm wine consumption affects the dental erosion in Maiwa, Enrekang.

Abstract

Matrix metalloproteinase (MMP) is a component extracelluler of matrix hydrolised which has an important role inthe process of biology and pathology. MMP has also a role in deficiency of dentine organic matrix due todemineralization by bacterial acid and tooth caries development or controlling. MMP in host originates from salivaand dentin. Saliva penetrate on the open dentinal lesion, and MMP in saliva had direct access to dentinedemineralization. It has been said that MMP saliva derived have a role in organic matrix destruction. Dentinalmatrix degradation is supposed acted by bacteria enzyme. It was clear that the cause of dentinal matrix degradationis MMP-2 and MMP-9 in gelatinolitic process, and MMP-8 in collagenitic process. This paper was aimed to explainabout enzyme that has an important role in dentine caries.
