Abstract

Metal ceramic crown is a very popular restoration used in dentistry for itsadvantageous properties. However, when metal ceramic crown is used at anteriorregion, grey area usually occured around the margin area and cause poor esthetics.The esthetic of metal ceramic crown can be improved by modifying the porcelainmargin. Therefore providing better translucency of cervical area the gingival will notaffected by metal coping. Margin preparation for porcelain should be shoulder typewith internal round angle and is made about 90-100° to root surface with 1-1.5 mmthickness. This case report discussed about the management of metal ceramic dowelcrown on anterior teeth with modification of porcelain margin to achieve restorationwith excellent esthetics.

Abstract

Microbicidal mechanism with producing free radical superoxide is one of neutrophil response to the bacterialinvasion. When overproducted, the free radical superoxide greater than antioxidan defense system and destroyed thecell itself. P.gingivalis had been knowed as the major bacteria that induce periodontal inflammation. These bacteriacan stimulated neutrophil activities. This was aimed to determinate the superoxide production from neutrophilstimulated with P.gingivalis. Neutrophil as samples were isolated from subject who fulfill the selected criteria. Theintracelluler superoxide production was determinated by nitroblue tetra zolium (NBT)test and the extracellulerproduction by spectrophotometer with wave length 580 nm. The result showed there was superoxide production fromneutrophil stimulated by P.gingivalis. It was concluded that the stimulation of P.gingivalis on neutrophil can promotesuperoxide production both intracelluler and extracelluler.

Abstract

Ludwig’s angina is an acute progressive cellulitis that involves the submental and submandibular spaces, bilateralsublingual space, which is caused by the Streptococci bacteria. Characteristics of this disease are swelling of thebilateral soft tissue submandibular, submental region and floor of the mouth, and elevation of the tongue. Earlydiagnosis and management of infection in children is important because a child has tendency for a progressiveworsening and lead to systemic infection. Infections involving the facial regions in children are rare cases wherebyonly one case is found in the last two years at the oral surgery department. This report shows a case of a seven-yearold girl with Ludwig’s angina that came to the emergency division of the oral surgery department with a chiefcomplaint of swelling at the chin and both cheeks. The complaint was first known as caries at 36, 74, and 75.Treatment includes antibiotic administration, incision and drainage, and extraction of the causative tooth. Patientwas hospitalized and returned home when was in good condition.

Abstract

Squamous Cell Carcinoma (SCC) is common is a malignant neoplasma originated from epithelial cells. It is usually involving the oral mucosa and underlying tissue of the tongue, hypopharynx, and also larynx. This report discussed about the management of SCC at tongue with hemiglosectomy and selective neck dissection managed in a male, 57 years old who referred with chief complaints of a mass on the tongue measuring 0.5cm that did not heal, Incision biopsy result confirms a squamous cell carcinoma with well differentiated on his tongue with enlarged lymph nodes level I ipsilateral. The patient was hospitalized and hemiglosectomy surgery with selective neck dissection was conducted, with the entire edge of the excision boundary has been free of tumor mass. Hemiglosectomy with selective neck dissection is selected depending on the size of the lesion, location of lesion and metastases occurrence.

Abstract

Stem cells provide new hope to fasten the healing process of various oral diseases including periodontal tissue(periodontitis). At present time, there is not yet found a material that can fix the teeth perfectly. Adult stem cells frombone marrow widely used for treatment. Numbers of stem cell are limited so growth factor is necessary to promoteproliferation of stem cells. Growth factor that has been used is expensive and difficult to get. Because of theseconstraints, it is necessary to develop alternative uses of medicinal plants as growth factor that accelerates woundhealing process. This study is aimed to proven what extent the potential Pegagan (Centella asiatica L) as a naturalgrowth factors to stem cells. The ability of proliferation and osteogenic differentiation in mesenchymal stem cells(MSC)was analyzed by adding terpenoid fraction of Centella asiatica. MTT assay data showed that the addition ofPegagan on cultured MSCs can increase stem cell proliferation. Pegagan increases the ability of MSC proliferationand differentiate into osteoblasts It is concluded that pegagan increase the ability of MSC proliferation anddifferentiate into osteoblasts.

Abstract

The hardness of denture base is influenced with the habit of most commonly consumed. Ulee Kareng Aceh coffeewas atype of robusta coffee which is acidic. The purpose of this study was to determine the change of surfacehardness of acrylic denture base after being immersed in ulee kareng coffee for 3 days. Twelve specimens (5 mmdiameter and 2 mm thickness)were randomly distributed in two groups: control (immersion in distilled water) andexperimental (immersion in Ulee Kareng Aceh coffee). Hardness was measured before and after immersion usingKnoop Microhardness tester (Shimadzu). Data were statistically analyzed by Mann Whitney and Wilcoxon test (α =0.05). The results of the study showed a significant difference between the groups P < 0.05. The surface hardness ofacrylic denture base decreased after immersion for both group P < 0.05.

Abstract

Recurrent Aphthous Stomatitis (RAS) is a disorder in the oral cavity, with a characterized symptom as ulceration, recurrent and very painfull. The etiology is idiopathic, with multifactorial predisposition. Exfoliative cheilitis is a persistent lesion on the lip, with a characterized cracking and desquamative, with crustae and inflammation. An elderly male (72 yrs) suffered with ulcer on his oral cavity, cracking lips and pain on both of his cheeks, skin, since 5 years ago. The pain is recurrent. On the clinical examination, there were some desquamation, both on the skin and vermilion border, whether on the inner lips (labial fold mucosa), there were ulcers with diameter about 1 cm. The laboratory test was within normal limits, except the LED was 40 mm/hour (n:<15). The diagnosis was Recurrent Aphthous Stomatitis (RAS) for the ulcer and Exfoliative cheilitis for the cracking lips. The treatment he received was a gargle liquid, topical corticosteroid and supplement. The skin’s disorder was revered to the skin and genital disease department, for further management. As a dental general practioner, had to be very careful and familiar for every changes that may be occur both in the outer or inner oral cavity. Other disorder that need refferal, had to be done with team work, to the colleague from the right connection.

Abstract

Allergic stomatitis or stomatitis venenata is a hypersensitivity reaction caused by an allergen from medicines, foods,dental materials (restoration material, prosthetic, orthodontic appliance, mercury, acrylic, cobalt). This report willdiscusses the management of food allergic stomatitis. The common cause of food allergy in adult patients are orange(citrus fruit), nuts, fish, marine fish and wheat. Oral manifestations of allergic stomatitis begin with multiple vesiclesand becomes fibrin-covered ulcers with erythematous edges accompanied by inflammation and pain. The patient was a53-year-old woman who has suffered stomatitis since 6 months ago, and had seen dentist in private practice andgovernment dental-practice (hospitals and health centers). The patient has an a allergy history. Intra oral examinationfound the presence of multiple ulcers on the buccal mucosa upper right, cheek mucosa lower right and tongue. Thefinal diagnosis of the case is allergic stomatitis due to daily food alergic base on anamesis, clinical examination andlaboratory tests. Treatment to prevent allergic stomatitis causes, symptomatic therapy, and topical corticosteroids. Itwas concluded that management of allergic stomatitis should avoid the food as primary causes, giving mouthwashcontaining analgesic/anti-inflammatory and topical corticosteroids.

Abstract

Abnormalities of the teeth that have had periapical endodontic treatment generally due to recurrent infections of root canal. Infection of the root canal is caused by leakage due to poor quality of coronal restoration and iatrogenicfactors. Root canal treatment of the tooth with periapical cyst generally do conventionally. Medication materialsused for healing of periapical lesions is calcium hydroxide, since it has properties not irritating, alkaline pH andantibacterial capabilities, so it is a biological stimulator for the formation of hard tissue in the area of damage andis expected to accelerate the healing process. It was reported the case of a 23-year-old student came to the Dentalclinic of Conservation Specialist RSGM Sekeloa with complaints lower right back teeth has restored with amalgam,no pain, but less comfortable when chewing. The results obtained after treatment with calcium hydroxide andmedikamen gutta percha filling with resin-based root canal cement. Approximately 4 months later, the periapicalradiolucent area had thinned. So it was concluded that conventional endodontic treatment proved to heal periapicallesions including cysts, as long as the treatment is taken adequately.

Abstract

The orthodontic “finishing” phase is recognized for the many details to accomplish anexcellent result. A high percentage of finishing-phase difficulties arise because of toothsize imbalance that could have been considered during the initial diagnosis and treatmentplanning. The aim of this study was to determine whether the Bolton's analysis appliesfor the crowded and non crowded dentitions. This study involved 60 subjects (30crowded and 30 non-crowded; 29 males and 31 females). Criteria for sample selectioncomprised absence of interproximal caries/filling, no residual crown-bridge restoration,no tooth deformity, aged over 17 years, never undergone orthodontic treatment, and thepresence of teeth from right first molar through left first molar, both upper and lower.Tooth size measurements were performed on study models. The statistical analysis usedwere descriptive and t-test. The results of the study were as follows: the mesiodistal toothwidths on males were bigger that those on females, the mesiodistal tooth widths oncrowded group were bigger than those on normal group but the difference wasinsignificant statistically (p>0.05); anterior ratio in crowded group was smaller that thatin normal group; overall ratio in crowded group was bigger than that in normal group butthe difference was insignificant (p>0.05). Anterior ratio of the two groups was biggerthan Anterior Bolton Mean, it means that the discrepancy was present on the lower arch.Overall ratio of the two groups was smaller than Overall Bolton Mean, it means that thediscrepancy was present on the upper arch.
