Abstract

Lung adenocarcinoma is often associated with pulmonary complications such as hydropneumothorax and pleural effusion, and cardiac complications such as cardiac tamponade and pericardial effusion, but are very rare. In our case, a 52-year-old woman presented with worsening shortness of breath for 1 month. The patient underwent thoracentesis after chest X-ray showed right pleural effusion. Pleural fluid cytology showed pulmonary adenocarcinoma. Chest CT scan showed hydropneumothorax and pericardial effusion. In conclusion, pleural effusion is common in lung malignancies. In this case, hydropneumothorax occurred due to repeated thoracentesis of massive pleural effusion. Pericardial effusion is uncommon. This is due to the presence of metastases in the pericardium that obstruct lymphatic drainage, so pericardial fluid analysis is needed.

Abstract

One in four girls and one in 13 boys globally experience child sexual abuse (CSA), a public health crisis. Snakes and ladders-based reproductive health education to combat CSA. This study examined the impact of snakes and ladders game modification on CSA anticipation knowledge. The study was quasi-experimental with pre- and post-test control groups. Sample size was 118. The experimental group received a modified snakes and ladders game comprising 59 students, whereas the control group received a brochure. The sampling method is sequential. This study included grade 4-6 primary pupils. The snakes and ladders modification group averaged pretest 68.3 SD 16.4 and posttest 82.0 SD 12.5. The leaflet group averaged 67.8 SD 11.4 pre-test and 76.4 SD 14.1 post-test. The snake and ladder modification group increased 13.6 and the leaflet group 8.5. An average difference of 5.1 was found between the snakes and ladders modification group and the leaflet, with a p-value of 0.037 < It can be concluded that snake modification increases female students' average knowledge.

Abstract

Tuberculosis recurrence is caused by reinfection with Mycobacterium tuberculosis that has already been experienced or infection with a new strain of bacteria. The increasing incidence of TBMDR in the community accelerates the spread of TBMDR. Objective: To identify TBMDR treatment failure in relapse cases, and category 1 and 2 treatment failure. Method: Retrospective. Secondary data from the Ministry of Health SITB, 2017-2019 period shows that the sample size of relapses, category 1 and 2 treatment is 466 patients. Results: the relapse bivariate test and treatment failure categories 1 and 2 obtained a p-value of <0.05 (age 17-about 60 years and second-line sensitivity test). They are affected by HIV status. The regression results of the second sensitization test strongly influenced TB-relapsed patients to fail, dropout, and complete treatment compared to cured treatment patients, still-relapsed 17-44 years old patients, and category 1 and 2 treatment failure were more likely to be LFU than recover, and the second sensitivity test affected treatment failure and completion rather than recovery, although HIV status did not affect treatment outcomes. Conclusion: Strategy to reduce the rate of recurrence and conversion failure by examining genetic mutations (biomarkers) of M. tuberculosis. Abstract

Abnormalities in platelets can lead to severe consequences renal and systemic blood flow. The aim of this study is to determine the predictive ability of platelet indices (mean platelet volume, platelet-lymphocyte ratio, and platelet count) in relation to changes in blood pressure before and immediately after hemodialysis. This study included 98 patients undergoing hemodialysis at the RSU UMM Malang, aged ≥18 years, hemodialysis duration > 1 month, and signing informed consent. The predictor variable were MPV, PLR, and platelet count and the outcome was the incidence of IDHT. There was a significant difference in the blood pressure of patients undergoing hemodialysis before compared to immediately after hemodialysis, for both systolic and diastolic blood pressure (p < 0.001). To assess the sensitivity and specificity of the accuracy of the platelet count, MPV, and PLR markers in predicting IDHT, ROC curve was created based on the difference in systolic blood pressure before-after hemodialysis. From the receiver operating characteristic (ROC) curve, it was found that the MPV value was the highest above the diagonal line compared to PLR and platelet count with an MPV cut-off value was 9.15. This indicates that MPV is the best predictive marker compared to the other models.

Abstract

Vernal keratoconjunctivitis is an ophthalmological condition that presents significant challenges, especially in children and adolescents, due to chronic inflammation of the conjunctiva and cornea. This inflammation leads to distressing symptoms such as itching, irritation, and photophobia, which severely impact the quality of life. Traditional treatments for this condition often involve antiallergic medications like dexamethasone, prednisolone, and quercetin. However, understanding the precise mechanisms and comparative effectiveness of these treatments is crucial for improving therapeutic outcomes. In this study, protein structure data for dexamethasone (4UDA), prednisolone (8CC1), and quercetin (1JUH) were sourced from the Protein Data Bank (PDB). The crystal structure of the Histamine H1 Receptor (8X5X) was also retrieved from PDB. In silico modeling was carried out using the Cluspro web server to simulate the interactions between these drugs and the receptor. The analysis revealed that quercetin exhibited the lowest Weighted Score of -1810 in cluster 2, outperforming dexamethasone (-1223) and prednisolone (-1372.4), indicating a higher binding potency to the Histamine H1 Receptor. These findings suggest that quercetin has superior potential compared to dexamethasone and prednisolone for binding to Histamine H1 Receptors in the context of vernal keratoconjunctivitis.

Abstract

Introduction: Infertility is the inability to achieve pregnancy after 12 months of regular unprotected intercourse, affecting millions globally. In Indonesia, it affects 10-15% of reproductive-aged couples. Infertility can be classified as primary (no prior pregnancies) or secondary (previous pregnancies). HSG is commonly used to assess uterine and tubal abnormalities, key contributors to infertility. Methodes: Descriptive qualitative study are done to examine correlation of age, uterine abnormalities, and fallopian tube abnormalities from HSG findings with the prevalence of infertility from patients in Radiology Department from Prof. Dr. I.G.N.G. Ngoerah Central General Hospital, covering the period from January 2021 to June 2024.Results: 66 patients were collected as samples. 47 patients (71.21%) have primary infertility, while 19 patients (28.79%) have secondary infertility. 9 patients (13.63%) have uterus abnormalities, while 21 patients (31.81%) have fallopian tube abnormalities.Discussion: Hysterosalpingography (HSG) is a key diagnostic tool in evaluating infertile patients, providing insights into uterine and fallopian tube morphology and patency. It detects structural abnormalities like retroflexed uterus, didelphys, or blocked tubes, which can hinder sperm-egg interaction, embryo implantation, or cause recurrent miscarriages, affecting fertility. Abstract

Self-esteem is an individual's self-evaluation that expresses approval or disapproval and identifies the level of individual confidence in their abilities, meaningfulness, success, and self-worth, while therapeutic communication plays a role in helping solve problems, healing, clarifying and reducing emotional and mental burdens, and helping to take effective action for patients. The purpose of this study was to determine the effect of therapeutic communication on the level of self-esteem in the elderly in Laut Dendang Village, Percut Sei Tuan District. This study used a quasi-experimental method (pretest-posttest). The study population was 62 people. The sample was taken using purposive sampling as many as 17 respondents. The results of the Paired T-test test showed the effect of therapeutic communication on the level of self-esteem in the elderly in Laut Dendang Village, Percut Sei Tuan District. The results of data analysis show a p-value of 0.000 with p ≤ 0.05 and a mean value (2.100). There was an increase of 2.1% after the intervention. In conclusion, there is an effect of therapeutic communication on the level of self-esteem in the elderly in Laut Dendang Village, Percut Sei Tuan District. This communication greatly affects self-confidence values related to feelings of weakness and helplessness

Abstract

Biofilm formation is a virulence factor contributing to the pathogenicity and antibiotic resistance of Klebsiella pneumoniae. This process occurs due to biofilm genes in clinical isolates, especially the mrkA, mrkD, and fimH genes. These three genes are related the quality of biofilms formed, which lead to chronic and prolonged infections. This study was conducted to determine the presence of mrkA, mrkD, and fimH genes as biofilm formers in Klebsiella pneumoniaeclinical isolates. The study used a descriptive in vitro laboratory study, with total of 24 Klebsiella pneumoniae clinical isolates from the collection of the Department of Microbiology, Faculty of Medicine, Universitas Tanjungpura. Isolates were subculture in MacConkey Agar, Gram staining, followed by DNA extraction and gene detection using PCR. The results showed the presence of the mrkA gene in 91.6% of the isolates, while the mrkD and fimH genes were each in 87.5% of the isolates. The high percentage of biofilm gene detected, should increase health workers and stakeholder awarness about the impact.

Abstract

Drug-induced liver injury is the most common cause of acute liver failure in the United States. Hepatocyte injury can be caused by the consumption of high doses of aspirin, leading to the formation of free radicals. Golden sea cucumber (Stichopus hermanii) can play a role in prevention of free radicals through antioxidant. The study used a post test only control group design involving 32 wistar male rats divided into 4 groups : negative control (K-), positive control (K+), and two groups treated with Stichopus hermanii extract at doses of 486 mg/kg BB (KP1) and 972 mg/kg BB (KP2) for 23 days. The results of the study with Man-Whitney test showed that neither the groups treated with Stichopus hermanii extract at both doses of 486 mg/kg BB nor 972 mg/kg BB doses of Stichopus hermanii extract led to a significant reduction in ALT levels when compared to positive control. There was no significant difference in ALT levels between the two extract doses. The conclusion of this study is the administration of Stichopus hermanii extract at doses of 486 mg/kg BB dan 972 mg/kg BB for 23 days can not reduce ALT levels in aspirin-induced experimental animal.

Abstract

Visual impairment in children still becomes a burden globally. It can affect children’s learning process, career options, and psychosocial development eventually. This study aims to determine the prevalence of visual impairment among schoolchildren in Lengkong Subdistrict, Bandung. This was a retrospective descriptive study using secondary data from visual impairment screening program of schoolchildren aged 6-14 years, which was conducted by Health Promotion Unit of Bandung Eye Center Hospital from August to November 2023. The sample for this study was collected using a total sampling technique. A total of 907 data consisting of Uncorrected Visual Acuity (UCVA), Presenting Visual Acuity (PVA), age, and gender were collected and analysed using Microsoft Excel 2019. The prevalence of visual impairment in schoolchildren was 16.2% with UCVA and 11.1% with PVA, moderate visual impairment was the major classification. Female students were more likely to have visual impairment compared to male students. The highest number of visual impairments with UCVA and PVA compared to age distribution was 12 years old. These results need to become school and government’s concern to create further policy regarding children’s eye health to support an optimal learning process.
